Purpose: The purpose of this study was to clarify the incidence of the transverse bundle (TB) of the ulnar collateral ligament (UCL), the relationships between TB morphology and morphology of the anterior bundle (AB) or posterior bundle (PB) of the UCL, and the relationship between the TB and the posterior common tendon (PCT). Methods: This study examined 38 elbows from 23 cadavers. TB, AB, and PB were classified morphologically. The TB was classified as: type I, TB does not continue the entire length of the AB; or type II, TB continues the entire length of the AB. The AB and PB were classified as: type I, could be separated as single bundles; or type II, could not be separated. We also observed specimens by focusing on the continuity between the TB and PCT. Fisher's exact test was used to examine the relationship between TB type and AB or PB type. Results: A TB was identified in all 38 elbows (100%), and continued to the AB in all specimens. No significant relationship was evident between TB type and AB or PB type. Continuity of TB fibers and the PCT was seen in 26 elbows (72%). Conclusion: This study suggested that the morphology of the transverse bundle may be unrelated to the morphology of the anterior bundle or posterior bundle. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this basic research study was to clarify the morphological characteristics of the posterior oblique fibers (POL) of the ulnar collateral ligament using a large number of specimens. Methods: This study examined 50 arms from 25 Japanese cadavers. Type classification was performed by focusing on the positional relationship between POL morphology and the joint capsule. The morphological features measured were fiber bundle length, width, and thickness. Results: The POL was classified as follows: Type I, the POL's anterior and posterior edges are located on the surface of the joint capsule and can be separated as a single fiber bundle; Type II-a, the POL anterior edge can be separated, but the posterior edge cannot be separated; Type II-b, the POL posterior edge can be separated, but the anterior edge cannot be separated; and Type III, the POL cannot be separated from the joint capsule. Type I was seen in 23 elbows (46%), Type II-a in 6 elbows (12%), Type II-b in 7 elbows (14%), and Type III in 14 elbows (28%). Conclusion: The results of this study suggested that the POL could be classified into an independent type and an unclear type, and the presence of the unclear type was one of the factors that caused morphological variation. Purpose: The flexor digitorum superficialis muscle (FDS) is considered the most important of the forearm flexors for maintaining elbow valgus stability. However, the relationships between the origin structure of each finger of the FDS and the anterior oblique ligament (AOL) of the ulnar collateral ligament and the common tendon (CT) in the proximal part, and morphological features are unclear. The purpose of this study was to clarify the relationships between the origin structure of each finger of the FDS and the AOL and the CT, as well as to clarify the morphological features of the muscle belly of each finger of the FDS. Methods: This study examined 20 elbows. The origin of each finger was examined. Muscle mass, muscle fiber bundle length, and the pennation angle of each finger were also measured. Results: In all cases, the third and fourth digits originated from the radius, the anterior common tendon (ACT), and the posterior common tendon (PCT). The second and fifth digits (18 elbows) or an independent fifth digit (2 elbows) originated from the ACT, the PCT, the AOL, and other soft tissues of the elbow. Muscle mass and muscle fiber bundle length in the muscle belly of the third and fourth digits were significantly heavier and longer, respectively, than in the muscle belly of the second and fifth digits. Background: Chronic ulnar nerve entrapment worsened by elbow flexion is the most common injury, but rare painful conditions may also be related to ulnar nerve instability. The posterior bundle of the medial collateral ligament (pMCL) and the retinaculum, respectively form a soft floor and a ceiling for the cubital tunnel. The aim of our study was to dynamically assess these soft structures of the cubital tunnel focusing on those involved in the biomechanics of the ulnar nerve. Methods: Forty healthy volunteers had a bilateral ultrasonography of the cubital tunnel. Elbows were scanned in full extension, 45° and 90°, and maximal passive flexion. Morphological changes of the nerve and related structures were dynamically assessed on transverse views. Results: Both the pMCL and the retinaculum tightened with flexion. During elbow flexion, the tightening of the pMCL superficially moved the ulnar nerve remote from the osseous floor of the retroepicondylar groove. A retinaculum was visible in all 69 tunnels with stable nerves (86.3%), tightened in flexion, but absent in 11 tunnels with unstable nerves (13.7%). The retinaculum was fibrous in 60 elbows and muscular in nine, the nine muscular variants did not significantly influence the biomechanics of stable nerves. Stable nerves flattened in late flexion between the tightened pMCL and retinaculum, whereas unstable nerves transiently flattened when translating against the anterior osseous edge of the groove. Conclusion: The retinaculum and the pMCL are key structures in the biomechanics of the ulnar nerve in the cubital tunnel of the elbow. [ABSTRACT FROM AUTHOR]

The development of perforator flaps' concept based on knowledge on vascular anatomy of the skin represents a major improvement in reconstructive surgery. Succeeding description about vascular territories and anatomical basics of the main donor sites, the study of hidden donor sites, such as medial upper arm, constitutes a new step and an additional refinement. 20 upper limbs of 10 fresh adult cadavers were studied with colored latex injections. The origin and distribution of the perforator arteries of the superior ulnar collateral artery and the brachial artery were investigated. The use of Fuji films is simple but their manipulation and result interpretation seem to be difficult in the framework of medical research. The reliability and reproducibility of Fuji films have been proved by many previous studies. This study was undertaken to know precisely the articular zones of the elbow and to determine the compressive stress these areas undergo during different activities, in order to assess the importance of different articular contact areas. These data indicate the need for better-adapted elbow prosthesis and can be eventually used to design more durable prosthesis for the elbow. The compressive stress on the radial head was less than 25% in extension. The stress on the radial head varied from the neutral position (23% of the stress), to full pronation (11% of the stress) and to full supination (6% of the stress). The Humero-ulnar compartment had the maximum impact. Coronoid process seemed to be a fundamental element of the elbow joint in extension (60% of total compressive stress). The Medial humero-ulnar compartment was less stressed than the lateral compartment. The radial head does not seem to play a major role in the stability of the elbow in extension if the ulnar collateral ligament exists. The ulnar collateral ligament is essential to the elbow joint stability. The lifespan of a non-constrained prosthesis would depend on the existence of the couple: radial head/ulnar collateral ligament; the absence of radial head could compromise the humero-ulnar stability. This work paved the way for the designing of new non-constrained elbow prosthesis with the reconstruction of the radial head. [ABSTRACT FROM AUTHOR]
