STUDY DESIGN: A porcine cadaveric biomechanical study.OBJECTIVE: To biomechanically evaluate a novel Cable Anchor System as semi-rigid junctional fixation technique for the prevention of proximal junctional failure after adult spinal deformity surgery and to make a comparison to alternative promising prophylactic techniques.SUMMARY OF BACKGROUND DATA: The abrupt change of stiffness at the proximal end of a pedicle screw construct is a major risk factor for the development of proximal junctional failure after adult spinal deformity surgery. A number of techniques that aim to provide a gradual transition zone in range of motion (ROM) at the proximal junction have previously been studied. In this study, the design of a novel Cable Anchor System, which comprises a polyethylene cable for rod fixation, is assessed.METHODS: Ten T6-T13 porcine spine segments were subjected to cyclic 4 Nm pure-moment loading. The following conditions were tested: uninstrumented, 3 level pedicle screw fixation (PSF), and PSF with supplementary Cable Anchors applied proximally at 1-level (Anchor1) or 2-levels (Anchor2), transverse process hooks (TPH), and 2-level sublaminar tapes (Tape2). The normalized segmental range of motion in the junctional zone was compared using one-way analysis of variance and linear regression.RESULTS: Statistical comparison at the level proximal to PSF showed significantly lower ROMs for all techniques compared to PSF fixation alone in all movement directions. Linear regression demonstrated a higher linearity for Anchor1 (0.820) and Anchor2 (0.923) in the junctional zone in comparison to PSF (1-level: 0.529 and 2-level: 0.421). This linearity was similar to the compared techniques (TPH and Tape2).CONCLUSION: The Cable Anchor System presented in this study demonstrated a gradual ROM transition zone at the proximal end of a rigid pedicle screw construct similar to TPH and 2-level sublaminar tape semi-rigid junctional fixation constructs, while providing the benefit of preserving the posterior ligament complex.Level of Evidence: 5.

Study design A Biomechanical in vitro Investigation. Objective To evaluate the function and stability of self-designed biomimetic artificial atlanto-odontoid joint(BAAOJ) replacement on the atlantoaxial joint. Summary of background data Upper cervical fusion surgery is a common treatment for various atlantoaxial disorders, and favorable clinical outcome has been achieved. However, the fusion surgery results in loss of atlantoaxial motion as well as adjacent segments degeneration, reducing the quality of life of patients and might produce severe neurological symptoms. Non-fusion technology is expected to solve the above problems, but various designed devices have certain defects and are still in the exploratory phase. Methods Biomechanical tests were conducted on 10 fresh human cadaveric craniocervical specimens in the following sequence:(1) intact condition, (2) after the BAAOJ arthroplasty, (3) after BAAOJ fatigue test, (4) after odontoidectomy, and (5) after anterior rigid plate fixation. Three-dimensional movements of the C1-C2 segment were evaluated to investigate the function and stability of BAAOJ arthroplasty compared with the intact condition after the BAAOJ fatigue test, odontoidectomy, and rigid plate fixation. Results Comparing the BAAOJ implantation to the intact state, the ROM and neutral zone (NZ) were slightly reduced in all directions (P>0.05). Compared with the rigid plate fixation, the BAAOJ implantation significantly increased the ROM and NZ in all directions, especially in the axial rotation (P Conclusion We designed a BAAOJ for correcting atlantoaxial disorders arising from atlantoaxial instability. As a non-fusion device, the most critical feature of BAAOJ replacement is the retention of flexion-extension, lateral bending, and axial rotation range of motion similar to the normal state. It can also stabilize the atlantoaxial complex, and the BAAOJ itself has a good initial stability.Level of Evidence: 4.

Study design A randomized controlled trial. Objective To evaluate the effects of inelastic and elastic lumbosacral orthoses (LSOs) on the prevention of low back pain (LBP) in healthy nurses. Summary of background data Over 80% of people experience LBP during their lifetime. LSOs, as a device used to maintain lumbar stability, were widely used in LBP management. The effects of inelastic LSOs (iLSOs) and elastic LSOs (eLSOs) on the prevention of LBP are controversial. Methods A randomized clinical trial recruiting healthy nurses was conducted from November 2011 to June 2013 at a tertiary hospital in China. A total of 300 eligible participants aged 20 to 25 years were randomly assigned to iLSO, eLSO, or control groups. The intervention period was six months, and follow-ups were continued for an additional six months. Participants in both iLSO and eLSO groups were required to wear LSOs daily. Outcomes included the incidence of LBP, changes in trunk muscle endurance, and spinal range of motion (ROM) assessed at baseline, 6 months, and 12 months from the starting date. The incidence of LBP among groups was analyzed by ANOVA. Wilcoxon rank-sum test, Kruskal-Wallis H test, et al were used for secondary outcomes comparison across groups. Results 278 out of 300 participants (92.7%) completed the trial. No statistically significant differences were observed in LBP incidence among the three groups. No difference was observed in abdominal/back muscle endurance among groups at six months. In secondary outcome analysis regarding spinal ROM, flexion and extension improved in iLSO groups at 12 months compared with that at baseline (flexion, p = 0.01; extension, p = 0.01), whereas only extension motion improved at 12 months in the eLSO group (p = 0.00). Conclusions Six-month wearing of LSO showed neither a significant difference in preventing LBP nor causing adverse effects to participants.Level of Evidence: 1.

Study design Retrospective Cohort Study. Objectives This study compared the function and radiographical outcomes of the patients who underwent C2 dome-like expansive laminoplasty to those C2 expansive open-door laminoplasty for the treatment of OPLL with C2 involved. Summary of background data There are few comparative studies of these two surgical methods. C2 dome-like and C2 expansive open-door laminoplasty are posterior approaches for posterior longitudinal ligament ossification with C2 level and above. Methods This study performed a retrospective cohort analysis of 59 patients with OPLL up to C2 which cause compression symptoms. Thirty-one patients underwent C2 dome-like expansive laminoplasty with C3-7 expansive open-door laminoplasty (Group Dom) and 28 underwent C2-7 expansive open-door laminoplasty (Group Exp). The preoperative and postoperative space available for cord (SAC) of C2 segment, cervical curvature index of C2-7, C2-7 range of motion, Japanese orthopedic association (JOA) score, visual analog scale (VAS) score, and neck disability index (NDI) were used to assess clinical outcomes and statistically analyzed. Results The cervical curvature index, JOA score, and NDI significantly changed at the final follow-up in two groups with no significant intergroup differences. There were no significant differences in preoperative SAC and VAS between the two groups. At the final follow-up, the SAC of C2/3 in Group Exp was significantly larger than Group Dom, while the VAS and range of motion of Group Dom became significantly better than Group Exp. Conclusion The C2 dome-like expansive laminoplasty can reduce postoperative neck pain more obviously and achieve better cervical curvature. C2 expansive open-door laminoplasty can get more adequate decompression in the spinal canal, which may be recommend to the patients with OPLL occupying more than 50% of the vertebral canal at C2/3, or with developmental spinal stenosis.Level of Evidence: 3.

Study design Biomechanical study using cadaveric cervical spines. Objective To evaluate joint mobility and stiffness at the craniovertebral junction. Summary of background data Data on the intersegmental kinematics of the craniovertebral joints are available in the literature with a widespread range of values. The effect that alar ligament injuries have on intersegmental kinematics remains unclear and requires further biomechanical investigation. Methods Ten occipito-atlanto-axial (C0-C1-C2) human specimens were articulated to flexion, extension, bilateral lateral bending, and bilateral axial rotation. The moment-rotation response was continuously tracked through the entire range of motion before and after unilateral alar ligament transection of the right side. Results The intersegmental (C0-C1/C1-C2) moment-rotation response was continuously quantified in full flexion (7.2 ± 6.6°/12.1 ± 5.8°), extension (11.1 ± 6.4°/3.0 ± 2.8°), lateral bending to the right (3.1 ± 2.2°/1.6 ± 1.2°) and left sides (3.3 ± 1.6°/2.1 ± 1.5°), and axial rotation to the right (1.2 ± 3.5°/32.3 ± 9.3°) and left sides (2.7 ± 2.6°/25.3 ± 8.3°). Unilateral alar ligament transection increased the range of motion of C0-C2 in the three planes of movement; however, intersegmental motion alterations were not always observed. Conclusion Increases in the range of extension and lateral bending at C0-C1, which had not been reported previously, were observed. Further, the range of rotation on the right and left sides increased, in conjunction with the increased ranges at C0-C1 and C1-C2.Level of Evidence: N/A.

STUDY DESIGN Retrospective study. OBJECTIVE The aim of this study was to evaluate the long-term clinical and radiographic outcomes of cervical arthroplasty using the ProDisc-C Vivo prosthesis. SUMMARY OF BACKGROUND DATA Previous reports have shown that cervical arthroplasty with ProDisc-C artificial disc has acceptable clinical outcomes at 5-year and 10-year follow-ups. METHODS Clinical and radiographic evaluations, including dynamic flexion-extension lateral images, were performed at baseline and at the 5-year follow-up. RESULTS Twenty-eight patients who underwent single-level ProDisc-C Vivo arthroplasty were followed-up for a mean period of 65 months. The range of motion at the operated level was 8.9 ± 2.3° at baseline and 8.3 ± 4.8° at the final follow-up (p = 0.494). Fourteen of 28 levels (50%) developed heterotopic ossification (HO). According to McAfee's classification, one level was classified as grade I, nine levels as grade II, two levels as grade III, and two levels as grade IV. Only four of 28 levels (14.3%) had severe HO. Among patients with cervical spondylotic myelopathy, mJOA score was 13.9 ± 2.5° at baseline and 15.9 ± 1.0° at the final follow-up (p = 0.001

STUDY DESIGN A biomechanical comparison study using finite element method. OBJECTIVE The aim of this study was to investigate effects of different dynamic stabilization devices, including pedicle-based dynamic stabilization system (PBDSS) and interspinous process spacer (ISP), used for topping-off implants on biomechanical responses of human spine after lumbar interbody fusion. SUMMARY OF BACKGROUND DATA Topping-off stabilization technique has been proposed to prevent adjacent segment degeneration following lumbar spine fusion. PBDSS and ISP are the most used dynamic stabilizers for topping-off instrumentation. However, biomechanical differences between them still remain unclear. METHODS A validated, normal FE model of human lumbosacral spine was employed. Based on this model, rigid fusion at L4-L5 and moderately disc degeneration at L3-L4 were simulated and used as a comparison baseline. Subsequently, Bioflex and DIAM systems were instrumented at L3-L4 segment to construct PBDSS-based and ISP-based topping-off models. Biomechanical responses of the models to bending moments and vertical vibrational excitation were computed using FE static and random response analyses, respectively. RESULTS Results from static analysis showed that at L3-L4, the response parameters including annulus stress and range of motion were decreased by 41.6% to 85.2% for PBDSS-based model and by 6.3% to 67% for ISP-based model compared with rigid fusion model. At L2-L3, these parameters were lower in ISP-based model than in PBDSS-based model. Results from random response analysis showed that topping-off instrumentation increased resonant frequency of spine system but decreased dynamic response of annulus stress at L3-L4. PBDSS-based model generated lower dynamic stress than ISP-based model at L3-L4, but the dynamic stress was higher at L2-L3 for PBDSSbased model. CONCLUSION Under static and vibration loadings, the PBDSSbased topping-off device (Bioflex) provided a better protection for transition segment, and likelihood of degeneration of supraadjacent segment might be relatively lower when using the ISPbased topping-off device (DIAM).Level of Evidence: 5.

STUDY DESIGN A prospective multicenter study. OBJECTIVE To evaluate and compare the surgical outcomes after open-door (OD) and double-door (DD) laminoplasties in subjects with cervical ossification of the posterior longitudinal ligament (OPLL). SUMMARY OF BACKGROUND DATA Although previous studies compared clinical results after OD and DD laminoplasties, they were performed at a single institution with a relatively small sample size targeting mixed pathologies, including cervical spondylotic myelopathy. METHODS This study was performed by the Japanese Multicenter Research Organization for Ossification of the Spinal Ligament. A total of 478 patients with myelopathy caused by cervical OPLL from 28 institutions were prospectively registered from 2014 to 2017 and followed up for 2 years. Of these, 41 and 164 patients received OD and DD laminoplasties, respectively. Demographic information, medical history, and imaging findings were collected. Clinical outcomes were assessed using the cervical Japanese Orthopaedic Association, Japanese Orthopaedic Association Cervical Myelopathy Evaluation Questionnaire, and visual analog scale scores. RESULTS Age, sex, symptom duration, and comorbidities were not significantly different between the groups. Segmental ossification was the most frequent in both the groups. No significant differences in K-line type, canal occupying ratio, C2 to C7 angles, and range of motion were found. Both the procedures reduced the cervical range of motion postoperatively. A comparable frequency of perioperative complications was observed between the groups. The cervical Japanese Orthopaedic Association scores showed a similar improvement at 2 years postopera- tively. The reduction in visual analog scale score for neck pain was favorable in the OD group (P = 0.02), while other pain assessments did not show any significant differences between the groups. The functional outcomes assessed using the Japanese Orthopaedic Association Cervical Myelopathy Evaluation Questionnaire presented equivalent effective rates. CONCLUSION The results demonstrated almost comparable surgical outcomes between OD and DD laminoplasties. Lamino- plasty is a valuable technique as a therapeutic option for cervical OPLL.Level of Evidence: 2.

STUDY DESIGN Prospective cohort study. OBJECTIVE The aim of this study was to identify surgical factors that affect adjacent segment kinematics after anterior cervical discectomy and fusion (ACDF) as measured by biplane radiography. SUMMARY OF BACKGROUND DATA Previous studies investigated the effect of surgical factors on spine kinematics as a potential etiology for adjacent segment disease (ASD). Those studies used static flexion-extension radiographs to evaluate range of motion. However, measurements from static radiographs are known to be unreliable. Furthermore, those studies were unable to evaluate the effect of ACDF on adjacent segment axial rotation. METHODS Patients had continuous cervical spine flexion/exten- sion and axial rotation movements captured at 30 images per second in a dynamic biplane radiography system preoperatively and 1 year after ACDF. Digitally reconstructed radiographs generated from subject-specific CT scans were matched to biplane radiographs using a previously validated tracking process. Dynamic kinematics, postoperative segmental kyphosis, and disc distraction were calculated from this tracking process. Plate-to-disc distance was measured on postoperative radiographs. Graft type was collected from the medical record. Multivariate linear regression was performed to identify surgical factors associated with 1-year post-surgery changes in adjacent segment kinematics. A secondary analysis was also performed to compare adjacent segment kinematics between each of the surgical factors and previously defined thresholds believed to be associated with adjacent segment degeneration. RESULTS Fifty-nine patients completed preoperative and postoperative testing. No association was found between any of the surgical factors and change in adjacent segment flexion/exten- sion or axial rotation range of motion (all P > 0.09). The secondary analysis also did not identify differences between adjacent segment kinematics and surgical factors (all P > 0.07). CONCLUSION Following ACDF for cervical spondylosis, factors related to surgical technique were not associated with short-term changes in adjacent segment kinematics that reflect the hypermobility hypothesized to lead to the development of ASD.Level of Evidence: 2.

STUDY DESIGN Biomechanical study of range of motion (ROM) at the vertebral levels adjacent to the construct of posterior pedicle screw-rod fixation with different types of lumbar interbody fusion techniques (LIF). OBJECTIVE To investigate the differences in adjacent segment mobility among three types of LIF: lateral lumbar interbody fusion (LLIF), transforaminal lumbar interbody fusion (TLIF), and posterior lumbar interbody fusion (PLIF). SUMMARY OF BACKGROUND DATA Previous studies have concluded that LLIF, TLIF, and PLIF with posterior pedicle screw-rod fixation (PSR) provide equivalent stability in cadaveric specimens and are comparable in fusion rate and functional outcome. However, long-term complications, such as adjacent segment degeneration associated with each type of interbody device, are currently unclear. Little is known about the biomechanical effects of interbody fusion technique on the mobility of adjacent segments. METHODS Normalized ROM data at the levels adjacent to L3-L4 PSR fixation with three different types of lumbar interbody fusion approaches (LLIF, TLIF, and PLIF) were analyzed. Study design Retrospective cohort study. Objective The purpose of this study was to analyze the long-term results for patients with lumbar spinal stenosis (LSS) treated with dynamic stabilization (DS) and to consider how we can improve the results. Summary of background data Few studies have reported long-term outcomes of DS surgery for LSS with or without spondylolisthesis. Methods A single-center, single-surgeon consecutive series of LSS patients who underwent DS surgery with at least five years of follow-up were retrospectively reviewed. Twenty-seven patients were included in the LSS group and 38 patients in the spondylolisthesis group. Patient characteristics, operative data, radiographic parameters, clinical outcomes, and complications were analyzed at baseline and follow-up. Results In the LSS group, all radiographic parameters (e.g., disc height, segmental lordosis, segmental range of motion [ROM] at the index level and proximal adjacent level, global lordosis, and global ROM) were maintained well until the last follow-up. In the spondylolisthesis group, global lordosis decreased from 36.5° ± 8.2° to 32.6° ± 6.0° at the last follow-up (p = 0.039), and global ROM decreased from 22.1° ± 6.9° to 18.8° ± 7.1° at the last follow-up (p = 0.012). In both groups, back pain, leg pain, and Oswestry Disability Index scores showed significant and sustained improvements. Screw loosening occurred in three patients (11.1%) in the LSS group and five patients (13.2%) in the spondylolisthesis group. Symptomatic adjacent segment degeneration (ASD) occurred in two patients (7.4%) in the LSS group and three patients (7.9%) in the spondylolisthesis group. Conclusions Decompression and DS surgery for LSS with or without spondylolisthesis showed favorable long-term surgical outcomes with an acceptable rate of complications and ASD. However, an improved physiological DS system should be developed.Level of Evidence: 4.

STUDY DESIGN A retrospective, radiographic comparative study conducted in a single academic institution. OBJECTIVE This study aims to compare fulcrum extension with conventional extension imaging to determine maximum "hip lordosis" (HL), an important novel patient-specific parameter in spinal realignment surgery, as well as understand the extension capabilities of the lower lumbar spine, which together, are key contributors to whole-body balancing. SUMMARY OF BACKGROUND DATA Recent literature recognizes the hip as an important contributor to whole-body lordosis beyond a compensator for spinal imbalance. METHODS Patients >45 years' old with mechanical low back pain due to degenerative spinal conditions were included and grouped based on the imaging performed-fulcrum or conventional extension. All imaging was performed using EOS under standardized instructions and visual aids. Radiographic parameters include global lumbar angle (GLA), inflexion-S1 (Inf-S1) angle, segmental lumbar angles, pelvic incidence (PI), sacral slope (SS), pelvic tilt (PT), femoral alignment angle (FAA), HL and spinocoxa angle (SCA). Unpaired t test was used to compare between radiographic parameters. RESULTS One hundred patients (40 males and 60 females, mean age 63.0 years) underwent either fulcrum or conventional extension EOS® imaging. Both groups had comparable baseline radiographic parameters. Fulcrum extension gave a larger mean GLA (-60.7° vs. -48.5°, P = 0.001), Inf-S1 angle (-58.8° vs. -48.8°, P = 0.003), SCA (-36.5° vs. -24.8°, P < 0.001), L4/5 and L5/S1 lordosis (-20.7° vs. -17.7°, P = 0.041, and -22.3° vs. -17.1°, P = 0.018, respectively), compared to conventional extension. PI, SS, PT, FAA, and HL were similar between both extension postures. CONCLUSION Fulcrum extension, compared to conventional extension, is better at generating lordosis in the lower lumbar spine, thus improving preoperative assessment of stiffness or instability of the lumbar spine. Both extension methods were equally effective at determining the patient-specific maximum HL to assess the flexibility and compensation occurring at the hip, potentially guiding surgical management of patients with degenerative spines.Level of Evidence: 3.

STUDY DESIGN Prospective, observational study. OBJECTIVE The aim of this study was to collect objective and especially subjective data on changes in cervical spine mobility after single-level anterior cervical decompression and fusion (ACDF) and to investigate the impact on quality of life and activities of daily living (ADLs). SUMMARY OF BACKGROUND DATA Although there are several studies dealing with the objective change in mobility after single-level ACDF, there are few data on how spondylodesis of a motion segment affects subjective restriction of cervical spine mobility. METHODS Patients undergoing first-time, single-level ACDF for a symptomatic spondylotic process were eligible. Data were collected before surgery, at 3-month, and 1-year follow-up. Patients were assessed via clinical scores (pain intensity, Short-Form 8 [SF-8], among others) and asked for impairment in ADLs due to restriction of cervical spine mobility. The subjective restriction was acquired by a five-step patient-reported score. The range of motion was measured by the CROM device. RESULTS Data of 97 patients could be evaluated. Study design A retrospective study of prospectively collected data. Objective This study aimed to examine how radiological parameters affect dynamic changes in the cross-sectional area of the spinal cord (CSA) in cervical spondylotic myelopathy (CSM) patients and how they correlate with the severity of myelopathy, by evaluating multi-modal spinal cord evoked potentials (SCEPs). Summary of background data Appropriate assessments of dynamic factors should reveal hidden spinal cord compression and provide useful information for choosing surgical procedures. Methods Seventy-nine CSM patients were enrolled. They were examined with kinematic CT myelography (CTM), and the spinal levels responsible for their CSM were determined via SCEP examinations. The C2-7 angle, C2-7 range of motion, and percentage of slip were measured on the midsagittal view during flexion and extension, and the CSA was measured on the axial view in each neck position using kinematic CTM. The patients who exhibited the smallest CSA values during extension and flexion were classified into Groups E and F, respectively. Results Fifty-two (65.8%) and 27 (34.2%) cases were included in Groups E and F, respectively. The preoperative JOA score did not differ significantly between the groups; however, the preoperative lower-limb JOA score of Group F was significantly lower than that of Group E (2.24 ± 0.82 vs. 2.83 ± 1.09, P = 0.016). In the multiple logistic regression analysis, a small C2-7 angle during extension (β = 5°, odds ratio: 0.69, 95% confidence interval [CI]: 0.54-0.90) and the slip percentage during flexion (β = 5%, odds ratio: 1.42, 95% CI: 1.09-1.85) were identified as significant predictors of belonging to Group F. Conclusion Exhibiting more severe spinal cord compression during neck flexion was associated with a small C2-7 angle and anterior spondylolisthesis. The neurological status of the patients in Group F was characterized by severe lower limb dysfunction because of a disturbed blood supply to the anterior column.Level of Evidence: 4.

STUDY DESIGN Retrospective matched cohort analysis. OBJECTIVE The aim of this study was to investigate the impact of adjacent level ossification development (ALOD) on the affected as well as the next-level discs with regards to range of motion (ROM) and degenerative changes. SUMMARY OF BACKGROUND DATA Although ALOD is not a rare condition in patients who undergo anterior cervical fusion procedures, there has been little discussion to date about its clinical implications. METHODS Patients who underwent anterior cervical instrumented fusion with a minimum 2-year follow-up were reviewed. Twelve patients with each respective ossification grade (totally 48 patients) were matched based on age, sex, and number of fusion levels. On the preoperative and final follow-up x-rays, disc height, osteophytes, ROM, and maximal listhesis were assessed at the segments which were one- (the adjacent segment) and two-level cranial (the next segment) to the uppermost fused disc. Then, the patients were divided into two groups according to ALOD degree: group 1 (grade 0-1, N = 24) versus group 2 (grade 2-3, N = 24). The changes of all variables were compared between the two groups. RESULTS The mean ROM of the adjacent segment increased by 3.6 degree in group 1 and conversely decreased by 2.8 degree in group 2 (P = 0.002). The mean ROM increase of the next segment was significantly greater in group 2 than in group 1 (4.5 vs. 1.2 degree, P = 0.016). Study design An in vitro biomechanical study. Objective The purpose of this study is to discuss whether pedicle screw systems can control spinal rotational instability in a functional spinal unit of lumbar spine on human cadaver. Summary of background data Rotational experiments using deer lumbar cadaveric models showed that rotational range of motion (ROM) of the model fixed by a pedicle screw system with crosslinking after total facetectomy for both the sides was larger than that in the intact model, and stated that spinal rotational instability could not be controlled using a pedicle screw system. Methods A rotation experiment using 10 functional spinal units (L3-4) of lumbar spine on human cadavers was performed by preparing the four models (intact model, damaged model, pedicle screw model, and crosslink (CL) model) in stages, then calculating and comparing rotational ROM among the four models. Methods. Patients who underwent anterior cervical discectomy with arthroplasty for a cervical radiculopathy due to a herniated disc from the The Netherlands Cervical Kinematics (NECK) trial (activC; metal endplates with a polyethylene inlay and a keel for primary stability) and the PROCON trial (Bryan; metal-on-polymer with titanium coated endplates without a keel) were analyzed for HO at 12 and 24 months postoperatively. HO was scored according to the McAfee–Mehren classification. Segmental ROM was defined by a custom developed image analysis tool, and global cervical ROM was measured by Cobb's angle. Clinical outcome was evaluated by means of the neck disability index (NDI) as well as physical-component summary (PCS) and mental-component summary (MCS). Results. At 2-year follow-up, the occurrence of HO was 68% in patients treated with the activC prosthesis (severe HO 55%), which was comparable with 85% (P = 0.12) in patients with the Bryan disc (severe HO 44%; P = 0.43). The HO progression was similar between groups. Clinically, the patients had comparable NDI, PCS, and MCS at 2-year follow-up, and comparable improvement of clinical outcomes. The global ROM in the Bryan group (56.4 ± 10.8°) was significantly higher than in the activC group (49.5 ± 14.0, P = 0.044) at 2-year follow-up. Conclusion. In comparison of two cervical disc prostheses the development of HO is independent on their architecture. Although global ROM was higher in the Bryan prosthesis group, this difference was not deemed clinically important, particularly because the clinical condition of patients with and without severe HO was comparable. STUDY DESIGN A biomechanical analysis correlating internal disc strains and tissue damage during simulated repetitive lifting. OBJECTIVE To understand the failure modes during simulated safe and unsafe repetitive lifting. SUMMARY OF BACKGROUND DATA Repetitive lifting has been shown to lead to lumbar disc herniation (LDH). In vitro studies have developed a qualitative understanding of the effect of repetitive loading on LDH. However, no studies have measured internal disc strains and subsequently correlated these with disc damage. METHODS Thirty human cadaver lumbar functional spinal units were subjected to an equivalent of 1 year of simulated repetitive lifting under safe and unsafe levels of compression, in combination with flexion (13-15°), and right axial rotation (2°) for 20,000 cycles or until failure. Safe or unsafe lifting were applied as a compressive load to mimic holding a 20 kg weight either close to, or at arm's length, from the body, respectively. Maximum shear strains (MSS) were measured, and disc damage scores were determined in nine regions from axial post-test magnetic resonance imaging (MRI) and macroscopic images. RESULTS Twenty percent of specimens in the safe lifting group failed before 20,000 cycles due to endplate failure, compared with 67% in the unsafe group. Over half of the specimens in the safe lifting group failed via either disc protrusion or LDH, compared with only 20% via protrusion in the unsafe group. Study design A controlled cross-sectional study. Objective The aim of this study was to compare the kinematics of lumbar axial rotation while sitting in an upright and forward bending position in men suffering with and without nonspecific chronic low back pain (NSCLBP). Summary of background data Lumbar rotation while sitting is an important factor in the mechanism of low back pain. Nevertheless, its kinematics has scarcely been investigated. Methods Range of motion (ROM in), average velocity (AV), maximum velocity (MV), and maximal acceleration (MA) of lumbar rotation while sitting in an upright (UP-sitting) and full forward bending position (FFB-sitting) were examined using an industrial lumbar motion monitor in 50 men (25 with NSCLBP and 25 controls). Pain level and the Rolland Morris questionnaire (RMQ) were also included. Results All examined kinematical parameters were significantly lower in men with NSCLBP compared with the controls (↓ROM = 16%-29%; ↓AV = 35%-53%; ↓MV = 3%-46%; ↓MA = 7%-44%) and significantly decreased when moving from UP-sitting to FFB-sitting. In the UP-sitting, the ROM and AV in both groups and the right rotation-MV in the NSCLBP group were always greater during the right rotation compared to the left (NSCLBP = ROM: Δ3.92°, AV: Δ2.74°, MV:Δ3.61°; controls = ROM: Δ3.46°, AV: Δ1.72°). The left rotation-MV was significantly greater compared to the right only in FFB-sitting in the controls (Δ3.03°). In all kinematical parameters in the NSCLBP group, no correlations were found in the visual analogue scale (VAS) levels (4.43 ± 1.47) or RMQ total score (12.32 ± 5.44). Conclusion The kinematic parameters of lumbar rotation were reduced in men with NSCLBP compared with controls both in an UP-sitting and FFB-sitting. In both groups, NSCLBP and controls, asymmetry in lumbar rotation kinematics was indicated as well as a decrease when moving from UP-sitting to FFB-sitting. Level of evidence 3.

Summary of background data Facet joint violation by pedicle screws is not a rare adverse event in instrumented lumbar fusion surgery, and one of the most common types is the screw head blocking the superior-adjacent facet. However, its contribution to biomechanical instability at the supradjacent level is unknown. Methods The range of motion (ROM) of 12 lumbar spines (L4-S1) were measured in flexion-extension, lateral bending, and axial rotation for L4/5. All specimens were randomly divided into two groups: the control group and the blocking group, each with 6 specimens. Spine were tested on intact and instrumented specimens, respectively. The relative ROM changes were compared between the blocking and control groups. Results In the blocking group, the supradjacent-level flexion-extension ROM significantly decreased under all moments (7.5, 6.0, 4.5 Nm) relative to the intact spine and a significant decrease in the lateral bending relative ROM was found at 4.5 Nm. In the control group, no significant change of supradjacent-level ROM was found relative to the intact noninstrumented spine at each moment. When performing flexion-extension, the relative ROM change between the 2 groups was significantly different at 4.5 Nm. When performing lateral bending, the relative ROM change between the 2 groups was significantly different at moments of 6.0 and 4.5 Nm. Conclusion When screw heads blocked superior-adjacent bilateral facets, the supradjacent-level flexion-extension ROM and lateral bending ROM decreased. OBJECTIVE The present study aimed to investigate the relationship between the FR phenomenon asymmetry of lumbar muscles and trunk lateral range of motion (ROM) asymmetry in nonspecific chronic low back pain (NSCLBP) patients. SUMMARY OF BACKGROUND DATA Imbalance in trunk muscle activation between right and left sides can induce pain by loading the spine incorrectly, especially in patients with NSCLBP. A previous study reported a greater asymmetry in the FR phenomenon of the erector spinae in NSCLBP patients than in asymptomatic participants (APs). Imbalance of muscle properties, such as trunk ROM, has been suggested as a possible cause of this observed asymmetry. METHODS Twenty-eight NSCLBP patients and 22 AP performed 3 standing maximal trunk flexions. Surface electromyography was recorded bilaterally for erector spinae longissimus and lumbar multifidus. A FR ratio was calculated for each muscle. The fingertip-to-thigh test was performed to assess trunk lateral ROM. Study design In vitro biomechanical evaluation of a novel self-adaptive unidirectional ratchet growing rod (RGR) system. Objective The aim of this study was to propose and biomechanically validate a novel RGR construct in vitro using porcine thoracic spines and calculate the tensile force required to elongate the RGR with springs, without springs, and with soft tissue encapsulation (induced in vivo in rabbits). Summary of background data Literature lacks clear consensus regarding the implant of choice for early-onset scoliosis. Multiple systems are currently available, and each has its own advantages and disadvantages. Therefore, studying novel designs that can credibly accommodate growth and curb deformity progression is of principle importance. Methods In vitro biomechanical motion tests were done using six porcine thoracic spines with pedicle screws at T3 and T8. A pure moment of ±5 Nm was loaded in lateral bending (LB) and flexion-extension. Range of motion (ROM) and neutral zone (NZ) of each specimen was determined after connecting the free movable growing rods (FGRs), RGRs, and standard rods (SRs). Tensile tests were done to measure the force required to elongate the RGR with springs, without springs, and with soft tissue encapsulation (induced in vivo in rabbits). Results Global ROM, implanted T3-T8 ROM, and the NZ of specimens with FGRs and RGRs were significantly higher than that with SRs. The RGRs favored unidirectional elongation in both LB and flexion. The tensile forces required for elongating the RGR without springs, with springs, and with soft tissue capsulation (by a scaled unit of 3 mm) were 3 ± 1.3 N, 10.5 ± 0.4 N, and 48.4 ± 14.4 N, respectively. Conclusion The RGR could stabilize and favor unidirectional elongation of the implanted spinal column when appropriate forces were present. There was no device failure as far as we have studied and it is anticipated that, with further safety and feasibility assessment, RGRs could be adapted for clinical use. Level of evidence N/A.

Retrospective study.To investigate whether loss of cervical lordosis (LCL) after laminoplasty can be predicted from specific preoperative dynamic radiograph measurements.Recent studies have focused on the correlation between LCL after laminoplasty and T1 slope. These studies explain this correlation through the injury of the posterior neck muscular-ligament complex (PMLC); however, this muscle injury model could not explain the less kyphotic change in high T1s patients, as reported in some studies as controversy. We have focused on the PMLC constriction reservoir which was represented by extension function (EF).We retrospectively analyzed 50 consecutive patients who underwent open-door laminoplasty (1-year follow-up). EF is defined as extension C2-7 Cobb angle (CA) minus neutral C2-7 CA (Ext CA - CA). LCL is defined as follow-up CA minus preoperative CA (CA [FU] - CA [PRE]), and significant kyphotic change was defined as LCL smaller than -10°.The distribution of LCL was -3.70 ± 7.98 and the significant kyphotic change occurred in 20% of the patients (10/50). EF, C2-7 sagittal vertical axis (PRE), and C2 slope (PRE) were found to be risk factors for LCL by multiple linear regression analysis. The receiver operating characteristic curve analysis revealed that EF could predict the significant kyphotic change well than previously known risk factors. The cutoff value of EF was 14°. No significant kyphotic change occurred at EF greater than or equal to 14°. Upon limiting the number of patients with preoperative straight curvature (n = 28), there is also no significant kyphotic change occurred in any patient whose EF was greater than or equal to 14°.In our study sample, we found that there is no relation between T1 slope and LCL. We have identified a new factor, EF, that could predict LCL after laminoplasty. No significant kyphotic changes after laminoplasty occurred particularly when the EF was greater than or equal to 14°.3.

Study design A biomechanical in vitro study using human cadaveric spines. Objective The aim of this study was to compare atlantoaxial stability and stiffness of a C1 lateral mass - C2 short pedicle - C3 lateral mass screw-rod construct versus C1 lateral mass - C2 pedicle screw-rod construct. Summary of background data The C1 lateral mass - C2 pedicle screw-rod construct provides excellent atlantoaxial fixation, but C2 pedicle screw placement is associated with risk of vertebral artery injury. The use of shorter C2 pedicle screws may mitigate the risk of vascular injury but may result in reduced C1-C2 stabilization. Extending C1 lateral mass - C2 short pedicle screw-rod construct with C3 lateral mass screws may mitigate the risk of vascular injury without compromising C1-C2 fixation. Methods Seven cervical spines were tested with internal control experimental design in the following sequence: intact state, and following creation of type II odontoid fracture, the specimen was instrumented with C1 lateral mass - C2 pedicle screw fixation (C2PED), C1 lateral mass - C2 short pedicle screw fixation (C2SPED), and C1 lateral mass - C2 short pedicle - C3 lateral mass screw fixation (C2SPED-C3LM). For each condition, the angular stiffness and range of motion (ROM) with 1.5-Nm load in flexion/extension (FE), lateral bending (LB), and right/left axial rotation (RAR/LAR) were quantified. Results Instrumented conditions demonstrated significantly lower C1-C2 angular ROM and greater stiffness than the intact state. Compared with C2PED, C2SPED-C3LM demonstrated significantly lower C1-C2 ROM during FE and LB, significantly greater C1-C2 stiffness in flexion and right/left LB, similar C1-C2 ROM and stiffness in RAR/LAR, and similar stiffness in extension. C2SPED-C3LM demonstrated significantly greater atlantoaxial stabilization in the sagittal and coronal planes than C2PED construct. Conclusion Compared with C2PED, C2SPED-C3LM may be a suitable alternative surgical strategy for atlantoaxial instability that provides superior atlantoaxial fixation. Level of evidence N/A.

Study design Finite element analysis. Objective To determine and compare the biomechanical stability of the bicortical short C2 pars screw fixation for high-riding vertebral artery (HRVA) with the C2 pedicle screw and C2 translaminar screw fixation in finite element models. Summary of background data Fixation of C2 is technically demanding in the case of HRVA. However, there is no consensus on the alternative technique for the C2 screw fixation for HRVA in the literature. Methods A finite element (FE) model of the upper cervical spine (C0-C2) with HRVA had been developed. C1 pedicle screw was applied at C1 by using notching technique. Bicortical short C2 pars screws, C2 pedicle screws, and C2 translaminar screws were used in each model. Then a vertical load of 50 N and a 1.5 Nm torque were applied to the C0 to simulate flexion, extension, lateral bending, and axial rotation respectively. Results Compared with C2 pedicle screw fixation, the bicortical short C2 pars screw fixation increased the range of motion (ROM) by -1.45%, 2.13%, 62.0%, and 22.0% under flexion, extension, lateral bending, and axial rotation, respectively; However, the C2 translaminar screw fixation increased the ROM by 43.6%, 17.8%, 423.4%, and 19.9%, respectively. In terms of the peak von Mises stress, compared with C2 pedicle screw fixation, bicortical short C2 pars screw decreased 46.1%, 41.6%, 71.3%, and -12.5% under flexion, extension, lateral bending, and axial rotation, respectively; C2 translaminar screw decreased -2.66%, -4.87%, 73.0% and -10.1%, respectively. Conclusion For a patient with HRVA, bicortical short C2 pars screw fixation provides sufficient stability and exhibited a smaller von Mises distribution on the screw-rod construct, indicating it could be an effective C2 internal fixation method for HRVA to promote C1-C2 stability and avoid the vertebral artery injury.Level of Evidence: N/A.

Study design A multicenter, retrospective study. Objective To clarify the clinical and radiological effects of removing interspinous contextures in lumbar decompression surgery for patients with lumbar spinal stenosis. Summary of background data There have seldom been reports that have compared both clinical results and radiological changes among minimally invasive decompression methods. Methods Consecutive 52 patients underwent lumbar spinous process-splitting laminectomy (LSPSL), following which 50 patients underwent trans-interspinous lumbar decompression (TISLD). All patients presented with cauda equina type of lumbar spinal stenosis and underwent a minimum 1-year follow-up. The Japanese Orthopaedic Association (JOA) score and Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) score were evaluated to determine the extent of daily living activities. Propensity score (PS)-matched analysis was used to match patients' age and preoperative JOA scores between the two groups. Lumbar lordosis (LL), disc height (DH), segmental range of motion (ROM), and vertebral anterior translation were measured by functional lumbar lateral X-ray, and changes between preoperative and 1-year postoperative values were evaluated. Results Twenty-nine pairs of patients were selected by PS-matching. Mean JOA scores increased from 14.4 to 23.5 (mean recovery rate was 62.3%) in the LSPSL group and from 14.0 to 23.2 (61.3%) in the TISLD group at preoperative and 1-year follow-up, respectively. There were no significant differences in clinical results and changes in LL, ROM, and vertebral anterior translation in each group. The DH at L4/5 level at 1-year after surgery revealed significant decrease in the TISLD group compared to the LSPSL group. There was a correlation between preoperative DH and DH decrease in the LSPSL group, but not in TISLD group. Conclusions Removal of interspinous contextures did not influence clinical outcomes at 1 year after surgery, but it may be likely to cause disc height loss when it applied at the L4/5 level.Level of Evidence: 3.

Study design An in-vitro biomechanical study of human cadaver sacroiliac joints. Objective Our study aimed to develop a more comprehensive understanding of the native motion of the SIJ within the context of spinal kinematics and spinal implant evaluation. Summary of background data Increasing attention has been given to the sacroiliac joint (SIJ) as a source of low back pain, despite its limited range of motion. We sought to characterize the rotational and translational motion in each axis utilizing standard pure moment flexion-extension (FE), lateral bending (LB), and axial rotation (AR) testing. Methods Sixteen sacroiliac joints were evaluated from eight lumbosacral cadaver specimens (six females, two males) from subjects aged 28 to 57 years (mean age 46.8) with body mass index (BMI) 22 to 36 (mean BMI 30). Single leg stance was modeled by clamping the blocks on one ischium in a vise and letting the contralateral ischium hang freely. Pure moment loading was applied in FE, right/left AR, and right/left LB. Relative motions were collected with infrared markers. Results The on-axis ratio was significantly lower in LB than in FE (P = 0.012) and in AR (P = 0.017). The rotation deviation angle measured 13.9 ± 9.1° in FE, 17.1 ± 8.7° in AR, and 35.7 ± 25.7° in LB. In LB the rotational deviation angle is significantly higher than both FE and AR (P = 0.003 and P = 0.011, respectively). In-plane translation was significantly higher (P = 0.005) in FE loading than in LB loading. Conclusion A nontrivial amount of rotation and translation occurred out of the expected axis of motion. The largest amount of off-axis rotation was observed in lateral bending. Relative to resultant translation, in-plane translation was lowest in lateral bending. Our results indicate that rotation of the SIJ is not fully described with the in-plane metrics which are normally reported in evaluation of fusion devices. Future studies of the SIJ may need to consider including off-axis rotation measurements when describing SIJ kinematics.Level of Evidence: 5.

Study design Biomechanical spine model. Comparison of stress in the implant and the adjacent cranial segment was done with conventional rigid versus dynamic stabilization system (DS) fixation. Objective The aim of this study was to study stress at the proximal end of spinal fixation with a novel DS. Summary of background data High stress at the implant bone junction may cause proximal junctional failure (PJF) in adult deformity surgery. Methods Five life-size spine models were instrumented with pedicle screws and a 5.5-mm Titanium rod from T8-S1. The same models were subsequently instrumented with a similar rod and DS between T8-9 pedicle screws. The spine model was loaded with 25 Nm static load cranial to the proximal fixation in six directions. Strains were measured from the proximal screws. Disc pressure was measured from the proximal instrumented segment (T8-9) and cranial adjacent segment (T7-8). Results Rigid fixation produced highest strain at T8, followed by T10 then T9. In contrast, DS fixation produced highest strain at T10, followed by T9 then T8. Strain at T8 was significantly less with DS fixation than rigid fixation (P = 0.019). The T10 screw strain was not significantly higher with DS stabilization compared to rigid fixation (P = 0.091). Rigid fixation allowed no load-sharing or pressure rise at T8-9 but an abrupt rise at T7-8. DS system permitted load-sharing and pressure rise in T8-9; the difference compared to rigid fixation was significant in flexion loading (P = 0.04) and similar trend but not significant in extension (P = 0.09). DS system produced a rise in the adjacent segment disc pressure (T7-8), which was smaller than rigid fixation but not significant. Conclusion Long spinal fixation using rigid rods produces maximum stress at the proximal end screw and increases adjacent disc pressure, possibly leading to PJF. Dynamic stabilization at the cranial end segment may prevent PJF by reducing these factors.Level of Evidence: N/A.

Study design Case report (level V evidence). Objective We report a case of a 33-year-old man with Marfan syndrome that visited our clinic for left knee pain and stiffness. Radiographs of the left knee and lumbar spine demonstrated a spinal rod in the posterolateral left knee and its origin being a broken rod from his previous unilateral spinal fusion 17 years prior. Summary of background data Spinal arthrodesis is a common treatment modality for a wide range of spinal pathologies including infection, trauma, congenital and developmental deformities, and degenerative conditions. A rare complication that may arise from said procedure is implant migration, most often a result of pseudoarthrosis. Methods Description of the case report. Results Patient was taken to the operating room 2 weeks later for an uneventful removal of the implant and immediate improvement with pain and range of motion. Conclusion Spinal implant migration is a rare complication most often due to implant failure from pseudoarthrosis. STUDY DESIGN Multicenter retrospective study. OBJECTIVE Flexion-extension radiographs are frequently used to assess motion in patients with degenerative spondylolisthesis. However, they expose patients to additional radiation and increase cost. The aim of this study is to determine if flexion-extension radiographs provide additional information not seen on upright neutral radiographs and supine magnetic resonance imaging (MRI) that may guide surgical decision making. SUMMARY OF BACKGROUND DATA Supine MRI and upright neutral radiographs are routinely performed in patients with degenerative spondylolisthesis. It is unclear whether additional flexion-extension views play a significant role in surgical planning for this patient population. METHODS From the Quality Outcomes Database, patients who had surgery for grade 1 degenerative spondylolisthesis were identified. Magnitude of slip on pre-op supine MRI, upright neutral, flexion, and extension radiographs were measured. Additional motion was defined as 3 mm or more slip difference between radiographs. For the purpose of this analysis, patients with a slip of 7 mm or more on upright neutral radiographs were assumed to require a fusion. RESULTS A total of 191 patients were identified. Mean age was 61.6 years (114 females, 60%). Only 31 patients (16%) had additional motion on flexion-extension views not seen on upright neutral x-rays versus supine MRI. Of these 31 patients, 19 had slips less than 7 mm on upright x-ray, generating equipoise for fusion. CONCLUSION Flexion-extension radiographs may play a limited role in management of degenerative spondylolisthesis. STUDY DESIGN Outcome study to determine the construct validity and reliability of the Japanese version of the Lumbar Stiffness Disability Index. OBJECTIVE The aim of this study was to evaluate the psychometric measurement properties of the Japanese version of the Lumbar Stiffness Disability Index (J-LSDI) following lumbar spinal surgery in order to assess its construct validity and reliability. SUMMARY OF BACKGROUND DATA The LSDI was designed and validated as a tool to assess functional impacts of lumbar spine stiffness and diminished spinal flexibility. A Japanese version has been developed, but its construct validity and reliability have not been evaluated. METHODS A Pearson correlation coefficient was calculated using flexion and extension range of motion, Oswestry Disability Index (ODI) and Tampa Scale for Kinesiophobia (TSK) as external standards to evaluate construct validity. An intraclass correlation coefficient (ICC) and a Bland-Altman analysis were used to evaluate test-retest reliability. RESULTS A total of 244 patients following lumbar spinal surgery participated in the study. Fifty one of the 244 patients participated in the reliability study. The ICC of the J-LSDI for test-retest reliability was 0.89 (95% confidence interval, 0.79-0.93). There was no systematic error found in the results of the Bland-Altman analysis. One hundred ninety-three of the 244 patients participated in the validity study. There were significant correlations between the J-LSDI and trunk flexion range of motion (r = -0.66), extension (r = -0.51), ODI (r = 0.62), and TSK (r = 0.38). CONCLUSION The construct validity and reliability of the Japanese version of the LSDI were confirmed. The J-LSDI can be used to evaluate lumbar stiffness and associated disability in Japanese patients following lumbar spinal surgery.Level of Evidence: 2.

Comparative study.The aim of this study was to compare cervical alignment among three-level total disc replacement (TDR), two prosthesis with one cage (2D1C), one prosthesis with two cages (1D2C), and anterior cervical discectomy and fusion (ACDF), then identify the importance of cervical balance of cervical spondylotic myelopathy (CSM).There were few long-term comparisons on cervical alignment and cervical balance among three-level TDR, 2D1C, 1D2C, and ACDF for CSM.Twenty-eight patients with TDR, 15 with 2D1C, 36 with 1D2C, and 32 cases with ACDF were included with a mean follow-up of 90.9 ± 8.9 months. C2-C7 cervical lordosis (CL), C2-C7 sagittal vertical axis (SVA), T1 slope (T1S) were measured on x-ray at preoperation, immediate postoperation, and final follow-up, as well as range of motion (ROM), upper/lower adjacent ROM (UROM/LROM), and adjacent segment degeneration (ASD); cervical balance was assessed by T1S minus CL (T1SCL; 20°). Clinical outcomes included neck disability index (NDI) and Japanese Orthopedic Association (JOA) score.NDI and JOA improved (P 0.01)at postoperation and final-visit with no difference among four groups. ROM decreased mostly in ACDF group, although with a comparable inter-group UROM/LROM and ASD. All groups gained equal CL-improvement at final-visit. SVA and T1S together with their change were of no differences among groups (P0.05). There was a correlation among alignment parameters and between CL and ROM. The inter-group capacity of balance maintaining and imbalance correction was comparable (P0.05). The change of T1SCL was not correlated to NDI and JOA (P0.05).Adjacent segments were seldom affected. Cervical alignment was equivalently rebuilt among TDR, 2D1C, 1D2C, and ACDF. It was not essential to pay excessive attention to balance.3.

OBJECTIVE The aim of this study was to evaluate the impact of cervical disc degeneration (CDD) severity on 2-year postoperative outcomes following laminoplasty. SUMMARY OF BACKGROUND DATA The impact of CDD on postoperative outcomes of cervical laminoplasty has not been well established. METHODS A total of 144 patients who underwent open-door laminoplasty for cervical spondylotic myelopathy (CSM) were enrolled. Six cervical discs were independently analyzed for degeneration severity using a previously reported grading system (grade 0: none, grade 3: severest). The relationship between the segmental range of motion (ROM) and the severity of CDD was evaluated. Subsequently, after dividing overall patients into mild and severe CDD groups by the average of CDD scores, the mixed-effect model was applied to assess 2-year postoperative outcomes, including physician-assessed myelopathy scores, patient-reported outcomes, and preoperative radiographic parameters. Finally, as additional analysis, the severe CDD group was further divided into two groups: group 1 included patients with a grade 3 CDD change in their most stenotic level and group 2 included the others. OBJECTIVE To compare the mid-term outcomes of hybrid surgery (HS) and anterior cervical discectomy and fusion (ACDF) for the treatment of contiguous two-segment cervical degenerative disc diseases. SUMMARY OF BACKGROUND DATA HS has become one of the most controversial subjects in spine communities, and the comparative studies of HS and ACDF in the mid- and long-term follow-up are rarely reported. METHODS From 2009 to 2012, 42 patients who underwent HS (n = 20) or ACDF (n = 22) surgery for symptomatic contiguous two-level cervical degenerative disc diseases were included. Clinical and radiological records, including Japanese Orthopedic Association (JOA), Neck Disability Index (NDI), Visual Analogue Scale (VAS), local cervical lordosis, and range of motion (ROM), were reviewed retrospectively. Complications were recorded and evaluated. RESULTS Mean follow-up was 77.25 and 79.68 months in HS group and ACDF group, respectively (P > 0.05). Objective The aim of this study was to investigate the incidence of progression in radiographic adjacent-level degeneration (ΔALD) from preoperative assessment to 5 years after total disc replacement (TDR) and the relationship of these changes with range of motion and clinical adjacent-level disease. A secondary objective was to compare adjacent-level degeneration (ALD) outcomes between TDR and fusion. Summary of background data Fusion is associated with high rates of ALD in symptomatic lumbar disc degeneration. TDR may reduce this risk. Methods In total, 175 patients with single-level, symptomatic, lumbar disc degeneration who had received activL or ProDisc-L and had a preoperative and 5-year postoperative radiograph available were included. Over 5-year follow-up, ΔALD was defined as an increase in ALD of ≥1 grade and clinical ALD was defined as surgical treatment at the level adjacent to an index TDR. Matching-adjusted indirect comparisons were conducted to compare ALD outcomes after TDR (current trial) with those after fusion (published trial). Results At 5-year follow-up, 9.7% (17/175) of TDR patients had ΔALD at the superior level. In patients with preoperative ALD at the superior level, most (88% [23/26]) showed no radiographic progression over 5 years. The rate of clinical ALD was 2.3% (4/175) and none of these patients had ALD at baseline. For each degree of range of motion gained at the TDR level, there was a consistent decrease in the percentage of patients with ΔALD. After matching and adjustment of baseline characteristics, TDR had a significantly lower likelihood of ΔALD than fusion (odds ratio 0.32; 95% confidence interval 0.13, 0.76). Conclusion The rates of ΔALD and clinical ALD in this TDR population were similar to those previously reported in the literature for TDR at 5-year follow-up. Study design Microstructural investigation of compression-induced herniation of a lumbar disc held in a concordant complex posture. Objective To explore the significance of loading rate in a highly asymmetric concordant posture, comparing the mechanisms of failure to an earlier study using a nonconcordant complex posture. Summary of background data A recent study with a nonconcordant complex posture (turning in the opposite direction to that which the load is applied) demonstrated the vulnerability of the disc to loading that is borne by one set of oblique-counter oblique fiber sets in the alternating lamellae of the annulus, and aggravated by an elevated loading rate. Given the strain rate-dependent properties of the disc it might be expected that the outcome differs if the posture is reversed. Methods Forty-one motion segments from ovine 16 spines were split into two cohorts; adopting the previously employed low rate (40 mm/min) and surprise rate (400 mm/min) of loading. Both groups of damaged discs were then analyzed microstructurally. Results With the lower rate loading the concordant posture significantly reduced the load required to cause disc failure than earlier described for nonconcordant posture (6.9 vs. 8.4 kN), with more direct tears and alternate lamella damage extending to the anterior disc. Contrary to this result, with a surprise rate, the load at failure was significantly increased with the concordant posture (8.08 vs. 6.96 kN), although remaining significantly less than that from a simple flexed posture (9.6 kN). Analysis of the damage modes and postures suggest facet engagement plays a significant role. Conclusion This study confirms that adding shear to the posture lowers the load at failure, and causes alternate lamella rupture. Load at failure in a complex posture is not determined by loading rate alone. Rather, the strain rate-dependent properties of the disc influence which elements of the system are brought into play. Study design A retrospective analysis of functional limitations due to stiffness after long-level spinal instrumented fusion surgery to correct lumbar degenerative flat back was performed. Objective To analysis the functional limitations in patients treated surgically for adult lumbar degenerative flat back (ALDFB) with long-level instrumented fusion to the sacrum or pelvis. Summary of background data Long-level instrumented fusion for ALDFB decreases back pain and spinal deformity. On the contrary, this surgery considerably eliminates spinal range of motion. This may have the potential to impair function and ability to perform activities of daily living (ADLs). Methods Consecutive 44 patients who underwent long-level instrumented fusion to the sacrum or pelvis for ALDFB were retrospectively included in this study. All patients were followed up for a minimum of 13 months. The Lumbar Stiffness Disability Index for Korean Lifestyle and Oswestry Disability Index were administered and analyzed to assess the impact of spinal stiffness on daily living. Cohorts were defined based on the upper instrumented vertebrae (above T10 [group 1] or below L1 [group 2]) and lower instrumented vertebrae (S1 pedicle screw [group S] or iliac bolt screw [group I]). Results All patients showed deteriorated postoperative ADLs compared to preoperative values. Group 1 showed deterioration postoperatively compared to group 2. Group 1 showed deteriorated postoperative ADLs compared to preoperative values. In group 2, question 5 and 7 showed deterioration postoperatively compared to preoperative values, and question 2 and 10 showed improvement postoperatively compared to preoperative values. Group I showed deterioration postoperatively compared to group S. Conclusion This study will hopefully allow surgeons to provide patients with ALDFB with a more informed explanation of expected surgery effects on specific ADLs. Level of evidence 3.

Study design A literature review. Objective The aim of this study was to review the contemporary methods for spinal flexibility assessment on the patients with adolescent idiopathic scoliosis (AIS). Summary of background data Spinal flexibility is one of the essential parameters for clinical decisions on the patients with AIS. Various methods of spinal flexibility assessment are proposed, but which method(s) could better reveal spinal flexibility or predict treatment effect is unclear. Methods The databases of AbleData, IBSS, Academic Search Premier, MEDLINE/PubMed, CINAHL, Recal Legacy, REHABDATA, Embase, and Web of Science were searched. The study inclusive criteria were (1) prospective cohort study; (2) investigated spinal flexibility on AIS patients; (3) published in English 1996 to 2016. Results Totally 15 articles were included and 11 methods of spinal flexibility assessment were introduced in this review. Traction methods revealed higher spinal flexibility on the patients with severe curves (>65°) but lower spinal flexibility on the patients with moderate curves (40°-65°), comparing with lateral bending methods. Among lateral bending methods, fulcrum bending flexibility is higher on thoracic (T) curves, whereas supine with lateral bending flexibility is higher on thoracolumbar or lumbar (TL/L) curves. For predicting postoperative correction, fulcrum bending flexibility showed higher correlation with postoperative correction on moderate curves and traction flexibility showed higher correlation on severe curves, compared with supine with lateral bending method. Conclusion Curve magnitude and location are two important parameters in selecting appropriate method for spinal flexibility assessment and treatment effect prediction. The traction method should be considered for the patients with severe curves, while the lateral bending method is suggested for the patients with moderate curves. The fulcrum bending method is recommended to assess T curve flexibility, whereas the supine with lateral bending method is for the assessment of TL/L curve flexibility. A comprehensive guideline for selecting spinal flexibility assessment method(s) should be established via future studies. Level of evidence 4.

Study design A cervical spine biomechanical investigation using multibody dynamics. Objective To develop a comprehensive cervical spine multibody dynamics model incorporated with the force-dependent kinematics (FDK) approach, and to study the influence of soft tissue deformation on the joint loading prediction. Summary of background data Musculoskeletal multibody dynamics models have been widely used to analyze joint loading. Current cervical spine musculoskeletal models, however, neglect the joint internal motion caused by soft tissue deformation. A novel FDK approach is introduced, which can predict joint internal motion and spinal joint loading simultaneously. Methods A comprehensive cervical spine musculoskeletal model with the posterior facet joints and essential ligaments was developed. To quantify the influence of soft tissue structures on joint loading prediction, four different models with different features were created. These newly developed models were validated, under flexion-extension movement. The predicted intervertebral disc loads (from C3-C4 to C5-C6) were compared with the published cadaveric experimental results. Moreover, the predicted facet joint forces, ligament forces, and anterior-posterior translations of instantaneous centers of rotation were also studied. Results The obtained intervertebral disc loads were varied among different models. Model 3 provided the closest prediction of joint loading to the experimental results. Moreover, the facet joint and ligament forces were in similar range of magnitude as literature findings. The predicted instantaneous centers of rotation translational changes were in accordance with the in vivo kinematics observation. Conclusion In the present study, a validated cervical spine musculoskeletal model was developed, using multibody dynamics and FDK approach. It can simulate the function of musculature and consider joint internal motion, and thus provides more reliable joint loading prediction. This newly developed cervical model can be used as an efficient tool to study the biomechanical behaviors of human cervical spine, and to understand the fundamental pathologies of spinal pains. A prospective radiographic analysis of adolescent idiopathic scoliosis (AIS) patients managed with alternate-level pedicle screw fixation was performed. The objective of this study was to characterize segmental curve flexibility and to determine its predictive value in curve correction in AIS patients. Little is known regarding the distinct segmental curve characteristics and their ability to predict curve correction in patients with AIS. The segmental Cobb angle was measured on posteroanterior standing radiographs and on fulcrum bending radiographs. Radiographs were analyzed preoperatively and at 2 years postoperatively and the curve was divided into upper, mid, and lower segments based on predefined criteria. The segmental flexibility and the segmental fulcrum bending correction index (FBCI) were calculated. Eighty patients were included with mean age of 15 years. Preoperative mean segmental Cobb angles were 18, 31, and 17 degrees in the upper, mid, and lower segments, respectively. OBJECTIVE: To quantify the ability of five commonly used immobilizers to restrict cervical spine movement, including intervertebral movement, in three directions. SUMMARY OF BACKGROUND DATA: Evidence about the ability of many clinically used cervical immobilizers to restrict cervical movement is limited. Furthermore, their effect on intervertebral movement is unknown. METHODS: Radiographic inert beads were implanted in the cervical vertebral bodies of five fresh-frozen human cadavers. After application of different immobilizers (Stifneck, Sternal Occipital Mandibular Immobilizer (SOMI), halo-traction, spineboard, halo-vest) and controlled flexion-extension, lateral bending, and rotation torques, radiostereometric analysis was used to determine the overall and intervertebral three-dimensional movement of each vertebral level. Restriction of cervical movement was described as a mean restriction percentage (MRP) and classified on an arbitrary basis (poor: 80%). RESULTS: Most of the restriction of flexion/extension was observed at C0-C1, while most rotational restriction was seen at C1-C2. Lateral bending was restricted at C1 to C7.The Stifneck provided the least immobilization with a moderate restriction of flexion-extension (MRP: 41%, SD: 14%), fair restriction of lateral bending (MRP: 29%, SD: 13%), and substantial restriction of rotation (MRP: 64%, SD: 15%). The halo-vest was the most restrictive immobilizer and reduced movement of the cervical spine substantially for flexion-extension (MRP: 70%, SD: 11%), substantially for lateral bending (MRP: 77%, SD: 14%), and nearly complete for rotation (MRP: 92%, SD: 3%). CONCLUSION: The restriction of movement from lowest to highest was: Stifneck, SOMI, halo-traction, head blocks on a spine board, and halo-vest. Notably, the standard deviations of the restrictions were smaller for the cranio-thoracic devices than for the cervico thoracic devices. With this new knowledge of external immobilizers and their ability to restrict intervertebral cervical movement, their indication and application in clinical practice can be improved for all patients with (suspected) cervical injury. STUDY DESIGN A cross-sectional study of prospectively collected data. OBJECTIVE To compare lumbar spine alignment in six common postures, and estimate loss in range of motion (ROM) relative to standing. SUMMARY OF BACKGROUND DATA Ideal position for fusion of lumbar spine remains unknown. Although surgical fusion is necessary for deformity correction and symptom relief, the final position in which the vertebrae are immobilized should provide maximum residual function. METHODS Data were collected prospectively from 70 patients with low back pain recruited over a year. All subjects had x-rays performed in slump sitting, forward bending, supine, half squatting, standing, and backward bending postures. ROM quantified in terms of sagittal global and segmental Cobb angles was measured from L1 to S1. Loss of ROM relative to standing was calculated for each posture. Analysis of variance and unpaired t tests were used to identify differences in alignment between postures. Study design Biomechanical study utilizing human cadaveric cervical spines. Objective To quantitatively assess the effects on intervertebral motion of isolated unilateral cervical facet fracture, and after disruption of the intervertebral disc at the same level. Summary of background data Clinical evidence has indirectly suggested that cervical facet fractures involving 40% of the height of the lateral mass can cause instability of the involved segment. No study to date has demonstrated the kinematic effects of such an injury in a cadaveric model of the cervical spine. Methods Nine six-segment cervical spines were defrosted and fixated to a spine motion simulator capable to apply unconstrained bending moments in the three anatomical planes. The spines were subjected to a maximum torque of 2 N · m in flexion, extension, left and right lateral bending, and of 4 N · m in left and right axial rotation. Each spine was tested in the intact configuration (INTACT), and following two increasing degrees of injury at C4-C5: fracture of the facet (CF1), and CF1 with disruption of the intervertebral disc at the same level (CF2). Intervertebral kinematics was tracked via clusters of active markers fixated on each vertebra. Differences in kinematics between INTACT and the two injured configurations were assessed via one-way Analysis of Variance (P Results No significant differences were detected between INTACT and CF1 across all kinematic parameters (P > 0.05) at C4-C5. CF2, however, resulted in significant increase of flexion, left axial rotation, and left lateral bending with respect to INTACT (flexion at C4-C5: INTACT = 8.7° ± 3.5°; CF2 = 14.3 ± 5.7; P Conclusion Our findings suggest that superior articular facet fractures alone involving 40% of the lateral mass may not necessarily result in intervertebral instability under physiologic loading conditions. Prospective, multicenter.To determine if stiffness significantly affects function or satisfaction after pan-lumbar arthrodesis.The Lumbar Stiffness Disability Index (LSDI) is a validated measure of the effect of lumbar stiffness on functional activities. Data suggests that patients undergoing fusion of the entire lumbar spine are at greatest risk of functional limitations from stiffness.The LSDI, Short Form 36, Scoliosis Research Society-22, and Oswestry Disability Index were administered preoperatively and at 2-year minimum follow-up to 103 spinal deformity patients from 11 centers. Patients were separated according to the proximal arthrodesis level; upper thoracic (T2-5) to pelvis (UT-Pelvis) or thoraco-lumbar (T10-T12) to pelvis (TL-Pelvis). Outcome scores were compared using Student t test or Tukey-Kramer Honest Significant Difference Analysis of Variance. Regression analysis of final LSDI scores versus Scoliosis Research Society-22 Satisfaction scores was performed.Mean ages, baseline values, and final scores of all outcome parameters were statistically equivalent in the two groups. Final LSDI scores did not change significantly from baseline in the UT-Pelvis (P = 0.478) or TL-Pelvis (P = 0.301) groups. In contrast, highly significant improvements (P ≤ 0.0001) from baseline were seen in both groups for other health-related QoL measures. The 2-year Satisfaction scores were statistically equivalent in the two groups, and the correlation between final LSDI and Satisfaction scores in the entire cohort was not significant (R = 0.013, P = 0.146).Patients undergoing pan-lumbar arthrodesis for adult spinal deformity did not experience substantial increases in disability due to stiffness of the low back, although they did report significant improvements in other health-related QoL measures. Further, LSDI scores did not correlate with patient satisfaction. There were no significant differences in perceived stiffness effects whether arthrodesis stopped in the thoracolumbar or upper thoracic regions. We hope these results will be useful to spine surgeons and patients during preoperative planning and discussions.2.
