PURPOSE Despite the high success rate of implant-supported fixed restorations in dentistry, there is a lack of evidence on the marginal seal for dental cement. Thus, this study aimed to evaluate the marginal seal of implant-supported crowns and partial dentures cemented using four different dental cements. MATERIALS AND METHODS The study evaluated the marginal seal of implant-supported crowns and partial dentures cemented using zinc phosphate, resin-modified glass-ionomer, self-adhesive resin, and noneugenol, acrylic-urethane polymer-based temporary dental cements. After cementation and thermal cycling procedures, the samples were incubated in Escherichia coli suspension for 5 days at 37°C under an aerobic environment. After debonding the restorations under sterile conditions, sterile cotton swabs were used to obtain microbial samples from the inner surface of each restoration and abutment surface. To analyze the contamination, each sample was immersed in a brain-heart infusion culture medium and incubated at 37°C for 24 hours, and then, the colony-forming units were counted and recorded. RESULTS Regarding the number of colonies for Escherichia coli, the results revealed no substantial difference between the crowns and the fixed partial restorations (P = .25). However, the differences in the level of contamination between the cement groups were significant (P ≤ .001). The self-adhesive resin cement samples showed the lowest level of contamination, followed by the zinc phosphate and resin-modified glass-ionomer cements. The difference in the level of contamination between these groups was not significant. The temporary cement group exhibited significantly higher numbers of bacterial colonies in comparison to the other cement groups. CONCLUSION Self-adhesive resin cement has better biologic properties for retaining implant-supported restorations than other types of dental cement.

Purpose: This numerical study examined the efficacy and challenges of using resonance frequency analysis to identify the stability of implants placed in mandibles. The study also examined the feasibility of using angular stiffness as an alternative index to quantify dental implant stability in mandibles. Materials and methods: A finite element model consisting of a mandible, an implant, an abutment, and a bonding layer (between the implant and the mandible) was created in commercially available software ANSYS. The level of osseointegration was modeled by varying the stiffness of the bonding layer. Three sets of boundary conditions were imposed on the mandible: fixed, rotationally free, and rotationally restrained. Three implant locations were studied: central, premolar, and molar positions. An alternative abutment mimicking SmartPeg and eight different implant lengths were also included. A modal analysis and a static analysis were conducted to calculate resonance frequencies and angular stiffness, respectively. Results: Two types of vibration modes were found. One was jawbone modes, for which the mandible deformed significantly but not the bonding layer. Resonance frequencies of the jawbone modes were not sensitive to the level of osseointegration. The other was implant modes, for which the bonding layer deformed significantly but not the mandible. Among multiple implant modes obtained, only one was trackable as the level of osseointegration increased. The resonance frequency of the trackable implant mode was very sensitive to the implant location as well as boundary conditions, but not as much to the level of osseointegration. In contrast, angular stiffness was sensitive to the level of osseointegration but not as much to boundary conditions. Conclusion: The efficacy of using resonance frequency analysis to quantify the stability of a dental implant is questionable. Its high sensitivity to implant locations and boundary conditions as well as its low sensitivity to the level of osseointegration cause huge uncertainties in correlating measured resonance frequencies to implant stability. Angular stiffness is a much more reliable indicator because of its high sensitivity to the level of osseointegration and low sensitivity to boundary conditions.

This study sought to define the tissue responses at different implant-abutment interfaces by studying bone and peri-implant mucosal changes using a 5-year prospective randomized clinical trial design study. The conus interface was compared with the flat-to-flat interface and platform-switched implant-abutment systems.One hundred forty-one subjects were recruited and randomized to the three treatment groups according to defined inclusion and exclusion criteria. Following implant placement and immediate provisionalization in healed alveolar ridges, clinical, photographic, and radiographic parameters were measured at 6 months and annually for 5 years. The calculated changes in marginal bone levels, peri-implant mucosal zenith location, papillae lengths, and peri-implant Plaque Index and bleeding on probing were statistically compared.Forty-eight conus interface implants, 49 flat-to-flat interface implants, and 44 platform-switched implants were placed in 141 subjects. Six platform-switched interface and eight flatto- flat interface implants failed, most of them within 3 months. After 5 years, 33 conical interface, 28 flat-to-flat interface, and 27 platform-switched interface implants remained for evaluation. Calculation of marginal bone level change showed a mean marginal bone loss of -0.16 ± 0.45 (-1.55 to 0.65), -0.92 ± 0.70 (-2.90 to 0.20), and -0.81 ± 1.06 (-3.35 to 1.35) mm for conical interface, flat-to-flat interface, and platform-switched interface implants, respectively (P.0005). The peri-implant mucosal zenith changes were minimal for all three interface designs (0.10 mm and +0.08 mm, P.60). Only 16% to 19% of the surfaces had presence of bleeding on probing, with no significant differences (P.81) between groups. Interproximal tissue changes were positive and similar among the implant interface designs.Over 5 years, the immediate provisionalization protocol resulted in stable peri-implant mucosal responses for all three interfaces. Compared with the flat-to-flat and platform-switched interfaces, the conical interface implants demonstrated significantly less early marginal bone loss. The relationship of marginal bone responses and mucosal responses requires further experimental consideration.

Purpose: To examine the effect of the attachment features (abutment height, retention, and freedom of rotation) of implant overdentures on the bending strain around implants under an overdenture. Materials and Methods: Mandibular and maxillary edentulous models were fabricated and covered with silicone rubber. Strain gauges were attached to the four sides of each implant. Two implants (tissue-level implant, 4.1-mm diameter, 10-mm length) were placed bilaterally in the area between the mandibular canines and lateral incisors. A maxillary conventional denture and a mandibular overdenture were made to fit the edentulous models. Three levels of peri-implant bone resorption were created sequentially: no bone resorption, 0.8 mm, and 1.5 mm. Three kinds of attachments (magnetic, stud, and ball attachments) were used. Bending strains generated from a 98-N occlusal load through the maxillary denture were measured using a sensor interface, and the data were analyzed using the Kruskal-Wallis and Bonferroni tests. Multiple regression analysis was used to find the relationship between independent variables (peri-implant bone resorption level, abutment height, retention, and freedom of rotation) and the dependent variable (bending strain). Results: For all attachment types, the bending strains were smallest with no bone resorption and largest with 1.5-mm bone resorption (P < .05). Multiple regression analysis showed that abutment height had the greatest impact on the reduction of bending strain (beta = .413), followed by freedom of rotation (beta = −.349), and retention (beta = .107). Conclusion: Low abutment height, large freedom of rotation, and low retention can minimize bending strain around implants.

PURPOSE To evaluate the resistance to fracture in cantilevered fixed dental prostheses (cFDPs) of single implant-supported zirconia cantilevers in the anterior region. MATERIALS AND METHODS Thirty-two cemented and solely screw-retained cFDPs consisting of an implant-supported crown replacing the central incisor and an attached cantilever unit in the position of the lateral incisor in the maxilla were constructed by computer-aided design (CAD) and machined by computer-aided manufacturing (CAM). For the cemented solution, a cFDP was designed on top of a customized abutment luted to an adhesive base. For screw-retained cFDPs, abutment, cement gap, and restoration of the cementable design were combined. All cFDPs were veneered manually on the facial side. Half of the samples underwent artificial aging (thermocycling and chewing simulation) before fracture tests were conducted with loads applied to the pontic either parallel to the implant axis (axial loading on the pontic) or tilted lingually by α = 45 degrees (oblique loading on the pontic). Thus, there were eight groups differing in cFDP design, artificial aging, and load application (n = 8/group). If fracture (Fu,total) occurred within the implant components, the adhesive base was replaced by a cast CoCr base, and the cFDP's fracture resistance (Fu,cFDP) was also determined. Using statistical analyses (SPSS 24, IBM), factors affecting fracture resistance were identified. RESULTS Fu,total was mainly correlated to screw fractures and therefore not affected by cFDP design. Oblique loading on the pontic (Fu,total = 231 N - 352 N), however, led to a significant (P < .001) decrease in ultimate load compared with axial loading on the pontic (Fu,total = 611 N - 815 N). In relation to Fu,total, Fu,cFDP was approximately twice as high for both loading conditions. CONCLUSION When relating the results to maximum occlusal forces exerted in the maxillary anterior region, single implant-supported cFDPs can be a viable restorative treatment option.

Purpose Short implants are increasingly being used in posterior regions to avoid grafting procedures and their related risks, costs, and morbidity. However, studies focusing on implants measuring less than 6 mm long remain scarce, and the literature does not yet have a definition of the limits inherent with their use. This study attempts to investigate whether extra-short 4-mm implants are capable of receiving a maxillary single crown. Materials and methods This study evaluates, through the finite element method, the stress distribution generated at implant lengths of 4 mm and 10 mm caused by different crown heights (10, 12.5, and 15 mm) in the posterior maxilla region when submitted to axial (200 N) and oblique (100 N) loads. Results The 4-mm implant showed a similar level of performance as that of the 10-mm implant when submitted to the axial load. However, the oblique load proved to be highly detrimental to both implants, inducing stresses of up to three times higher than those achieved in conjunction with the axial load, especially in the surrounding bone and the abutment. Conclusion The use of 4-mm short implants to support single crowns in the posterior maxilla region may be feasible in habitual conditions; however, the risk associated with significant oblique loads should be mitigated by adopting a mutually protected occlusion approach and using acrylic occlusal devices if necessary.

PURPOSE To report the success of a repair service set for the retrieval of fractured screws and to compare three clinical assessments to an in vitro assessment to verify the completeness of removal of the fractured screws. MATERIALS AND METHODS Twelve clinicians were asked to remove fractured implant abutment screws from prepared specimens by means of a repair service set. The completeness of the removal of any abutment screw material was measured by the fit of an impression post, a dental surgery microscope, and the use of an elastomeric impression material. RESULTS After the participants attempted to remove the fractured screw with the repair service set, 100%, 83.3%, 66.7%, and 75% of the fractured screws were considered successful in the impression post, dental surgery microscope, silicone replica, and stereomicroscope assessments, respectively. CONCLUSION The retrieval of fractured screws succeeded in 75% of the cases. The silicone replica technique and the dental surgery microscope had similar diagnostic values to the stereoscopic microscope assessment.

PURPOSE This systematic review evaluated the mean survival rate and marginal bone loss (MBL) of dental implants with ≤ 6 mm in length, across a time frame of 5 years. The overall prosthetic and biologic complications were evaluated, and their survival rates obtained. In addition, the complication rates of the splinted vs nonsplinted implants were assessed. MATERIALS AND METHODS An electronic literature search in PubMed (MEDLINE) and EMBASE (OVID) and Cochrane were performed, in addition to a manual search through all periodontics and implantology-related journals, up to October 2017, to identify relevant articles. RESULTS Out of 515 potentially eligible articles, 19 investigations assessing a total of 910 extra-short (≤ 6 mm) implants were included and further evaluated. After 5 years of follow-up, a mean survival rate of 94.1% (90% in the maxilla and 96% in the mandible) and a maximum bone loss of 0.53 mm were demonstrated. Additionally, a statistically significant difference in terms of bone loss was observed between tissue-level (0.12 mm) and bone-level implants (0.36 mm) at 12 months (P < .01), but not between internal and external abutment connections (P = .17). The most commonly reported prosthetic complication was screw loosening. Finally, splinted implants showed less overall prosthetic complications (RR = 3.32; 95% CI: 1.9 to 5.7), screw loosening (RR = 15.2; 95% CI: 5.92 to 39.31), and implant failure (RR = 1.96; 95% CI: 0.8 to 4.8) than nonsplinted implants. CONCLUSION Extra-short implants are a viable treatment alternative in ridges exhibiting atrophy, demonstrating a satisfactory survival rate, as well as a low rate of prosthetic and biologic complications across a 5-year follow-up. Additionally, splinting extra-short implants is associated with fewer prosthetic complications and lower implant failure rate compared with nonsplinted implants.

Purpose The aim of this work was a finite element analysis of the effect of a sloped platform in tilted implants in the All-on-4 treatment concept on the level of maximum stresses in compact bone. Materials and methods Finite element modeling of the stresses at tilted microthread implants with standard and sloped abutment platform under four loading conditions and two levels of osseointegration was performed. Results The sloped abutment platform in tilted implants positioned at rim bone level resulted in a two to four times reduction of the maximum von Mises stresses in the adjacent cortical bone compared with the standard platform. These stresses were well below the yield stress of the cortical bone. The main importance of the proposed sloped platform in tilted implants is substantial stress reduction in the case of immediate loading. The stress reduction also results in the reduction of the deformation of the framework of the All-on-4 concept. Conclusion The suggested modification of the slope of the abutment platform on tilted implants is recommended as a technically simple and highly effective solution for significant reduction of maximum stresses in the cortical bone around the implant.

Purpose To quantify the rate of maintenance interventions of mandibular prostheses: complete dentures and overdentures retained by two implants, with either internal hex or external hex connections. Materials and methods This study analyzed patients with edentulous mandibles treated with distinct treatment protocols: a control group received complete dentures; a second group received Locator-retained overdentures supported by two external hex connection implants; and a third group received overdentures supported by internal hex connection implants. All of the maintenance interventions associated with the prosthetic treatment after delivery were recorded (starting from 2000 until the final assessment in 2012). Moreover, patient satisfaction was assessed using a 0 to 10 visual analog scale. Results The final sample was composed of 136 patients: 56 in the control group, 31 in the hex implant overdenture group, and 49 in the internal connection overdenture group followed over a period of 60 months (ranging from 3 to 144 months). Prosthetic adjustment was the most frequent intervention carried out, particularly within the control group (a mean rate of 1.1 ± 1.9 times/subject/year) in comparison with the internal connection (0.6 ± 0.8 times/subject/year) and hex implant overdenture groups (0.3 ± 0.2 times/subject/year). Relining was also frequently required, occurring on average every 4 years in the control group and every 7 years in the overdenture groups. Abutment loosening was more frequent in the hex implant overdenture group than in the internal connection overdenture group (8/31 versus 8/49), as well as abutment wear (5/31 versus 5/49). Gingival inflammation was significantly more common in the hex implant overdenture group (19/31 patients), as compared with the controls (16/56 subjects), and satisfaction was higher in patients in the two overdenture groups (mean Oral Satisfaction Scale [OSS] score of 8.1 ± 1.9 for external and 8.4 ± 1.6 for internal connections) than in the control group (5.3 ± 3.3). Conclusion Locator-retained overdentures needed less than half the number of maintenance interventions when compared with the control group, and maintenance was required every 2 to 4 years.

Purpose The objective of this investigation was to assess the microbiologic contamination in the inner surface of titanium implants prior to prosthetic abutment placement. Materials and methods The study population consisted of partially edentulous individuals who had previously received at least one internal hexagon titanium dental implant. A bacterial sample of the inner surface of the individual dental implant was taken after surgical reopening for healing abutment placement. The samples were allocated in order to evaluate three distinctive variables as follows: (1) location (mandible vs maxilla), (2) early exposure of implants to the oral cavity (cover screw) throughout the healing stage (exposed vs not exposed), and (3) existence or lack of keratinized mucosa (KM). The microorganism species detected were examined by checkerboard DNA-DNA hybridization. Results A total of 32 partially edentulous patients with 78 implants placed in both the maxilla and mandible were enrolled: 8 men and 24 women, ranging in age from 27 to 64 years (mean age: 47.7 years). Bacteria were detected in 20 patients, distributed in 41 implants. Spontaneous early implant exposure and absence of KM did not increase bacterial contamination in the inner surface of implants. A significant increase in the detection of 22 bacterial species was found in the mandible when compared with the maxilla. Conclusion Microbial biofilm accumulation in the implant's internal surface might happen before healing abutment placement. Exposure of implants to the oral cavity and absence of KM were not directly related to a greater microbial biofilm count. The results suggested that submerged healing does not protect implants against bacterial colonization.

PURPOSE The purpose of this study was to evaluate mouthguard shock absorption ability, strain, and stress generation during impact on dental implants placed in the anterior maxilla. The mouthguard material was also characterized. MATERIALS AND METHODS Sixty experimental models were created and divided into six groups (n = 10): implant type, external hexagon (EH) and Morse taper (MT); without and with two mouthguards (MTG), EVA custom-fitted (Ct-MTG) and standard self-adapted (St-MTG). The Shore A hardness, EVA sheet and mouthguard thickness, and elastic modulus were measured to characterize the mouthguard material. A strain gauge was attached to the palatal surface of the implant abutment, which was subjected to an impact test. Distortion of the abutment and implant was determined after impact from centroid displacements in standardized radiographic images. Two-dimensional finite element models were created to represent the six groups and were submitted to nonlinear dynamic impact analysis. The data were statistically analyzed using analysis of variance (ANOVA) and Tukey test (α = .05). RESULTS Ct-MTG resulted in higher Shore A hardness (P < .001). After the heat-forming process, the St-MTG maintained the original thickness, but the Ct-MTG thickness had decreased. The elastic modulus of EVA was 18.1 ± 0.5 MPa. The mouthguard presence reduced strain values significantly (P < .001), particularly for Ct-MTG. There was no significant difference between implant connection types EH and MT (P = .547). CONCLUSION The external hexagon abutment resulted in higher stress and micro-displacement values. The mouthguard was able to absorb 40% to 46% of the energy caused by the impact on the dental implant.

Purpose The reliability of combining natural teeth and implants in one removable prosthesis is controversial. This systematic review was conducted to evaluate the prognosis of combined tooth/implant-supported double-crown-retained removable dental prostheses (DCR-RDPs) and to compare them with solely implant-supported prostheses with a minimum observation period of 3 years. Materials and methods Electronic database (PubMed, Embase, Central, and SCI) and manual searches up to August 2016 were conducted to identify human clinical studies on tooth/implant-supported DCR-RDPs. Literature selection and data extraction were accomplished by two independent reviewers. Meta-analyses of survival and complication rates were performed separately for combined tooth/implant-supported and solely implant-supported DCRRDPs. Results Among the initially identified 366 articles, 17 were included in a quantitative analysis. The estimated overall cumulative survival rate (CSR) for implants in combined tooth/implant-supported DCRRDPs was 98.72% (95% confidence interval [95% CI]: 96.98% to 99.82%), and that for implants in solely implant-supported DCR-RDPs was 98.83% (95% CI: 97.45% to 99.75%). The summary CSR for abutment teeth was 92.96% (95% CI: 85.38% to 98.12%). Double-crown-retained dentures with both abutment types showed high CSRs, most of which were approximately 100%. Regarding prosthetic maintenance treatment, the estimated incidence for patients treated with combined tooth/implant-supported RDPs was 0.164 (95% CI: 0.089 to 0.305) per patient per year (T/P/Y) and that for patients restored with solely implant-supported RDPs was 0.260 (95% CI: 0.149 to 0.454) T/P/Y. Based on four studies with combined tooth/implant-supported DCR-RDPs, no intrusion phenomena were encountered. Conclusion Subject to the limitations of the present review, combining remaining teeth and implants in DCR-RDPs is a reliable and predictable treatment modality for partially edentulous patients. Comparable high survival rates and minor biologic or technical complications are observed for combined tooth/implant-supported and solely implant-supported DCR-RDPs. Due to the heterogeneity of the included studies, the results must be interpreted with caution.

Purpose The aim of this study was to evaluate the microgap between the abutment and implant as well as the circularity of implant platforms and associating conformational errors with bacterial microleakage in tapered connection implant systems. Materials and methods Four brands of implants with a tapered abutment connection were tested. Bacterial leakage was assessed using 0.3 μL of Escherichia coli suspension inoculated into the abutment screw chamber of the implants, which were then torqued and incubated at 37°C for 14 days. All specimens used for the microbiologic experiment were then cut lengthwise, and the microgap was measured at three points on each side of the sample using scanning electron microscopy (up to 5,000× magnification). Microtomography was used to assess implant platform circularity to validate the microscopic findings qualitatively. Results Two samples from the Nobel Biocare system, four from the Ankylos (Dentsply) system, four from the Neodent (Straumann) system, and five from the Conexao system were positive for bacterial leakage, with no significant difference between groups. The Neodent system had the highest mean microgap values (5.84 ± 9.83 μm), followed by the Nobel Biocare systems (5.17 ± 4.10 μm), Ankylos (3.47 ± 3.28 μm), and Conexao (2.72 ± 3.19 μm), with no significant difference between systems. All systems showed conformational errors of circularity on microtomography images. Conclusion The tapered connection systems evaluated herein were not able to halt bacterial leakage, nor were they free from conformational errors.

Purpose: The aim of this randomized clinical trial was to radiographically compare peri-implant marginal bone level changes between immediately loaded implants without the removal of an abutment placed at the time of surgery (IL), delayed loaded nonsubmerged implants (NS), and delayed loaded submerged implants (SI). Materials and Methods: Patients' edentulous sites were randomly allocated to the three groups of implants so that each patient received one implant of each group. Digital periapical radiographs were obtained at baseline as well as three (T1), six (T2), and nine (T3) months after implant insertion. The mesial and distal bone crest levels adjacent to each implant were measured, and the mean values were calculated for each implant at each period. The data were statistically analyzed by repeated measures analysis of variance (ANOVA) and the Tukey test (P < .05). Results: The patient sample comprised 15 subjects (13 women and 2 men; mean age: 47.7 years) with partially edentulous sites in the mandibular posterior area. Forty-five implants were inserted (15 IL implants, 15 NS implants, and 15 SI implants). There was no statistically significant difference between the three groups with regard to changes in the marginal bone level during the follow-up period. The study achieved a 93.3% cumulative survival rate for both delayed loaded groups (NS and IS) and a 100% survival rate for the IL group. Conclusion: In the 9-month period following the implants, no statistically significant differences were found between immediately and delayed loaded implants or between submerged and nonsubmerged implants in bone level changes in patients with partial posterior mandibular edentulism.

The goal of this study was to explore an innovative approach to single-tooth replacement using an individually custom-fabricated, root-analog, hybrid dental implant, in the esthetic zone, to avoid the microgap and micromovements between the implant and abutment. Moreover, the use of burs to prepare the implant recipient site is not necessary in this technique, reducing the bone removal, heating, and trauma. The process requires capturing accurate root geometry through combined computer-aided design/computer-assisted manufacturing (CAD/CAM) and a three-dimensional (3D) visualization (digital volume tomography [DVT]) of the tooth in situ, which might result in reduced remodeling after insertion. A good esthetic and functional outcome was obtained. The use of a root-shaped tooth analog implant might be in selected cases a viable alternative to current threaded cylindrical and cone-shaped implants. The new concept avoids the microgap between the implant and the abutment and reduces the trauma to the tissue and bone.

Purpose: To assess the estimated cumulative survival (ECS) and explore the technical and biologic complications of 256 TiUnite implants (Nobel Biocare) supporting one-piece cast abutment/metal-ceramic implant-supported single crowns (ISCs) in situ for up to 14 years. Materials and Methods: A prospective sequentially recruited cohort of 207 patients received 256 metal-ceramic ISCs on TiUnite implants between 2001 and 2014. All but 24 patients with 27 crowns were clinically evaluated between January 2014 and April 2015 in conjunction with or in addition to their tailored maintenance program. Radiographs were obtained, and any previously recorded treatments associated with the crowns were tabulated. The ECS and standard errors were calculated with the life table actuarial method and Greenwood's formula, respectively. The log rank test was applied to assess differences between anterior and posterior crowns. Complication incidence, severity, and economic burden, measured in time/cost accounting units (TAUs), were tallied and compared descriptively. Independent groups were compared with the Mann-Whitney U test and related groups with the Wilcoxon Signed Rank Test. Results: The mean clinical service time of the crowns was 5.61 years (44 ≥ 10 years). The 14-year ECS was 95.95% ± 3.20% with no significant difference between anterior and posterior prostheses. Only seven implants lost marginal bone ≥ one thread from the time of crown insertion. There were 30 nonterminal complications (16 biologic, 14 mechanical). The associated economic burden was low (n = 35 TAUs). Conclusion: High gold-alloy one-piece cast abutment/metal-ceramic ISCs on TiUnite implants exhibited excellent longevity and few complications over 14 years.

Purpose: Misfit is a risk factor for rehabilitation with implants, and its detection is of fundamental importance to the success of treatment with implants. The use of appropriate radiographic imaging is key for a good prognosis. The aim of this study was to compare the efficacy of film and digital radiographs for the detection of misfit at the implant-abutment interface. Materials and Methods: Digital and conventional (manual and automatic processing) radiography was performed in seven test specimens, each one with a different vertical misfit between the abutment and the platform of the implant. Scanning electron microscopy was used to confirm the misfit and to measure it. Five dental radiologists independently and blindly evaluated the images. Cohen's kappa with linear weighting was calculated to determine interexaminer and intraexaminer concordance. Statistical analyses were performed using the Cochran's Q test and the receiver operating characteristic curve (ROC). Results: Interexaminer analysis showed that the kappa value was equal to 0.74, whereas the average kappa value in the intraexaminer evaluation was 0.90. Digital imaging showed the largest area on the ROC graph, and conventional images with manual processing showed the smallest area. The images obtained through conventional radiography with both manual and automatic processing showed statistically significant differences from the measurement of the gold standard (P < .05). Conclusion: Digital imaging can be used to evaluate misfit at the implant-abutment interface. Conventional systems of radiographic imaging do not provide sufficient information to evaluate misfit at the implant-abutment interface.

Purpose: The aim of this study was to evaluate the surface characteristics and gingival fibroblast adhesion of disks composed of implant and abutment materials following brief and repeated instrumentation with instruments commonly used in procedures for implant maintenance, stage-two implant surgery, and periimplantitis treatment. Materials and Methods: One hundred twenty disks (40 titanium, 40 titaniumzirconium, 40 zirconia) were grouped into treatment categories of instrumentation by plastic curette, titanium curette, diode microlaser, rotary titanium brush, and no treatment. Twenty strokes were applied to half of the disks in the plastic and titanium curette treatment categories, while half of the disks received 100 strokes each to simulate implant maintenance occurring on a repetitive basis. Following analysis of the disks by optical laser profilometry, disks were cultured with human gingival fibroblasts. Cell counts were conducted from scanning electron microscopy (SEM) images. Results: Differences in surface roughness across all instruments tested for zirconia disks were negligible, while both titanium disks and titaniumzirconium disks showed large differences in surface roughness across the spectrum of instruments tested. The rotary titanium brush and the titanium curette yielded the greatest overall mean surface roughness, while the plastic curette yielded the lowest mean surface roughness. The greatest mean cell counts for each disk type were as follows: titanium disks with plastic curettes, titanium-zirconium disks with titanium curettes, and zirconia disks with the diode microlaser. Conclusion: Repeated instrumentation did not result in cumulative changes in surface roughness of implant materials made of titanium, titanium-zirconium, or zirconia. Instrumentation with plastic implant curettes on titanium and zirconia surfaces appeared to be more favorable than titanium implant curettes in terms of gingival fibroblast attachment on these surfaces.

Purpose: To evaluate the soft tissue response in posterior teeth adjacent to interdental single implants comparing intrasulcular and trapezoidal incision, and to study their evolution over time. Materials and Methods: A controlled randomized clinical trial was carried out in the Oral Surgery and Implantology Unit of a University Clinic. All the included patients received an interdental single implant (Frontier 2.45, Ilerimplant; Global Medical Implants). The incision type was randomized by sealed envelopes into two groups using the SPSS statistical package (SPSS): (1) intrasulcular or (2) trapezoidal incision. Probing depth and gingival recession at the mesial and distal teeth adjacent to the implant were measured before implant placement, 1 month after surgery, the day of the abutment connection, and at 6 months and 1 year postloading. Scar formation and papilla index were measured 1 month after surgery, and at 6 months and 1 year postloading. Results: Forty patients with one implant per patient were included: 20 in the intrasulcular and 20 in the trapezoidal group. No statistical differences were found between incision types in the measured parameters (probing depth, recession, and interproximal papilla). When analyzing periodontal changes of the total sample, significant differences were found between implant placement and the 1-year follow-up in recession, scar formation, and papilla index. Conclusion: The incision type used to place a single interdental implant did not significantly influence the periodontal parameters of the adjacent teeth. Considering the whole sample, the values between implant placement and 1 year postloading showed significant differences in recession, scar formation, and papilla index over time.

Purpose: Since bacterial leakage along the implant-abutment interface may be responsible for peri-implant infections, a realistic estimation of the interface gap width during function is important for risk assessment. The purpose of this study was to compare two methods for investigating microgap formation in a loaded dental implant, namely, microcomputed tomography (micro-CT) and three-dimensional (3D) nonlinear finite element analysis (FEA); additionally, stresses to be expected during loading were also evaluated by FEA. Materials and Methods: An implant-abutment complex was inspected for microgaps between the abutment and implant in a micro-CT scanner under an oblique load of 200 N. A numerical model of the situation was constructed; boundary conditions and external load were defined according to the experiment. The model was refined stepwise until its load-displacement behavior corresponded sufficiently to data from previous load experiments. FEA of the final, validated model was used to determine microgap widths. These were compared with the widths as measured in micro-CT inspection. Finally, stress distributions were evaluated in selected regions. Results: No microgaps wider than 13 μm could be detected by micro-CT for the loaded implant. FEA revealed gap widths up to 10 μm between the implant and abutment at the side of load application. Furthermore, FEA predicted plastic deformation in a limited area at the implant collar. Conclusion: FEA proved to be an adequate method for studying microgap formation in dental implant-abutment complexes. FEA is not limited in gap width resolution as are radiologic techniques and can also provide insight into stress distributions within the loaded complex.

Purpose: The aim of this noninterventional, retrospective case series was to evaluate the outcome of immediately loaded implants in patients with failing dentitions that require bone tabling using a bone reduction guide and a surgical guide manufactured directly on three-dimensional (3D) stereolithographic models. Materials and Methods: Consecutive patients with failing dentition and at least two remaining teeth who were treated in a single center between December 14, 2009 and September 23, 2013 were eligible. All patients receiving implants loaded with a hybrid prosthesis on the same day as extraction with their surgery planned on 3D models and performed using a surgical guide manufactured in a laboratory on the planning model were included. Patients who had undergone bone grafting procedures were excluded. Descriptive statistical analyses of available data were performed, including life-table calculations to derive a cumulative survival rate (CSR). Results: Two hundred twenty-eight patients (105 females and 123 males) received 1,657 implants (NobelActive) in 321 arches, in most cases 5 implants (range, 5 to 7) per arch. Ten preexisting implants were used. The mean insertion torque was 60.02 ± 13.1 Ncm (range, 15 to 75 Ncm). The definitive abutment was placed on the same day as surgery in all cases, and the definitive prosthesis (n = 304) was delivered after a mean of 7.9 ± 2.6 months. All implants were followed for 20.01 ± 11.3 months (range, 0 to 52 months) from implant insertion. Four implants (3 patients) had delayed loading, and one implant was left as a sleeping implant. Eight implants among six patients failed, two of the implants after prosthesis delivery. The CSR of the placed implants was 99.4% at implant level and 96.2% at patient level. Conclusion: Planning on 3D models to remove bone and place implants using custom-made bone reduction and surgical guides with immediate loading on the same day as extraction of remaining teeth was safe and effective for implant survival and rehabilitation of patients with periodontitis and failing dentition.

Purpose: To determine the effect of maintaining torque after mechanical cycling of abutment screws that are coated with diamondlike carbon and coated with diamondlike carbon doped with diamond nanoparticles, with external and internal hex connections. Materials and Methods: Sixty implants were divided into six groups according to the type of connection (external or internal hex) and the type of abutment screw (uncoated, coated with diamondlike carbon, and coated with diamondlike carbon doped with diamond nanoparticles). The implants were inserted into polyurethane resin and crowns of nickel chrome were cemented on the implants. The crowns had a hole for access to the screw. The initial torque and the torque after mechanical cycling were measured. The torque values maintained (in percentages) were evaluated. Statistical analysis was performed using one-way analysis of variance and the Tukey test, with a significance level of 5%. Results: The largest torque value was maintained in uncoated screws with external hex connections, a finding that was statistically significant (P = .0001). No statistically significant differences were seen between the groups with and without coating in maintaining torque for screws with internal hex connections (P = .5476). Conclusion: After mechanical cycling, the diamondlike carbon with and without diamond doping on the abutment screws showed no improvement in maintaining torque in external and internal hex connections.

PURPOSE The implant-abutment interface may affect peri-implant mucosal architecture, and influence health and esthetics. The goal of this 1-year follow-up report of a 5-year clinical investigation was to examine the peri-implant mucosal tissue responses to different implant-abutment interface designs. MATERIALS AND METHODS Subjects requiring an anterior maxillary implant were recruited. Tooth extractions, with or without preservation or ridge augmentation procedures, were performed as required. After 5 months of healing, one of three different implant-abutment combinations (conical interface [CI] n = 48); flat-to-flat interface [FI] n = 49); or platform switch interface [PS] n = 44) was placed and provisionalized. Twelve weeks later, permanent crowns were placed and data gathered throughout the first year. Peri-implant mucosal architecture and bone levels were evaluated clinically, photographically, and radiographically. RESULTS At 1 year, seven FI and six PS implants failed and two FI and two PS implant participants were lost to follow-up, resulting in survival rates of 100% (CI), 85.7% (FI), and 86.4% (PS) (90.8% overall). Marginal bone level changes were -0.22 mm (CI, P < .05), -1.2 mm (FI, P < .05), and -1.32 mm (PS, P < .05) after 1 year. Marginal bone level stability (≤ 0.5-mm bone loss or gain) was recorded for 87% (CI), 8% (FI), and 27% (PS) of implants. Measurement of midbuccal mucosal zenith and papilla positions revealed no change in the mucosal positions and 0.2 to 0.3 mm of gain in papilla dimensions in all groups. CONCLUSION Significant differences in marginal bone loss were observed among the three implant-abutment interfaces. At 1 year follow-up, changes in the buccal mucosal zenith position or papilla dimensions were not discernable. A continued longitudinal evaluation of peri-implant bone and mucosal changes around these different interfaces is ongoing.

Technical complications in implant prosthetic cases represent a major challenge in dentistry. This case report describes minimally invasive management to recover an implant with a fractured remnant of a zirconia abutment, including provisional rehabilitation during a sequential treatment protocol in the esthetic zone. A patient was treated with a screw-retained one-piece implant-supported reconstruction made of a customized zirconia abutment with direct ceramic veneering in the maxillary right central incisor position. During the prosthetic try-in, a fracture in the apical portion of the abutment was evident. The first rescue attempt led to fracture of the retrieval instrument. Immediately, an individualized wired construction was applied to bond the existing fractured reconstruction to the neighboring teeth to maintain the peri-implant mucosal architecture. Because the implant screw canal was blocked, a customized round bur had to be manufactured and was placed in the implant axis with a specific bracket tool from the service set to protect the interior implant threads. Then, the drills of the service set were guided by the newly created access to remove the fractured remnants. The implant screw was retapped and the area rinsed with chlorhexidine solution. All remnants were removed without the need for surgical intervention. Neither the implant connection nor the bone-to-implant interface was damaged. The stepwise treatment approach with the customized round bur combined with the system-specific drills of the service set saved the blocked implant so that the patient could be successfully rehabilitated with a new implant reconstruction.

Purpose: The primary objective of this study was to investigate the fracture resistance of experimental mini-implants with a reduced abutment height. The secondary objective was to assess the effects of implant diameter and bone level on the load limit, using finite element simulations. Materials and Methods: Two Ti-6A1-4V 1.8-mm–diameter implants were subjected to monotonic bending testing and fatigue tests incorporating 5 × 106 cycles (ISO 14801): a commercially available implant (c18), and an experimental implant with a reduced abutment height (e18). The load limit was estimated using the finite element models based on the maximum stress at failure in the experiments. For simulations, implants with increased diameters of 2.1 and 2.4 mm were also modeled, and the load limit was estimated for all models in a bone model. Results: In the bending test, e18 revealed a higher mean load at yield stress than c18, and this was attributed to the reduced height of the former. An endurance limit of 140 N was detected for both c18 and e18 in the fatigue test, while the load limit of e18 was higher than that of c18. The estimated load limit increased as the implant diameter or the bone level increased, with the highest value of 510 N observed at a diameter of 2.4 mm. Conclusion: A higher load limit was evident in the experimental mini-implant with a reduced abutment height. PURPOSE To assess the survival outcomes and reported complications of screw- and cement-retained fixed reconstructions supported on dental implants. MATERIALS AND METHODS A Medline (PubMed), Embase, and Cochrane electronic database search from 2000 to September 2012 using MeSH and free-text terms was conducted. Selected inclusion and exclusion criteria guided the search. All studies were first reviewed by abstract and subsequently by full-text reading by two examiners independently. Data were extracted by two examiners and statistically analyzed using a random effects Poisson regression. RESULTS From 4,324 abstracts, 321 full-text articles were reviewed. Seventy-three articles were found to qualify for inclusion. Five-year survival rates of 96.03% (95% confidence interval [CI]: 93.85% to 97.43%) and 95.55% (95% CI: 92.96% to 97.19%) were calculated for cemented and screw-retained reconstructions, respectively (P = .69). Comparison of cement and screw retention showed no difference when grouped as single crowns (I-SC) (P = .10) or fixed partial dentures (I-FDP) (P = .49). The 5-year survival rate for screw-retained full-arch reconstructions was 96.71% (95% CI: 93.66% to 98.31). All-ceramic reconstruction material exhibited a significantly higher failure rate than porcelain-fused-to-metal (PFM) in cemented reconstructions (P = .01) but not when comparing screw-retained reconstructions (P = .66). Technical and biologic complications demonstrating a statistically significant difference included loss of retention (P ≤ .01), abutment loosening (P ≤ .01), porcelain fracture and/or chipping (P = .02), presence of fistula/suppuration (P ≤ .001), total technical events (P = .03), and total biologic events (P = .02). CONCLUSIONS Although no statistical difference was found between cement- and screw-retained reconstructions for survival or failure rates, screw-retained reconstructions exhibited fewer technical and biologic complications overall. There were no statistically significant differences between the failure rates of the different reconstruction types (I-SCs, I-FDPs, full-arch I-FDPs) or abutment materials (titanium, gold, ceramic). The failure rate of cemented reconstructions was not influenced by the choice of a specific cement, though cement type did influence loss of retention.

Purpose: The objectives of this review were to (1) identify if prosthodontic parameters influence the esthetic outcome of implant-supported restorations and (2) make clinically relevant recommendations based upon the findings. Materials and Methods: Electronic and manual searches of dental literature were performed to collect information on esthetic outcomes based on objective criteria. The prosthodontic parameters included optimal three-dimensional implant position, the utilization of provisional restorations, the timing of provisional restoration with regard to implant placement, the choice of prosthodontic platform size and form, the abutment and definitive restoration material, and the mode of prosthesis retention. Regions including maxillary and mandibular anterior teeth and premolars were considered. All levels of evidence, including case studies, were accepted. Results: From 472 titles, 152 full-text articles were evaluated and 58 records included for data extraction (15 randomized controlled trials, 6 cohort studies, and 37 case series studies). Considerable heterogeneity in study design was found. A meta-analysis of controlled studies was not possible. It was consistently reported that facial malpositioning of implants increases the likelihood of mucosal recession. No studies directly compared esthetic outcomes associated with the use or non-use of provisional restorations. The literature contains a greater number of case series studies evaluating esthetic outcomes for protocols including, rather than excluding, provisional restorations. It is not possible to identify any significant variation in esthetic outcomes based on the character of the abutment platform from the current literature. Based on the findings, no significant difference can be established between all-ceramic and metal-ceramic prostheses with regard to esthetic indices over short observation periods. No firm conclusions relating esthetic benefits for cement in comparison to screw retention can be identified. Conclusions: There is a need for RCTs comparing accepted procedures in routine practice. The utilization of provisional restorations remains strongly recommended in order to trial the planned definitive restoration, to facilitate maturation of healing tissues and for patient convenience. Implant positioning according to the planned prosthesis remains a requirement to achieve a long-lasting esthetic outcome. The majority of studies reported on single-tooth replacement, and many of the outcomes may not be relevant or applicable to the large number of esthetic indications involving more than one tooth.

PURPOSE: To evaluate the marginal and internal adaptation of five-unit implant-supported fixed partial dentures with different superstructure and abutment materials with two different implant systems. MATERIALS AND METHODS: Two mandibular epoxy resin models (one for each implant system) were fabricated, and implants were inserted in the canine, first premolar, and first molar regions. Thirty impressions of each model were then made. The 30 models of each group were divided into three subgroups: group 1, titanium abutment with metal (nickel-chromium) framework; group 2, titanium abutment with zirconium framework; and group 3, zirconium abutment with zirconium framework. The replica technique was used to examine the marginal and internal gap values. For each restoration, 20 measurements were performed, and the data were evaluated statistically using analysis of variance and the least significant difference post hoc test. RESULTS: The highest values for internal adaptation were observed at the occlusal surface in all groups. There were statistically significant differences in marginal measurements between subgroups. The smallest gaps were found in group 1 (79.361 μm), and the largest gaps were found in group 3 (131.242 μm). CONCLUSIONS: In this study, marginal and internal gap measurements were 79 to 131 μm. The marginal discrepancy of the tested materials could be considered clinically acceptable.

Purpose The aim of this case series is to propose an approach to help maintain autogenous bone grafts. This is done by applying a collagen membrane (CM) and anorganic bovine bone (ABBM) at the time of implant surgery, rather than at the time of ridge augmentation, to avoid volume loss after implants are inserted. Materials and methods Ten patients with severe horizontal bone atrophy were consecutively enrolled in this study. A staged approach was chosen for implant placement following horizontal ridge augmentation. A block graft was harvested from the retromolar area and secured to the recipient site with fixation screws; contour overbuilding was avoided. The width of the ridge was measured before and after horizontal augmentation. After 4 months of healing, implants were inserted, the augmented site was relined with ABBM, and CM was applied to prevent bone volume loss. Another 4 months later, at the time of abutment placement, cone beam computed tomography was performed to quantify the end result. Results The mean horizontal ridge width prior to treatment was 2.1 ± 0.5 mm. Mean postsurgical crest width was 6.9 ± 0.5 mm. After the 4-month healing period, the mean alveolar crest width was 6.6 ± 0.6 mm. At the time of abutment connection, the mean width of the regenerated ridge, as measured on three-dimensional cone beam images, was 7.7 ± 0.8 mm. Conclusions Minimal bone loss occurred in unprotected autogenous bone grafts with respect to alveolar bone contour (0.25 ± 0.29 mm). After the implants were inserted, no further remodeling/resorption occurred with sites treated by ABBM and CM relining; moreover, an additional increase in alveolar crest width was evident. The nonresorbable ABBM osseointegrated clinically and radiologically, preventing bone loss prior to implant loading. This layer appeared to maintain the regenerated crest volume.

Purpose: Three-dimensional finite element analysis was conducted to evaluate and compare the stress distribution in the abutment and retention screw of implant-supported single crowns with platform switching and with a conventional platform under vertical and oblique loading. Materials and Methods: Two finite element models were created simulating an osseointegrated implant (4 × 10 mm, platform 4.1 mm) embedded in jawbone. One model simulated a 4-mm-diameter abutment connection (conventional model) and the other represented a 3.8-mm-diameter abutment connection (platform-switched model). A crown with a cobalt-chromium framework and feldspathic porcelain veneering was applied to the titanium abutment. Static vertical and oblique loads were applied to the crown, with a maximum load of 150 N. Results: In both models, the highest stress values occurred in the abutment during vertical and oblique loading. Nevertheless, the von Mises stresses in the abutment and the retention screw were lower in the platform-switched model than in the conventional model. During axial loading, the abutment and screw supported slightly less stress in the conventional model than in the platform-switched model. Increases in the angle of force application caused a progressive increase in stresses in the abutment and screw in both models. The maximum stress was distributed at the margin and transgingival area of the abutment and on two-thirds of the flat area and the first threads of the retention screw in both models. Conclusions: Platform switching reduced the stress values on the abutment and retention screw of a single-unit prosthesis during oblique loading. Regardless of whether platform switching was employed, the stress on the abutment and screw gradually increased as the loading direction changed from vertical to 45 degrees oblique. PURPOSE The crestal bone level around a dental implant may influence its strength characteristics by offering protection against mechanical failures. Therefore, the present study investigated the effect of simulated bone loss on modes, loads, and cycles to failure in an in vitro model. MATERIALS AND METHODS Different amounts of bone loss were simulated: 0, 1.5, 3.0, and 4.5 mm from the implant head. Forty narrow-diameter (3.0-mm) implant-abutment assemblies were tested using compressive bending and cyclic fatigue testing. Weibull and accelerated life testing analysis were used to assess reliability and functional life. Statistical analyses were performed using the Fisher-Exact test and the Spearman ranked correlation. RESULTS Compressive bending tests showed that the level of bone loss influenced the load-bearing capacity of implant-abutment assemblies. Fatigue testing showed that the modes, loads, and cycles to failure had a statistically significant relationship with the level of bone loss. All 16 samples with bone loss of 3.0 mm or more experienced horizontal implant body fractures. In contrast, 14 of 16 samples with 0 and 1.5 mm of bone loss showed abutment and screw fractures. Weibull and accelerated life testing analysis indicated a two-group distribution: the 0- and 1.5-mm bone loss samples had better functional life and reliability than the 3.0- and 4.5-mm samples. CONCLUSION Progressive bone loss had a significant effect on modes, loads, and cycles to failure. In addition, bone loss influenced the functional life and reliability of the implant-abutment assemblies. Maintaining crestal bone levels is important in ensuring biomechanical sustainability and predictable long-term function of dental implant assemblies.

PURPOSE: Scalloped implants were developed to better maintain marginal bone. Therefore, this study evaluated marginal bone levels (MBLs) and soft tissue around single implants with scalloped necks in the esthetic zone prospectively over a 3-year period. MBLs were compared to those of conventional flat-platform implants at 1 and 3 years. MATERIALS AND METHODS: Patients received anterior single-tooth implants and were divided into two groups; the test group included implants with a scalloped neck design and the control group included conventional rough-surface implants with external hex. MBLs were compared radiographically at 1 and 3 years after crown insertion. For the test group, soft and hard tissue assessments were made at six different time points, beginning at abutment connection. Mesial and distal interproximal papillae and probing depths were assessed clinically. Data were analyzed with the Student t test and repeated measures analysis of variance on ranks (P < .05). RESULTS: Twenty-four patients (17 men, 7 women) took part; each group included 12 implants. The test group had significantly more marginal bone resorption than the control group at each time point (P < .001). With respect to changes in MBL values over time, a significant difference was detected between groups mesially (P < .001) but not distally. Statistical analyses demonstrated obvious significant increases with time (P < .001) in bone resorption and mesial papilla index. A significant increase over time in probing depths was demonstrated distally (P = .003) but not mesially. CONCLUSIONS: The scalloped implant did not stabilize interproximal bone remodeling and caused more bone loss than conventional implants. Soft tissue levels were not maintained around the scalloped implants.

Purpose Misfit in the conical implant-abutment interface plays an important role on the mechanical behavior of the implant when masticatory forces are applied. The origin of the misfit adopted in this work is a conical angle difference between implant and abutment, which can be due to a combination of design decisions and manufacturing tolerances. The goal of this work was to investigate the effects of the implant-abutment conical angle difference in the following mechanical features: interfacial microgap, preload loss on the bolt, stress level in the bone, and abutment removal force and/or torque. Materials and methods A simplified three-dimensional nonlinear monoparametric finite element model of an OsseoSpeed TX 4.5 S 9-mm implant (Astra Tech) with a tapered implant-abutment interface was built to evaluate the variability of the mechanical features cited above with the conical angle difference, keeping constant the overall geometry, load and boundary conditions, material properties, frictional behavior, and mesh structure. Results As the conical angle difference increased, the following effects were observed: the microgap decreased and remained almost constant for values over a given positive angle difference, the stress level in the bone increased sensitively, the removal force and/or torque needed to separate the abutment from the implant varied slightly, and the bolt preload loss increased. Conclusions In light of the results provided, the conical angle difference in the implant-abutment interface had a significant influence on the overall mechanical behavior of the implant. Among the four mechanical features considered, the interfacial microgap and the bone stress were demonstrated to be the most sensitive to the conical angle difference, and therefore the most relevant when selecting an optimum value in the design process of a conical interface.

PURPOSE The aim of this study was to compare the influence of abutment screw preload on the marginal bone stress around a conventional external-hex implant system and one with a new implant/abutment joint design. MATERIALS AND METHODS Two implant/bone models, each consisting of three parts and fabricated of titanium and bone (cortical), were built and arranged with computer-aided design software. The first model represented an external-hex implant system, while the second had a tapered extension over the external hex, 1.5 mm high, so that the apical extension of the abutment screw in the assembly did not go beyond the lower level of the implant shoulder. Meshing and generation of boundary conditions, loads, and interactions were performed for the two models. All parts were meshed independent of each other. The sole load applied on the model was a torque of 32 Ncm on the abutment screw about its axis of rotation. RESULTS The first model showed deformation of the implant collar, and the resulting von Mises stress was 60 MPa in the marginal bone. In contrast, no deformation was observed in the second model. CONCLUSIONS The new implant/abutment joint eliminated the deformation at the implant collar area caused by the application of tightening torque and thus eliminated the resulting stresses in the marginal bone.

PURPOSE The objective of this study was to evaluate sinus floor augmentation with simultaneous implant placement using platelet-rich fibrin (PRF) as the only grafting material. MATERIALS AND METHODS This study included patients who underwent sinus floor augmentation with simultaneous implant placement using PRF as the sole filling material between July 2009 and January 2011 at the Department of Oral and Maxillofacial Surgery, Nagasaki University Hospital. For each patient, presurgical and postsurgical (6 months after the surgery) radiography and computed tomographic scanning were performed to assess bone formation at the implant sites. The density (in Hounsfield units [HU]) of the newly formed bone and the bone height from the sinus floor to the alveolar crest where implants were inserted were measured using implant planning software (Simplant, Materialise Dental). RESULTS Nine sinus floor augmentations were performed, and 17 implants were placed in six patients. The mean residual bone height between the sinus floor and alveolar crest was 4.28 ± 1.00 mm (range, 1.9 to 6.1 mm) prior to surgery and 11.8 ± 1.67 mm (range, 9.1 to 14.1 mm) after surgery. The alveolar bone ridge was wide enough for implant placement in all cases. The mean density of the newly gained bone around the implants was 323 ± 156.2 HU (range, 185 to 713 HU). All implants were clinically stable at the time of abutment insertion, 6 months after sinus augmentation. CONCLUSION Sinus elevation with simultaneous implant placement using PRF as the only filling material may promote natural bone regeneration.

PURPOSE: To evaluate the mechanical properties and behavior of a self-locking taper connection with three different techniques: three-dimensional finite element analysis (FEA), ultimate failure loading, and cyclic loading analysis. MATERIALS AND METHODS: The implant-abutment complex was embedded vertically in the center of an acrylic resin support block (Young's modulus > 3 GPa). All materials used in this study were assumed to be homogenous and isotropic, but while the resin was assumed to be linearly elastic, the titanium was assumed to have a multilinear behavior to better represent the implant system in its plastic phase and to compare as closely as possible the numeric simulation with the experimental tests. An 800-N 30-degree off-axis load was applied to the occlusal surface of the abutment. In addition to the FEA, static and dynamic tests were carried out. RESULTS: The greatest von Mises stresses were concentrated in the coronal portion of the abutment's tapered connection, while at the implant neck they were lower and less extensive than the abutment ones. Experimental results confirmed the FEA findings, in which the structural limit of the system was reached, with permanent deformation of the abutment that exceeded a predefined limit, rather than fracture. CONCLUSION: Within the limitations of the reported analyses, these static and dynamic tests appear to supply congruent results, thus allowing evaluation of the mechanical behavior of a self-locking tapered-connection implant system. High resistance to an off-axis load was exhibited, exceeding that usually offered by screw-retained implant systems, thus indicating good stability of the implant-abutment connection.
