Introduction: Authors submitting to Orthopaedics & Traumatology: Surgery & Research (OTSR) must fulfill the criteria of scientific integrity (SI), including issues of plagiarism and disclosure of interest. These data have not been assessed for OTSR and we therefore conducted a retrospective study of (1) potential plagiarism rates, (2) deficient disclosure of interest (for French authors), and (3) correlation between the two., Hypothesis: Suspected plagiarism rates exceed 20% and the non-disclosure rate exceeds 80%., Material and Methods: We analyzed 1 year's submissions to OTSR: January 24, 2022 to January 23, 2023. Articles were checked for plagiarism, using iThenticate software (Turnitin, Oakland, CA, USA), with a threshold of > 20% matching. Conflicts of interest, for French authors, investigated on the French Ministry of Health Transparence website (www.transparence.sante.gouv.fr), with non-disclosure defined by undeclared amounts exceeding €1000., Results: In total, 851 submissions were analyzed. iThenticate identified 152 (17.7%) with > 20% matching to an already published article. This likely plagiarism varied (p<0.01) between geographic origins of submissions. In the 289 submissions by French authors, there were 275 (95%) failures to report amounts exceeding €1000. Combined non-disclosure and plagiarism was found in only 3 articles submitted by French authors (3/289: 1.03%)., Discussion: OTSR applies the Recommendations for the Conduct, Reporting, Editing and Publication of Scholarly Work in Medical Journals (www.icmje.org), adheres to the guidelines of the Committee On Publication Ethics (COPE) and the French Health Authority (HAS), and has an ethics committee. Plagiarism is a major SI breach, suspected in 20% of submissions. Also, 95% of French authors failed to properly disclose their interests on submitting an article, although this declaration is mandatory in France and is an integral aspect of SI. There are also other forms of misconduct, such as failure to comply with the French "Jardé" law on research involving human subjects, failure to obtain review board approval, unjustified claims to authorship or deficient archiving, that were not analyzed here., Conclusions: Although overall plagiarism rates were under 20% for submissions to OTSR, some geographic areas were more concerned than others. Also, despite reminders by the editorial board, only 5% of French authors made full disclosures; this is a major breach of SI, requiring correction on the part of French authors., Level of Evidence: IV; retrospective study., (Copyright © 2023. Background: Ramp lesions are found in 16% to 40% of patients undergoing anterior cruciate ligament reconstruction. The repair technique traditionally involves using a suture hook through a posteromedial portal, with the arthroscope positioned in the intercondylar view via an antero-lateral portal. Ramp lesions may be difficult to visualize and repair, even with a 70° arthroscope. The objective of this study was to assess the feasibility of suturing ramp lesions via dual posteromedial portals for the arthroscope and instruments., Hypothesis: Dual posteromedial arthroscopic portals allow good visualisation and high-quality suturing of ramp lesions, without inducing specific iatrogenic injuries., Material and Methods: We used 11 fresh cadaver knees. Two posteromedial portals were created under visualisation via an arthroscope introduced through an antero-lateral portal: one was the traditional instrumental portal and the other, located more proximally, was the optical portal. A 2-cm long ramp lesion was created. A suture hook was used to place one or two stitches of PDS n°0 suture. A probe was used to test the quality and stability of the suturing. The posteromedial plane was then dissected to evaluate the anatomical relationships of the portals., Results: The dual posteromedial approach allowed the visualisation and hook suturing of the ramp lesions in all 11 cases. A single stitch was placed in 4 cases and two stitches in 7 cases. The suture was always of good quality and stable when tested with the probe. The dissection found no injuries to nerves, blood vessels, or tendons., Conclusion: Ramp lesions can be repaired through a dual posteromedial arthroscopic approach. This surgical technique provides good visibility of these lesions and allows high-quality suturing, with no specific iatrogenic injuries. Background: Surgical treatment of multiligament knee injuries (MLKIs) leads to better outcomes but there are controversies about optimal surgical strategies. Debates remain about timing of surgery: acute, staged or delayed and about graft choice: autograft, allograft or a combination of both. Therefore, we performed a retrospective study aiming to evaluate postoperative laxity using stress radiographs and clinical outcomes after one-stage reconstructions of injured ligaments using non-irradiated, fresh-frozen allografts., Hypothesis: MLKIs treated by one-stage reconstructions using non-irradiated, fresh-frozen allograft may lead to satisfactorily postoperative laxity and clinical outcomes., Methods: Between November 2013 and July 2015, 23 patients with MLKIs underwent one-stage reconstruction using allograft. Knee injuries were defined according Schenk classification of Knee Dislocation (KD). Patients were evaluated using the Knee injury and Osteoarthritis Outcome Score (KOOS), the Lysholm Knee Scoring Scale, and the International Knee Documentation Committee (IKDC) Subjective Knee Evaluation Form at a minimum follow-up of 24 months. Postoperative anterior, posterior, varus, and valgus laxities were assessed using stress radiographs and expressed as side-to-side differences (SSD) in millimeters., Results: Three of 23 patients were lost to follow-up. There were 6 KD-I, 12 KD-III, and 2 KD-IV lesions, 12 lateral-side and 10 medial-side lesions, and 13 acute and 7 chronic cases. Three patients had associated neurovascular injuries. Mean follow-up was at 29.4±6.1 months. Mean valgus SSD was 0.2mm±1.4mm (range, -2.1-2.2mm), mean varus SSD was 1.4mm±2.5mm (range, -1.7-6.0mm), mean posterior SSD was 7.2mm±3.9mm (range, 1.2-16.0mm), mean anterior SSD was 3.6mm±5.1mm (range, -4.8-16.8mm). Overall IKDC ratings were: 4 grade A, 3B, 7C, and 6D. Three patients complained of postoperative instability, with an IKDC rating of D. The mean subjective IKDC score was 67.2±19.6, the mean Lysholm Knee Scoring Scale was 77.3±16.5, and the mean KOOS results were 78.5±16.6 for pain, 67.7±17.4 for symptoms, 86.5±14.2 for daily activities, 56±25.4 for sports, and 47.2±28.6 for quality of life. Nineteen of 20 patients returned to sport-6 to the same level. One patient underwent an arthroscopic arthrolysis due to postoperative arthrofibrosis., Conclusions: Using non-irradiated allografts for one-stage reconstructions of all the injured ligaments in MLKIs is effective and safe. Introduction: In 2008, the French National Authority for Health (HAS) recommended that "conservative" treatments be adopted for meniscal lesions. This recommendation and the lack of superiority of meniscectomy over non-operative treatment for meniscus degeneration have modified the treatment pathway. However, the impact of these findings on French clinical practice is not known. The objective of this study was to evaluate the change over time in the number of alternative surgical procedures (meniscectomy and meniscus repair) and regional variation in France using data from the French agency for information on hospital care (ATIH)., Hypothesis: We hypothesized that the number of meniscectomy procedures will decrease, and the number of repair procedures will increase over time at various healthcare facilities., Patients and Methods: Between 2005 and 2017, the number of hospitalizations in the Medicine-Surgery-Obstetrics wards for meniscectomy (NFFC003 and NFCC004) or meniscus repair (NFEC001 and NFEC002) was evaluated overall and then based on whether the stay occurred in public or private sector hospitals in France. Data were extracted from the ATIH database and the findings were (1) related to French demographics during the period in question; (2) separated into public or private sector hospitals; (3) distributed into various regions in France and; (4) stratified by patient age., Results: Between 2005 and 2017, 1,564,461 meniscectomy and 63,142 meniscus repair procedures were done in France. Over this period in the entire country, the meniscectomy rate gradually decreased from 19.80/10,000 inhabitants in 2005 to 15.77/10,000 inhabitants in 2017 (21.4% reduction) (p<0.0001) while the meniscus repair rate increased from 0.42/10,000 inhabitants in 2005 to 1.36/10,000 inhabitants in 2017 (320% increase) (p<0.0001). The largest meniscectomy reduction effort occurred in private sector hospitals, going from 15.79 to 12.01/10,000 inhabitants in 12 years; the decrease was smaller in public hospitals (going from 4.01 to 3.77/10,000 inhabitants) (p<0.0001 in both cases). The change in the procedure ratio between private and public hospitals was asymmetric, with the meniscus repair/meniscectomy ratio clearly increasing more in public hospitals (4% to 12.6%) between 2005 and 2017 than in private hospitals (1.6% to 6.6%) (p<0.0001). We found large regional differences: regions in Eastern France had higher meniscectomy rates, while regions in Western France had higher meniscus repair rates. When the analysis of procedures between 2008 and 2017 was stratified by age, a similar increase in repair procedures was found in all age brackets. Conversely, the reduction in meniscectomy was most apparent before 40 years of age, and the number of meniscectomy procedures was stable after 60 years of age., Conclusion: These findings suggest there has been a significant shift in the surgical management of meniscal injuries towards more conservative treatments. Background: Single-stage bilateral knee arthroplasty, even when unicompartmental, remains controversial, chiefly due to the risk of peri-operative complications. The primary objective of this study was to compare the short-term complication rate and cost of single- vs. two-stage bilateral unicompartmental knee arthroplasty (UCA). The secondary objective was to compare total hospital stay lengths and motion-range recovery., Hypothesis: Single-stage bilateral UCA is a cost-saving alternative that is not associated with higher complication rates compared to two-stage bilateral UCA., Material and Method: This single-centre retrospective comparative study included 70 patients of any age managed between 2010 and 2016. Among them, 44 (88 UKAs) had single-stage surgery (1S group) and 26 (52 UCAs) two-stage surgery (2S group). The two groups were comparable for age, body mass index, gender distribution, compartment replaced, ASA score, and Charlson comorbidity index. The following were evaluated: operative time, haemoglobin level before and after surgery, major and minor complication rates, motion-range recovery, and the radiographic hip-knee-ankle (HKA) angle. Costs were estimated based on the standard codes assigned to the procedures by the national statutory health insurance system (GHM 08C24 for knee arthroplasty to treat knee osteoarthritis and NFKA006 for unicompartmental tibio-femoral or femoro-patellar arthroplasty), modulated according to the concomitant diagnoses., Results: No differences were found for the haemoglobin level change, time to motion-range recovery, or HKA angle. The complication rates per patient were not significantly different between the groups: major complications, 9.1% (n=4) in the 1S group and 15.4% (n=4) in the 2S group (p=1.00); minor complications, 4.5% (n=2) in the 1S group and 3.8% (n=1) in the 2S group (p=1.00). Cost of the total hospital stay was significantly higher in the 2S group than in the 1S group (11,766.7€) and 5626.4€, respectively; p<0.001). Mean total hospital stay duration per patient was 6.7 days with single-stage surgery and 13.4 days with two-stage surgery., Discussion: Single-stage bilateral UCA is not associated with a higher rate of peri-operative complications compared to the two-stage alternative and is substantially less costly. Introduction: The rotational position of the femoral component is a primary driver of success in total knee arthroplasty. However, distal femoral torsion (DFT) varies greatly between individuals. Measuring DFT preoperatively by CT in combination with computer-assisted surgery can significantly improve the rotational positioning of the femoral component. However, a preoperative CT scan is costly and exposes the patient to radiation. These are doubled when the patient is undergoing bilateral arthroplasty. The aim of this study was to determine the DFT in both knees of a patient undergoing bilateral arthroplasty. We hypothesized that DFT was symmetric between a patient's two knees and was independent of frontal alignment., Material and Methods: In this retrospective study of TKA cases performed between December 2008 and March 2015, 82 patients (mean age 73years) who underwent two-stage bilateral TKA (164 knees) were included. A preoperative CT scan of each knee was performed to measure the DFT using the surgical posterior condylar angle (PCA) described by Yoshioka. Two observers performed the measurements twice each, to allow calculation of the intraclass and interclass correlation coefficients., Results: The mean PCA was 5.4° (±1.48) in the right knee and 5.4° (±1.45) in the left knee, with a left/right difference ranging from 0 to 2.2° (p=0.8). In the entire cohort, 84.6% of patients had a left/right difference of less than 1°. We found no significant differences in DFT in knees with large or small frontal deformity (deformity<10°, p=0.7; deformity>10°, p=0.5) or the presence of varus or valgus (p=0.9). The intraclass correlation coefficient was excellent (94%) and the interclass correlation coefficient was moderate to good (60% for left knees, 53% for right knees)., Discussion: Based on CT scan measurements, the DFT in both knees of an arthritic patient is comparable and this measurement is reproducible. Introduction: Using locking plates in opening-wedge high tibial osteotomy (OWHTO) via a medial opening theoretically allows early weight-bearing without need for bone or bone-substitute grafting. It incurs a risk of non-consolidation in case of large correction (>10°), although rates and risk factors of non-union are not known. The present retrospective study compared OWHTO with correction <10° versus >10°, with a view to determining: (1) complications rates (non-union) according to degree of correction, and (2) risk factors for such complications., Hypothesis: OWHTO with correction greater than 10° without graft shows normal consolidation and allows early weight-bearing., Material and Method: Forty-one patients treated by OWHTO for medial osteoarthritis of the knee between January 2101 and November 2015 were included in a retrospective study. HKA angle was assessed by long-leg axis radiographs, preoperatively and at 3 months. Clinical and radiological follow-up at 6 weeks, 3 months and 6 months assessed consolidation in terms of >40% filling of the osteotomy site. Partial (contact) weight-bearing was allowed from the first postoperative day, with full weight-bearing at 6 weeks., Results: Mean patient age was 59±5 years. Mean body-mass index (BMI) was 30.3±5.2; 17 patients (41.5%) had BMI >30. Mean initial HKA angle was 173.5°±3° (range, 167-178°) and mean correction was 10.7°±2.7° (range, 5-15°). There were 27 corrections of 10° or more, and 14 less than 10°. At 3 months, mean HKA was 182.9°±2.5° (range, 178-187°). Twelve cases showed lateral tibial cortex fracture after opening. Thirty-six patients (87.8%) showed consolidation, at a mean 5±3 months. Five patients showed osteotomy site non-union; in all these cases, the lateral cortex was broken initially (P=0.003); all had BMI >30 (mean, 37.2±3.8; P<0.03); none were smokers. On univariate analysis, lateral tibial cortex fracture (OR=10; 95% CI, (1.59-196.30)), BMI >30 (OR=1.18; 95% CI, (1.03-1.41)) and correction ≥10° (OR=10.50; 95% CI, (2.49-53.86)) were associated with delayed consolidation. On multivariate analysis, only degree of osteotomy was significantly associated with delayed consolidation (OR=11.51; 95% CI, (2.13-95.74))., Discussion/conclusion: Obesity and initial lateral cortex fracture appeared as risk factors for non-consolidation of OWHTO with large correction. Osteochondritis dissecans progresses to osteoarthritis if integration of the fragment is not obtained. The prognosis of osteochondritis dissecans is more severe in adults, as spontaneous integration due to physeal closure does not occur. Hybrid fixation consists in combining screw fixation of the fragment with mosaicplasty through the fragment to promote integration into the native condyle. Meniscectomy remains one of the most frequent orthopedic procedures, despite meniscal sparing having been advocated for several decades now. Incidence is excessive in the light of scientifically robust studies demonstrating the interest of meniscal repair or of nonoperative treatment for traumatic tear and of nonoperative treatment for degenerative meniscal lesions. It is high time that the paradigm shifted, in favor of meniscal preservation. In traumatic tear, and most particularly longitudinal vertical tear in vascularized zones, repair shows a high success rates in terms of recovery time, functional outcome and cartilage protection. Leaving the meniscus alone may be an option in asymptomatic lesions of the lateral meniscus during anterior cruciate ligament (ACL) reconstruction. Posterior ramp lesions (in associated ACL tear), traumatic root tears and radial lesions are also excellent indications for repair, although it has to be borne in mind that the natural history of these lesions is not completely understood and nonoperative treatment also may be considered. Degenerative meniscal lesions are frequently revealed by MRI in middle-aged or elderly subjects. They are closely related to tissue aging and thus probably to osteoarthritic processes. Meniscectomy was long considered the treatment of choice. All but 1 of the 8 recent randomized studies reported non-superiority of arthroscopy over nonoperative treatment, which should thus be the first-line choice, with arthroscopic meniscectomy reserved for cases of failure, or earlier in case of "considerable" mechanical symptoms. Horizontal cleavage in young athletes is a particular case, requiring meniscal repair, to avoid a meniscectomy, which would inevitably be extensive in a young active patient. Background: Bimeniscal lesions are common in patients with anterior cruciate ligament (ACL) tears. However, bimeniscal repair is rarely performed during ACL reconstruction., Objective: To assess outcomes after ACL reconstruction with bimeniscal repair., Hypothesis: Bimeniscal lesions, even when repaired, are associated with poorer outcomes of ACL reconstruction., Material and Methods: A retrospective case-control design was used. The cases were 15 patients who underwent ACL reconstruction, without procedures on any other ligaments, combined with bimeniscal repair, between May 2009 and May 2013 (3.2% of all ACL reconstructions during the study period). This group (2-Mc group) was matched on age, gender, body mass index, and time to surgery to 30 patients who underwent ACL reconstruction and had no meniscal lesions (0-Mc group) and to 30 patients who underwent ACL reconstruction and repair of the medial meniscus (1-Mc group). After a mean follow-up of 3.6 years, clinical outcomes were assessed based on the KOOS, Lysholm, and IKDC scores and knee laxity based on TELOS and GNRB measurements. The primary outcome measure was the rate of ACL re-rupture. Secondary outcome measures were functional outcomes and rate of delayed meniscectomy., Results: The ACL re-rupture rate was significantly higher in the 2-Mc group than in the 0-Mc and 1-Mc groups pooled (20%, vs. 1.7%; P=0.02). The functional scores showed no significant differences across groups. Post-operative differential laxity was significantly greater in the 2-Mc group (3.3mm by TELOS, P=0.02; and 2.5mm by GNRB, P=0.03) than in the 0-Mc and 1-Mc groups pooled. Delayed meniscectomy was performed in none of the 2-Mc group patients and in 2 of the 1-Mc group patients., Conclusion: ACL reconstruction combined with bimeniscal repair is a rarely performed procedure. It is associated with a high ACL re-rupture rate and greater differential laxity. Background: Implantation of the Actifit ® polyurethane meniscal scaffold in patients who have pain after partial meniscectomy provides short-term pain relief and better function. But there is a lack of information about medium-term outcomes. The objective of this longitudinal study was to evaluate the MRI and functional outcomes after a minimum follow-up of 5 years. It was hypothesized that the results are stable over time., Material and Methods: Fifteen consecutive patients (8 men, 7 women; mean age 30 years, range 19-47, mean BMI 25) were enrolled in the study between February 2008 and January 2011. Five patients also underwent ACL reconstruction and one underwent mosaicplasty. Nine lateral and six medial Actifit ® implants were evaluated prospectively before the surgery and at a minimum of 12, 24 and 60 months' follow-up using a visual analogue scale (VAS) for pain, the objective and subjective IKDC scores, radiological and MRI analysis with measurement of the ICRS score, Genovese score and extrusion., Results: The mean follow-up was 6 years (range 5-8.1). Two patients were lost to follow-up. Three patients were re-operated at 7, 19 and 30 months because they had not improved functionally and the implant appeared damaged on MRI. Partial removal of the implant did not improve the functional outcomes (mean subjective IKDC pre- and post-revision: 37.0 vs. 34.9). Two patients were lost to follow-up. In intention to treat (13 cases), the pain (VAS) and subjective IKDC score were improved between the preoperative period and the last follow-up (5.46 vs. 2.92, P=0.007 and 51.2 vs. 66.1, P=0.05). In per protocol (10 cases, failures excluded), the pain (VAS) and subjective IKDC score were improved (5.3 vs. 1.9, P=0.0009; 49.6 vs. 75.4, P=0.002) along with the pain, daily activities and quality of life components of the KOOS (60.6 vs. 86.0, P=0.0008; 70.3 vs. 90.2, P=0.001; 42.7 vs. 71.0, P=0.0058). The functional scores were stable between months 12, 24 and 60. ICRS cartilage score and mean meniscal extrusion were unchanged at the last follow-up (1.6 vs. 1.6 and 2.41 vs. 2.79). In all patients, the meniscal implant had an intermediate signal and reduced size on MRI., Discussion: Despite an abnormal MRI appearance suggesting the meniscal scaffold is not fully mature after 5 years, the functional scores and cartilage status are stable at this time point. Introduction: Massive rotator cuff tear repair results are variable. The main purpose of this study was to compare functional outcome between two procedures: open repair by autologous quadriceps-patella tendon patch, and arthroscopic suture. The study hypothesis was that there is no significant difference in results between the two techniques., Material and Methods: A retrospective study included all patients younger than 70 years operated on from 1995 to 2013 for massive rotator cuff tear. Exclusion criteria comprised history of dislocation, fracture or surgery or osteoarthritis in the affected shoulder, and infra- and supra-spinatus fatty degeneration equal to or greater than stage 3. Two consecutive groups were distinguished: group 1, from 1995 to 2003, comprised 23 patients (24 shoulders; mean age, 55.8 years) treated by open repair using quadriceps tendon autograft; group 2, from 2003 to 2013, comprised 27 patients (29 shoulders: mean age, 60.3 years) treated by arthroscopic repair., Results: Preoperatively, mean Constant score was 42.9 in group 1 and 45.7 in group 2 (P=0.36), pain score 5.5/15 and 7.6/15 (P=0.08), strength 3.0kg and 2.4kg (P=0.30), and subacromial space 6.3 and 6.7mm (P=0.05), respectively. At respectively 58 and 55 months' mean follow-up, Constant score was 71.1 in group 1 and 71.8 in group 2 (P=0.086), pain 11.9/15 and 12.7/15 (P=0.76), gain in strength 1.4kg and 2.3kg (P=0.0006), and subacromial space 7.1mm and 6.3mm (P=0.29), respectively. The complications rate was 70% in group 1 and there were no specific complications in group 2., Conclusion: Functional improvement was significant and comparable between the 2 groups. Background: Arthroscopic meniscal repair limits the medium-term risk of radiological osteoarthritis. Magnetic resonance imaging (MRI) cannot provide an accurate assessment of meniscal healing but may show harbingers of osteoarthritis such as meniscal extrusion. The objective of this study was to assess long-term meniscal extrusion after meniscal repair., Hypothesis: Arthroscopic meniscal suture is not followed by meniscal extrusion and can, therefore, provide good knee function in the long-term., Methods: Consecutive patients who underwent arthroscopic meniscal suture on a stable or stabilised knee were included retrospectively. MRI was performed to measure absolute meniscal extrusion (AME), relative meniscal extrusion (RME), anterior sagittal extrusion (ASE), posterior sagittal extrusion (PSE), coronal cartilage coverage index (cCCI), and sagittal cartilage coverage index (sCCI)., Results: After a mean follow-up of 8.8±0.87 years, there was no evidence of meniscal extrusion in these patients with stable or stabilised knees: AME, 1.7±1.03 and 2.3±0.93mm, RME, 17±0.10% and 28±0.12%, ASE, 2.52±1.43 and 1.71±2.42mm, PSE, 0.29±3.49 and 0.22±2.35mm, cCCI, 23±0.08% and 20±0.09%, and sCCI, 49±0,10% and 53±0.09%., Conclusion: In the long-term after meniscal repair, osteoarthritis is limited and meniscal function seems preserved., Level of Evidence: IV, retrospective study., (Copyright © 2017 Elsevier Masson SAS. Background: When managed conservatively, lateral epicondylitis often subsides only after considerable time, during which social and occupational activities are severely disrupted. If conservative management fails, a recently introduced option is arthroscopic release of the extensor carpi radialis brevis (ECRB). The primary objective of this study was to compare clinical outcomes of this procedure according to preoperative symptom duration., Hypothesis: Earlier arthroscopic release is associated with better functional outcomes., Material and Method: Consecutive patients with arthroscopically managed lateral epicondylitis were included in a retrospective study. Arthroscopy was performed only after at least 6 months of conservative treatment. The criteria to evaluate the clinical outcomes were the Nirschl and Quick-DASH scores, muscle strength, time to pain relief, and percentage of functional recovery., Result: Thirty-five patients were evaluated at a median of 4 years (range: 1-12 years) after surgery. Mean preoperative symptom duration was 18 months (range: 6-106 months) with a mean sick leave duration of 2.3±4.9 months. Postoperatively, mean time to recovery was 37.5 days (range: 7 days to 5 years) and mean sick leave duration was 2.4±2.4 months. The mean Quick-DASH score was 15.9±19.1. The Nirschl score improved significantly, from 26.4±7.9 to 66.3±16.3. The initial muscle strength deficit was 10.1±33.2% and muscle strength at last follow-up was increased by 4.3±30.3%. Symptom duration showed no correlations with any of the clinical outcome measures., Discussion and Conclusions: Outcomes after arthroscopic release were not associated with symptom duration in this study. Introduction: To reduce the size of the surgical incision, modular mini-keel tibial components have been developed with or without extensions for the Nexgen™ MIS Tibial Component. Although a smaller component could theoretically result in defective fixation, this has never been evaluated in a large comparative series. Thus, we performed the following case control study to: (1) evaluate intermediate-term survival of a modular "mini-keel" tibial component compared to a reference standard keel component from the same line of products (Nexgen LPS-Flex Tibial Component, Zimmer); (2) to identify any eventual associated factors if the frequency of loosening was increased., Hypothesis: The rate of revision for aseptic tibial loosening is comparable for both components., Materials and Methods: This comparative, retrospective, single center series of 459 consecutive total knee arthroplasties (TKA) was performed between 2007 and 2010: with 212 modular "mini-keel" (MK) tibial components and 247 "standard" (S) components. Survival, rate of revision for aseptic tibial loosening and identification of a radiolucent line were analyzed at the final follow-up., Results: After a median follow-up of 5years, the rate of revision for tibial aseptic loosing was significantly higher in the MK group with 12 cases (5.7%) and 4 cases in the S group (1.6%) (P=0.036). The use of the MK component appears to be a prognostic factor for surgical revision (hazard ratio=3.86 (1.23-11.88), P=0.02) but not for the development of a radiolucent line (HR=1.75 (0.9-3.4), P=0.097). The mean delay before revision was 38months (8-64) in the MK group and 15.2months (8-22) in the S group (P=0.006). Introduction: Theoretically, the properties of second-generation metal-on-metal (MoM) bearings are better for wear, osteolysis and longevity. However, follow-up studies of more than 10 years are rare, in particular with hybrid fixation (cemented stainless steel stems and cementless cup), therefore we evaluated the results of this combination after a mean follow-up of 12.8 years: (1) to analyze the survival rate, (2) to compare it with the survival rate in the same series after 6.4 years (95.8% cup, 94.8% stem), (3) to evaluate clinical and radiographic outcome and (4) to analyze these failures., Hypothesis: The number of revisions would increase after 10 years., Materials and Methods: We evaluated 106 total hip arthroplasties (THA) (Cedior™ press-fit cup with cemented Acora™ (n=50) and Exafit™ (n=56) stems and 28mm Metasul™ bearings performed between January 1999 and December 2002. The survival rate was calculated by the Kaplan-Meier method. The clinical assessment included the Postel Merle d'Aubigné (PMA) and Oxford scores. The radiographic assessment evaluated radiolucencies and osteolysis on standard X-rays. A histological analysis was only performed during revision THA., Results: After a mean 12.8 years of follow-up (10-16), 53 THA were reviewed in 48 patients. Thirteen THA revisions (14%) were performed including 11 aseptic revisions (5 stem fractures, 2 cases of impingement and 3 loosenings [1 bipolar and 3 cups] and one case of osteolysis). Overall survival, taking into account revision for aseptic loosening, was 87.6% (CI 95%=77.3 to 99.3%). The mean PMA and Oxford scores at the final follow-up were 17.6±0.8 points (16-18) and 16.5±5.2 points (12-38) respectively. The radiological follow-up mainly identified radiolucencies around the stem in Gruen zones 1 and 7 (17 and 21% respectively)., Discussion: The survival rate of hybrid MoM THA in this series decreased after 10 years and is lower than studies evaluating cementless THA with the 28-mm Metasul™ bearings (90.9 to 100% survival). Although the clinical results are satisfactory, survival is lower. Recent publications on patient-specific instrumentation for total knee arthroplasty have reported considerable variability in the axial positioning of the cutting guides for the femoral component. These personalized cutting guides are manufactured based on bone shape data, generated from magnetic resonance imaging (MRI) or computed tomography (CT). The goal of this study was to compare the reproducibility and accuracy of distal femoral torsion (DFT) values measured using these two imaging modalities. We hypothesized that MRI does not reproducibly and consistently measure DFT and is not as accurate as CT scan., Methods: Anonymized radiology records from 54 patients that included MRI and CT scans of the knee were read in random order by two observers, on two separate occasions. These records were from patients being considered for a meniscal or osteochondral graft and who had their knee explored, but who had not undergone femoral or tibial surgery and were free of osteoarthritis. The DFT was estimated using the posterior condylar angle (PCA), using both its anatomical and surgical definitions. The intra- and inter-observer reproducibility of the MRI and the differences relative to CT scan measurements were analysed., Results: The average intra-observer difference for the MRI evaluation of the anatomical PCA was 0.8±1.2°; it was 0.4±0.9° for the surgical PCA. More than 1° difference from the average was found in 8 cases (14%) using the anatomical PCA measurement and 4 cases (7.4%) when using the surgical PCA (P=0.4). The intraclass correlation coefficients (ICCs) were 0.67 (95% CI: 0.33-0.85) and 0.74 (95% CI: 0.47-0.89) for the anatomical and surgical PCA, respectively. The average inter-observer difference for the MRI evaluation of the anatomical PCA was 1.6±1.4°; it was 1.5±1.0° for the surgical PCA. More than 1° difference from the average was found in 27 cases (50%) using the anatomical PCA measurement and 22 cases (40%) when using the surgical PCA (P=0.4). The ICCs were 0.31 (95% CI: 0.14-0.65) and 0.48 (95% CI: 0.06-0.75) for the anatomical and surgical PCA, respectively. The average differences between the CT and MRI measurements were 1.4±1.1° (0.2-5°) and 1.1±0.8° (0-3.6°) for the anatomical and surgical PCA, respectively. Greater than 1° difference between CT and MRI was found in 29 records (54%) for the anatomical PCA and in 18 records (33%) for the surgical PCA (P=0.03)., Conclusion: DFT measurement on MRI is more reproducible and consistent when using the surgical PCA. The management of chronic extensor mechanism disruption can be complex. One of the options is allograft reconstruction. The goal of this study was to present the surgical procedure and provide preliminary results with this technique. The allograft uses the whole extensor mechanism (anterior tibial tubercle, patellar ligament, patella, quadriceps tendon). The native patella can be completely removed if the quality of the bone is poor, otherwise a bone groovecan be created to receive the allograft. The allograft is tightly tensioned with the knee in full extension. This surgical technique was performed 5 times with a minimum follow-up of 1 year. Active extension was recovered in all cases. Background: Meniscal substitutes have been suggested for the treatment of knee pain after partial meniscectomy. However, despite the partial nature of the initial meniscectomy, secondary extrusion of the substitute is common. The primary objective of this study was to evaluate morphological outcomes of meniscal substitute implantation and their potential associations with preoperative meniscal extrusion., Hypothesis: Preoperative absolute meniscal extrusion in the coronal plane predicts poorer morphological and clinical outcomes., Material and Methods: Consecutive patients who received an Actifit(®) meniscal substitute between 2008 and 2011 were included prospectively. After 1 year and 2 years, the IKDC score and KOOS were determined and magnetic resonance imaging performed. The morphological evaluation consisted in measuring meniscal extrusion and cartilage coverage by the substitute in the coronal and sagittal planes., Results: Twenty patients were included. Among them, 3 required subsequent removal of the substitute. The mean subjective IKDC score increased from 48.1 preoperatively to 56.4 after 2 years. Over the same period, the function/sports/recreational activities component of the KOOS improved significantly (42.9 vs. 55.0, P=0.04). Positive correlations between preoperative and 1-year values were demonstrated for both cartilage coverage in the coronal and the sagittal planes (P=0.03 and P=0.04, respectively) and coronal absolute meniscal extrusion (P=0.05). No significant differences were found between preoperative and 2-year values of cartilage coverage in the coronal and sagittal planes (P=0.38). There was a negative correlation linking preoperative meniscal extrusion in the coronal plane to 1-year cartilage coverage in the coronal and sagittal planes (P=0.01 and P=0.04, respectively). Preoperative absolute meniscal extrusion in the coronal plane correlated negatively with the subjective IKDC score after 1 year (P=0.02)., Discussion: Preoperative meniscal extrusion in the coronal plane strongly predicts clinical and morphological outcomes. Introduction: Surgical quality and risk management is a major public health issue. The consequences of unplanned return to theater are social, occupational, financial and even legal. Unscheduled revision surgery is a major adverse event, resulting from serious complications - some of which are thought to be avoidable. The present study sought to assess and analyze the incidence of unplanned return to theater in an orthopedic surgery department. The study hypothesis was that some of the complications involved could be avoided., Patients and Method: A mixed retrospective-prospective study examined a consecutive series of 10,158 patients operated on in an orthopedic and traumatologic surgery department between January 2011 and December 2013. Patients undergoing revision surgery for reasons directly related to the primary procedure were analyzed. Patients were distributed among the following subgroups: infection, implant dislocation; hemorrhagic complication, mechanical complication, problem of primary technique, stiffness, wound healing disorder. Specific indicators of dysfunction liable to have contributed to onset of the complication were applied in each subgroup, to determine the avoidable or unavoidable nature of the event., Results: Two hundred and twenty-four patients (2.2%) underwent revision surgery for reasons directly related to the primary procedure. One hundred and eight cases (48.2%) were considered to have been avoidable: 48 infections (21.4%), 27 implant dislocations (12%), 15 hemorrhagic complications (6.7%), 66 mechanical complications (29.5%), 35 technical problems at primary surgery (15.6%), 21 cases of stiffness (9.3%), and 12 cases of delayed wound healing (5.3%). Mean time to revision surgery was 2.7 ± 2.6 months. Extending the time-window to 1 year recruited extra cases: in 31.7% of cases, onset was after the 90th postoperative day, which is the usual deadline. The rate of unplanned return to theater was higher after unscheduled (traumatic: 3.2%) than scheduled surgery (1.7%, P < 0.001)., Conclusion: Return to surgery in orthopedic and traumatologic surgery is underestimated. Annual incidence was 2.2%, and twice as high (3.2%) following traumatologic compared to scheduled surgery (1.7%). Analysis found that almost half the cases were avoidable. They represent a relevant and easily assessed indicator of treatment quality and associated risk management. Post-traumatic knee stiffness and loss of range of motion is a common complication of injuries to the knee area. The causes of post-traumatic knee stiffness can be divided into flexion contractures, extension contractures, and combined contractures. Post-traumatic stiffness can be due to the presence of dense intra-articular adhesions and/or fibrotic transformation of peri-articular structures. Various open and arthroscopic surgical treatments are possible. A precise diagnosis and understanding of the pathology is mandatory prior to any surgical treatment. Failure is imminent if all pathologies are not addressed correctly. From a general point of view, a flexion contracture is due to posterior adhesions and/or anterior impingement. On the other hand, extension contractures are due to anterior adhesions and/or posterior impingement. This overview will describe the different modern surgical techniques for treating post-traumatic knee stiffness. Any bony impingements must be treated before soft tissue release is performed. Intra-articular stiff knees with a loss of flexion can be treated by an anterior arthroscopic arthrolysis. Extra-articular pathology causing a flexion contracture can be treated by open or endoscopic quadriceps release. Extension contractures can be treated by arthroscopic or open posterior arthrolysis. Unlabelled: Meniscus allograft transplantation (MAT) is used to treat patients with knee pain after total or subtotal meniscectomy. The graft can be inserted during open or arthroscopic surgery. The objectives are anatomic horn positioning and strong fixation to the bone and capsule of an appropriately sized graft., Hypothesis: Arthroscopic MAT with trans-tibial bone fixation ensures better mid-term functional outcomes and limits allograft extrusion., Patients and Methods: We conducted a retrospective single-centre study of 23 consecutive patients who underwent MAT between 2001 and 2010. Among them, 11 had open surgery and anchoring of the horns without tunnels and 12 had arthroscopically-assisted surgery with bony fixation of the horns through trans-tibial tunnels. The two groups were comparable at baseline. Mean follow-up was 66.1 months. Post-operative outcomes were assessed using the IKDC score and KOOS, standard radiographs of both knees, and either magnetic resonance imaging or computed arthrotomography. We measured joint space narrowing, meniscal extrusion in the sagittal and coronal planes; and the degree of cartilage coverage by the graft using an index developed for this study., Results: The overall failure rate was 17.4% (4/23, two cases each of complete and partial graft removal). Joint space narrowing increased by 28% versus the pre-operative value (P=0.009). IKDC and KOOS values were not significantly different between the two groups. Absolute meniscus extrusion was greater in the arthroscopy group (4mm vs. 3mm, P=0.03)., Discussion: Osteoarthritis of the transplanted compartment is unavoidable. Open surgery is associated with less meniscal extrusion. The clinical outcomes are independent from the technique used. Background: Segmental tissue loss in the lateral meniscus is associated with pain and increased risk of osteoarthritis even when indications have been carefully considered., Hypothesis: Repairing the defect using a novel biodegradable scaffold will reduce pain and restore the knee function., Methods: In this prospective multicenter study, a total of 54 patients (37 males/17 females; mean age: 28 years [16-50]) were enrolled. All patients presented with postmeniscectomy syndrome and segmental lateral meniscus loss, and were treated with a polyurethane biodegradable scaffold (Actifit(®), Orteq) implanted arthroscopically. Clinical outcomes were assessed at 6, 12 and 24 months using Visual Analogue Scale (VAS), International Knee Documentation Committee Score (IKDC) and Knee Injury and Osteoarthritis Outcome Score (KOOS)., Results: VAS decreased from 5.5 at baseline to 3.6 at 6 months, 3.4 at 12 months and 2.9 at 24 months. IKDC improved from 47.0 at baseline to 60.2, 67.0 and 67.0 at 6, 12 and 24 months. All KOOS subscores improved between baseline and 24 months., Discussion: Clinical results of this study demonstrate clinically and statistically significant improvements of pain and function scores (VAS, IKDC, and all KOOS subscales except sport), at the 6 months follow-up and on all clinical outcomes at the 2-year follow-up. Since the outcome of partial trapeziectomy is not always satisfactory, we proposed using arthroscopy to perform partial trapeziectomy and to perform ligament reconstruction with the abductor pollicis longus tendon. A tendon strip was left intact at its insertion at the base of the first metacarpal and then slipped into the trapeziectomy space. It was fixed into a blind tunnel at the base of the second metacarpal with a bioabsorbable interference screw. Forty-nine patients were surgically treated between 2006 and 2009. With an average follow-up of 3.6 years, 83.5% of patients were satisfied with the procedure; 96% of thumbs were stable and 74% were pain free. The average recovery time was 4.5 months. There were four cases of radial nerve branch irritation and one case of flexor carpi radialis tendinitis; there were no cases of complex regional pain syndrome. This minimally-invasive technique is a less-aggressive treatment approach for thumb carpometacarpal joint arthritis, which simplify postoperative recovery., (Copyright © 2012. Introduction: Discoid medial meniscus is a rare abnormality, with incidence estimated at 0.12%. The present study describes this congenital abnormality anatomically and reports clinical results in four symptomatic cases managed by surgery., Materials and Method: A retrospective study included three patients (2 female, 1 male), one of whom had bilateral pathology. Mean age at consultation was 18.5 years (range, 13 to 28 yrs). Presenting symptoms were knee pain, associated with acute locking (1 case) or recurrent effusion (1 case). Plain X-rays were normal. MRI found discoid medial meniscus in all four cases, with intrameniscal hypersignal on T2-weighted sequences., Results: Arthroscopy confirmed the discoid abnormality of the medial menisci. Meniscal tear was systematically associated: horizontal in two cases and vertical in the other two. Three cases showed insertion defect of the anterior horn of the discoid medial meniscus. All two cases were managed by meniscoplasty, removing the central part of the meniscus and sparing its peripheral part. Meniscal repair was associated in one case. Subjective results were assessed by KOOS score. At a mean 23 months' follow-up (range, 7 to 54 months), mean KOOS score was 82.7 (range, 77.6 to 86.4): 88 ± 5 for pain, 89 ± 8 for other symptoms, 98 ± 1 for function, 69 ± 17 for sports activity, and 69 ± 16 for quality of life., Conclusion: Symptomatic discoid medial meniscus is frequently associated with bone insertion abnormality of the anterior horn. Meniscal tear is consistently present and revelatory, indicating meniscal tissue fragility, as in the lateral meniscus. Meniscoplasty, possibly with associated meniscal repair if the remaining meniscal wall is unstable, provides satisfactory but imperfect results while avoiding total meniscectomy, which would be disabling in this age group., (Copyright © 2011 Elsevier Masson SAS. Introduction: The importance of a dysplastic trochlea as a component of patellar instability has long been recognized. An original trochleoplasty technique consisting in retro-trochlear recession wedge osteotomy was described by Goutallier et al. The aim is not to fashion a groove but to reduce the bump without modifying patellofemoral congruence., Patients and Methods: This retrospective study reports the operative technique and short-term outcomes of a consecutive case series of 17 patients (19 knees) who underwent recession wedge trochleoplasty for patellofemoral instability associated with severe trochlear dysplasia. Other contributing factors of patellar instability were also corrected as part of the surgical procedure: tibial tuberosity transfer (n=18), MPFL reconstruction (n=8)., Results: Minimum follow-up was 12 months (mean, 34 months; range, 12 to 71 months). The trochlear prominence was reduced from a mean 4.8mm (range, 0 to 8mm) to -0.8mm (range, -8 to 6mm). Patellar tilt was reduced from a mean 14° (range, 6° to 26°) to 6° (range, -1° to 24°). Two cases showed recurrent patellofemoral instability. Mean Kujala, KOOS and IKDC score were respectively 80 (± 17), 70 (± 18) and 67 (± 17) at last follow-up. Three patients required further operations, apart from removal of metal screws: arthroscopic arthrolysis for stiffness (n=1), revision for tibial tuberosity non-union (n=1), and supratrochlear exostosectomy (n=1)., Discussion: Recession wedge trochleoplasty is a feasible additional procedure addressing bony trochlear abnormality in the surgical treatment of patellar instability. Our attitude is to perform it never in isolation but associated to realignment of the extensor apparatus according to the à la carte surgery concept. Introduction: Various surgical techniques have been described to set the rotational alignment of the tibial baseplate during total knee arthroplasty. The self-positioning method ("self-adjustment") aligns the tibial implant according to the rotational alignment of the femoral component which is used as a reference after performing repeated knee flexion/extension cycles. Postoperative computed tomography scanning produces accurate measurements of the tibial baseplate rotational alignment with respect to the femoral component., Hypothesis: The rotational positioning of the tibial baseplate matches the rotation of the femoral component with parallel alignment to the prosthetic posterior bicondylar axis., Patients and Methods: A 3-month follow-up CT scan was carried out after primary total knee arthroplasty implanted in osteoarthritic patients with a mean 7.8° varus deformity of the knee in 50 cases and a mean 8.7° valgus deformity of the knee in 44 cases. The NexGen LPS Flex (Zimmer) fixed-bearing knee prosthesis was used in all cases. An independant examiner (not part of the operating team) measured different variables: the angle between the anatomic transepicondylar axis and the posterior bicondylar axis of the femoral prosthesis (prosthetic posterior condylar angle), the angle between the posterior bicondylar axis and the posterior marginal axis of the tibial prosthesis, the angle between the posterior marginal axis of the tibial prosthesis and the posterior marginal axis of the tibial bone and finally the angle between the anatomic transepicondylar axis and the posterior marginal axis of the tibial prosthesis., Results: For the genu varum and genu valgum subgroups, the mean posterior condylar axis of the femoral prosthesis was 3.1° (SD: 1.91; extremes 0° to 17.5°) and 4.7° (SD: 2.7; extremes 0° to 11°) respectively. The tibial baseplate was placed in external rotation with respect to the femoral component: 0.7° (SD : 4.45; extremes -9.5° to 9.8°) and 0.9° (SD: 4.53; extremes -10.8° to 9.5°), but also to the native tibia: 6.1° (SD: 5.85; extremes -4.6° to 22.5°) and 12.5° (SD: 8.6; extremes -10° to 28.9°). The tibial component was placed in internal rotation relative to the anatomic transepicondylar axis: 1.9° (SD : 4.93; extremes -13.6° to 7°) and 3° (SD : 4.38; extremes -16.2° to 4.8°)., Discussion: The tibial component is aligned parallel to the femoral component whatever the initial frontal deformity (P≅0.7). However, a difference was observed between the rotational alignment of the tibial baseplate and the native tibia depending on the initial deformity and could be attributed to the morphological variations of the bony tibial plateau in case of genu valgum., Conclusion: The self-positioning method is a reproducible option when using this type of implant since it allows the tibial component to be positioned parallel to the posterior border of the femur., (Copyright © 2011 Elsevier Masson SAS. Introduction: Mucoid degeneration of the anterior cruciate ligament (ACL) is a little-known entity. The clinical presentation is one of posterior pain with limited flexion. Its interstitial nature within the ACL structure contrasts with synovial cyst of the ACL. Arthroscopic treatment may include ACL resection, which raises the questions about the harmlessness of this procedure and the risk of anterior instability., Hypothesis: Arthroscopic resection of ACL mucoid degeneration is effective for treating pain and flexion limitation, but at the expense of anterior laxity., Patients and Methods: This bicentric, retrospective cohort study with an average follow-up of 6years involved 27 patients (29 knees) presenting with symptomatic ACL mucoid degeneration validated by magnetic resonance imaging (MRI). Noninfiltrating synovial cysts of the ACL were excluded. Average patient age was 49 (22 to 68) years. Preoperative assessment included a questionnaire, clinical examination (Lachman and pivot shift tests), MRI and standard radiography. Arthroscopic examination analyzed the ACL aspect and its associated lesions (meniscus, cartilage). Anatomopathology samples were collected in 18 cases. Postoperative follow-up included standard radiography and dynamic examination, measuring laxity with a Telos device., Results: Pain was posterior in 23 knees (80%). Fourteen knees (48%) had limited flexion, on average 97 degrees. Twelve partial and 17 total resections were performed. Twenty knees (69%) had associated cartilaginous lesions and 19 (66%) had meniscal lesions. Meniscectomy was undertaken in 11 cases (41%). Posterior pain disappeared in 27 cases (93%), on average 3.7 weeks after the procedure. Average improvement in flexion was 21.5 degrees (0 to 60 degrees). Twenty-eight knees (97%) showed soft and/or delayed stops on postoperative Lachman testing. Average postoperative differential laxity on the Telos device was 8.3 mm (5 to 13 mm). Average postoperative International Knee Documentation Committee (IKDC) and Knee Injury and Osteoarthritis Outcome Scores (KOOS) were 71.2 (42.5 to 92.0) and 78.2 (26.4 to 99). Two patients uderwent secondary ligamentoplasty., Discussion: Treatment of ACL mucoid degeneration by arthroscopic resection is effective for posterior pain and flexion limitation. It results in postoperative laxity, but rarely in frank instability. Therefore, indications for ACL resection must be carefully selected. Background: Navigation tracker pins rigidly fixed to bone is a prerequisite for computer-assisted total knee arthroplasty. The first cases of fracture on navigation tracker pin sites have recently been reported., Hypothesis: The risk of fracture depends first on diaphyseal placement of the tracker pins, and second on "transcortical" tangential route of the tracker pin as well as failure to obtain rigid fixation., Material and Methods: In a continuous series of 385 total knee arthroplasties, five patients (four women, one man) on average 73.2 years old (range: 65-79 years old) have sustained femoral fractures at the tracker pin site (incidence 1.3%). We investigated the demographic and radiological factors contributing to this complication., Results: The patients with fractures were obese or overweight with an average body mass index of 32.56 (range: 25.14-39.45) but this was not statistically different from the BMI of the population of patients without fractures. The average delay from arthroplasty to fracture was 12.6 weeks (range 7-21). The fracture was always preceded by several days of thigh pain and occurred after a minor trauma. The fractures were always simple originating from the tracker pin site. In four out of five cases, the tracker pins were placed in the diaphyseal femur, and in all cases at least one pin was transcortical. Closed endomedullary nailing or ORIF were performed in five cases, with no bone graft. Union was obtained with functional results that were equivalent to those before the fracture., Discussion: Fractures at the navigation tracker pin site are a complication which must be understood and explained to patients undergoing computer-assisted TKA because of the 1.3% incidence described in our series. The circumstances systematically associated with this type of fracture were: occurrence a certain amount of time after arthroplasty in obese patients who had pain before the fracture occurred. These fractures are favored by suboptimal placement of the tracker pins, especially in the lower diaphysis of the femur and transcortical fixation of at least one of the pins. Treatment included stable osteosynthesis and did not affect the results of total knee arthroplasty. The development of thigh pain some time after surgery in high risk patients (obesity, tracker pin site in the lower diaphysis, transcortical fixation) should suggest this diagnosis and weight-bearing should be avoided because these fractures are assimilated with stress fractures. Bicortical metaphyseal fixation should be the preferred tracker pin positioning for navigated total knee arthroplasty., Level of Evidence: Level IV retrospective study., (Copyright 2010 Elsevier Masson SAS. Introduction: Substantial flexion after total knee arthroplasty (TKA) is required for certain categories of patients who wish to squat or kneel in their daily life. Many factors influence this postoperative flexion, including the prosthesis design. It is therefore valuable to in vivo analyze these factors on three knee prosthesis designs through a study of their intraoperative flexion., Hypothesis: The posterior-stabilized (PS) knee prostheses provide better intraoperative flexion than the ultracongruent (UC) model. Of the currently available PS models, the high-flexion ones have better intraoperative flexion than standard models. Our main focus endpoint was the intraoperative flexion achieved, before soft-tissues closure, during TKA surgical procedure., Patients and Methods: This was a controlled study. Seventy-two osteoarthritic knees requiring TKA were included to compare three selected prosthesis models: the SAL ultracongruent and two PS models (the standard LPS and the LPS Flex). This was a single-operator study, with patients divided into three homogenous, comparable groups, in which intraoperative measurement of flexion was performed using computer-assisted navigation. Statistical analysis allowed comparison of the three models., Results: Intraoperatively, after prosthesis implantation, before soft-tissues closure, the mean flexion of the LPS-Flex was 134 degrees versus 124 degrees for the SAL (p=0.0004); the mean flexion of the standard LPS model was 130 degrees versus 124 degrees for the SAL (p=0.14); the PS Flex model showed no significant difference (p=0.26) in flexion (134 degrees) compared to the standard model (130 degrees). The SAL ultracongruent model seemed to be a factor reducing the intraoperative flexion by 8 degrees compared to the PS models (p<10(-4))., Discussion: In this study, the PS designs (standard or Flex) provided better intraoperative flexion than the SAL ultracongruent design. Background: Metal-on-metal bearings in total hip arthroplasty (THA) were introduced to reduce the production of wear debris and debris-induced periprosthetic osteolysis. Analysis of various series according to the type of selected acetabular fixation highlights different evolutions: favourable results with uncemented cups contrasting with loosening and radiolucent lines (RLL) evolution for cemented cups., Hypothesis: Combining metal-on-metal bearings to uncemented cups does not increase the osteolysis risk at a minimum 5 years' follow-up., Materials and Methods: From January 1999 to December 2002, 106 Metasul THAs were implanted in 95 patients using a Hardinge anterolateral approach (40 women and 55 men with an average age of 59.2 years). The cups were of cementless, hydroxyapatite-coated Cedior type (Zimmer) housing a Metasul insert in a polyethylene sandwich. The femoral stem used was the cemented Acora, then the Exafit (Zimmer) type with Metasul 28-mm head mounted on a 8/10 Morse taper. Patients were evaluated clinically using the Postel-Merle-d'Aubigné (PMA) scoring system and radiologically using various markers: cup inclination angle, eventual RLL presence, appearance of osteolysis images, ectopic ossifications and finally, eventual implant migration. In the eventuality of suspected RLL evolution or osteolysis, advanced imaging was performed; joint aspiration liquid and cobalt serum level were studied., Results: We reviewed 94 prostheses (85 patients) with an average follow-up of 6.4 years (4.3 to 9.3 years, median of 6.3 years). The rate of patient loss from follow-up and death was 12.1%. The PMA score of non-revised patients increased from 11.4+/-1.5 to 17.6+/-0.2 at follow-up. We numbered ten re-operations not attributable to the type of bearings used. Three revisions were directly related to the metal-on-metal bearing: two metallosis due to impingement and one case of hypersensitivity. Cup inclination angle was 45.7+/-5.49 degrees . No implant migration was noted during the follow-up duration. Only one cup undergoing revision presented a substantial osteolysis. On the femoral side, with non-revised implants, we observed 13 cement/bone RLL images around nine femoral stems and six calcar resorptions. The survival rate at more than 6 years was 95.8% (91.8-99.8) for the cups and 94.8% (90.3-99.2) for the femoral stems (95% confidence interval)., Discussion: The present study confirms our hypothesis: with Metasul bearings equipped cementless cups, the rates of aseptic loosening, RLL or acetabular osteolysis are low and remain stable over time, unlike the results observed for Metasul series with polyethylene directly cemented on bone. Specific complications, such as metallosis due to impingement and hypersensitivity, can rarely be encountered.
