The new posterior vertical mattress suture technique is technically easier to perform and provides more secure fixation than the arthroscopic simple or horizontal stitch techniques and may contribute to restoring function of the medial meniscus.

Arthroscopic transpatellar screw fixation with a cannulated screw and washer is a simple, effective, and safe technique providing stable fracture fixation to allow immediate mobilization with minimal loss of extension in type II and III tibial eminence fractures in adults.

Synovial chondromatosis is an uncommon, benign neoplastic process typically affecting adult men and most commonly involving the knee, hip, and elbow joints. Intra-articular primary synovial chondromatosis in the knee joint may be localized or generalized. The symptoms associated with localized synovial chondromatosis may include diffuse pain, swelling, restricted motion, crepitus with symptoms of locking and giving way, and posteromedial tenderness mimicking medial meniscus tear. Diagnosis is usually established with magnetic resonance imaging (MRI) and confirmed by histological examination. To our knowledge, synovial chondromatosis of the knee joint in the posteromedial corner has not been reported in the literature. A 53-year-old woman presented with pain in the posteromedial joint line, mild effusion, and occasional locking. Diagnostic evaluation included radiographs, routine blood investigations, and MRI. Radiographs showed no abnormalities. Magnetic resonance imaging showed a small cystic lesion including multiple calcifications between the posterior horn of the medial meniscus and the posterior capsule. Arthroscopic synovectomy and removal of small loose bodies through the trans-septal portal was performed, which relieved her symptoms completely. Arthroscopy using the posterior trans-septal portal allows for better visualization and access, and might be useful to successfully treat localized synovial chondromatosis within the knee joint.

Congenital habitual patellar dislocation is a rare condition of the knee where the patella dislocates during flexion and relocates during extension. The congenital form is permanent, irreducible, and presents at birth. It is characterized by a short quadriceps and a major patellofemoral dysplasia and short height. This article presents a rare case of a 27-year-old woman with recurring bilateral habitual dislocation of the patella after a failed previous proximal and distal realignment procedure. Clinical examinations of both knees revealed genu valgus knees and lateral joint pain that recurred after several previous operations. Radiographs of both knees showed patellar dislocation and genu valgum associated with patellofemoral dysplasia and osteoarthritis of the lateral compartment. Long-leg standing radiographs showed an anatomic tibiofemoral angle of right 13° and left 6° valgus and a mechanical tibiofemoral angle of right 8° and left 2° valgus and weight-bearing line of 65% on the right and 48% on the left. The authors performed a distal femoral closing wedging osteotomy to correct the valgus deformity, and then percutaneous lateral release and medial reefing were performed to stabilize the patellas of both knees simultaneously.

A retrospective analysis was performed based on the medical records and imaging studies of 16 wrists (14 patients) with isolated partial intercarpal ligament tears (scapholunate ligament: 6 wrists, lunotriquetral ligament, 10 wrists) treated with arthroscopic debridement and thermal shrinkage. Three wrists had Geissler grade 1 tears and 13 wrists had grade 2 tears. Mean follow-up was 52.8 months. Overall pain visual analog scale scores improved significantly ( P The results of this study suggest that arthroscopic debridement and thermal shrinkage provide symptomatic pain relief and prevention of intercarpal instability for a significant period of time in patients with partial intercarpal ligament tear.

Chondral fractures of the patella are associated with acute dislocation of the patella. Osteochondral fracture in patellar dislocation is located in the medial facet of the patella. This article presents a case of a 15-year-old female ballerina with isolated displaced osteochondral fracture of the patella without patellar dislocation. She had no history of trauma. A Merchant’s view of both knees showed mild subluxation of the patella, a small fragment on the lateral aspect of the knee, and a small defect of the centromedial patella. Axial magnetic resonance imaging (MRI) revealed an osteochondral fragment measuring 13 mm medial to the patella. However, the medial patellofemoral ligament and medial retinaculum were intact. An effusion on the medial side of the patella consistent with hemarthrosis was observed. An absence of a contusion or bone bruise on the lateral femoral condyle was shown. The loose body was removed arthroscopically. Intraoperative findings included a 1.5×2 cm osteochondral fragment. It is unusual that the osteochondral patellar defect site in this patient was in the inferior and central areas of the patella. Patellar chondral fractures without dislocation or patella fracture are rare. Therefore, the possibility of a trivial trauma leading to an osteochondral fracture should be kept in mind in adolescent and young adults who present with knee pain and hemarthrosis.

The popliteal artery is vulnerable to injury during surgeries performed around the knee joint. Pseudoaneurysm of the popliteal artery following a high tibial osteotomy is rare. Few case reports describe the development of this complication after a lateral closing wedge high tibial osteotomy. Our patient underwent an uneventful medial opening wedge high tibial osteotomy and autogenous bone grafting fixed with dual plating for medial osteoarthritis of the knee. The procedure was performed under tourniquet control, which was released only once after the wound closure. Postoperatively, the dressing was soaked, and a large volume of hemorrhagic collection was present in the suction drain. The patient experienced decreased sensation over the sole, which was successfully treated conservatively with medication. Other clinical parameters like motor function and distal pulses were normal. The patient was discharged after 2 weeks. Two days later, the patient presented with pain and numbness over the entire lower limb and a pulsatile swelling in the popliteal fossa. A femoral angiogram revealed a pseudoaneurysm arising from the popliteal artery just below the osteotomy site. Open vascular surgery with resection of the pseudoaneurysm and end-to-end anastomosis using contralateral saphenous vein interposition graft was performed. During the vascular surgery, a pinhead-sized tear was clearly identified on the anterior wall of the popliteal artery, which may have occurred while using the oscillating saw during opening wedge high tibial osteotomy. Careful placement of retractors around the osteotomy site during sawing and flexing the knee to displace the popliteal artery away are recommended to prevent this complication. To our knowledge, this is the first report of a popliteal artery pseudoaneurysm occurring after a medial opening wedge high tibial osteotomy.

The OrthoPilot TKA navigation system (B. Braun Aesculap, Tuttlingen, Germany) offers software to optimize soft tissue balance using gap balance techniques. However, there are no studies on the reliability of the navigation-guided gap technique. The goal of this study is to establish the reliability of the navigation-guided gap technique. The investigators measured flexion and extension gap in the medial and lateral sides of the knee joint after bone resection to evaluate the reliability of navigation-guided soft tissue balancing. Gap data from 100 cases of navigation-guided total knee arthroplasty were analyzed. We defined trapezoidal gap (unsatisfactory soft tissue balance) as a gap difference3 mm between the medial and lateral sides in extension and a 5 mm difference in 90 degrees of flexion. Furthermore, gap difference between flexion and extension greater than 3 mm on the medial side and 5 mm on the lateral side was also considered a trapezoidal gap. Among 100 cases, 84 showed rectangular (acceptable) gap, and 16 showed trapezoidal gap. We also evaluated the correlation between clinical results including range of motion and soft tissue balance as well as characteristics of trapezoidal gap. This study suggests that the navigationguided gap technique is a reliable method for optimizing soft tissue balance.

Calcific tendinitis results from the deposition of calcium hydroxyapatite crystals in periarticular muscular attachments. It is a rare cause of knee pain commonly affecting patients aged 40 to 70 years. Although commonly seen in the shoulder, it should be kept in mind in nontraumatic cases, particularly when the pain is severe and localized to the lateral aspect of the knee. The exact mechanism of hydroxyapatite deposition is unclear, although genetic and metabolic factors have been suspected. A 45-year-old man presented with severe pain in the lateral aspect of his knee with local tenderness over the lateral epicondyle. Radiographs revealed multiple calcific deposits just below the lateral epicondyle of the femur. Magnetic resonance imaging showed multiple areas of low-signal present intra-articularly near the popliteus tendon that was suspected to be calcification. Erythrocyte sedimentation rate and C-reactive protein were slightly raised and other blood investigations including uric acid were within normal limits. Due to failure of conservative treatment, arthroscopy was performed through standard anteromedial and anterolateral portals. Arthroscopy revealed reddish synovial congestion in the lateral gutter. Partial synovectomy was performed with a shaver through a superolateral portal and the calcific deposit was found to lie between the popliteus tendon and the lateral collateral ligament. This was excised and sent for biopsy. Histopathological evaluation revealed the presence of hydroxyapatite crystals within degenerated tendon thereby confirming the diagnosis of calcific tendinitis. Immediate resolution of symptoms following excision allowed the patient to perform activities of daily living immediately postoperatively without pain.
