In the event of a fission-based weapon or improvised nuclear device (IND) detonation, dose coefficients can be harnessed to provide dose assessments for defense, emergency preparedness, and consequence management, as well as to prospectively inform the assessment of radiation biomarkers and development of medical prophylaxis countermeasures for defense and homeland security stakeholders and decision-makers. Although dose coefficients have previously been calculated for this group, they would apply specifically to the studied population, the 1945 Japanese cohort, after which their anthropomorphic computational phantoms were modeled. For this reason, applications to other populations may be limited, and instead, an assessment of a more standardized population is desired. We employed a series of computational human phantoms representing international reference individuals: UF/NCI voxel phantom series containing newborn, 1-, 5-, 10-, 15-, and 35-year-old males and females. Irradiation of the phantoms was simulated using the Monte Carlo N-Particle transport code to determine organ dose coefficients under four idealized irradiation geometries at three distances from the detonation hypocenter at Hiroshima and Nagasaki using DS02 free-in-air prompt neutron and photon fluence spectra. Through these simulations, age-specific dose coefficients were determined for individual organs. Various articulated PIMAL stylized phantoms were simulated as well to estimate the effect of body posture on dose coefficients and determine the effect of posture on dosimetric estimation and reconstruction. Results additionally demonstrate that 137Cs and the Watt fission spectra are not ideal general surrogate sources for fission weapons, which may be considered for experimental testing of medical countermeasures. Supplementary data provided tabulates the compilation of organ dose-rate coefficients in this study.

International audience; Within the European Epidemiological Study to Quantify Risks for Paediatric Computerized Tomography (EPI-CT study), a cohort was assembled comprising nearly one million children, adolescents and young adults who received over 1.4 million computed tomography (CT) examinations before 22 years of age in nine European countries from the late 1970s to 2014. Here we describe the methods used for, and the results of, organ dose estimations from CT scanning for the EPI-CT cohort members. Data on CT machine settings were obtained from national surveys, questionnaire data, and the Digital Imaging and Communications in Medicine (DICOM) headers of 437,249 individual CT scans. Exposure characteristics were reconstructed for patients within specific age groups who received scans of the same body region, based on categories of machines with common technology used over the time period in each of the 276 participating hospitals. A carefully designed method for assessing uncertainty combined with the National Cancer Institute Dosimetry System for CT (NCICT, a CT organ dose calculator), was employed to estimate absorbed dose to individual organs for each CT scan received. The two-dimensional Monte Carlo sampling method, which maintains a separation of shared and unshared error, allowed us to characterize uncertainty both on individual doses as well as for the entire cohort dose distribution. Provided here are summaries of estimated doses from CT imaging per scan and per examination, as well as the overall distribution of estimated doses in the cohort. Doses are provided for five selected tissues (active bone marrow, brain, eye lens, thyroid and female breasts), by body region (i.e., head, chest, abdomen/pelvis), patient age, and time period (1977-1990, 1991-2000, 2001-2014). Relatively high doses were received by the brain from head CTs in the early 1990s, with individual mean doses (mean of 200 simulated values) of up to 66 mGy per scan. Optimization strategies implemented since the late 1990s have resulted in an overall decrease in doses over time, especially at young ages. In chest CTs, active bone marrow doses dropped from over 15 mGy prior to 1991 to approximately 5 mGy per scan after 2001. Our findings illustrate patterns of age-specific doses and their temporal changes, and provide suitable dose estimates for radiation-induced risk estimation in epidemiological studies.

Epidemiological investigation is an important approach to assessing the risk of late effects after radiotherapy, and organ dosimetry is a crucial part of such analysis. Computed tomography (CT) images, if available, can be a valuable resource for individualizing the dosimetry, because they describe the specific anatomy of the patient. However, CT images acquired for radiation treatment planning purposes cover only a portion of the body near the target volume, whereas for epidemiology, the interest lies in the more distant normal tissues, which may be located outside the scan range. To address this challenge, we developed a novel method, called the Anatomically Predictive Extension (APE), to extend a partial-body CT image stack using images of a computational human phantom matched to the patient based on their height and weight. To test our method, we created five APE phantoms from chest and abdominal images extracted from the chest-abdomen-pelvis (CAP) CT scans of five patients. Organ doses were calculated for simple chest and prostate irradiations that were planned on the reference computational phantom (assumed patient geometry if no CT images are available), APE phantoms (patient-phantom hybrid given a partial-body patient CT) and full patient CAP CT scans (ground truth). The APE phantoms and patient CAP CT scans resulted in nearly identical dosimetry for those organs that were fully included in the partial-body CT used to construct the APE. The calculated doses to these same organs in the reference phantoms differed by up to 20% and 52% for the chest and prostate cases, respectively. For organs outside the scan coverage, the reference phantom showed, on average, dose differences of 31% (chest case) and 41% (prostate case). For the APE phantoms, these values were 26% (chest) and 17% (prostate). The APE method combines patient and phantom images to improve organ dosimetry both inside and outside the scan range. We intend to use the APE method for estimating dose for organs peripheral to the treatment fields; however, this method is quite generalizable with many potential applications.

Owing to recent advances in computational dosimetry tools, an update is warranted for the dosimetry system for atomic bomb survivors that was established by the Joint U.S.Japan Working Group on the Reassessment of Atomic Bomb Dosimetry in 2002 (DS02). The DS02 system, and its predecessor, DS86, at the Radiation Effects Research Foundation (RERF), are based on adjoint Monte Carlo particle transport simulations coupled with stylized computational human phantoms. In our previous studies, we developed the J45 series of computational voxel phantoms representative of 1945 Japanese adults, children and pregnant females. The dosimetric impact of replacing the DS02/DS86 stylized phantoms by the J45 phantom series was also discussed through computation of organ doses for several idealized exposure scenarios. In the current study, we investigated the possible impact of introducing not only the J45 phantom series but also various methodological upgrades to the DS02 dosimetry system. For this purpose, we calculated organ doses in adults for 12 representative exposure scenarios having realistic particle energy and angular fluence, using different combinations of phantoms and dose calculation methods. Those doses were compared with survivor organ doses given by the DS02 system. It was found that the anatomical improvement in the J45 phantom series is the most important factor leading to potential changes in survivor organ doses. However, methodological upgrades, such as replacement of the adjoint Monte Carlo simulation with kerma approximation by the forward Monte Carlo simulation with secondary electron transport, can also improve the accuracy of organ doses by up to several percent.In addition, this study established a series of response functions, which allows for the rapid conversion of the unidirectional quasi-monoenergetic photon and neutron fluences from the existing DS02 system to organ doses within the J45 adult phantoms. The overall impact of introducing the response functions in the dosimetry system is not so significant, less than 10% in most cases, except for organs in which the calculation method or definition was changed, e.g., colon and bone marrow. This system of response functions can be implemented within a revision to the DS02 dosimetry system and used for future updates to organ doses within the Life Span Study of the atomic bomb survivors.

One of the largest sources of data on radiation exposure in humans is the study of the atomic bomb survivors at Hiroshima and Nagasaki, Japan performed by the Radiation Effects Research Foundation (RERF). As part of their retrospective dosimetry efforts for the atomic bomb survivors, RERF published two core systems: Dosimetry System 1986 (DS86) and Dosimetry System 2002 (DS02). Due to computing limitations at the time, only three stylized phantoms (an infant, child and adult) were used in DS86 and DS02 to represent the entire Japanese population. In this study, we sought to evaluate the dosimetric differences that should be expected from using an updated and age-expanded phantom series with the survivor cohort. To this end, we developed a new series of hybrid phantoms, based on the Japanese population of 1945, which has greater anatomical realism and improved age resolution over those used by RERF. These phantoms were converted to voxel format and compared to their older counterparts through the calculation of organ dose coefficients using DS02 free-in-air particle fluences at three distances from the bomb hypocenter. From the photon portion of the spectra, organ dose differences of up to nearly 25% are expected between the old and new series, while organ dose differences of up to nearly 70% are expected from the neutron portion. We also compared organ dose coefficients among themselves to determine the accuracy in the use of one organ dose as the epidemiological surrogate to another. Certain organ-surrogate pairs were shown to be inappropriate, such as the use of colon dose for breast risk analyses. Overall, our new series of phantoms provides significant improvements to survivor organ dosimetry, especially to those survivors who were previously misrepresented in body size by their stylized phantom and to those who experienced a highly-directional irradiation field.
