Financial sanctions are often thought of as the “soft alternative” to armed conflict and are widely used in the 21st century. Nonetheless, sanctions are often criticized for being non-specific in their action, and having impact beyond their intended remit. One often-overlooked area affected by sanctions are academic systems of research and education. Sanctions place “invisible barriers” for research in these countries by limiting access to necessary resources and curtailing their effective use. In this paper we present a national survey of Sudanese academics focused on the impact of 20 years of economic sanctions on their work. It identifies key areas of academic research and education that have been impacted by international sanctions. Moreover, these data highlight how the impact of sanctions on academia is likely to persist long after they are formally lifted. The paper concludes by problematising the current interpretation of jus post bellum, or moral behaviour after conflict. It suggests that the responsibility to make reparations in the form of support for academic systems applies to countries who impose economic sanctions. [ABSTRACT FROM AUTHOR]

Background: The population of Ghana is increasingly becoming urbanized with about 70% of the estimated 27 million people living in urban and peri-urban areas. Nonetheless, eight out of the ten regions in Ghana remain predominantly rural where only 32% of the national health sector workforce works. Moreover, the rural-urban disparities in the density of health tutors (staff responsible for pre-service training of health professionals) are enormous. This paper explores perceived needs of health tutors in rural and urban health training institutions in Ghana. Methods: This is a descriptive qualitative study conducted in the Greater Accra and Northern regions of Ghana. The Study used the deductive thematic and sub-thematic analysis approaches. Five health training institutions were randomly sampled, and 72 tutors engaged in separate focus group discussions with an average size of 14 participants per group in each training institution. Results: Perceived rural-urban disparities among health tutors were found in the payment of extra duty allowances; school infrastructure including libraries and internet connectivity; staff accommodation; and opportunities for scholarships and higher education. Health tutors in rural areas generally expressed more frustration with these work conditions than those in urban areas. Conclusions: There is the need to initiate and sustain work incentives that promote motivation of rural health tutors to control ongoing rural-urban migration of qualified staff. It is recommended the following incentives be prioritized to promote retention of qualified health tutors in rural health training schools: payment of research, book and rural allowances; early promotion of rural staff; prioritizing rural tutors for scholarships, and introduction of national best health tutor awards. We propose a novel laboratory experiment to document the pressure to share income within social networks in Africa. We find that the redistributive pressure exerted via the possibility of receiving a claim increases altruism, while the possibility of hiding from such claim reduces it. Our results indicate that sharing norms are crucial drivers of giving to other members of the network. We also find that pressure to share has a detrimental effect on the undertaking of profitable but risky investments. [ABSTRACT FROM AUTHOR]

Background: Despite considerable efforts to prevent HIV and other sexually transmitted infections (STI) among female sex workers (FSW), other sexual and reproductive health (SRH) needs, such preventing unintended pregnancies, among FSW have received far less attention. Programs targeting FSW with comprehensive, accessible services are needed to address their broader SRH needs. This study tested the effectiveness of an intervention to increase dual contraceptive method use to prevent STIs, HIV and unintended pregnancy among FSW attending services in drop-in centers (DIC) in two cities in Kenya. The intervention included enhanced peer education, and routine screening for family planning (FP) needs plus expanded non-condom FP method availability in the DIC. Methods: We conducted a two-group, pre-/posttest, quasi-experimental study with 719 FSW (360 intervention group, 359 comparison group). Participants were interviewed at baseline and 6 months later to examine changes in condom and non-condom FP method use. Results: The intervention had a significant positive effect on non-condom, FP method use (OR = 1.38, 95%CI (1.04, 1.83)), but no effect on dual method use. Consistent condom use was reported to be high; however, many women also reported negotiating condom use with both paying and non-paying partners as difficult or very difficult. The strongest predictor of consistent condom use was partner type (paying versus non-paying/emotional); FSW reported both paying and non-paying partners also influence non-condom contraceptive use. Substantial numbers of FSW also reported experiencing sexual violence by both paying and non-paying partners. Conclusions: Self-reported difficulties with consistent condom use and the sometimes dangerous conditions under which they work leave FSW vulnerable to unintended pregnancy STIs/HIV. Adding non-barrier FP methods to condoms is crucial to curb unintended pregnancies and their potential adverse health, social and economic consequences. Background: Research on contraceptive behaviour changes over time in Uganda is scarce, yet it has among the highest fertility and maternal mortality rates of any country in the East African region. Understanding temporal patterns of contraceptive use for both women and men is vital in evaluating the effectiveness of family planning interventions and strategies, and identifying those with the most unmet need. Using repeated nationally representative cross-sectional samples, this study charts the changes in Uganda’s population-based contraceptive use over recent years. Methods: Five Demographic and Health Survey datasets for Uganda over 21 years, from 1995 to 2016, were sourced and interrogated. Eligible participants included all women aged 15–49 years and men aged 15–54 years. Responses to questions on modern and any (modern or traditional) contraceptive use were analysed. Stratified by gender, weighted regression analyses were employed to detect change over time. The patterns associated with key demographic variables were also investigated. Results: Overall, 50,027 women and 14,092 men were included within the study. In 2016, 30.3% of women and 39.9% of men were using any contraceptive method, a significant non-linear increase from 13.4% of women and 20.3% of men in 1995. Furthermore, 27.3% of women and 35.9% of men were using modern contraceptive methods in 2016, an increase from 7.4% of women and 10.4% of men in 1995. All considered demographic variables were significantly associated with contraceptive use for both women and men (all P<0.001); and for women, all variables differentially changed over time (all P<0.001). Conclusion: This study showed a significant increase and dynamism across key demographic variables in contraceptive uptake by both women and men. Sustained family planning programs and interventions have successfully resulted in behaviour change across the Ugandan population. A confounding factor for healthcare programmes in African countries is the inability of essential health tools to reach targeted locations and populations, due to poor Logistics Management Information System (LMIS). In a bid to contribute towards addressing these challenges, a pilot study was undertaken to evaluate the tracking ability, reliability and applicability of EASE App, a novel Smart Phone based Application. The App is designed to provide real-time tracking and tracing of commodities as well as curation of data in a cloud based database with restricted access which can be linked with other databases. In this study, NIPRIMAL was labelled with QR codes, and tracked within the Federal Capital Territory, Abuja, Nigeria, using the smartphone based EASE App. Data collected showed that the “EASE App” tracking had accuracy of 100% for date and time of scan, operators’ codes and product identity; and 92.83±1.69% and 99.83±0.27% accuracy for GPS mapping label for the city and country, respectively. The GPS mapping label for specific streets, roads or districts, gave an accuracy of about 64.28±3.14%. The technology was able to provide real-time data on user unique identity, user location as well as date/time of use, and the feedback report indicated that it was readily deployable and easy to use. The results demonstrate that the “EASE App” is a promising technology that can support supply chain and related data management challenges in resource poor settings. The potential benefit of the EASE App in strengthening LMIS and distribution chain system in Africa as well as future optimization of the App are discussed. [ABSTRACT FROM AUTHOR]

Background: Strong primary health care (PHC) is essential for achieving universal health coverage, but in many low- and middle-income countries (LMICs) PHC services are of poor quality. Facility management is hypothesized to be critical for improving PHC performance, but evidence about management performance and its associations with PHC in LMICs remains limited. Methods: We quantified management performance of PHC facilities in Ghana and assessed the experiences of women who sought care at sampled facilities. Using multi-level models, we examined associations of facility management with five process outcomes and eight experiential outcomes. Findings: On a scale of 0 to 1, the average overall management score in Ghana was 0·76 (IQR = 0·68–0·85). Facility management was significantly associated with one process outcome and three experiential outcomes. Controlling for facility characteristics, facilities with management scores at the 90th percentile (management score = 0·90) had 22% more essential drugs compared to facilities with management scores at the 10th percentile (0·60) (p = 0·002). Positive statistically non-significant associations were also seen with three additional process outcomes—integration of family planning services (p = 0·054), family planning types provided (p = 0·067), and essential equipment availability (p = 0·104). Compared to women who sought care at facilities with management scores at the 10th percentile, women who sought care at facilities at the 90th percentile reported 8% higher ratings of trust in providers (p = 0·028), 15% higher ratings of ease of following provider’s advice (p = 0·030), and 16% higher quality rating (p = 0·020). However, women who sought care in the 90th percentile facilities rated their waiting times as worse (22% lower, p = 0·039). Interpretation: Higher management scores were associated with higher scores for some process and experiential outcomes. Large variations in management performance indicate the need to strengthen management practices to help realize the full potential of PHC in improving health outcomes. [ABSTRACT FROM AUTHOR]

Background: TB persists despite being relatively easy to detect and cure because the journey from the onset of symptoms to cure involves a series of steps, with patients being lost to follow-up at each stage and delays occurring among patients not lost to follow-up. One cause of drop-off and delay occurs when patients delay or avoid returning to clinic to get their test results and start treatment. Methods: We fielded two SMS interventions in three Cape Town clinics to see their effects on whether people returned to clinic, and how quickly. One was a simple reminder; the other aimed to overcome “optimism bias” by reminding people TB is curable and many millions die unnecessarily from it. Recruits were randomly assigned at the clinic level to a control group or one of the two SMS groups (1:2:2). In addition to estimating effects on the full sample, we also estimated effects on HIV-positive patients. Results: SMS recipients were more likely to return to clinic in the requested two days than the control group. The effect was smaller in the intent-to-treat analysis (52/101 or 51.5% vs. 251/405 or 62.0%, p = 0.05) than in the per-protocol analysis (50/97 or 51.5% vs. 204/318 or 64.2%, p = 0.03). The effect was larger among HIV-positives (10/35 or 28.6% vs. 97/149 or 65.1%, p<0.01). The effects of SMS messages diminished as the interval increased: significant effects at the 5% level were found at five and 10 days only among HIV-positives. The second SMS message had larger effects, albeit not significantly larger, likely due in part to lack of statistical power. Conclusions: At 2 U.S. cents per message, SMS reminders are an inexpensive option to encourage TB testers to return to clinic, especially when worded to counter optimism bias. [ABSTRACT FROM AUTHOR]

Background: Method-specific contraceptive prevalence varies widely globally, as huge variations exist in the use of different types of contraception, with short-term methods being the most common methods in sub-Saharan Africa (SSA). Evidence is scanty on the trends, patterns and determinants of long-acting reversible contraceptive (LARC) methods in SSA. This study aimed to address this knowledge gap. Methods: Using a pseudo longitudinal research design and descriptive and inferential statistics, we analysed Demographic and Health Survey data of eight countries selected on the basis of contraceptive prevalence rates across SSA. Multinomial logistic regression modelling was used to tease out the predictors of the uptake of LARC methods in the selected countries. Results: Findings exhibit a steady but slow upward trend in LARC methods across selected countries, as a marginal increase was recorded in LARC uptake over a 10-year period in many of the selected countries. Results established significant predictors of LARC methods uptake, including fertility-related characteristics, age, level of education, work status, wealth index and exposure to mass media. This study underscored the need to address various barriers to the uptake of LARC methods in SSA. It is recommended that governments at different levels undertake to cover the costs of LARC methods in order to increase access and uptake. [ABSTRACT FROM AUTHOR]

Increasing modern contraceptive use is important for improving maternal and child health and achieving economic growth and development goals. However, pervasive high unmet need for modern contraceptives in sub-Saharan Africa warrants new understandings of the drivers of modern contraceptive use. A deviance approach (i.e., examining how women’s experiences/characteristics differ from other women in their community) provides an innovative framework for capturing heterogeneity among women in a community. This framework can inform public health programming by both exploring how women avoid adverse health outcomes and understanding the needs of harder-to-reach populations who may experience health risks, despite living in communities where others do not experience vulnerability. Using data from Demographic and Health Surveys from 29 sub-Saharan African countries, we examine how a woman’s deviation from community norms around socioeconomic characteristics and gender and fertility norms and behaviors is associated with modern contraceptive use. Random-effects logistic regression models were fitted for each country to examine relationships between modern contraceptive use and deviance. Some deviance factors were associated with modern contraceptive use in only a few countries, while others were significant across many countries. Cross-country consistency in the direction of the relationship between deviance and modern contraceptive use varied by the specific deviance factor, with some relationships being consistent across countries, and other relationships being more varied. For example, having more education than the community norm was associated with increased modern contraceptive use across countries; however, marrying older than other women in the community was associated with an increase in modern contraceptive use in some countries and a decrease in others. More work is needed to understand the role of deviance on modern contraceptive use; however, this study suggests that using context-specific deviance approaches may be important for further elucidating experiences of modern contraceptive use. Robust evidence underpinning the role of beekeeping in poverty alleviation is currently lacking. This study estimated the production potential for beekeepers in Northern Uganda by quantifying current production assets (equipment and knowledge) and impact on rural income streams range of proposed interventions. Intervention scenarios evaluated the economic benefits to be derived from different hive types combined with year-round provision of a nectar source (Calliandra calothyrsus) planted at varying density. Findings show that the type and number of beehive combinations used influenced the amount of revenue streams generated by the beekeepers. Addition of 20 log hives increased incomes 10 times, 20 KTBs increased revenues 16 times and Langstroth 18 times. Adding Calliandra trees as a forage source to the baseline scenario yielded revenues up to 17.6 times higher than the baseline. Implying that good management plus the introduction of a reliable nectar source, to off-set dry season challenges (absconding), could improve beekeeping productivity in Northern Uganda. Further research is required to validate in situ the impact of modelled scenarios on both honey yield and other ecosystem service benefits. [ABSTRACT FROM AUTHOR]

Rigorous impact evaluations on agricultural interventions in the developing world have proliferated in research of recent years. Whereas increased care in causal identification in such analyses is beneficial and has improved the quality of research in this field, much of the literature still fails to investigate the costs needed to achieve any benefits identified. Such understanding, however, would be crucial for drawing policy and programmatic conclusions from the research and for informing the allocation of public investments. Cost-effectiveness analysis (CEA) subjects both the cost side and the effects side of agricultural and rural interventions to technical scrutiny and unifies both sides in order to compare the relative cost-effectiveness of different modalities of a programme, of efforts to reach different target groups, or of efforts to achieve different outcomes. CEAs, while present in the health and education sectors, remain rare in agricultural and rural development research. This study contributes to filling the knowledge gap by conducting CEAs in a particular type of programmatic work in the agricultural sector—namely, interventions conducted as field experiments that bring a gender lens to community-based advisory services in African rural areas. Specifically, we consider two such programmes—one in Mozambique in which such advisory services aim to improve sustainable land management (SLM) practices in agricultural production, and the other in Tanzania to advise farmers on their land rights. Using CEA methods combined with econometric analysis based on randomised controlled trials, we find that the gendered modality is consistently more cost-effective than the basic modality when considering varied outcomes and target groups. However, for any given modality, it is more cost-effective to improve outcomes for men than for women. The structure of costs in the agricultural extension programme further allowed for a simulation of how cost-effectiveness would change if the programme were scaled up geographically. The results show that expansion of the basic modality of the SLM programme leads to improvements in cost-effectiveness, while the gendered modality displays nonlinear changes in cost-effectiveness along the expansion path, first worsening with initial scale-up and subsequently improving with further expansion. Road traffic accidents are the major cause of mortality among people aged 15–29 years in Africa. World Health Organisation (WHO) and the World Bank launched a Decade of Action for Road Safety in 2011 with the goal of halving the number of injuries and deaths on the roads. No progress has been reported in Low and Middle Income Countries (LMICs) and the number of deaths remains very high. To reach the target set, there is a need for interventions in several areas. This scoping review proposes to produce a synthesis by identifying the kinds of interventions and outcomes which have been carried out on the African continent. Using the scoping studies method, 23 articles were selected and analysed. The study shows that interventions were developed in four fields: road safety policy, health education, safety equipment and data collection. It shows also that there were records of interventions in only twelve countries, mostly in Eastern and Southern Africa. The main conclusion of this study reveals both a lack of road safety interventions and shortcomings in the assessment of those performed and selected for our study. [ABSTRACT FROM AUTHOR]

Epidemiological transitions are occurring throughout Africa. To inform public health programs and policies, longitudinal cohorts investigating non-communicable diseases are needed. However, loss-to-follow up is a major problem. In preparation for a longitudinal study, we conducted a randomized controlled trial to test communication-based retention strategies (message content and delivery methods) among a pilot cohort of South African healthcare workers (n = 1536; median age = 36; women = 1270). Two messaging formats across three delivery modes were tested. Response rates were analyzed by intervention, survey return date and method using chi-square tests and univariate logistic regression. Sixty-seven of 238 (17.4%) control group participants and 238 of 1152 (24.6%) intervention group participants were retained (OR 1.54: CI 1.15–2.07; P = 0.004). Odds of being retained were 1.68 times greater for participants who received regular contact and themed messages compared to control (CI 1.22–2.32; P = 0.001). Neither health status nor clinical condition affected response rates (P>0.05). Time-to-first contact did not impact response rates (P>0.05). Message content and delivery method influenced response rates compared to the control, however no difference was found between intervention groups. Although greater retention is required for valid cohort studies, these findings are the first to quantitatively assess retention factors in Africa. [ABSTRACT FROM AUTHOR]

Globally, it is recognized that blue carbon ecosystems, especially mangroves, often sequester large quantities of carbon and are of interest for inclusion in climate change mitigation strategies. While 19% of the world’s mangroves are in Africa, they are among the least investigated of all blue carbon ecosystems. We quantified total ecosystem carbon stocks in 33 different mangrove stands along the Atlantic coast of West-Central Africa from Senegal to Southern Gabon spanning large gradients of latitude, soil properties, porewater salinity, and precipitation. Mangrove structure ranged from low and dense stands that were <1m in height and >35,000 trees ha-1 to tall and open stands >40m in height and <100 ha-1. Tremendous variation in ecosystem carbon (C) stocks was measured ranging from 154 to 1,484 Mg C ha-1. The mean total ecosystem carbon stock for all mangroves of West-Central Africa was 799 Mg C ha-1. Soils comprised an average of 86% of the total carbon stock. The greatest carbon stocks were found in the tall mangroves of Liberia and Gabon North with a mean >1,000 Mg C ha-1. The lowest carbon stocks were found in the low mangroves of the semiarid region of Senegal (463 Mg C ha-1) and in mangroves on coarse-textured soils in Gabon South (541 Mg C ha-1). At the scale of the entirety of West-Central Africa, total ecosystem carbon stocks were poorly correlated to aboveground ecosystem carbon pools, precipitation, latitude and soil salinity (r2 = ≤0.07 for all parameters). Based upon a sample of 158 sites from Africa, Asia and Latin America that were sampled in a similar manner to this study, the global mean of carbon stocks for mangroves is 885 Mg C ha-1. The ecosystem carbon stocks of mangroves for West-Central Africa are slightly lower than those of Latin America (940 Mg C ha-1) and Asia (1049 Mg C ha-1) but substantially higher than the default Intergovernmental Panel on Climate Change (IPCC) values for mangroves (511 Mg C ha-1). This study provides an improved estimation of default estimates (Tier 1 values) of mangroves for Asia, Latin America, and West Central Africa. [ABSTRACT FROM AUTHOR]
