BACKGROUND:Prior acute coronary syndrome (ACS) registries in Saudi Arabia might not have accurately described the true demographics and cardiac care of patients with ACS. We aimed to evaluate the clinical characteristics, management, and outcomes of a representative sample of patients with acute myocardial infarction (AMI) in Saudi Arabia. METHODS:We conducted a 1-month snap-shot, prospective, multi-center registry study in 50 hospitals from various health care sectors in Saudi Arabia. We followed patients for 1 month and 1 year after hospital discharge. Patients with AMI included those with or without ST-segment elevation (STEMI or NSTEMI, respectively). This program survey will be repeated every 5 years. RESULTS:Between May 2015 and January 2017, we enrolled 2233 patients with ACS (mean age was 56 [standard deviation = 13] years; 55.6% were Saudi citizens, 85.7% were men, and 65.9% had STEMI). Coronary artery disease risk factors were high; 52.7% had diabetes mellitus and 51.2% had hypertension. Emergency Medical Services (EMS) was utilized in only 5.2% of cases. Revascularization for patients with STEMI included thrombolytic therapy (29%), primary percutaneous coronary intervention (PCI); (42.5%), neither (29%), or a pharmaco-invasive approach (3%). INTRODUCTION:MicroRNAs (miRNAs) are small non-coding RNA that plays a vital role in cancer progression. Neo-adjuvant chemotherapy (NAC) has become the standard of care for locally advanced breast cancer. The aim of this study was to evaluate miRNA alterations during NAC using multiple samples of tissue and serum to correlate miRNA expression with clinico-pathological features and patient outcomes. METHODS:Tissue and serum samples were collected from patients with locally advanced breast cancer undergoing NAC at four time points: time of diagnosis, after the first and fourth cycle of doxorubicin/cyclophosphamide treatment, and after the fourth cycle of docetaxel administration. First, we evaluated the miRNA expression profiles in tissue and correlated expression with clinico-pathological features. Then, a panel of four miRNAs (miR-451, miR-3200, miR-21, and miR-205) in serum samples was further validated using quantitative reverse-transcription polymerase chain reaction (RT-qPCR). The alterations in serum levels of miRNA, associations with clinical and pathological responses, correlation with clinico-pathological features, and survival outcomes were studied using Friedman, Mann-Whitney U, and Spearman, Wilcoxon signed-ranks tests. P≤0.05 was considered statistically significant. RESULTS:We analyzed 72 tissue samples and 108 serum samples from 9 patients and 27 patients, respectively. MicroRNA expression profiling of tumor versus normal tissue revealed more than 100 differentially expressed miRNAs. Serum miR-451 levels were significantly decreased during treatment, and higher serum levels were associated with improved clinical and pathological responses and disease-free survival. This is one of the early reports on miR-3200 in response to treatment in breast cancer, as serum levels of miR-3200 found to decline during NAC, and higher serum levels were associated with lower residual breast cancer burden and relapse rates at time of diagnosis. CONCLUSION:Variations in serum miRNA levels during NAC treatment may be therapeutically significant for predicting response and survival outcomes.

Endothelial dysfunction is a key initiating event in vascular disease in chronic kidney disease (CKD) patients and haemodialysis (HD) patients exhibit significant vascular abnormalities. Some mathematical characteristics are studied, including hazard function, Pth percentile, moment generating function and Shannon entropy. Simulation study is performed to examine the behaviour of parameter estimates. The proposed model is fitted on three real data sets to check its efficacy. Additionally, TTT (total time on test) plot is drawn to study the failure rate of the three data sets. Truncated models are imperative to efficiently analyze the finite data that we observe in almost all the real life situations. In this paper, a new truncated distribution having four parameters named Weibull-Truncated Exponential Distribution (W-TEXPD) is developed. The proposed model can be used as an alternative to the Exponential, standard Weibull and shifted Gamma-Weibull and three parameter Weibull distributions. The statistical characteristics including cumulative distribution function, hazard function, cumulative hazard function, central moments, skewness, kurtosis, percentile and entropy of the proposed model are derived. The maximum likelihood estimation method is employed to evaluate the unknown parameters of the W-TEXPD. A simulation study is also carried out to assess the performance of the model parameters. The proposed probability distribution is fitted on five data sets from different fields to demonstrate its vast application. Utilization of metal-oxide nanoparticles (NPs) in enhanced oil recovery (EOR) has generated substantial recent research interest in this area. Among these NPs, zinc oxide nanoparticles (ZnO-NPs) have demonstrated promising results in improving oil recovery due to their prominent thermal properties. These nanoparticles can also be polarized by electromagnetic (EM) field, which offers a unique Nano-EOR approach called EM-assisted Nano-EOR. However, the impact of NPs concentrations on oil recovery mechanism under EM field has not been well established. For this purpose, ZnO nanofluids (ZnO-NFs) of two different particle sizes (55.7 and 117.1 nm) were formed by dispersing NPs between 0.01 wt.% to 0.1 wt.% in a basefluid of sodium dodecylbenzenesulfonate (SDBS) and NaCl to study their effect on oil recovery mechanism under the electromagnetic field. This mechanism involved parameters, including mobility ratio, interfacial tension (IFT) and wettability. The displacement tests were conducted in water-wet sandpacks at 95˚C, by employing crude oil from Tapis. Three tertiary recovery scenarios have been performed, including (i) SDBS surfactant flooding as a reference, (ii) ZnO-NFs flooding, and (iii) EM-assisted ZnO-NFs flooding. Compare with incremental oil recovery from surfactant flooding (2.1% original oil in place/OOIP), nanofluid flooding reaches up to 10.2% of OOIP at optimal 0.1 wt.% ZnO (55.7 nm). Meanwhile, EM-assisted nanofluid flooding at 0.1 wt.% ZnO provides a maximum oil recovery of 10.39% and 13.08% of OOIP under EM frequency of 18.8 and 167 MHz, respectively. By assessing the IFT/contact angle and mobility ratio, the optimal NPs concentration to achieve a favorable ER effect and interfacial disturbance is determined, correlated to smaller hydrodynamic-sized nanoparticles that cause strong electrostatic repulsion between particles., Competing Interests: The authors have declared that no competing interests exist.

Recent developments propose renewed use of surface-modified nanoparticles (NPs) for enhanced oil recovery (EOR) due to improved stability and reduced porous media retention. The enhanced surface properties render the nanoparticles more suitable compared to bare nanoparticles, for increasing the displacement efficiency of waterflooding. However, the EOR mechanisms using NPs are still not well established. This work investigates the effect of in-situ surface-modified silica nanoparticles (SiO2 NPs) on interfacial tension (IFT) and wettability behavior as a prevailing oil recovery mechanism. For this purpose, the nanoparticles have been synthesized via a one-step sol-gel method using surface-modification agents, including Triton X-100 (non-ionic surfactant) and polyethylene glycol (polymer), and characterized using various techniques. These results exhibit the well-defined spherical particles, particularly in the presence of Triton X-100 (TX-100), with particle diameter between 13 to 27 nm. To this end, SiO2 nanofluids were formed by dispersing nanoparticles (0.05 wt.%, 0.075 wt.%, 0.1 wt.%, and 0.2 wt.%) in 3 wt.% NaCl to study the impact of surface functionalization on the stability of the nanoparticle suspension. The optimal stability conditions were obtained at 0.1 wt.% SiO2 NPs at a basic pH of 10 and 9.5 for TX-100/ SiO2 and PEG/SiO nanofluids, respectively. Finally, the surface-treated SiO2 nanoparticles were found to change the wettability of treated (oil-wet) surface into water-wet by altering the contact angle from 130° to 78° (in case of TX-100/SiO2) measured against glass surface representing carbonate reservoir rock. IFT results also reveal that the surfactant treatment greatly reduced the oil-water IFT by 30%, compared to other applied NPs. These experimental results suggest that the use of surface-modified SiO2 nanoparticles could facilitate the displacement efficiency by reducing IFT and altering the wettability of carbonate reservoir towards water-wet, which is attributed to more homogeneity and better dispersion of surface-treated silica NPs compared to bare-silica NPs., Competing Interests: The authors have declared that no competing interests exist.

Recently, nano-EOR has emerged as a new frontier for improved and enhanced oil recovery (IOR & EOR). Despite their benefits, the nanoparticles tend to agglomerate at reservoir conditions which cause their detachment from the oil/water interface, and are consequently retained rather than transported through a porous medium. Dielectric nanoparticles including ZnO have been proposed to be a good replacement for EOR due to their high melting point and thermal properties. But more importantly, these particles can be polarized under electromagnetic (EM) irradiation, which provides an innovative smart Nano-EOR process denoted as EM-Assisted Nano-EOR. In this study, parameters involved in the oil recovery mechanism under EM waves, such as reducing mobility ratio, lowering interfacial tensions (IFT) and altering wettability were investigated. Two-phase displacement experiments were performed in sandpacks under the water-wet condition at 95°C, with permeability in the range of 265-300 mD. A crude oil from Tapis oil field was employed; while ZnO nanofluids of two different particle sizes (55.7 and 117.1 nm) were prepared using 0.1 wt. % nanoparticles that dispersed into brine (3 wt. % NaCl) along with SDBS as a dispersant. In each flooding scheme, three injection sequential scenarios have been conducted: (i) brine flooding as a secondary process, (ii) surfactant/nano/EM-assisted nano flooding, and (iii) second brine flooding to flush nanoparticles. Compare with surfactant flooding (2% original oil in place/OOIP) as tertiary recovery, nano flooding almost reaches 8.5-10.2% of OOIP. On the other hand, EM-assisted nano flooding provides an incremental oil recovery of approximately 9-10.4% of OOIP. By evaluating the contact angle and interfacial tension, it was established that the degree of IFT reduction plays a governing role in the oil displacement mechanism via nano-EOR, compare to mobility ratio. These results reveal a promising way to employ water-based ZnO nanofluid for enhanced oil recovery purposes at a relatively high reservoir temperature.

Global gene expression was monitored in long-term stationary phase (LSP) cells of E. coli K12 MG1655 and compared with stationary phase (SP) cells that were sub-cultured without prolonged delay to get an insight into the survival strategies of LSP cells. The experiments were carried out using both LB medium and LB supplemented with 10% of glycerol. In both the media the LSP cells showed decreased growth rate compared to SP cells. DNA microarray analysis of LSP cells in both the media resulted in the up- and down-regulation of several genes in LSP cells compared to their respective SP cells in the corresponding media. In LSP cells grown in LB 204 genes whereas cells grown in LB plus glycerol 321 genes were differentially regulated compared to the SP cells. Comparison of these differentially regulated genes indicated that irrespective of the medium used for growth in LSP cells expression of 95 genes (22 genes up-regulated and 73 down-regulated) were differentially regulated. These 95 genes could be associated with LSP status of the cells and are likely to influence survival and growth characteristics of LSP cells. This is indeed so since the up- and down-regulated genes include genes that protect E. coli LSP cells from stationary phase stress and genes that would help to recover from stress when transferred into fresh medium. The growth phenotype in LSP cells could be attributed to up-regulation of genes coding for insertion sequences that confer beneficial effects during starvation, genes coding for putative transposases and simultaneous down-regulation of genes coding for ribosomal protein synthesis, transport-related genes, non-coding RNA genes and metabolic genes. As yet we still do not know the role of several unknown genes and genes coding for hypothetical proteins which are either up- or down-regulated in LSP cells compared to SP cells.

This study demonstrates the effects of simulated microgravity on E. coli K 12 MG1655 grown on LB medium supplemented with glycerol. Global gene expression analysis indicated that the expressions of hundred genes were significantly altered in simulated microgravity conditions compared to that of normal gravity conditions. Under these conditions genes coding for adaptation to stress are up regulated (sufE and ssrA) and simultaneously genes coding for membrane transporters (ompC, exbB, actP, mgtA, cysW and nikB) and carbohydrate catabolic processes (ldcC, ptsA, rhaD and rhaS) are down regulated. The enhanced growth in simulated gravity conditions may be because of the adequate supply of energy/reducing equivalents and up regulation of genes involved in DNA replication (srmB) and repression of the genes encoding for nucleoside metabolism (dfp, pyrD and spoT). In addition, E. coli cultured in LB medium supplemented with glycerol (so as to protect the cells from freezing temperatures) do not exhibit multiple stress responses that are normally observed when cells are exposed to microgravity in LB medium without glycerol.

The present study was focused on evaluating the potential of Emblica officinalis against cariogenic properties of Streptococcus mutans, a causative microorganism for caries. The effect of crude extract and ethanolic fraction from Emblica officinalis fruit was analysed against S. mutans. The sub-MIC concentrations of crude and ethanolic fraction of E. officinalis were evaluated for its cariogenic properties such as acid production, biofilm formation, cell-surface hydrophobicity, glucan production, sucrose-dependent and independent adherence. Its effect on biofilm architecture was also investigated with the help of confocal and scanning electron microscopy (SEM). Moreover, expression of genes involved in biofilm formation was also studied by quantitative RT- PCR. This study showed 50% reduction in adherence at concentrations 156 µg/ and 312.5 µg/ml of crude extract and ethanolic fraction respectively. However, the biofilm was reduced to 50% in the presence of crude extract (39.04 µg/ml) and ethanolic fraction (78.08 µg/ml). Furthermore, effective reduction was observed in the glucan synthesis and cell surface hydrophobicity. The qRT-PCR revealed significant suppression of the genes involved in its virulence. Confocal and scanning electron microscopy clearly depicted the obliteration of biofilm structure with reference to control. Hence, this study reveals the potential of E. officinalis fruit extracts as an alternative and complementary medicine for dental caries by inhibiting the virulence factors of Streptococcus mutans.
