International audience; The aim of our study was to compare the performance of three different instruments measuring clinical and cognitive dimensions of insight. Data on 182 outpatients with schizophrenia and one-year follow-up assessments was drawn from the FACE-SZ cohort. Awareness of clinical state (« clinical insight ») was measured using both a clinician-rated measure (the Scale to assess Unawareness of Mental Disorder (SUMD)), and a self-report measure (the Birchwood Insight Scale (BIS). Cognitive insight was measured using a self-report measure (the Beck Cognitive Insight Scale (BCIS)). For each scale, change in insight was examined at the follow-up. Correlations between SUMD and BIS subscales measuring same dimensions were significant. BIS-BCIS correlations were weak for all combinations except between BIS illness dimension and BCIS composite score. At the follow-up, BIS and SUMD awareness of treatment need improved whereas illness and symptom awareness increased only on the SUMD. Conversely, cognitive insight composite scores decreased. Despite relatively good overall agreement between the two clinical insight instruments, considerable variability for similar insight dimensions measured by different instruments was found. Agreement between cognitive and clinical insight is moderate. Our study strengthens the argument that insight is harder to operationalize than other symptoms and may explain why it is so seldom explicitly targeted in schizophrenia treatment.

International audience; Poor insight is found in up to 80% of schizophrenia patients and has been associated with multiple factors of which cognitive functioning, social and environmental factors. Few studies have explored associations between patient insight and that of their biological parents', and the influence of parental factors. Insight was assessed in 41 patients and their biological parents with Amador's Scale for the assessment of Unawareness of Mental Disorder (SUMD). Parents' knowledge about schizophrenia and critical attitudes were assessed with validated self-report questionnaires. Both groups underwent cognitive assessments for working memory and executive functioning. Insight in patients and their parents was not associated for any of the SUMD dimensions but a significant correlation was found between patient and parent awareness of treatment effect for patient-parent dyads with frequent daily contact. Low parental critical attitude was associated with higher patient awareness of symptoms and a high parental memory task score with high patient insight. Our study is the first to suggest a possible influence of parental factors such as critical attitudes and cognitive performance on patient insight.

International audience; This study explored the mediation effect of metacognitive beliefs on the relationship between intrusive thoughts and emotional distress in schizophrenia (N=49) and obsessive-compulsive disorder (OCD) (N=35). Intrusive thoughts impact on anxiety and depression through beliefs about uncontrollability and danger of thoughts in schizophrenia. Negative beliefs in general mediated the effect of intrusive thoughts on anxiety in obsessive-compulsive disorder. The results suggest that metacognitive beliefs may be a vulnerability factor for emotional and psychological disorder.

International audience; Expressive behaviour plays a crucial role in the success of social interactions. Abnormality of expressive behaviour has been reported in interpersonal interactions of patients suffering from schizophrenia and social phobia, two debilitating mental disorders with important social deficits. However, no study has compared the expressive behaviour in these two disorders. Thirty schizophrenia patients, 21 social phobia patients and 30 healthy controls were evaluated and compared on expressive, cognitive and clinical dimensions. Expressive behaviour was assessed using the Motor Affective subscale of the Motor-Affective-Social-Scale (MASS). Covariables include the Positive and Negative Syndrome Scale (PANSS), the anxiety level Liebowitz-Social-Anxiety-Scale (LSAS) and cognitive tasks. After controlling for depression, schizophrenia and social phobia patients both exhibited significantly fewer expressive behaviours compared to healthy controls. Moreover, our results showed specific signatures: schizophrenia patients performed fewer spontaneous gestures (hand gestures and smiles) whereas social phobia patients had an impaired ability to produce voluntary smiles in comparison to healthy controls. Interestingly, poor social functioning was significantly correlated with a decrease of expressive behaviour for schizophrenia patients. Expressive behaviour is impaired in different ways in social phobia and schizophrenia and is associated in schizophrenia with poorer social functioning. The Motor Affective subscale of the MASS is an interesting tool for assessing the dysfunction of interpersonal expressive behaviour in mental disorders.

International audience; We investigated the links among decision-making assessed by the Iowa Gambling Task (IGT) and the Game of Dice Task (GDT), and the four facets of impulsivity (urgency, lack of premeditation, lack of perseverance, and sensation seeking, UPPS) proposed by Whiteside and Lynam (2001) in a sample of 107 healthy volunteers. Hierarchical regressions controlling for age and gender indicated that sensation seeking and urgency were linked to disadvantageous decisions on the GDT while no association was found between IGT performance and the UPPS. Sensation seeking and urgency facets of impulsivity are related in healthy individuals, to decision-making processes where potential consequences of different options and their subsequent probabilities rely on explicit information. In healthy controls, there is little overlap between decision-making influenced by both implicit and explicit information and impulsivity as measured by the UPPS. These findings add evidence to the notion that self-reported trait impulsivity is associated with the decision making process. Decisions made under risk seemed to be differentially associated with specific facets of impulsivity.

In this study, we examined the association between insight and decision-making capacity in schizophrenia using the Iowa Gambling Task (IGT). No association was found between insight and IGT scores. Our results suggest that impaired decision-making ability in schizophrenia patients cannot be solely predicted by lack of insight.

Past research suggests that unawareness of illness in schizophrenia is associated with deficits in executive functions; however, the relationships between executive processes and the various dimensions of insight are still unclear. Recent models of executive functioning have proposed that four executive processes – inhibition, updating, shifting and dual task coordination – are moderately related yet separable. In this study, we proposed to investigate and clarify the relationships between insight dimensions and the aforementioned four executive components. A total of 60 patients were administered the Test for Attentional Performance and the Scale to Assess Unawareness of Mental Disorder. The effect of potential confounding variables such as medication, symptomatology, demography, psycho-affective state, and general processing speed were also examined in a preliminary statistical analysis. We found that both awareness of disorder and awareness of response to medication were significantly related to Updating. Awareness of the social consequences of the disease was significantly related to Updating, Divided Attention and Inhibition Processes. The analysis indicates that poor insight in schizophrenia may be partially related to executive dysfunction. Finally, our study emphasizes the possible role of neuropsychological intervention in improving patients' insight into illness.
