Enrolling patients to the standard of care (SOC) arm in randomized clinical trials, especially for rare diseases, can be very challenging due to the lack of resources, restricted patient population availability, and ethical considerations. As the therapeutic effect for the SOC is often well documented in historical trials, we propose a Bayesian platform trial design with hybrid control based on the multisource exchangeability modelling (MEM) framework to harness historical control data. The MEM approach provides a computationally efficient method to formally evaluate the exchangeability of study outcomes between different data sources and allows us to make better informed data borrowing decisions based on the exchangeability between historical and concurrent data. We conduct extensive simulation studies to evaluate the proposed hybrid design. We demonstrate the proposed design leads to significant sample size reduction for the internal control arm and borrows more information compared to competing Bayesian approaches when historical and internal data are compatible. We develop the scale transformed power prior for settings where historical and current data involve different data types, such as binary and continuous data. This situation arises often in clinical trials, for example, when historical data involve binary responses and the current data involve some other type of continuous or discrete outcome. The power prior, proposed by Ibrahim and Chen, does not address the issue of different data types. Herein, we develop a new type of power prior, which we call the scale transformed power prior (straPP). The straPP is constructed by transforming the power prior for the historical data by rescaling the parameter using a function of the Fisher information matrices for the historical and current data models, thereby shifting the scale of the parameter vector from that of the historical to that of the current data. Examples are presented to motivate the need for such a transformation, and simulation studies are presented to illustrate the performance advantages of the straPP over the power prior and other informative and noninformative priors. In clinical trials, there often exist multiple historical studies for the same or related treatment investigated in the current trial. Incorporating historical data in the analysis of the current study is of great importance, as it can help to gain more information, improve efficiency, and provide a more comprehensive evaluation of treatment. Enlightened by the unit information prior (UIP) concept in the reference Bayesian test, we propose a new informative prior called UIP from an information perspective that can adaptively borrow information from multiple historical datasets. We consider both binary and continuous data and also extend the new UIP to linear regression settings. Extensive simulation studies demonstrate that our method is comparable to other commonly used informative priors, while the interpretation of UIP is intuitive and its implementation is relatively easy. One distinctive feature of UIP is that its construction only requires summary statistics commonly reported in the literature rather than the patient‐level data. By applying our UIP to phase III clinical trials for investigating the efficacy of memantine in Alzheimer's disease, we illustrate its ability to adaptively borrow information from multiple historical datasets. The power prior is a popular tool for constructing informative prior distributions based on historical data. The method consists of raising the likelihood to a discounting factor in order to control the amount of information borrowed from the historical data. However, one often wishes to assign this discounting factor a prior distribution and estimate it jointly with the parameters, which in turn necessitates the computation of a normalizing constant. In this article, we are concerned with how to approximately sample from joint posterior of the parameters and the discounting factor. We first show a few important properties of the normalizing constant and then use these results to motivate a bisection‐type algorithm for computing it on a fixed budget of evaluations. We give a large array of illustrations and discuss cases where the normalizing constant is known in closed‐form and where it is not. We show that the proposed method produces approximate posteriors that are very close to the exact distributions and also produces posteriors that cover the data‐generating parameters with higher probability in the intractable case. Our results suggest that the proposed method is an accurate and easy to implement technique to include this normalization, being applicable to a large class of models. They also reinforce the notion that proper inclusion of the normalizing constant is crucial to the drawing of correct inferences and appropriate quantification of uncertainty. [ABSTRACT FROM AUTHOR]

When designing a clinical trial, borrowing historical control information can provide a more efficient approach by reducing the necessary control arm sample size while still yielding increased power. Several Bayesian methods for incorporating historical information via a prior distribution have been proposed, for example, (modified) power prior, (robust) meta-analytic predictive prior. When utilizing historical control borrowing, the prior parameter(s) must be specified to determine the magnitude of borrowing before the current data are observed. Thus, a flexible prior is needed in case of heterogeneity between historic trials or prior data conflict with the current trial. To incorporate the ability to selectively borrow historic information, we propose a Bayesian semiparametric meta-analytic-predictive prior. Using a Dirichlet process mixture prior allows for relaxation of parametric assumptions, and lets the model adaptively learn the relationship between the historic and current control data. Additionally, we generalize a method for estimating the prior effective sample size (ESS) for the proposed prior. This gives an intuitive quantification of the amount of information borrowed from historical trials, and aids in tuning the prior to the specific task at hand. We illustrate the effectiveness of the proposed methodology by comparing performance between existing methods in an extensive simulation study and a phase II proof-of-concept trial in ankylosing spondylitis. In summary, our proposed robustification of the meta-analytic-predictive prior alleviates the need for prespecifying the amount of borrowing, providing a more flexible and robust method to integrate historical data from multiple study sources in the design and analysis of clinical trials. The recent 21st Century Cures Act propagates innovations to accelerate the discovery, development, and delivery of 21st century cures. It includes the broader application of Bayesian statistics and the use of evidence from clinical expertise. An example of the latter is the use of trial-external (or historical) data, which promises more efficient or ethical trial designs. We propose a Bayesian meta-analytic approach to leverage historical data for time-to-event endpoints, which are common in oncology and cardiovascular diseases. The approach is based on a robust hierarchical model for piecewise exponential data. It allows for various degrees of between trial-heterogeneity and for leveraging individual as well as aggregate data. An ovarian carcinoma trial and a non-small cell cancer trial illustrate methodological and practical aspects of leveraging historical data for the analysis and design of time-to-event trials. In developing products for rare diseases, statistical challenges arise due to the limited number of patients available for participation in drug trials and other clinical research. Bayesian adaptive clinical trial designs offer the possibility of increased statistical efficiency, reduced development cost and ethical hazard prevention via their incorporation of evidence from external sources (historical data, expert opinions, and real-world evidence), and flexibility in the specification of interim looks. In this paper, we propose a novel Bayesian adaptive commensurate design that borrows adaptively from historical information and also uses a particular payoff function to optimize the timing of the study's interim analysis. The trial payoff is a function of how many samples can be saved via early stopping and the probability of making correct early decisions for either futility or efficacy. We calibrate our Bayesian algorithm to have acceptable long-run frequentist properties (Type I error and power) via simulation at the design stage. We illustrate our approach using a pediatric trial design setting testing the effect of a new drug for a rare genetic disease. Information from historical trials is important for the design, interim monitoring, analysis, and interpretation of clinical trials. Meta‐analytic models can be used to synthesize the evidence from historical data, which are often only available in aggregate form. We consider evidence synthesis methods for trials with recurrent event endpoints, which are common in many therapeutic areas. Such endpoints are typically analyzed by negative binomial regression. However, the individual patient data necessary to fit such a model are usually unavailable for historical trials reported in the medical literature. We describe approaches for back‐calculating model parameter estimates and their standard errors from available summary statistics with various techniques, including approximate Bayesian computation. We propose to use a quadratic approximation to the log‐likelihood for each historical trial based on 2 independent terms for the log mean rate and the log of the dispersion parameter. A Bayesian hierarchical meta‐analysis model then provides the posterior predictive distribution for these parameters. Simulations show this approach with back‐calculated parameter estimates results in very similar inference as using parameter estimates from individual patient data as an input. Noninferiority trials have recently gained importance for the clinical trials of drugs and medical devices. In these trials, most statistical methods have been used from a frequentist perspective, and historical data have been used only for the specification of the noninferiority margin Δ>0. In contrast, Bayesian methods, which have been studied recently are advantageous in that they can use historical data to specify prior distributions and are expected to enable more efficient decision making than frequentist methods by borrowing information from historical trials. In the case of noninferiority trials for response probabilities π1 ,π2 , Bayesian methods evaluate the posterior probability of H1 :π1 >π2 -Δ being true. To numerically calculate such posterior probability, complicated Appell hypergeometric function or approximation methods are used. Further, the theoretical relationship between Bayesian and frequentist methods is unclear. In this work, we give the exact expression of the posterior probability of the noninferiority under some mild conditions and propose the Bayesian noninferiority test framework which can flexibly incorporate historical data by using the conditional power prior. Further, we show the relationship between Bayesian posterior probability and the P value of the Fisher exact test. From this relationship, our method can be interpreted as the Bayesian noninferior extension of the Fisher exact test, and we can treat superiority and noninferiority in the same framework. Our method is illustrated through Monte Carlo simulations to evaluate the operating characteristics, the application to the real HIV clinical trial data, and the sample size calculation using historical data. Meta-analyses of clinical trials often treat the number of patients experiencing a medical event as binomially distributed when individual patient data for fitting standard time-to-event models are unavailable. Assuming identical drop-out time distributions across arms, random censorship, and low proportions of patients with an event, a binomial approach results in a valid test of the null hypothesis of no treatment effect with minimal loss in efficiency compared with time-to-event methods. To deal with differences in follow-up-at the cost of assuming specific distributions for event and drop-out times-we propose a hierarchical multivariate meta-analysis model using the aggregate data likelihood based on the number of cases, fatal cases, and discontinuations in each group, as well as the planned trial duration and groups sizes. Such a model also enables exchangeability assumptions about parameters of survival distributions, for which they are more appropriate than for the expected proportion of patients with an event across trials of substantially different length. Borrowing information from other trials within a meta-analysis or from historical data is particularly useful for rare events data. Prior information or exchangeability assumptions also avoid the parameter identifiability problems that arise when using more flexible event and drop-out time distributions than the exponential one. We discuss the derivation of robust historical priors and illustrate the discussed methods using an example. The power prior has been widely used in many applications covering a large number of disciplines. The power prior is intended to be an informative prior constructed from historical data. It has been used in clinical trials, genetics, health care, psychology, environmental health, engineering, economics, and business. It has also been applied for a wide variety of models and settings, both in the experimental design and analysis contexts. In this review article, we give an A-to-Z exposition of the power prior and its applications to date. We review its theoretical properties, variations in its formulation, statistical contexts for which it has been used, applications, and its advantages over other informative priors. We review models for which it has been used, including generalized linear models, survival models, and random effects models. Statistical areas where the power prior has been used include model selection, experimental design, hierarchical modeling, and conjugate priors. Frequentist properties of power priors in posterior inference are established, and a simulation study is conducted to further examine the empirical performance of the posterior estimates with power priors. Results from clinical trials are never interpreted in isolation. Previous studies in a similar setting provide valuable information for designing a new trial. For the analysis, however, the use of trial-external information is challenging and therefore controversial, although it seems attractive from an ethical or efficiency perspective. Here, we consider the formal use of historical control data on lesion counts in a multiple sclerosis trial. The approach to incorporating historical data is Bayesian, in that historical information is captured in a prior that accounts for between-trial variability and hence leads to discounting of historical data. We extend the meta-analytic-predictive approach, a random-effects meta-analysis of historical data combined with the prediction of the parameter in the new trial, from normal to overdispersed count data of individual-patient or aggregate-trial format. We discuss the prior derivation for the lesion mean count in the control group of the new trial for two populations. For the general population (without baseline enrichment), with 1936 control patients from nine historical trials, between-trial variability was moderate to substantial, leading to a prior effective sample size of about 45 control patients. For the more homogenous population (with enrichment), with 412 control patients from five historical trials, the prior effective sample size was approximately 63 patients. Although these numbers are small relative to the historical data, they are fairly typical in settings where between-trial heterogeneity is moderate. Many epidemiological studies use a nested case-control (NCC) design to reduce cost while maintaining study power. However, because of the incidence density sampling used, reusing data from NCC studies for analysis of secondary outcomes is not straightforward. Recent methodological developments have opened the possibility for prior NCC data to be used to complement controls in a current study, thereby improving study efficiency. However, practical guidelines on the effectiveness of prior data relative to newly sampled subjects and the potential power gains are still lacking. Using simulated cohorts, we show in this paper how the efficiency of NCC studies that use a mixture of prior and newly sampled subjects depends on the number of newly sampled controls and prior subjects as well as the overlap in the distributions of the matching variables. We explore the feasibility and efficiency of a current study that gathers no controls, relying instead on prior data. Using the concept of effective number of controls, we show how researchers can assess the potential power gains from reusing prior data. We apply the method to analyses of anorexia and contralateral breast cancer in the Swedish population and show how power calculations can be done using publicly available software.
