Air embolism is often an iatrogenic complication that may occur in venous or arterial circulation depending on the port of entry. We present a case of a 40-year-old female who had a venous air embolism in the pulmonary artery as a consequence of the injection of a contrast agent. She experienced dyspnea and chest pain following a contrast-enhanced chest computed tomography imaging. She was successfully treated and discharged from our hospital. Congenital portosystemic shunt (CPS) is a rare vascular anomaly resulting in diversion of splanchnic or portal blood into the systemic circulation. Other vascular malformations associated with this entity are uncommon. A 4-year-old female child with a diagnosis of acute viral hepatitis had an incidental finding of extrahepatic CPS on a Doppler abdominal ultrasound. Contrast-enhanced computed tomography showed a dilated portal vein having H-type side-to-side communication with a hypoplastic intrahepatic portion of the inferior vena cava and a prominent dilated azygos vein. There was retroaortic left renal vein drained into the IVC which was visualized in its entirety. Echocardiography findings were normal and the patient was discharged after symptomatic treatment that achieved improvement. With the expansion of abdominal imaging, incidental cases of CPS are increasingly being diagnosed in children. We report a case of a 61-year-old woman who presented to the emergency department with high inflammatory parameters and acute renal failure. Marked enlarged kidneys with persistent nephrogram were found on contrast-enhanced computed tomography . Renal biopsy showed acute interstitial nephritis. We report the case of a 71-year-old woman with a skin ulcer derived from an abscess around the tibia. The abscess resulted in periprosthetic joint infection and osteomyelitis 11 years after total knee arthroplasty. The first symptom was a skin ulcer of the lower leg. Magnetic resonance imaging revealed a circumferential mass around the proximal tibia. A skin biopsy taken around the ulcer showed thrombosis and degenerated collagen. Contrast-enhanced computed tomography showed a circumferential mass around the proximal tibia with ring enhancement. Biopsies of the skin ulcer and circumferential mass showed an abscess caused by Enterococcus faecium and methicillin-resistant Staphylococcus epidermis . We conducted debridement of the abscess, a gastrocnemius flap and split-thickness skin grafting and a 2-stage revision of the total knee component with a hinged prosthesis. Two years later, the infection did not reoccur and the patient can walk without a cane. This case is unique as abscess around proximal tibia caused necrotic skin ulcer and appearance of abscess was fibrous and different from typical bacterial abscesses containing pus or fluid. Ureteral herniation into the scrotum is rare and often associated with congenital abnormalities or postoperative anatomic changes. A high index of suspicion is needed to avoid intraoperative ureteric injuries. A 50-year-old man with ureteric herniation into an inguinoscrotal hernia reported acute left flank pain and had a history of recurrent urinary tract infections. Contrast-enhanced computed tomographic evaluation revealed a left ureter extending from the pelvis into the scrotum through the inguinal canal. Screening ultrasound of the scrotum revealed the intrascrotal extension of the ureter along with the other hernial contents and diagnosed as a Paraperitoneal inguinal herniation of the ureter. Various cross-sectional imaging modalities, including Ultrasound, provide good anatomic details that can diagnose the presence of a herniated ureter., (© 2021 The Authors. Severe anaphylactic reactions to an intravenous nonionic iodine contrast medium (NICM) are uncommon but can result in permanent morbidity or death if not managed appropriately. An anaphylactic reaction to an NICM typically manifests as clinical symptoms that include an itchy nose, sneezing, and skin redness. To our knowledge, a rapid change in the caliber of the inferior vena cava (IVC) during multiphasic contrast-enhanced computed tomography (CT) has not been reported. Here, we report the computed tomographic findings in three cases of hypovolemic shock caused by an anaphylactic reaction to an NICM. We suspect that a decrease in caliber of the IVC during multiphasic contrast-enhanced CT may be a predictor of an allergic-like reaction to an NICM. Patients in whom physicians and radiographers detect a rapid caliber change in the IVC during multiphasic contrast-enhanced CT should be managed carefully.
