Purpose: Phase difference enhanced (PADRE) imaging can enhance myelin density and delineate the superior cerebellar peduncle (SCP). We aimed to determine if SCP atrophy was distinguishable on PADRE imaging and evaluate its diagnostic performance compared with previous MRI progressive supranuclear palsy (PSP) findings., Methods: Two reviewers measured the SCP widths on PADRE in 20 PSP and 31 Parkinson's disease (PD) patients. The SCP and middle cerebellar peduncle (MCP) widths and the pons and midbrain areas were measured on 3D-T1WI, and the ratio of the area of the pons to the area of the midbrain, the MCP/SCP ratio, and the magnetic resonance parkinsonism index (MRPI) were calculated. We used the Steel-Dwass test to compare PSP, PD, and HS, and receiver operating characteristic curve (ROC) analyses to assess the sensitivity and specificity for diagnosing PSP from PD. A comparison of ROC curves was performed between the SCP on PADRE and these 3D-T1WI parameters., Results: In radiologist 1, the SCP on PADRE in PSP (1.1 ± 0.3 mm) was significantly smaller than those in PD (2.4 ± 0.4 mm) (P < 0.001); the area under the curve (AUC) was 0.97. At a 1.75-mm cutoff value, the diagnostic sensitivity and specificity for differentiating PSP from PD were 93.5% and 100%, respectively. The AUC of the SCP on PADRE was significantly higher than the 3D-T1WI parameters (the SCP, MCP, pons area, MCP/SCP ratio, and MRPI)., Conclusion: Assessing SCP with PADRE imaging may yield high diagnostic accuracy for discriminating PSP from PD., (© 2023. Purpose: Model-based iterative reconstruction (MBIR) yields higher spatial resolution and a lower image noise than conventional reconstruction methods. We hypothesized that thin-slice MBIR designed for brain CT could improve the detectability of acute ischemic stroke in the middle cerebral artery (MCA) territory., Methods: Included were 41 patients with acute ischemic stroke in the MCA territory; they were seen at 4 medical centers. The controls were 39 subjects without acute stroke. Images were reconstructed with hybrid IR and with MBIR designed for brain CT at slice thickness of 2 mm. We measured the image noise in the ventricle and compared the contrast-to-noise ratio (CNR) in the ischemic lesion. We analyzed the ability of reconstructed images to detect ischemic lesions using receiver operating characteristics (ROC) analysis; 8 observers read the routine clinical hybrid IR with 5 mm-thick images, while referring to 2 mm-thick hybrid IR images or MBIR images., Results: The image noise was significantly lower on MBIR- than hybrid IR images (1.2 vs. 3.4, p < 0.001). The CNR was significantly higher with MBIR than hybrid IR (6.3 vs. 1.6, p < 0.001). The mean area under the ROC curve was also significantly higher on hybrid IR plus MBIR than hybrid IR (0.55 vs. 0.48, p < 0.036). Sensitivity, specificity, and accuracy were 41.2%, 88.8%, and 65.7%, respectively, for hybrid IR; they were 58.8%, 86.1%, and 72.9%, respectively, for hybrid IR plus MBIR., Conclusion: The additional thin-slice MBIR designed for brain CT may improve the detection of acute MCA stroke., (© 2021. Purpose: The typical MRI findings in corticobasal degeneration (CBD), which have been described in previous reports, may be non-specific. We evaluated cerebral gyri (CG) using quantitative susceptibility mapping (QSM) images of patients with CBD, progressive supranuclear palsy (PSP), and Parkinson's disease (PD) to determine the possibility of discriminating them on an individual basis., Methods: After reviewing the normal appearances on QSM on 16 healthy subjects, two radiologists assessed abnormal findings from 12 CBD, 14 PSP, and 30 PD patients. For conventional MRI, two radiologists independently reviewed typical CBD findings that have been previously reported. We also investigated three autopsy cases including one each of CBD, PSP, and PD to reveal the histopathological basis of MRI findings., Results: CBD-specific findings included three layers; a higher susceptibility layer in superficial GM, a lower susceptibility layer, and a higher susceptibility layer in corticomedullary junction, with frequencies of 83% (10/12) in CBD, 21% (3/14) in PSP, and 0% (0/30) in PD patients. The typical CBD findings on conventional MRI were observed in only 42% (5/12) of CBD patients. Ferritin-positive microglia accumulated in the superficial gray matter (third cortical layer) and corticomedullary junction in CBD patients., Conclusions: The CG findings on QSM images may be more useful than those on conventional MRI for discriminating CBD from PD on an individual basis. Based on postmortem pathological data, cortical QSM hyperintensity might be an expression of ferritin-positive microglia.

Introduction: The purpose of this study was to evaluate the role of contrast-enhanced fast imaging with steady-state acquisition (CE-FIESTA) for assessing whether dural attachment in intracranial meningiomas is adhesive or not by correlation with intraoperative findings., Methods: Fourteen consecutive patients who were candidates for surgical treatment of meningiomas were prospectively analyzed with preoperative magnetic resonance imaging, including CE-FIESTA at 3 T. First, two neuroradiologists assessed several characteristics of the attachment of the meningioma to the dura mater or skull base on CE-FIESTA images. Second, the surgical findings of adhesion at the dural attachment of meningiomas were evaluated by two neurosurgeons. Finally, the CE-FIESTA findings were correlated with the surgical findings by one neurosurgeon and one neuroradiologist by consensus., Results: CE-FIESTA clearly depicted a hypointense marginal line at the attachment site of the meningioma. When CE-FIESTA revealed smooth marginal lines or hyperintense zones along the marginal lines, tumors were detached easily from the dura mater. On the contrary, when CE-FIESTA showed an irregularity, such as partial disruption of the marginal lines, vessels, or bony hyperostosis, the tumors tended to adhere firmly to the dura mater, which was found to contain small vessels and fine fibrous tissues., Conclusions: There seems to be an excellent correlation between the characteristics of dural attachment of meningiomas on CE-FIESTA images and intraoperative findings. Therefore, for operative planning, CE-FIESTA may provide useful information regarding the adhesiveness of dural attachment.

Voxel-based morphometry (VBM) done by means of MRI have provided new insights into the neuroanatomical basis for subjects with several conditions. Recently, VBM has been applied to investigate not only regional volumetric changes but also voxel-wise maps of fractional anisotropy (FA) computed from diffusion tensor imaging (DTI). The aim of this article is to review the recent work using VBM technique in particular focusing on schizophrenia, temporal lobe epilepsy (TLE), and Alzheimer's disease (AD)/mild cognitive impairment (MCI). In patients with schizophrenia, VBM approach detects the structural brain abnormalities that appear normal on conventional MRI. Moreover, this technique also has the potential to emerge as a useful clinical tool for early detection and monitoring of disease progression and treatment response in patients with schizophrenia or AD/MCI. In TLE, VBM approach may help elucidate some unresolved important research questions such as how recurrent temporal lobe seizures affect hippocampal and extrahippocampal morphology. Thus, in the future, large cohort studies to monitor whole brain changes on a VBM basis will lead to a further understanding of the neuropathology of several conditions.

Introduction: The purpose of this study was to compare the visibility of the normal subthalamic nucleus (STN) between fast spin-echo T2-weighted (FSE T2-W) images and fast short inversion time inversion-recovery (FSTIR) images, and to assess the age-related changes of the STN at 3.0 T., Methods: We evaluated high-resolution FSE T2-W and FSTIR images in 24 neurologically normal subjects. Using both sequences, we assessed the visibility of the margins of the STN, and calculated the difference in signal intensity between the STN and structures adjacent to the STN. Then, to assess the age-related changes of the STN, we calculated the contrast-to-noise ratio between the STN and the white matter, and measured the position, length, and height of the STN. All data were statistically evaluated., Results: The visibility of the lower margin of the STN was significantly better on the FSTIR images than on the FSE T2-W images (P = 0.0001), while the visibility of other margins was significantly better on the FSE T2-W images than on the FSTIR mages (P = 0.002). The difference in signal intensity between the STN and substantia nigra was significantly greater on the FSTIR images than on the FSE T2-W images (P < 0.0001). The distance from the midline to the lateral border of the STN increased with age (FSE T2-W images: left r = 0.4916, P = 0.015; right r = 0.4442, P = 0.030)., Conclusion: The combined reading of both FSE T2-W and FSTIR images at 3 T will improve the identification of the STN. The age-related positional change of the STN should be considered in target determination for deep brain stimulation procedures.

Introduction: Although radiation therapy plays an important role in the management of children with brain tumors, radiation-induced brain damage sometimes occurs after radiation therapy. In some pediatric patients who had undergone cranial radiation therapy, we noticed cystic changes in the brain on follow-up MRI. The purpose of this study was to determine the frequency, latency, relationship with patient age, radiation dose, and serial change in the cystic changes in children after cranial irradiation., Methods: We retrospectively studied MRI in 33 children who had undergone cranial irradiation for their primary brain tumors. Postirradiation cystic change in the brain on follow-up MRI was defined as a well-demarcated, oval-shaped, CSF-like signal intensity area, and no contrast enhancement., Results: Of the 33 patients, 6 (18.2%) had one or more cystic lesions. The latency of the cystic changes ranged from 1.5 to 7 years (mean 2.6 years). Cystic changes were found in the subcortical, periventricular and other deep white matter and the basal ganglia. The size of the lesions ranged from 1 to 10 mm at their first appearance; eight cystic lesions increased in size. None the cystic lesions reduced in size or resolved with time and none required intervention. All patients with cystic changes had received irradiation when they were 6 years of age or younger. The cystic changes occurred within the radiation field in patients treated with a radiation dose of 36 Gy or more., Conclusion: Asymptomatic brain parenchymal cystic changes appear to occur in children who have undergone cranial irradiation at 6 years of age or younger.

On T2-weighted MR images, the pontine tegmentum frequently shows a signal of high intensity in neurologically healthy individuals. We examined whether the signal intensity of the pontine tegmentum normally differs from that of the pontine base. We evaluated the signal intensity of the pontine tegmentum and pontine base on T2-weighted images from 38 neurologically healthy subjects. The subjects included 29 adults (16 males and 13 females, age range 23-48 years, mean age 39.5 years) and 9 children (4 boys and 5 girls (age range 4-9 years mean age 6.5 years). We compared the contrast-to-noise ratio (CNR) between the tegmentum and the base in the upper pons, midpons and lower pons, and evaluated the signal intensity ratio of the tegmentum to the base. The CNR was significantly higher for the tegmentum than the base at each level of the pons (P<0.0001), and the signal intensity ratio of the tegmentum to the base in the upper pons was significantly higher in children than in adults (P<0.005). On T2-weighted images, a high signal intensity of the pontine tegmentum is frequently seen in neurologically healthy subjects. This finding should not be considered abnormal, particularly in children.

Hypothalamic hamartomas are relatively rare, non-neoplastic congenital malformations. With conventional MR images alone, small hypothalamic hamartomas may be difficult to diagnose because of artifacts from cerebrospinal fluid. We present the usefulness of three-dimensional constructive interference in steady state sequence for evaluating small hypothalamic hamartomas in three pediatric patients.
