Simulation-based teaching is gaining increasing importance in veterinary education worldwide and is by now an integral part of all German veterinary educational institutions in the form of Skills Labs. Students and teachers of the University of Veterinary Medicine Hannover, Foundation (TiHo) showed a good acceptance for the establishment and curricular use of a Clinical Skills Lab (CSL). This study will now examine the perspectives of non-university stakeholders in order to assess the acceptance of such a facility among clinicians. In addition, experiences in implementing a CSL in veterinary teaching are taken into account by interviewing experts.Semi-structured guided interviews were conducted with alumni of the TiHo, senior veterinarians from various German clinics and practices and leading experts from some of the largest veterinary CSLs worldwide.The interviews revealed a need for improvement in the clinical-practical training of veterinary students among senior veterinarians and alumni. Respondents could imagine that teaching in the CSL may play a major role in this. Overall, the participants pleaded for a mandatory implementation of simulation-based teaching in the curriculum.The study suggests that the training of clinical-practical skills of veterinary students should be continuously adapted to the requirements of the veterinary profession in a clinical environment. Non-university stakeholders seem to support the use of a CSL to improve these skills and encourage its further implementation in the curriculum.Die simulationsbasierte Lehre hat in der tiermedizinischen Ausbildung weltweit immer mehr Bedeutung und ist mittlerweile in Form von Skills Labs ein fester Bestandteil an allen deutschen tiermedizinischen Bildungsstätten. Studierende und Dozierende der Stiftung Tierärztliche Hochschule Hannover (TiHo) zeigten eine gute Akzeptanz für den Aufbau und die curriculare Nutzung eines Clinical Skills Lab (CSL). Weiterführend sollen nun die Perspektiven hochschulexterner Interessenvertreter untersucht werden, um die Akzeptanz einer solchen Einrichtung unter klinisch tätigen Tierärzten/-innen zu erfassen. Zusätzlich werden durch die Befragung von Experten/-innen deren Erfahrungen bei der Implementierung eines CSL in die tiermedizinische Lehre berücksichtigt.Es wurden semistrukturierte Leitfadeninterviews mit Alumni der TiHo, mit leitenden Tierärzten/-innen verschiedener Kliniken und Praxen in Deutschland sowie mit leitenden Experten/-innen einiger der größten tiermedizinischen CSLs weltweit durchgeführt.Die Interviews zeigten einen Bedarf bei leitenden Tierärzten/-innen und Alumni nach einer Verbesserung der klinisch-praktischen Ausbildung von Tiermedizinstudierenden und die Befragten konnten sich vorstellen, dass die Lehre im CSL hierbei eine tragende Rolle spielen könne. Insgesamt plädierten die Teilnehmenden für eine verpflichtende Implementierung simulationsbasierter Lehre im Curriculum.Die Untersuchung legt nahe, dass die Ausbildung klinisch-praktischer Fertigkeiten von Studierenden der Tiermedizin laufend an die Anforderungen der klinischen tierärztlichen Berufstätigkeit angepasst werden sollte. Hochschulexterne Interessenvertreter scheinen den Einsatz eines CSL zur Verbesserung der entsprechenden Fertigkeiten zu unterstützen und eine weitergehende Implementierung in das Curriculum zu befürworten.

As part of a study on competence-based assessment, students of the University of Veterinary Medicine, Hannover (TiHo), were asked to fill in an online questionnaire regarding their exam preparation and common examination formats. The aim of this investigation was to attain the students' opinion on different exam formats as well as the intensity and method of their preparation in order to further improve and develop the assessment formats.Data was collected using an online questionnaire developed in LimeSurveyA total of 215 questionnaires were completed and entered the evaluation. It was shown that the duration of the exam preparation depended on the specific exam format. Students' preparation time for an oral exam was longer than for a written-electronic exam. In the students' view, the assessment contents of oral examinations appeared to be more "relevant for later professional life". Electronic exams, on the other hand, were perceived as being more "fair" and less stressful. 30.3 % of respondents indicated to be suffering from test anxiety.Based on these findings, new recommendations for the development of teaching/learning materials may be derived. The opinions of students collected in this investigation provide an up-to-date insight into their perception of examinations. These results may serve as a valuable contribution to the optimization of examination scenarios in veterinary education. In addition, such surveys contribute to university-wide quality management of the examination systems.Im Rahmen einer Studie zur Kompetenzorientierung in Prüfungen wurde an der Stiftung Tierärztliche Hochschule Hannover (TiHo) eine Befragung der Studierenden zur Prüfungsvorbereitung und zu den Prüfungsformaten vorgenommen. Das Ziel dieser Studie war die Bewertung der eingesetzten Prüfungsformate. Erfasst wurden auch die Intensität und Art der Vorbereitung auf die Prüfungen, um aus den gewonnenen Daten eine Weiterentwicklung von Prüfungsformaten umsetzen zu können.Die Datenerhebung erfolgte mithilfe eines mit LimeSurveyFür die Auswertung standen insgesamt 215 vollständig ausgefüllte Fragebögen zur Verfügung. Es zeigte sich, dass die Dauer der Prüfungsvorbereitung vom Prüfungsformat abhängt. So bereiten sich Studierende deutlich länger auf eine mündliche Prüfung vor als auf eine schriftlich-elektronische Prüfung. Die Prüfungsinhalte mündlicher Prüfungen erscheinen den Studierenden „berufsrelevant“, während elektronische Prüfungen als „gerechter“ und weniger belastend empfunden werden. 30,2 % der Befragten gaben an, unter Prüfungsangst zu leiden.Aus den Erkenntnissen können neue Handlungsempfehlungen zur Erstellung von Lehr-/Lernmaterialien abgeleitet werden. Das erhobene Meinungsbild der Studierenden bietet einen aktuellen Einblick in die Wahrnehmung von Prüfungen. Dies kann als wertvoller Beitrag zur Optimierung von Prüfungsszenarien in der tiermedizinischen Ausbildung dienen. Gleichzeitig stellen solche Umfragen einen Beitrag für das hochschulweite Qualitätsmanagement im Prüfungswesen dar.

Exercise induced metabolic myopathy in German Hunting Terrier dogs is an autosomal-recessively inherited disorder, caused by a nonsense variant of the gene encoding for the very long-chain acyl-CoA-dehydrogenase (VLCAD) enzyme. Clinical signs include exercise- induced fatigue, muscle pain and weakness. In the present study, the long-term course of this disease was investigated over a period of 1 year in 9 affected German Hunting Terriers. The dogs were treated symptomatically with oral L-carnitine, coenzyme Q10 and a special diet characterized by a low content of long-chain fatty acids and a high proportion of carbohydrates.In 9 affected dogs, the phenotype as well as clinical, laboratory parameters, and histopathological findings are described (time point 1) and compared to follow-up examinations 1 year later (time point 2). At both time points clinical and neurological examinations, complete blood cell count, clinical chemistry profile and the concentration of brain natriuretic peptide (NT-proBNP) were investigated.In the follow-up examinations, the same post-exercise clinical signs were present as in the initial presentation of the homozygous dogs. Dark-brownish discoloration of the urine, weakness, myalgia as well as stiff and tetraparetic gait were apparant. All hematological values and the concentration of NT-proBNP were within the relevant reference ranges. Plasma CK and ALT activities were compared between the first presentation and the follow- up examination and no significant differences were detected (pOral supplementation with L-carnitine, coenzyme Q10 and the special dietary management did not result in any improvement of clinical signs or laboratory parameters. No progression of the disease was observed. The prognosis for affected dogs remains cautious as long-term observations of affected dogs over several years are lacking. Our findings provide further important information on inherited disorders of mitochondrial β-oxidation in dogs, especially focused on the exercise induced metabolic myopathy in the German Hunting Terrier. This may provide new insights for novel treatment modalities in conjuntion with the development of improved breeding guidelines.Die belastungsabhängige metabolische Myopathie beim Deutschen Jagdterrier ist eine autosomal-rezessive Erbkrankheit, die aufgrund einer Punktmutation zu einem Enzymdefekt der sehr langkettigen Acyl-CoA-Dehydrogenase führt und klinisch durch belastungsabhängige Schwäche, schwere Myalgien und Myoglobinurie charakterisiert ist. In dieser Studie wurde der klinische Verlauf der Erkrankung bei 9 betroffenen Deutschen Jagdterriern über 1 Jahr untersucht. Die Behandlung der Hunde umfasste die orale Supplementierung von L-Carnitin und Koenzym Q10 sowie eine Diät mit hohem Kohlenhydratgehalt und einem niedrigen Anteil an langkettigen Fettsäuren.Die 9 Hunde mit nachgewiesenem Gendefekt wurden bei Auftreten der ersten Symptome (Messzeitpunkt 1, MZP1) und 1 Jahr später (Messzeitpunkt 2, MZP2) klinisch-neurologisch untersucht und es erfolgte eine Blutuntersuchung mit Analyse hämatologischer und klinisch-chemischer Parameter sowie der Konzentration des natriuretischen Peptids Typ B (NT-proBNP).Zum MZP2 zeigten die Hunde, wie bereits zum MZP1, nach Belastung ein steifes Gangbild, Muskelschwäche und -schmerz und hatten einen dunkelbraun verfärbten Urin. Bei den hämatologischen Parametern und der NT-proBNP-Konzentration ergaben sich, wie zum MZP1, keine Abweichungen vom Referenzbereich. Die Aktivitäten der Kreatinkinase und Alanin-Aminotransferase unterschieden sich statistisch nicht signifikant zwischen beiden MZP (pDie orale Supplementierung von L-Carnitin und Koenzym Q10 sowie spezielle diätetische Maßnahmen über 1 Jahr führten zu keiner Verbesserung der klinischen Symptomatik oder der untersuchten Laborparameter. Die Tiere zeigten keine progressive Verschlechterung der Symptomatik im Vergleich zur Erstvorstellung. Die Prognose ist jedoch als vorsichtig zu bezeichnen, da Daten zum Langzeitverlauf über einige Jahre fehlen. Unsere Ergebnisse bilden eine Grundlage für weitere Forschungen zu Lipidspeichermyopathien, insbesondere mit Fokus auf die belastungsabhängige metabolische Myopathie des Deutschen Jagdterriers, deren Therapie und eine entsprechende Zuchthygiene.

The implementation of lecture recordings at the University of Veterinary Medicine Hannover, Foundation, was accompanied by the current study investigating students' and teachers' acceptance of and requirements for this teaching format.Data were collected using questionnaires and guideline-based focus group surveys with students and teachers at the University of Veterinary Medicine Hannover, Foundation.Despite skepticism, teachers at the University are prepared to have their lectures recorded and prefer the option to provide the students with a short version of their lectures. All interviewed focus groups identified similar requirements for lecture recordings, including updating and editing options. Students wish to expand the lecture recording offer in veterinary medicine, while the teachers expressed their desire for support measures, including instructions and training.The present study will contribute to the optimization of lecture recording for students and teachers of veterinary medicine. The lecture recording offer can be adapted to the indicated requirements. Teachers who were skeptical of lecture recording have already been convinced to record. The use of mobile technology enables the expansion of the lecture recording possibilities. With the continuation of digitization, teaching material and didactics in veterinary medicine can be further enriched.Diese Studie wurde im Rahmen der Einführung von Vorlesungsaufzeichnungen an der Stiftung Tierärztliche Hochschule Hannover (TiHo) durchgeführt, um die Akzeptanz und speziellen Anforderungen von Dozierenden und Studierenden an Vorlesungsaufzeichnungen zu erheben.Die Datenerhebung erfolgte mithilfe von Fragebögen und leitfadengestützten Fokusgruppenbefragungen mit Studierenden und Dozierenden der TiHo.Dozierende der TiHo sind trotz Skepsis bereit, ihre Vorlesungen aufzeichnen zu lassen und präferieren die Option, den Studierenden eine Kurzversion zur Verfügung zu stellen. Alle befragten Fokusgruppen nannten ähnliche Anforderungen an Vorlesungsaufzeichnungen, beispielsweise Möglichkeiten zur Aktualisierung und Bearbeitung. Studierende wünschen sich den Ausbau des Angebots an Vorlesungsaufzeichnungen in der Tiermedizin, während die Dozierenden den Wunsch nach Unterstützungsmaßnahmen wie Anleitungen und Schulungen äußerten.Die Studie wird zur Optimierung der Vorlesungsaufzeichnungen für Studierende und Dozierende der Tiermedizin beitragen und das Angebot an Vorlesungsaufzeichnungen lässt sich an die genannten Anforderungen anpassen. Erste zuvor skeptische eingestellte Dozierende konnten bereits gewonnen und überzeugt werden, Aufzeichnungen durchzuführen. Der Einsatz mobiler Technik ermöglicht den Ausbau des Angebots. Mit der Fortführung der Digitalisierung der Lehre kann die tiermedizinische Hochschuldidaktik weiter bereichert werden.

During five and a half years of studying veterinary medicine, students should in addition to theoretical knowledge acquire sufficient practical skills. Considering animal welfare and ethical aspects, opportunities for hands-on learning on living animals are limited because of the high annual number of students. The first German veterinary clinical-skills lab, established in 2013 at the University for Veterinary Medicine Hannover, Foundation (TiHo), offers opportunities for all students to learn, train and repeat clinical skills on simulators and models as frequently as they would like, until they feel sufficiently confident to transfer these skills to living animals. This study describes the establishment of clinical-skills lab training within the students' practical education, using the example of the small-animal clinic of the TiHo.Two groups of students were compared: without skills lab training (control group K) and with skills lab training (intervention group I). At the end of both the training and a subsequent 10-week clinical rotation in different sections of the clinic, an objective structured clinical examination (OSCE) was performed, testing the students' practical skills at 15 stations. An additional multiple-choice test was performed before and after the clinical rotation to evaluate the increased theoretical knowledge.Students of group I achieved significantly (p ≤ 0.05) better results in eight of the 15 tested skills. The multiple-choice test revealed a significant (p ≤ 0.05) gain of theoretical knowledge in both groups without any differences between the groups. Students displayed a high degree of acceptance of the skills lab training.Using simulators and models in veterinary education is an efficient teaching concept, and should be used continually and integrated in the curriculum.

Granulomatous myelitis due to infection with Mycobacterium avium was diagnosed in a 4-year-old male neutered European mink (Mustela lutreola). The causative agent was detected by an acid-fast stain and further characterized by polymerase chain reaction and DNA sequencing of the PCR product. A thorough histological evaluation of the remaining organs revealed no granulomatous lesions or detectable acid-fast organisms. Although minks are generally highly susceptible for mycobacteria, localised infections, especially of the central nervous system, are unusual and may represent an atypical chronic form of the disease.

The aim of the study was to evaluate the glucose ratio (glucose level in the cerebrospinal fluid [CSF]/blood glucose level) as a quickly available marker for detecting bacterial meningoencephalomyelitis (BM).Blood and CSF samples of 328 dogs were reviewed and evaluated retrospectively. Following the neurological diagnosis, the dogs were assigned to seven different groups: steroid-responsive meningitis-arteritis (SRMA), intervertebral disc disease (IVDD), neoplasia of the central nervous system (N), idiopathic epilepsy (IE), bacterial meningoencephalomyelitis (BM), meningoencephalomyelitis of other origin (ME) and healthy dogs.The median of the CSF-glucose level (mmol/l) and the median of the glucose ratio in the SRMA group displayed the lowest values and differed significantly from the CSF-glucose levels of dogs in the groups IVDD, N, IE and healthy dogs (CSF-glucose level: p0.01; glucose ratio: p0.05). In the BM group, both parameters did not differ significant- ly from other groups, but displayed similar low levels as in the SRMA group. There was a negative correlation between the CSF cell count and the CSF-glucose ratio (Spearman correlation coefficient -0.322, p=0.01, R²=0.108).The CSF-glucose concentration cannot be used as a distinct marker to differentiate BM from other inflammatory CNS-diseases, especially from SRMA usually accompanied by severe pleocytosis. Low CSF-glucose levels appear to be caused by elevated CSF cell counts rather than by bacterial metabolism.For a definitive diagnosis of bacterial meningoencephalomyelitis in dogs, the detection of microorganisms remains necessary.

Thiamine deficiency in cats frequently leads to a dysfunction of the central nervous system including vestibular signs with fatal outcome in untreated cases. The aim of the present study was to directly measure thiamine concentrations using high pressure liquid chromatography (HPLC) in feline blood samples and to evaluate values in healthy and diseased cats.Blood samples (1 ml EDTA-whole blood) from 193 cats were analysed for total thiamine and thiamine diphosphate using HPLC. For the interpretation of the results cats were retrospectively assigned to six groups: A) healthy cats, B) cats with diseases of the gastrointestinal tract, C) cats with different traumas not affecting the gastrointestinal tract, D) cats with inappetence, cats with central vestibular signs and normal (E) or low values of thiamine (F), respectively.In animals of group F no obvious cause for the vestibular signs was found and spontaneous recovery after thiamine application occurred in three cats. Therefore thiamine deficiency was a highly likely clinical diagnosis. Total thiamine concentration (mean 48.2 µg/l, standard deviation ± 22.6) of group F significantly differered from the other groups (group A-D: p0.01, group E: p0.001). Comparable results were obtained for thiamine diphosphate. However, low total thiamine values were also found in cats with inappetence without any neurological signs.In the present study a method for direct measurement of thiamine formerly established for ruminants was evaluated for cats. A more accurate and objective clinical diagnosis of thiamine deficiency is feasible in cats with values less than 50 µg/l and typical clinical signs. In animals with values of total thiamine levels between 50-70 µg/l a prophylactic substitution of thiamine can be discussed.
