Acute lobar nephronia (ALN) refers to a renal mass caused by acute focal infection without liquefaction. The radiological findings in 12 patients with 13 episodes of ALN are described. A characteristic combination of uroradiological findings is (a) a relatively sonolucent mass which disrupts corticomedullary definition on ultrasonography; (b) a solid-appearing mass on other uroradiological studies; and (c) a positive gallium image in the region of the mass, which may be associated with increased activity elsewhere in the same or opposite kidney. The angiographic finding of significant venous narrowing within the mass associated with only minor arteriographic abnormalities is characteristic of ALN as well.

Segmental renal vein sampling was performed in 25 hypertensive patients. In those with no arteriographic abnormalities or stenosis of only the main renal artery, renin levels from segmental veins were the same as those from the ipsilateral main renal vein. In patients with an infarction of a renal segment or stenosis of a segmental renal artery, the renin level from the corresponding segmental vein was often much higher than that from the main renal vein. Segmental vein sampling may reveal a significant source of renin production not apparent in samples from the main renal vein.

Duodenal tumors are rare but duodenal inflammatory disease occurs frequently. In most cases it can be accurately diagnosed without angiography but in the occasional case that presents a diagnostic problem, angiography is required. Seven cases of duodenal inflammatory disease are reviewed. Careful attention to the shape and direction of vascular displacement and to separate evaluation of pancreatic and duodenal arteries accurately distinguished duodenal from pancreatic disease. This distinction can significantly influence subsequent therapy.

Duodenal varices were angiographically demonstrated in 18 cases of extrahepatic obstruction of the portal vein or obstruction of the splenic vein. In 11 cases, barium examination of the upper gastrointestinal tract revealed prominent filling defects in the duodenal bulb and the duodenal sweep. These abnormalities share common characteristics and differ in their appearance from more commonly diagnosed duodenal lesions; their demonstration should raise the suspicion of extrahepatic portal or splenic vein obstruction, which may be verified by angiography.
