Coronary angiography is clinically used worldwide to diagnose diseases of coronary arteries. Despite its effectiveness, this technique is quite invasive and it is associated with significant risks due to the arterial catheterisation needed to inject the contrast agent. A valid alternative is using the K-edge subtraction (KES) method, which is based on the subtraction of two images acquired at energies bracketing the K-edge of the contrast element. The enhanced sensitivity of KES allows the intravenous injection of the contrast agent, thus reducing the risks of catheterisation. This technique can be effectively implemented by using intense and quasi-monochromatic x-ray beams. Synchrotron radiation has been proven to work well for this purpose, but its cost and size prevent a widespread clinical application. Inverse Compton sources are among the most promising innovative sources of intense and quasi-monochromatic x-rays. These sources are intrinsically more compact than those based on synchrotron radiation. In this work, the potential application of inverse Compton radiation to KES angiography is investigated. To this purpose, after a short review of the physics behind the inverse Compton process, an analytical framework is described. The proposed model is based on the application of the KES algorithm to calculate the SNR of details inside a suitable mathematical phantom. That allowed us to identify the characteristics of an inverse Compton source required for KES imaging. In particular, it was estimated that a photon fluence of 10 8 ph mm -2 is necessary to detect signals of clinical interest. Novel sources based on inverse Compton promise to achieve this requirement with an acquisition time of few hundreds of ms. This feature, together with compactness, broad two-dimensional radiation field, absence of harmonic contamination and the ability to deliver high photon fluxes also at high energies, makes this kind of sources promising for KES angiography and other diagnostic applications.

We describe a spectral x-ray transmission method to provide images of independent material components of an object using a synchrotron x-ray source. The imaging system and process is similar to K-edge subtraction (KES) imaging where two imaging energies are prepared above and below the K-absorption edge of a contrast element and a quantifiable image of the contrast element and a water equivalent image are obtained. The spectral method, termed 'spectral-KES' employs a continuous spectrum encompassing an absorption edge of an element within the object. The spectrum is prepared by a bent Laue monochromator with good focal and energy dispersive properties. The monochromator focuses the spectral beam at the object location, which then diverges onto an area detector such that one dimension in the detector is an energy axis. A least-squares method is used to interpret the transmitted spectral data with fits to either measured and/or calculated absorption of the contrast and matrix material-water. The spectral-KES system is very simple to implement and is comprised of a bent Laue monochromator, a stage for sample manipulation for projection and computed tomography imaging, and a pixelated area detector. The imaging system and examples of its applications to biological imaging are presented. The system is particularly well suited for a synchrotron bend magnet beamline with white beam access.

This work reports on the development and performance evaluation of the VrPET/CT, a new multimodality scanner with coplanar geometry for in vivo rodent imaging. The scanner design is based on a partial-ring PET system and a small-animal CT assembled on a rotatory gantry without axial displacement between the geometric centers of both fields of view (FOV). We report on the PET system performance based on the NEMA NU-4 protocol; the performance characteristics of the CT component are not included herein. The accuracy of inter-modality alignment and the imaging capability of the whole system are also evaluated on phantom and animal studies. Tangential spatial resolution of PET images ranged between 1.56 mm at the center of the FOV and 2.46 at a radial offset of 3.5 cm. The radial resolution varies from 1.48 mm to 1.88 mm, and the axial resolution from 2.34 mm to 3.38 mm for the same positions. The energy resolution was 16.5% on average for the entire system. The absolute coincidence sensitivity is 2.2% for a 100-700 keV energy window with a 3.8 ns coincident window. The scatter fraction values for the same settings were 11.45% for a mouse-sized phantom and 23.26% for a rat-sized phantom. The peak noise equivalent count rates were also evaluated for those phantoms obtaining 70.8 kcps at 0.66 MBq/cc and 31.5 kcps at 0.11 MBq/cc, respectively. The accuracy of inter-modality alignment is below half the PET resolution, and the image quality of biological specimens agrees with measured performance parameters. The assessment presented in this study shows that the VrPET/CT system is a good performance small-animal imager, while the cost derived from a partial ring detection system is substantially reduced as compared with a full-ring PET tomograph.

Several groups have reported the development of dual modality Gamma camera/optical imagers, which are useful tools for investigating biological processes in experimental animals. While previously reported dual modality imaging instrumentation usually employed a separated gamma camera and optical imager, we designed a detector using a position sensitive photomultiplier tube (PSPMT) that is capable of imaging both gamma rays and optical photons for combined gamma camera and optical imager. The proposed system consists of a parallel-hole collimator, an array-type crystal and a PSPMT. The top surface of the collimator and array crystals is left open to allow optical photons to reach the PSPMT. Pulse height spectra and planar images were obtained using a Tc-99m source and a green LED to estimate gamma and optical imaging performances. When both gamma rays and optical photon signals were detected, the signal interferences caused by each other signal were evaluated. A mouse phantom and an ICR mouse containing a gamma ray and optical photon source were imaged to assess the imaging capabilities of the system. The sensitivity, energy resolution and spatial resolution of the gamma image acquired using Tc-99m were 1.1 cps/kBq, 26% and 2.1 mm, respectively. The spatial resolution of the optical image acquired with an LED was 3.5 mm. Signal-to-signal interference due to the optical photon signal in the gamma pulse height spectrum was negligible. However, the pulse height spectrum of the optical photon signal was found to be affected by the gamma signal, and was obtained between signals generated by gamma rays with a correction using a veto gate. Gamma ray and optical photon images of the mouse phantom and ICR mouse were successfully obtained using the single detector. The experimental results indicated that both optical photon and gamma ray imaging are feasible using a detector based on the proposed PSPMT.

A dedicated breast PET/CT system has been constructed at our institution, with the goal of having increased spatial resolution and sensitivity compared to whole-body systems. The purpose of this work is to describe the design and the performance characteristics of the PET component of this device. Average spatial resolution of a line source in warm background using maximum a posteriori (MAP) reconstruction was 2.5 mm, while the average spatial resolution of a phantom containing point sources using filtered back projection (FBP) was 3.27 mm. A sensitivity profile was computed with a point source translated across the axial field of view (FOV) and a peak sensitivity of 1.64% was measured at the center of the FOV. The average energy resolution determined on a per-crystal basis was 25%. The characteristic dead time for the front-end electronics and data acquisition (DAQ) was determined to be 145 ns and 3.6 micros, respectively. With no activity outside the FOV, a peak noise-equivalent count rate of 18.6 kcps was achieved at 318 microCi (11.766 MBq) in a cylindrical phantom of diameter 75 mm. After the effects of exposing PET detectors to x-ray flux were evaluated and ameliorated, a combined PET/CT scan was performed. The percentage standard deviations of uniformity along axial and transaxial directions were 3.7% and 2.8%, respectively. The impact of the increased reconstructed spatial resolution compared to typical whole-body PET scanners is currently being assessed in a clinical trial.

A compact dedicated 3D breast SPECT-CT (mammotomography) system is currently under development. In its initial prototype, the cone-beam CT sub-system is restricted to a fixed-tilt circular rotation around the patient's pendant breast. This study evaluated stationary-tilt angles for the CT sub-system that will enable maximal volumetric sampling and viewing of the breast and chest wall. Images of geometric/anthropomorphic phantoms were acquired using various fixed-tilt circular and 3D sinusoidal trajectories. The iteratively reconstructed images showed more distortion and attenuation coefficient inaccuracy from tilted cone-beam orbits than from the complex trajectory. Additionally, line profiles illustrated cupping artifacts in planes distal to the central plane of the tilted cone-beam, otherwise not apparent for images acquired with complex trajectories. This indicates that undersampled cone-beam data may be an additional cause of cupping artifacts. High-frequency objects could be distinguished for all trajectories, but their shapes and locations were corrupted by out-of-plane frequency information. Although more acrylic balls were visualized with a fixed-tilt and nearly flat cone-beam at the posterior of the breast, 3D complex trajectories have less distortion and more complete sampling throughout the reconstruction volume. While complex trajectories would ideally be preferred, negatively fixed-tilt source-detector configuration demonstrates minimally distorted patient images.

Modern positron emission tomography (PET) detectors are typically made from 2D modular arrays of scintillation crystals. Their characteristic flood field response (or flood histogram) must be segmented in order to correctly determine the crystal of annihilation photon interaction in the system. Crystal identification information thus generated is also needed for accurate system modeling as well as for detailed detector characterization and performance studies. In this paper, we present a semi-automatic general purpose template-guided scheme for the segmentation of flood histograms. We first generate a template image that exploits the spatial frequency information in the given flood histogram using Fourier-space analysis. This template image is a lower order approximation of the flood histogram, and can be segmented with horizontal and vertical lines drawn midway between adjacent peaks in the histogram. The template is then registered to the given flood histogram by a diffeomorphic polynomial-based warping scheme that is capable of iteratively minimizing intensity differences. The displacement field thus calculated is applied to the segmentation of the template resulting in a segmentation of the given flood histogram. We evaluate our segmentation scheme for a photomultiplier tube based PET detector, a detector with readout by a position-sensitive avalanche photodiode (PSAPD) and a detector consisting of a stack of photomultiplier tubes and scintillator arrays. Further, we quantitatively compare the performance of the proposed method to that of a manual segmentation scheme using reconstructed images of a line-source phantom. We also present an adaptive method for distortion reduction in flood histograms obtained for PET detectors that use PSAPDs.

In order to maintain overall navigation accuracy established by a calibration procedure in our CT-based registration-free navigation system, the CT scanner has to repeatedly generate identical volume images of a target at the same coordinates. We tested the positioning accuracy of the prototype of an advanced workplace for image-guided surgery (AWIGS) which features an operating table capable of direct patient transfer into a CT scanner. Volume images (N = 154) of a specialized phantom were analysed for translational shifting after various table translations. Variables included added weight and phantom position on the table. The navigation system's calibration accuracy was determined (bias 2.1 mm, precision +/- 0.7 mm, N = 12). In repeated use, a bias of 3.0 mm and a precision of +/- 0.9 mm (N = 10) were maintainable. Instances of translational image shifting were related to the table-to-CT scanner docking mechanism. A distance scaling error when altering the table's height was detected. Initial prototype problems visible in our study causing systematic errors were resolved by repeated system calibrations between interventions. We conclude that the accuracy achieved is sufficient for a wide range of clinical applications in surgery and interventional radiology.

Gamma camera imaging with Tc-99m marking is a widely used method to locate sentinel lymph nodes (SNs) in breast cancer patients. Prior to SN biopsy, the anterior and lateral location of the SN is marked on the patient's skin using an ink pen. The pen marks guide the surgeon during an operation. However, in many cases the marking is difficult due to limited space under the detectors of a gamma camera. The aim of this study was to improve the pen marking method. Eleven female patients were imaged 3-4 h after injection of Tc-99m labelled Nanocol. Injection was performed to parenchyma surrounding the breast tumour. To facilitate pen marking, two polycarbonate (PC) plates with 40 x 32 holes (spacing=10 mm) were engineered for anterior and lateral side imaging and then installed on the bed of a dual-head gamma camera. Two drops of Tc-99m were placed into the top corners of both the PC plates, in order to trace the corresponding x-y coordinates first from the acquired images and then from the plates. After imaging, the x-y coordinates of the SN(s) were determined from the anterior and lateral side images. Subsequently, the location of each SN was marked with an ink pen on the skin through the small holes in the PC plates. According to the surgeon's evaluation, the distance between the marks and the true location of the SNs was 4.5+/-6.9 mm. Measurements with a custom made phantom revealed that the accuracy of the novel method was significantly (P=0.06) higher as compared with the traditional method (2.7+/-3.0 mm versus 9.2+/-3.0 mm). In addition, we were not able to mark the weakest activity (0.02 MBq) with the traditional method. Taken together, the marking process was considerably easier with the novel method, it had better accuracy and sensitivity than the traditional method and the device is simple enough to be adapted for most gamma cameras.

Non-invasive imaging technologies are opening up new windows into mouse biology. We have developed a mouse imaging system that integrates positron emission tomography (PET) with x-ray computed tomography (CT), allowing simultaneous anatomic and molecular imaging in vivo with the potential for precise registration of the two image volumes. The x-ray system consists of a compact mini-focal x-ray tube and an amorphous selenium flat panel x-ray detector with a low-noise CMOS readout. The PET system uses planar arrays of lutetium oxyorthosilicate scintillator coupled to position-sensitive photomultiplier tubes. We describe the design of this dual-modality imaging system and show, for the first time, simultaneously acquired PET and CT images in a phantom and in mice.
