Retrospective descriptive study carried out to determine the characteristics of fungemia in 285 cancer patients hospitalized from 2012 to 2016 at the Instituto Nacional de Enfermedades Neoplásicas (INEN). Demographic, clinical and microbiological information was evaluated. Fungemia by C. albicans predominated in patients with solid tumors and without neutropenia, while those caused by C. tropicalis predominated in patients with hematological neoplasia and neutropenia. C. tropicalis was the agent isolated in most cases (47.0%). Fungemia increased over time in patients without neutropenia. Fungemia caused by C. albicans increases with age in patients with solid tumors without neutropenia. It is concluded that fungemia are mainly caused by C. tropicalis in patients with hematological neoplasia with neutropenia and by C. albicans in patients with solid tumors without neutropenia. In addition, fungemia in patients without neutropenia increases over time; and those caused by C. albicans increase with age in patients with solid tumors without neutropenia.

Con el objetivo de determinar las características clínicas y geriátricas en pacientes adultos mayores con neumonía, en un hospital de Buenos Aires en Argentina, se realizó un estudio transversal en el 2010, con 66 pacientes hospitalizados por neumonía. La edad media fue de 82 años, siendo el 54,5% hombres. El 47,0% de los ingresos fueron por neumonía adquirida en la comunidad. En cuanto a la valoración funcional el 31,8% eran dependientes para las actividades básicas de la vida diaria y 69,4% dependientes para actividades instrumentales de la vida diaria. Las características clínicas más frecuentes fueron fiebre, tos, disnea y expectoración. Se identificó el agente causal de la neumonía en el 34,8% de pacientes, siendo 18 bacterianas (27,2%) y cinco virales (7,5%). La mortalidad cruda fue del 28,8%, pero cercana al 50% en los que tenían síndrome confusional. Se concluye que la neumonía en adultos mayores tiene características especiales que se hacen visibles con una correcta valoración diagnóstica y geriátrica.

Se realizó un estudio retrospectivo y descriptivo en 210 pacientes hospitalizados con lesión medular del Instituto Nacional de Rehabilitación (INR), Callao-Perú (2000-2006) para conocer la etiología, características clínicas y socio-demográficas de los pacientes con lesiones medulares no traumáticas (LMNT). Se encontró una prevalencia de 27 % para LMNT siendo la edad promedio al inicio de la lesión 32,0 años; siendo varones el 50,5 %, y el 41,9 % tuvieron educación secundaría, el nivel de pobreza alcanzó el 90,5 %. Predominó la etiología infecciosa (viral y bacteriana) en 37,6 %, con un 11,9 % de infección por HTLVI. No obstante ser el INR un centro de referencia, nuestros resultados son preliminares, siendo necesario realizar mayores estudios para proponer estrategias de prevención y control, dado el alto costo del tratamiento integral de rehabilitación en estos pacientes.We performed a retrospective and descriptive cross-sectional; study in 210 hospitalized patients with spinal cord injury at the National Institute of Rehabilitation (INR), Callao, Peru from 2000-2006. The goal was to describe etiology, and clinical and socio-demographic characteristics of non traumatic spinal cord injuries (LMNT). We found a prevalence of 27 % for LMNT, average age at onset of 32.0 years, male gender 50.5 %, and secondary education completed in 41.9 %, poverty 90.5 %. The infectious etiology (viral and bacterial) was predominant in 37.6 %, with 11.9 infected with HTLVI. Although the INR is a reference center, the findings can’t be generalized because it isn’t a representative sample of the Peruvian population, further studies are necessary to propose strategies for prevention and control, considering the high cost of integral rehabilitation treatment in these patients.

Cross-sectional study that included patients over 18 years of age admitted to the emergency room in a national hospital, with AMS, within twenty-four hours of onset and that required observation. The demographic, clinical and functional assessment information were taken from the medical record. A follow-up was made at three months by telephone. Results. The study included 290 patients, representing 4.1% of the total patients in the emergency room. Overall mortality was 24.2%, whereas in the subgroup of older adults was 28.1%; 19.3% in adults and 15.2% in young adults. The average age was 63.5 years and the elderly group constituted nearly two thirds of the population. 51% were males. The average hospital stay was 7 days. The most frequently associated causes were infectious diseases (44.1%), neurological (40.3%), respiratory (28.3%), metabolic (18.6%), and cardiovascular (17.2%) disorders. Mortality was associated with a) GCS levels 7 (OR 2.9, p0.001) and b) functionality score of 5 on the modified Rankin scale (OR 8.2, p=0.03). Conclusions. AMS, present in 4% of emergency admissions, is associated with mortality in one in four people. The predominant cause infectious diseases. The commitment of the functionality was the main predictor of mortality.

In order to determine the clinical and geriatric characteristics of elderly adults with pneumonia in a hospital in Buenos Aires, Argentina, a cross-sectional study was carried out in 2010, included 66 patients hospitalized for pneumonia. The mean age of the study subjects was 82, 54.5% of whom were men. 47.0% of the hospitalizations were due to community-acquired pneumonia. As for the functional evaluation, 31.8% showed dependence in performing basic activities of daily living and 69.4% presented dependence in instrumental activities of daily living. The most frequent clinical characteristics observed were fever, cough, dyspnea and expectoration. The causative agent of pneumonia was identified in 34.8% of the cases, 18 of which were bacterial (27.2%) and 5 viral (7.5%). The crude mortality rate was 28.8%, but was close to 50% in those patients having confusional syndrome. We conclude that pneumonia in elderly patients has special characteristics, which can be observed with a proper diagnostic and geriatric evaluation. [ABSTRACT FROM AUTHOR]
