This paper highlights the important leadership role of the public health sector, working with other governmental sectors and nongovernmental entities, to advance environmental public health in Latin America and the Caribbean toward the achievement of 2030 Sustainable Development Goal 3: Health and Well-Being. The most pressing current and future environmental public health threats are discussed, followed by a brief review of major historical and current international and regional efforts to address these concerns. The paper concludes with a discussion of three major components of a regional environmental public health agenda that responsible parties can undertake to make significant progress toward ensuring the health and well-being of all people throughout Latin America and the Caribbean.En este artículo se destaca el importante papel de liderazgo que tiene el sector de la salud pública, en colaboración con otros sectores gubernamentales y entidades no gubernamentales, en el fomento de la salud pública medioambiental en América Latina y el Caribe con el fin de alcanzar para el 2030 el Objetivo de Desarrollo Sostenible número 3: salud y bienestar. Se abordan las amenazas de salud pública medioambiental actuales y futuras más urgentes, y se presenta una breve revisión de las principales medidas históricas y actuales adoptadas a nivel regional e internacional para abordar estas cuestiones. El artículo finaliza con un análisis de los tres principales componentes de un programa regional de salud pública ambiental que las partes responsables pueden ejecutar para realizar progresos significativos en la garantía de la salud y el bienestar de todos en América Latina y el Caribe.Este artigo destaca o importante papel de liderança do setor de saúde pública, trabalhando com outros setores governamentais e entidades não governamentais, para promover a saúde pública ambiental na América Latina e no Caribe a fim de atingir o Objetivo de Desenvolvimento Sustentável 3 para 2030: Saúde e Bem-Estar. São discutidas as ameaças ambientais mais urgentes para a saúde pública, atuais e futuras, seguidas por uma breve revisão dos principais esforços históricos e atuais, internacionais e regionais, para abordar essas preocupações. O artigo conclui com uma discussão de três componentes essenciais de uma agenda regional de saúde pública ambiental que as partes responsáveis podem adotar para avançar significativamente no sentido de garantir a saúde e o bem-estar de todas as pessoas na América Latina e no Caribe.

Due to its impact on human health and its capacity to cause damage, disasters are one of the global problems that most concern the international community. However, and in spite of the efforts made by many health systems, the reduction of vulnerabilities and the lack of strategies to avoid or minimize risks have not received enough attention yet. As Cuba is located in an area of permanent danger of disasters, its health sector has accumulated considerable experience in the management of risks and in the assurance of less risky future scenarios, with active participation of the community and a planned and organized process to address the impact of climate change. This paper summarizes some of these experiences and describes the dangers, vulnerabilities, and risks of disasters in Cuba, its Civil Defense System, and planning and organization in the health sector for disaster reduction, as well as the main effects and challenges of climate change in the health system. Lessons learned and good practices support the key role of human resources to reduce vulnerabilities; the greatest challenges are to avoid or minimize risks, to advance research, to train professionals for climate change, and to optimize the organization of health systems and services in the country.Devido ao seu impacto na saúde humana e na sua capacidade de causar danos, os desastres são um dos problemas globais mais preocupantes para a comunidade internacional. No entanto, e apesar dos esforços feitos por numerosos sistemas de saúde, a atenção necessária ainda não foi oferecida para a redução de vulnerabilidades e a falta de estratégias que evitem ou minimizem os riscos. Como Cuba está localizada em uma área de permanente perigo de desastres, seu setor de saúde acumulou uma experiência considerável no gerenciamento de riscos e na garantia de cenários futuros menos arriscados, com a participação ativa da comunidade e um processo planejado e organizado para enfrentar o impacto das mudanças climáticas. Este artigo resume algumas dessas experiências e descreve os perigos, vulnerabilidades e riscos de desastres em Cuba, seu Sistema de Defesa Civil e planejamento e organização no setor da saúde para a redução de desastres, bem como os principais efeitos e desafios das mudanças climáticas no sistema de saúde. As lições aprendidas e as boas práticas ratificam o papel determinante dos recursos humanos para reduzir vulnerabilidades; o maior desafio é evitar ou minimizar riscos, avançar pesquisa, preparar profissionais para mudanças climáticas e otimizar a organização de sistemas e serviços de saúde no país.

This paper opens a discussion on the main features of an investment case for obesity prevention and control, by scanning available evidence on existing approaches and by highlighting contextual considerations and evidence for Latin America and the Caribbean. We call on researchers and analysts in the field to update and broaden existing methods of economic analyses to better reflect the multisectoral nature of an investment case for obesity prevention and control. We also identify research gaps and further work required to advance methods and evidence towards investment cases throughout the Americas.Este artículo abre un debate sobre las principales características de los argumentos a favor de la inversión para prevenir y controlar la obesidad, al buscar evidencia sobre los métodos existentes y destacar las consideraciones relativas al contexto y la evidencia para América Latina y el Caribe. Instamos a los investigadores y analistas en el campo a que actualicen y amplíen los métodos existentes de análisis económico a fin de reflejar mejor la naturaleza multisectorial de los argumentos a favor de la inversión para la prevención y el control de la obesidad. También encontramos lagunas en la investigación y el trabajo adicional que se requiere para impulsar los métodos y la evidencia que respalden estos argumentos a favor de la inversión en toda América.Esta análise visa trazer à discussão as principais características de um caso de investimento para prevenção e controle da obesidade ao examinar comprovações sobre os enfoques existentes e destacar considerações contextuais e evidências para a América Latina e o Caribe. Fazemos um chamado aos pesquisadores e analistas no campo a atualizar e expandir a metodologia atual de análise econômica a fim de melhor refletir o caráter multissetorial de um caso de investimento para prevenção e controle da obesidade. Também identificamos lacunas de pesquisa e a necessidade de trabalhar mais para melhorar a metodologia e as evidências de casos de investimento nas Américas.

In the last 10 years, implementation research has come to play a critical role in improving the implementation of already-proven health interventions by promoting the systematic uptake of research findings and other evidence-based strategies into routine practice. The Alliance for Health Policy and Systems Research and the Pan American Health Organization implemented a program of embedded implementation research to support health programs in Latin America and the Caribbean (LAC) in 2014–2015. A total of 234 applications were received from 28 countries in the Americas. The Improving Program Implementation through Embedded Research (iPIER) scheme supported 12 implementation research projects led by health program implementers from nine LAC countries: Argentina, Bolivia, Brazil, Chile, Colombia, Mexico, Panama, Peru, and Saint Lucia. Through this experience, we learned that the “insider” perspective, which implementers bring to the research proposal, is particularly important in identifying research questions that focus on the systems failures that often manifest in barriers to implementation. This paper documents the experience of and highlights key conclusions about the conduct of embedded implementation research. The iPIER experience has shown great promise for embedded research models that place implementers at the helm of implementation research initiatives.
