Combined large-cell neuroendocrine carcinoma (LCNEC) and small-cell lung cancer (SCLC) is extremely rare, with only a few reports available in the literature. An accurate diagnosis is difficult to make due to the overlapping clinical features between LCNEC and SCLC, and a standardized treatment option is lacking. A 53-year-old female patient was admitted to Xiaoshan Affiliated Hospital of Wenzhou Medical University (Hangzhou, China) due to symptoms of dyspnea and phlegm, with blood in the sputum. Computed tomography revealed a 52×32×26-mm irregular soft-tissue mass in the left upper lung. Pathological examination of the biopsy specimen showed a poorly differentiated neuroendocrine carcinoma with compression injury, consistent with a mixed type of large and small cell carcinoma. The patient was administered chemotherapy, radiotherapy and targeted therapy, and as of October 2023, the patient had a survival period of 29 months. LCNEC combined with SCLC is a sporadic tumor with a high potential for malignancy. Multidisciplinary treatment and close follow-up are recommended. Pleomorphic undifferentiated sarcoma (PUS) of the mediastinal thymus is a rare type of cancer. In the present case report, a 67-year-old female patient presenting a mediastinal mass for >1 year was assessed for clinical characteristics, histopathological, immunohistochemical expression and gene mutation using fluorescence in situ hybridization (FISH), and relevant literature was reviewed. Histological analysis revealed nodular changes of different sizes in the thymus, which consisted of a mixture of pleomorphic and spindle cells. The pleomorphic cells with distinct atypia were giant cells and multinucleated cells with large cell sizes and frequent nuclear divisions. The spindle cells were mild to moderate atypical and arranged in a woven pattern, and nuclear division was rare. Immunohistochemical analysis indicated that vimentin was diffusively expressed in tumor cells. No amplification was found in CDX2 and MDM4 genes using the FISH analysis. In conclusion, mediastinal thymus neoplasm should be considered in the presence of PUS and it is an exclusionary diagnosis based on clinical and pathological examination of the patient. P40 and thyroid transcription factor-1 (TTF-1) dual expression in non-small cell lung cancer (NSCLC) is a rare occurrence. However, the presence of EML4-ALK and PIK3CA gene mutations in this type of cancer is unknown. The present study describes the case of a 38-year-old male patient who had never smoked. A 4.5-cm mass adjacent to his right upper mediastinum was detected by a computed tomography (CT) scan of the chest. Biopsy of the level four lymph nodes in the right mediastinum revealed microscopic morphological features typical of high-grade NSCLC. Immunohistochemical findings resembled those reported previously for several cases of NSCLC with the dual expression of P40 and TTF-1 markers. In addition, echinoderm microtubule-associated protein-like 4-anaplastic lymphoma kinase (EML4-ALK) and phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit a (PIK3CA) gene mutations were detected using high-throughput next-generation sequencing. To the best of our knowledge, this is the first report of NSCLC with the expression of P40 and TTF-1 as well as EML4-ALK and PIK3CA gene mutations. The present study reported a case of primary malignant melanoma of the bladder colliding with high-grade non-invasive urothelial papillary carcinoma with clinical, pathologic and immunohistochemical analysis, and reviewed the relevant literature. A 74-year-old male presented with hematuria; B ultrasound and computed tomography revealed a solid mass in the bladder and transurethral resection of the bladder lesion was performed. Microscopically, the tumors were composed of morphologically diverse malignant melanomas (95%) and high-grade non-invasive urothelial papillary carcinoma (5%), with no closely related or transitional regions. Immunohistochemistry indicated that malignant melanoma cells expressed HMB45, Melan-A and S-100, whereas they did not express any epithelial markers. The urothelial carcinomas expressed broad-spectrum cytokeratin and GATA3, and were negative for melanoma markers. The diagnosis of collision tumor between primary malignant melanoma of bladder and high-grade non-invasive urothelial papillary carcinoma depends on clinical and pathological examinations; this pathology is prone to recurrence and metastasis and has a poor prognosis. Sarcomatoid carcinoma is a rare disease, while larynx sarcomatoid carcinoma is a rarer type of cancer characterized by high malignancy and rapid growth. This type of cancer is prone to misdiagnosis and recurrence. The present case study aimed to provide a detailed report on the diagnosis and treatment of a male patient who underwent three tumor resections in the same location of the vocal cords within a short time. Immunohistochemical analysis was not performed after the first two surgeries at other hospitals, thus resulting in an inconclusive diagnosis. Within a short time, it was suspected that the patient experienced local recurrence, thus rapidly leading to the onset of grade III laryngeal obstruction symptoms. During his last visit to the Second Affiliated Hospital of the Army Military Medical University Outpatient Department (Chongqing, China), a biopsy was carried out and tissue samples were collected for immunohistochemical examination. The diagnosis of larynx sarcomatoid carcinoma was therefore verified. Subsequently, partial laryngectomy combined with laryngeal functional reconstruction and left neck lymph node dissection were performed. Post-surgery immunohistochemical analysis also verified larynx sarcomatoid carcinoma.
