Abstract

Bladder prolapse through a patent urachus is rare. We present a newborn with an unusual exophytic, erythematous umbilical mass. Voiding cystography readily demonstrated continuity of the bladder dome with the umbilical mass, confirming bladder prolapse through a patent urachus. The diagnosis of bladder prolapse was rapidly made in a second newborn with similar imaging and clinical findings and confirmed by cystography. We discuss the embryology of this condition including the association with a vesico-allantoic cyst in utero. Pre- and postnatal images are presented. Abstract

A patent urachus, which is typically diagnosed as leakage from the umbilicus postnatally, can present as an allantoic cyst in the umbilical cord antenatally. We report a case of a patent urachus with an allantoic cyst diagnosed via fetal MR imaging at 24 weeks' gestation. Abstract

Ultrasound of a patent urachus has been well described. However, ultrasound of the other congenital abnormalities affecting the umbilicus has not. Two cases are described, one of a vitelline (omphalomesenteric) duct and one of an umbilical granuloma, in which the ultrasound findings guided the child's management, preventing a minilaparotomy.

Abstract

From 1981 to 1989 seventeen cases of pediatric patients with urachal remnants have been treated at the Fukuoka Municipal Children's Hospital (2 patent urachus, 5 urachal cyst, 9 urachal sinus, 1 urachal diverticulum). The cases of patent urachus were discovered in the neonates due to a urine discharge from the umbilicus; in the older children, cysts or sinuses accompanied by an infection led to the diagnosis of the urachal anomaly. In 8 of 11 cases, fistulography established the diagnosis. In 9 of 12 cases, ultrasound imaging was diagnostically successful, as was CT in all 3 cases that were given scans.

Abstract

Sonographic air/CO2 contrast studies of the urinary bladder allow identification of abnormal peri- and intravesical low-echogenic structures and can differentiate between those that communicate with the bladder (refluxing ureters, bladder diverticula, patent urachus) from others that do not communicate (cysts of ovarian/enteric origin, urachus cyst). Ureteroceles with a refluxing/obstructed ureter can be more clearly delineated using gas contrast than with plain ultrasound. In a group of 217 children examined by bladder contrast sonography, 19 showed pathological peri- and intravesical findings, 17 of which were definitely evaluated by bladder contrast sonography. On the other hand, only 10 abnormalities of these 19 could be detected, and in not more than 8 was the final diagnosis made by plain sonography alone.
