Objective: To determine the emerging antibiotic resistance pattern of adult patients of MDR UTI cases caused by E.Coli. Study Design: Retrospective Data. Setting: Departments of Medicine and Obstetrics & Gynecology, Madinah Teaching Hospital / University Medical and Dental College, Faisalabad. Period: August 2016 to Sep 2018. Material & Methods: This retrospective study included 187 patients meeting the operational definition of multi-drug resistance cases of urinary tract infection caused by E. coli. All cases either male or female of age above 15 years with positive reports of urine culture and sensitivity for E.coli were included in the study. Along with the demographic variables urine WBC’s, results of urine culture and sensitivity were noted. Antibiotic sensitivity pattern of all the E.coli resistant UTI cases was analyzed. After identification of the microbe, antimicrobial sensitivity was accessed using Kirby–Bauer disc diffusion method on the Mueller–Hinton agar. The antimicrobial susceptibility test was performed against E. coli strains by using the following antibiotics: Piperacilin, Amoxicilin-Calvulanic, Pipmedic Acid, Cefipime, Ceftrixone, Cefuroxime, Cefoperazone-sulbactam, Ceftazidime, Cephradine, Ciprofloxacin, Ofloxacin, Levofloxacin, ofloxacin, Nitrofurantoin, Amikacin, Tombramycin, Gentamicin, Linezolid, Doxycycline, Sulphamethoxazole, Imipenem, Meropenem, Aztreonam in order to obtain an antibiogram. Results: In our data, 66.8% (n=125) were MDR UTI, and more than half of these cases were 63.2% (n=79) were females. Among the total 187 cases, 97.3% patients were resistant to most of the beta-lactam antibiotics, 95.7% were resistant to most of the quinolones and 68.4% were resistant to most of the aminoglycosides. Conclusion: This increasing antibiotic resistance is very alarming and steps should be taken to reduce the misuse of antibiotics. Studies should be done to control the risk factors leading to E. coli UTI. By following proper pharmacological guidelines, principles to use antibiotics and manage outpatient cases of UTIs, the incidence of MDR UTI can be controlled.

The study was to compare efficacy of parenteral versus oral vitamin D replacement in hypovitaminosis. Study Design: Randomized trial. Setting: Medical Outpatient Clinics of Madinah Teaching Hospital, Chiniot General Hospital and Maqsooda Zia Hospital, Faisalabad. Period: 6 months (Oct 2017 – Apr 2018). Material & Methods: 84 patients were included in the study. Baseline 25(OH) D levels were determined, and followed-up at 3rd and 6th weeks following vitamin D replacement. After giving the first dose of vitamin D (parenteral or oral), patients were given maintenance dose of calcium and vitamin D supplement as per recommended daily allowance (RDA). Patients with significant clinical improvement were also noted in both groups. Results: The change in vitamin D level after 3 weeks and 6 weeks of replacement through oral route and intramuscular (IM) route was compared; which was found to be statistically significant in both groups (p value < 0.05). Mean change in vitamin D levels after 6 weeks of replacement in all the patients was 17.96 + 13.0. In oral group, it was 13.5 + 10.07 and in IM group, it was 22.40 + 14.18. This clearly shows that it was higher in the IM group compared to the oral group. This difference was statistically significant (p=0.001). The percentage change in the serum 25-OH D level was 53% and 79% for oral group compared to 103% and 207% for the IM group, y after 3 and 6 weeks of replacement respectively. Conclusion: While managing hypovitaminosis D, IM route of administration is more effective. There was significant improvement in the serum 25OHD levels in the IM group. A larger randomized control trial should be done comparing the efficacy of oral and IM route of vitamin D replacement.

Objectives: To identify the bacterial spectrum of SBP in patients with cirrhosis of liver. Study Design: Descriptive Cross-sectional study. Setting: Department of Gastroenterology and Hepatology, Madina Teaching Hospital, Faisalabad. Period: 1st November, 2018 – 30th June, 2019. Material & Methods: Two hundred and fifty cirrhotic patients having ascites and clinical symptoms and signs of SBP, consecutively admitted in the ward or presented to OPD were included in the study. Results: A total of 250 patients included in this study. SBP was diagnosed according to the predetermined criteria in 140 patients. The culture-negative ascites was found in 77 (55%) and culture-positive ascites was found in 63 (45%) patients. The most common organism was E.Coli 28 (47.45%) followed by Enterococcus 10 (16.94%), Klebsiella Pneumoniae 6 (10.17%), Streptococcal Pneumoniae 5 (8.47%), S.aureus 5 (8.47%), Acinetobacter 3 (5.08%) and miscellaneous 2 (3.38%). Conclusion: The percentage of SBP is quite high among patients having cirrhosis and ascites and the Gram-negative bacterial spectrum is still much common in our region.

Hepatic encephalopathy (HE), a syndrome observed in some patients with cirrhosis, with depressed level of consciousness. Lactulose as well as lactitol has been used in the treatment of HE. Lactitol is comparable to lactulose in the treatment of HE with fewer side effects and better tolerated. However, literature showed equal efficacy of both drugs. So we conducted this trial to find better drug to implement its use in future. Objective: To compare the effectiveness of lactulose and lactitol in patients with acute hepatic encephalopathy. Study Design: Randomized Controlled Trial. Setting: Department of Medicine OPD and Emergency (East, West, North, South), Mayo Hospital, Lahore. Period: 6 Months June 2017 to Dec 2017. Material & Methods: 570 patients were included through non-probability, consecutive sampling after informed consent. Initial grade of HE was assessed and patients were randomly divided in two groups by using lottery method i.e. lactulose or lactitol. Patients were admitted to ward for management and kept under observation for 5 days. After 5 days, HE grades was measured again, then improvement in grade of HE (effectiveness) was measured. All data was entered and analyzed by using SPSS version 21.0. Chi-square was applied to compare both groups for effectiveness taking p-value≤0.05 as significant. Results: In this study the mean age of the patients was 44.22 ±11.81 years, the male to female ratio of the patients was 2.4:1. The mean duration of the cirrhosis of the patients was 3.73±1.61 months. For children below age of five urinary tract infection (UTI) is the foremost common sources of infection. To lessen the danger of renal scarring it is critical to have immediate diagnosis and treatment. Quick, cost-effective, methods for the analysis of UTI are needed as substitute to culture. Objectives: To determine the diagnostic accuracy of urine dipstick in detection of UTI in children keeping urine culture as a gold standard. Study Design: Cross sectional validation study. Setting: Outpatient Department, Madinah Teaching Hospital, Faisalabad. Period: 01-2018 to 07-2018. Material & Methods: After taking permission from the hospital ethical committee all female patients with urinary tract infection were included. A rapid dipstick analysis for leukocyte esterase and nitrite was done by a staff nurse using Meditest Combi 10 SGL. The reaction of dipstick strip was read visually by a trained nurse who has five year experience working in the Outpatient Department, Madinah Teaching Hospital, Faisalabad. All dipstick positive and dipstick negative samples underwent cultures by sending samples in a clean container to the pathology lab via a sanitary attendant within one hour of the urine collection. Results: In our study, 69.21%(n=281) were between 2 months to 3 years of age while 30.79%(n=125) were between 4-5 years of age, mean+sd was calculated as 2.63+1.36 years, frequency of UTI in children on urine culture as a gold standard was recorded in 20.69%(n=84), the diagnostic accuracy of urine dipstick in detection of UTI in children keeping urine culture as a gold standard was recorded as 80.95%, 62.42%, 35.98%, 92.63% and 66.26% for sensitivity, specificity, positive predictive value, negative predictive value and accuracy rate. Conclusion: We conclude that the diagnostic accuracy of urine dipstick in detection of UTI in children keeping urine culture as a gold standard is higher and this cost effective and easy technique may be used in our routine practice for prediction of UTI in children.

Objectives: To determine the frequency of esophageal variceal bleed in patients with upper GI bleed presenting in a tertiary care hospital. Study Design: Cross sectional survey. Setting: Department of Medicine, Mayo Hospital, Lahore and Madinah Teaching Hospital, Faisalabad. Period: 6 months from 01-06-2018 to 31-12-2018. Material & Methods: Two hundred patients with upper GI bleed were included in the study. All the patients had upper GI endoscopy to determine the esophageal variceal bleeding which was documented as frequency distribution table. Results: Esophageal variceal bleed was seen among 108 (54%) patients while it was not present among 92 (46%) patients. Conclusion: Esophageal variceal bleeding is frequently present among patients with upper GI bleeding and should be suspected in every patient with upper GI bleeding.

Urinary tract infections (UTIs) are a major burden to the health care as it is estimated that around 150 million UTIs occur yearly worldwide. Enterobacteriaceae are the most common agent causing serious urinary tract infections; and MDR cases are increasing day by day. Objectives: To determine the frequency and patient demographics of multidrug resistant urinary tract isolates of Escherichia Coli in a Tertiary Care Hospital. Study Design: Retrospective cross sectional study. Setting: Medicine Department of Madinah Teaching Hospital / The University of Faisalabad, Faisalabad. Period: From May 2016 to Sep 2018. Material & Methods: 187 patients of age 15 to 90 years with positive E. coli on urine culture and sensitivity were included in this Retrospective cross sectional study. All those patients with history of dysuria (pain during urination) or frequent urination (more than 7 times per day) were advised urine complete analysis and those with >5 WBCs or pus cells /HPF or having positive for leukocyte esterase and/or nitrite, were advised urine culture and sensitivity. Main outcome variable was the frequency of MDRE cases among the culture positive E. coli UTI patients. Results: Among all the cases of E. coli UTI, frequency of MDRE UTI was 66.8% (n=125) and rest 33.2% (n= 62) cases were not MDR UTI. 97.3% patients were resistant to lactam antibiotics, 95.7% were resistant to quinolones and 68.4% were resistant to aminoglycosides.
