Background Invasive lung adenocarcinoma (IAC) occurs in 22.8% to 40.4% of pure ground-glass opacity (GGO) cases. This study assessed the malignancy and survival outcomes of IAC manifesting as pure GGO with the aim of providing suggestions on T staging of these tumors. Methods From January 2010 to December 2012, the study focused on 109 cases of IAC that radiologically manifested as pure GGO. For comparison, 305 clinical stage IA part-solid IACs were also included. Clinicopathological characteristics, managements, and prognoses were evaluated. Results As compared with part-solid nodules, pure GGOs showed lower T stage, lower N stage, smaller invasive size, less invasive predominant components, and better survival. Long-term outcomes were independently influenced by whether the tumors presented as pure GGO. For the pure GGO group, the 5-year overall and disease-free survival rates were 100% and 99.1%, respectively. The pT stage, invasive size, and predominant component type did not influence survival. Conclusions IAC radiologically manifesting as pure GGO is a group of tumors with low-grade malignancies and excellent prognosis. External validation is needed to assess whether it should be restaged in the TNM classification of non-small lung cancers.

Some clinical N0 lung adenocarcinomas have been pathologically diagnosed as N1 or N2. To improve the preoperative diagnostic accuracy of lymph node disease, we developed a prediction model for lymph node metastasis in cT1 N0 M0 lung adenocarcinoma based on computed tomography texture analysis and clinical characteristics to estimate the probability of lymph node metastasis.The records of 501 consecutive patients with cT1 N0 M0 lung adenocarcinoma who underwent computed tomography scan and pulmonary resection with systematic lymph nodes dissection or lymph nodes sampling were reviewed. Each nodule was manually segmented, and its computerized texture features were extracted. Multivariate logistic regression with fivefold validation was used to estimate independent predictors and build the prediction model. The prediction model was then externally validated. A nomogram was developed based on logistic regression results.Among 501 patients, 41 were diagnosed with positive lymph nodes (8.18%). Four independent predictors were identified: the skewness and 90th percentile of computed tomography number, nodule compactness, and carcinoembryonic antigen level. This model showed good calibration (Hosmer-Lemeshow test, p = 0.337), with an area under the curve of 0.883 (95% confidence interval, 0.842 to 0.924; p0.001). The area under the curve was 0.808 (95% confidence interval, 0.735 to 0.880) when validated with independent data.A model based on computerized textures and carcinoembryonic antigen level can assess the lymph node status of patients with cT1 N0 M0 lung adenocarcinoma preoperatively, which could assist surgeons in making subsequent clinical decisions.

Background The prognostic significance of hilar structures invasion, which remains undefined for non-small cell lung cancer (NSCLC), may have potential application for cancer staging. Tumor extension along the bronchus and pulmonary vessels was examined for survival significance. Methods In all, 213 pathologically proved central-type stage I NSCLC cases were enrolled. Four study groups were assigned based on the extent of resections: standard lobectomy (group L, n=32), bronchoplastic procedures (group B, n=94), standard lobectomy combined with pulmonary angioplasty (group A, n=48), and bronchial sleeve resection combined with pulmonary artery angioplasty (group BA, n=39). Univariate and multivariate analysis were performed by the Kaplan-Meier method and the Cox regression model. Results There were 2 postoperative deaths (pulmonary embolism and serious pulmonary infection). Complications were noted in 39 patients (18.3%). Among these patients, the overall 5-year survival rate was 60.2% ± 0.05%, with a median survival time of 75.0 ± 7.5 months. The 5-year survival rates of subgroups were 79.5%, 59.7%, 59.0%, and 47.9%, respectively for groups L, B, A, and BA. Univariate analysis indicated tumor size, bronchial invasion, arterial involvement, and type of operation as closely associated with long-term survival. Multivariate analysis indicated that type of operation and tumor size were the most prominent prognostic factors of 5-year survival. Conclusions Proximal tumor extension into bronchus, invasions into extrapericardial pulmonary vessels, and tumor size were the most important risk factors for 5-year survival with central-type stage I NSCLC. Tumor extension in the hilum was highly related to prognosis and might provide pertinent information to accurately define a tumor ("T") subclass.
