Abstract

Behcet病は,再発性口腔内アフタ性潰瘍,皮膚症状,外陰部潰瘍,眼病変を4大主症状とする原因不明の炎症性疾患である.本症はHLA-B51との有意な相関が認められるが,その病態形成にあたっては, Tリンパ球の異常反応に基づく好中球機能の亢進が中心的役割を果たすものと考えられる.近年,本症の難治性病態の1つとして,進行性の痴呆を引き起こす慢性進行型の神経Behcetが注目されている.本症は,早期より髄液中IL-6の持続性上昇を示すことから,これにより早期診断を行った上で,メトトレキサートを中心とした治療を行う必要がある.

Abstract

症例は43歳,女性.両足底部のしびれ感と疼痛,両足関節痛にて発症.関節炎・リンパ球減少・抗核抗体陽性・抗DNA抗体陽性よりSLEと診断.抗リン脂質抗体陽性で,無症候性の多発性脳梗塞も認めた.針筋電図と左腓腹神経生検にて,神経栄養血管の血栓形成による二次的なポリニューロパチーと診断した.我々の調べた範囲では,抗リン脂質抗体に起因すると考えられる微少血管の血栓形成による二次的末梢神経障害の記載はなく,今後注意すべき病態と考え報告した.
