Objective: The objective was to compare the efficacy of three different lasers in disinfection of root canals inoculated with Enterococcus faecalis and Candida albicans biofilms., Background: Endodontic space disinfection depends on the type of root canal irrigant used and the way it is delivered and agitated because irrigants have limited ability to reach all parts of root canal system., Materials and Methods: Thirty single-rooted human teeth were selected. Root canals were instrumented and root surfaces were sealed using adhesive and the apical openings with adhesive and composite resin. Roots were fixed in Eppendorf tubes and sterilized in autoclave. The specimens were randomly divided into three experimental groups (n = 10) and inoculated with E. faecalis and C. albicans. After 7 days of incubation period, the number of E. faecalis and C. albicans colony-forming units (CFUs) was determined for each root canal. In the first experimental group, Er:YAG laser (0.3 W) with photon-induced photoacoustic streaming technique was used for root canal disinfection, in the second, Nd:YAG laser (1.5 W), and in the third, Er,Cr:YSGG (1.25 W) laser was used. After different root canal disinfection protocols, the number of E. faecalis and C. albicans CFUs was determined again for each root canal., Results: Er:YAG and Er,Cr:YSGG lasers eradicated significant number of E. faecalis and C. albicans CFUs (p < 0.05), while Nd:YAG laser irradiation did not result in statistically significant reduction (p > 0.05). Er,Cr:YSGG laser eradicated significantly more microorganisms in comparison with Er:YAG laser (p < 0.05)., Conclusions: Er,Cr:YSGG laser was the most efficient tool in eradication of E. faecalis and C. albicans biofilms.

Objective: The purpose of this study was to evaluate the effects of low-level laser irradiation (LLLI) on the in vitro growth characteristics and in vivo pathogenicity of Candida albicans in a murine model in the absence of a photosensitizer., Background Data: C. albicans is an opportunistic commensal organism that causes a wide variety of diseases in human beings, ranging from superficial infections to life-threatening invasive candidiasis. The incidence of C. albicans infection is increasing, because of the greater frequency of acquired immunodeficiency conditions. A high recurrence rate has been reported for vulvovaginal and oral candidiasis, despite the best available treatments. Therefore, the search for new treatment modalities seems quite rational., Methods: Candida culture plates were exposed to common clinical energies of LLLI: 3, 5, 10, and 20 J at 685 nm (BTL Laser 5000, Medicinos Projektai, Czech Republic, Prague, max power output 50 mW) and 3, 5, 10, 30, and 50 J at 830 nm (BTL Laser 5000, Medicinos Projektai, Czech Republic, Prague, max power output 400 mW)., Results: Following LLLI with energies >10 J at both 685 and 830 nm wavelengths, statistically significant effects were observed in vitro on the turbidimetric growth kinetics of C. albicans and in vivo on the survival rate of infected mice (p value ≤ 0.05). Therefore, this energy could be considered a threshold for clinical investigation., Conclusions: Translating our data into the clinical setting, it can be proposed that a direct laser-based approach without using a photosensitizing dye can significantly reduce the pathogenicity of Candida albicans. It can also be concluded that laser light at specific wavelengths could be a possible promising novel treatment for superficial and mucocutaneous C. albicans infections.

Objective: The aim of this study was to quantitatively investigate the efficiency of the ultraviolet (UV) and visible light in eradication of Candida albicans in vitro; in particular, to determine, for selected wavelengths, the specific eradication coefficients and thresholds in terms of energy density levels required to effect 3.0log10 and 4.0log10 reduction., Background Data: Oral candidosis is the most common infection of the oral cavity and is caused by Candida species. The widespread use of topical and systemic antifungal agents as conventional treatment for oral candidosis has resulted in the development of resistance in C. albicans. Therefore, it has become necessary to develop alternative therapies for the treatment of oral candidosis., Methods: C. albicans ATCC(®) 90028(™) was irradiated with 254 nm, 365 nm, 406 nm, 420 nm, and broadband Xe spectrum. For each wavelength, a fit of experimental data (survival fraction vs. applied energy density) with an exponential decay function enabled estimation of the specific eradication coefficients and thresholds., Results: Based on estimated specific efficiencies (Δ) and eradication thresholds (ET) of the investigated wavelengths, the ranking in eradication efficiency of C. albicans (most to least effective) is: 254 nm (Δ=6.1 mJ/cm(-2), ET99.99=56 mJ/cm(-2)), broadband Xe spectrum (Δ=27.7 mJ/cm(-2), ET99.99=255 mJ/cm(-2)), 365 nm (Δ=4.3 J/cm(-2), ET99.99=39 J/cm(-2)), 420 nm (Δ=0.65 J/cm(-2), ET99.99=6 J/cm(-2)), and 406 nm (Δ=11.4 J/cm(-2), ET99.99=104 J/cm(-2))., Conclusions: The results provide insight into the wavelength-dependent dynamics of eradication of C. albicans. For each investigated wavelength, the eradication coefficient and corresponding eradication threshold were estimated. The observed different eradication efficiencies are consequence of different spectrally dependent inactivation mechanisms. The established methodology enables unambiguous quantitative comparison of eradication efficiencies of optical radiation and selection of most effective wavelengths for clinical and therapeutic use.

Objective: The purpose of this in vitro study was to investigate the ability of Er,Cr:YSGG laser irradiation at different energy settings to eliminate Candida albicans, either with or without 5.25% sodium hypochlorite (NaOCl) treatment., Methods: In total, 90 extracted human single-rooted teeth were shaped by using a crown-down technique with HERO Shaper rotary instruments and irrigated with 1 mL of 2.5% NaOCl between each instrument. The specimens were autoclaved and incubated with a suspension of C. albicans (ATCC 90028). Then the specimens were divided into six treatment groups (n = 15 per group): group 1, NaOCl + 1-W laser; group 2, 1-W laser; group 3, NaOCl + 0.75-W laser; group 4, 0.75-W laser, group 5, NaOCl; and group 6, no treatment. Aliquots of the experimental teeth were transferred onto Sabouraud's dextrose agar plates with paper points, and colony-forming units (CFUs) were counted as a measure of antifungal activity. One root from each group, which was not submitted to the sampling procedure, was fixed in glutaraldehyde, split into two halves, and evaluated with scanning electron microscopy (SEM). Statistical analysis was performed by using analysis of variance (ANOVA) and the Dunnett t test. Significance was set at alpha = 0.05., Results: All the treatment groups resulted in a significant reduction of C. albicans, but no sterilization, and all the groups were significantly different from each other (p < 0.0001). Group 1 (NaOCl and 1-W laser combination) exhibited the greatest reduction, whereas group 5 (NaOCl) resulted in a minimal reduction in CFUs of C. albicans. The SEM observations were in accordance with the microbiologic analysis., Conclusions: The 5.25% NaOCl and 1-W laser treatment combination may be a valuable cleaning tool for root canals infected with C. albicans.

Objective: This study evaluated, in vitro, the degree of disinfection of the Er:YAG laser in root canals contaminated with Bacillus subtilis, Staphylococcus aureus, Enterococcus faecalis, Pseudomonas aeruginosa, and Candida albicans, for 28 days., Methods: Forty-six single-rooted human teeth were divided into five groups of eight teeth each; three teeth were used as negative controls and three as positive controls. After contamination, the root canals were prepared mechanically. Three groups were irradiated with Er:YAG laser at 100 mJ, varying the frequency (7, 10, and 16 Hz). Two groups were irrigated with 1.0% and 2.5% NaOCl solution. After treatment, two sterilized paper cones were placed in the root canals for 5 min. One cone was transferred to 2.0 mL of Letheen broth culture medium, incubated at 37 degrees C for 48 h, and then 0.1 mL of that solution was placed in 2.0 mL of brain heart infusion for 48 h to determine microbial growth. The other cone was transferred to a test pipette with peptone and water for serial dilution and spread in Müeller Hinton medium. After 24 h of incubation, the colony-forming units (CFUs) were counted., Results: There was a microbial reduction of 85.33% for the group irradiated with Er:YAG laser at 100 mJ/7 Hz, 74.58% at 100 mJ/10 Hz, and 89.50% at 100 mJ/16 Hz. For the groups irrigated with 1.0% and 2.5% NaOCl solution, 83.15% and 84.46% values of microbial reduction were obtained respectively., Conclusion: All the groups showed statistically similar results (p > 0.05%). No method totally eliminated microorganisms.

Objective: The purpose of our report is to present the effect of low-level laser therapy on Candida albicans growth and palatal inflammation in two patients with denture stomatitis., Background Data: The most common oral mucosal disorder in denture wearers is denture stomatitis, a condition that is usually associated with the presence of the yeast Candida albicans. Different treatment methods have been suggested to treat this symptom, none of which is proven to be absolutely effective., Methods: Two denture-wearing patients, both with palatal inflammation diagnosed as Newton type II denture stomatitis were treated with low-power semiconductor diode laser (BTL-2000, Prague, Czech Republic) at different wavelengths (685 and 830 nm) for 5 d consecutively. In both patients, palatal mucosa and acrylic denture base were irradiated in noncontact mode (probe distance of 0.5 cm from irradiated area) with different exposure times-5 min (830 nm, 3.0 J/cm2, 60 mW) and 10 min (685 nm, 3.0 J/cm2, 30 mW). The effect of laser light on fungal growth in vivo was evaluated after the final treatment using the swab method and semiquantitative estimation of Candida albicans colonies growth on agar plates. The severity of inflammation was evaluated using clinical criteria., Results: After lowlevel laser treatment, the reduction of yeast colonies on the agar plates was observed and palatal inflammation was diminished., Conclusion: LLLT is effective in the treatment of denture stomatitis. Further placebo controlled studies are in progress.
