The equalization cancellation model is often used to predict the binaural masking level difference. Previously its application to speech in noise has required separate knowledge about the speech and noise signals to maximize the signal-to-noise ratio (SNR). Here, a novel, blind equalization cancellation model is introduced that can use the mixed signals. This approach does not require any assumptions about particular sound source directions. It uses different strategies for positive and negative SNRs, with the switching between the two steered by a blind decision stage utilizing modulation cues. The output of the model is a single-channel signal with enhanced SNR, which we analyzed using the speech intelligibility index to compare speech intelligibility predictions. In a first experiment, the model was tested on experimental data obtained in a scenario with spatially separated target and masker signals. Predicted speech recognition thresholds were in good agreement with measured speech recognition thresholds with a root mean square error less than 1 dB. A second experiment investigated signals at positive SNRs, which was achieved using time compressed and low-pass filtered speech. The results demonstrated that binaural unmasking of speech occurs at positive SNRs and that the modulation-based switching strategy can predict the experimental results.

Automatic gain control (AGC) compresses the wide dynamic range of sounds to the narrow dynamic range of hearing-impaired listeners. Setting AGC parameters (time constants and knee points) is an important part of the fitting of hearing devices. These parameters do not only influence overall loudness elicited by the hearing devices but can also affect the recognition of speech in noise. We investigated whether matching knee points and time constants of the AGC between the cochlear implant and the hearing aid of bimodal listeners would improve speech recognition in noise. We recruited 18 bimodal listeners and provided them all with the same cochlear-implant processor and hearing aid. We compared the matched AGCs with the default device settings with mismatched AGCs. As a baseline, we also included a condition with the mismatched AGCs of the participants' own devices. We tested speech recognition in quiet and in noise presented from different directions. The time constants affected outcomes in the monaural testing condition with the cochlear implant alone. There were no specific binaural performance differences between the two AGC settings. The purpose of this study was to measure the binaural interaction component (BIC) derived from click-evoked auditory brainstem responses (ABRs) using stimuli configured to elicit the Precedence Effect. The hypothesis was that the contribution of binaural processing to echo suppression can be evidenced by a diminished or absent BIC associated with the echo. Ten normal-hearing young adults provided ABRs generated by sequences of click pairs. Results showed that BICs elicited by diotic clicks in isolation were obliterated when those diotic clicks were preceded by a click pair having an interaural time difference of 400 µs and where the interclick interval was 8.4 ms. The presence of the leading click pair increased the latency of the ABR generated by the lagging diotic click pair but did not decrease its amplitude. The results were interpreted as indicating a contribution of binaural processing at the level of the brainstem to echo suppression, at least for the conditions tested here.

For speech intelligibility in rooms, the temporal integration of speech reflections is typically modeled by separating the room impulse response (RIR) into an early (assumed beneficial for speech intelligibility) and a late part (assumed detrimental). This concept was challenged in this study by employing binaural RIRs with systematically varied interaural phase differences (IPDs) and amplitude of the direct sound and a variable number of reflections delayed by up to 200 ms. Speech recognition thresholds in stationary noise were measured in normal-hearing listeners for 86 conditions. The data showed that direct sound and one or several early speech reflections could be perfectly integrated when they had the same IPD. Early reflections with the same IPD as the noise (but not as the direct sound) could not be perfectly integrated with the direct sound. All conditions in which the dominant speech information was within the early RIR components could be well predicted by a binaural speech intelligibility model using classic early/late separation. In contrast, when amplitude or IPD favored late RIR components, listeners appeared to be capable of focusing on these components rather than on the precedent direct sound. This could not be modeled by an early/late separation window but required a temporal integration window that can be flexibly shifted along the RIR.

Informational masking (IM) can greatly reduce speech intelligibility, but the neural mechanisms underlying IM are not understood. Binaural differences between target and masker can improve speech perception. In general, improvement in masked speech intelligibility due to provision of spatial cues is called spatial release from masking. Here, we focused on an aspect of spatial release from masking, specifically, the role of spatial attention. We hypothesized that in a situation with IM background sound (a) attention to speech recruits lateral frontal cortex (LFCx) and (b) LFCx activity varies with direction of spatial attention. Using functional near infrared spectroscopy, we assessed LFCx activity bilaterally in normal-hearing listeners. In Experiment 1, two talkers were simultaneously presented. Listeners either attended to the target talker (speech task) or they listened passively to an unintelligible, scrambled version of the acoustic mixture (control task). Target and masker differed in pitch and interaural time difference (ITD). Relative to the passive control, LFCx activity increased during attentive listening. Experiment 2 measured how LFCx activity varied with ITD, by testing listeners on the speech task in Experiment 1, except that talkers either were spatially separated by ITD or colocated. Results show that directing of auditory attention activates LFCx bilaterally. Moreover, right LFCx is recruited more strongly in the spatially separated as compared with colocated configurations. Findings hint that LFCx function contributes to spatial release from masking in situations with IM.

In complex listening environments, children can benefit from auditory spatial cues to understand speech in noise. When a spatial separation is introduced between the target and masker and/or listening with two ears versus one ear, children can gain intelligibility benefits with access to one or more auditory cues for unmasking: monaural head shadow, binaural redundancy, and interaural differences. This study systematically quantified the contribution of individual auditory cues in providing binaural speech intelligibility benefits for children with normal hearing between 6 and 15 years old. In virtual auditory space, target speech was presented from + 90° azimuth (i.e., listener's right), and two-talker babble maskers were either co-located (+ 90° azimuth) or separated by 180° (–90° azimuth, listener's left). Testing was conducted over headphones in monaural (i.e., right ear) or binaural (i.e., both ears) conditions. Results showed continuous improvement of speech reception threshold (SRT) between 6 and 15 years old and immature performance at 15 years of age for both SRTs and intelligibility benefits from more than one auditory cue. With early maturation of head shadow, the prolonged maturation of unmasking was likely driven by children's poorer ability to gain full benefits from interaural difference cues. In addition, children demonstrated a trade-off between the benefits from head shadow versus interaural differences, suggesting an important aspect of individual differences in accessing auditory cues for binaural intelligibility benefits during development.

Acoustic hearing listeners use binaural cues—interaural time differences (ITDs) and interaural level differences (ILDs)—for localization and segregation of sound sources in the horizontal plane. Cochlear implant users now often receive two implants (bilateral cochlear implants [BiCIs]) rather than one, with the goal to provide access to these cues. However, BiCI listeners often experience difficulty with binaural tasks. Most BiCIs use independent sound processors at each ear; it has often been suggested that such independence may degrade the transmission of binaural cues, particularly ITDs. Here, we report empirical measurements of binaural cue transmission via BiCIs implementing a common “ n-of- m” spectral peak-picking stimulation strategy. Measurements were completed for speech and nonspeech stimuli presented to an acoustic manikin “fitted” with BiCI sound processors. Electric outputs from the BiCIs and acoustic outputs from the manikin’s in-ear microphones were recorded simultaneously, enabling comparison of electric and acoustic binaural cues. For source locations away from the midline, BiCI binaural cues, particularly envelope ITD cues, were found to be degraded by asymmetric spectral peak-picking. In addition, pulse amplitude saturation due to nonlinear level mapping yielded smaller ILDs at higher presentation levels. Finally, while individual pulses conveyed a spurious “drifting” ITD, consistent with independent left and right processor clocks, such variation was not evident in transmitted envelope ITDs. Results point to avenues for improvement of BiCI technology and may prove useful in the interpretation of BiCI spatial hearing outcomes reported in prior and future studies.

This paper reviews the hypothesis of harmonic cancellation according to which an interfering sound is suppressed or canceled on the basis of its harmonicity (or periodicity in the time domain) for the purpose of Auditory Scene Analysis. It defines the concept, discusses theoretical arguments in its favor, and reviews experimental results that support it, or not. If correct, the hypothesis may draw on time-domain processing of temporally accurate neural representations within the brainstem, as required also by the classic equalization-cancellation model of binaural unmasking. The hypothesis predicts that a target sound corrupted by interference will be easier to hear if the interference is harmonic than inharmonic, all else being equal. This prediction is borne out in a number of behavioral studies, but not all. The paper reviews those results, with the aim to understand the inconsistencies and come up with a reliable conclusion for, or against, the hypothesis of harmonic cancellation within the auditory system.

An aided sound-field auditory steady-state response (ASSR) has the potential to be used to objectively validate hearing-aid (HA) fittings in clinics. Each aided ear should ideally be tested independently, but it is suspected that binaural testing may be used by clinics to reduce test time. This study simulates dichotic ASSR sound-field conditions to examine the risk of making false judgments due to unchecked binaural effects. Unaided ASSRs were recorded with a clinical two-channel electroencephalography (EEG) system for 15 normal hearing subjects using a three-band CE-Chirp® stimulus. It was found that the noise corrected power of a response harmonic can be suppressed by up to 10 dB by introducing large interaural time differences equal to half the time period of the stimulus envelope, which may occur in unilateral HA users. These large interaural time differences also changed the expression of ASSR power across the scalp, resulting in dramatically altered topographies. This would lead to considerably lower measured response power and possibly nondetections, evidencing that even well fit HAs are fit poorly (false referral), whereas monaural ASSR tests would pass. No effect was found for simulated lateralizations of the stimulus, which is beneficial for a proposed aided ASSR approach. Full-scalp ASSR recordings match previously found 40 Hz topographies but demonstrate suppression of cortical ASSR sources when using stimuli in interaural envelope antiphase.

A study was conducted to examine the benefits afforded by a signal-processing strategy that imposes the binaural cues present in a natural signal, calculated locally in time and frequency, on the output of a beamforming microphone array. Such a strategy has the potential to combine the signal-to-noise ratio advantage of beamforming with the perceptual benefit of spatialization to enhance performance in multitalker mixtures. Participants with normal hearing and with hearing loss were tested on both speech localization and speech-on-speech masking tasks. Performance for the spatialized beamformer was compared with that for three other conditions: a reference condition with no processing, a beamformer with no spatialization, and a hybrid beamformer that operates only in the high frequencies to preserve natural binaural cues in the low frequencies. Beamforming with full-bandwidth spatialization supported speech localization and produced better speech reception thresholds than the other conditions.

The ability to attend to target speech in background noise is an important skill, particularly for children who spend many hours in noisy environments. Intelligibility improves as a result of spatial or binaural unmasking in the free-field for normal-hearing children; however, children who use bilateral cochlear implants (BiCIs) demonstrate little benefit in similar situations. It was hypothesized that poor auditory attention abilities might explain the lack of unmasking observed in children with BiCIs. Target and interferer speech stimuli were presented to either or both ears of BiCI participants via their clinical processors. Speech reception thresholds remained low when the target and interferer were in opposite ears, but they did not show binaural unmasking when the interferer was presented to both ears and the target only to one ear. These results demonstrate that, in the most extreme cases of stimulus separation, children with BiCIs can ignore an interferer and attend to target speech, but there is weak or absent binaural unmasking. It appears that children with BiCIs mostly experience poor encoding of binaural cues rather than deficits in ability to selectively attend to target speech.

A person’s ability to process temporal fine structure information is indispensable for speech understanding. As speech understanding typically deteriorates throughout adult life, this study aimed to disentangle age and hearing impairment (HI)-related changes in binaural temporal processing. This was achieved by examining neural and behavioral processing of interaural phase differences (IPDs). Neural IPD processing was studied electrophysiologically through steady-state activity in the electroencephalogram evoked by periodic changes in IPDs over time, embedded in the temporal fine structure of acoustic stimulation. In addition, behavioral IPD discrimination thresholds were determined for the same stimuli. To disen- tangle potential effects of age from those of HI, both measures were applied to six participant groups: young, middle-aged, and older persons, with either normal hearing or sensorineural HI. All participants passed a cognitive screening, and stimulus audibility was controlled for in participants with HI. The results demonstrated that HI changes neural processing of binaural temporal information for all age-groups included in this study. These outcomes were revealed, superimposed on age-related changes that emerge between young adulthood and middle age. Poorer neural outcomes were also associated with poorer behavioral performance, even though the behavioral IPD discrimination thresholds were affected by age rather than by HI. The neural outcomes of this study are the first to evidence and disentangle the dual load of age and HI on binaural temporal processing. A FDA clinical trial was carried out to evaluate the potential benefit of cochlear implant (CI) use for adults with unilateral moderate-to-profound sensorineural hearing loss. Subjects were 20 adults with moderate-to-profound unilateral sensorineural hearing loss and normal or near-normal hearing on the other side. A MED-EL standard electrode was implanted in the impaired ear. Outcome measures included: (a) sound localization on the horizontal plane (11 positions, -90° to 90°), (b) word recognition in quiet with the CI alone, and (c) masked sentence recognition with the target at 0° and the masker at -90°, 0°, or 90°. This battery was completed preoperatively and at 1, 3, 6, 9, and 12 months after CI activation. Normative data were also collected for 20 age-matched control subjects with normal or near-normal hearing bilaterally. The CI improved localization accuracy and reduced side bias. Word recognition with the CI alone was similar to performance of traditional CI recipients. The CI improved masked sentence recognition when the masker was presented from the front or from the side of normal or near-normal hearing. The binaural benefits observed with the CI increased between the 1- and 3-month intervals but appeared stable thereafter. In contrast to previous reports on localization and speech perception in patients with unilateral sensorineural hearing loss, CI benefits were consistently observed across individual subjects, and performance was at asymptote by the 3-month test interval. Cochlear implant settings, consistent CI use, and short duration of deafness could play a role in this result.

Current clinical practice in programming a cochlear implant (CI) for individuals with single-sided deafness (SSD) is to maximize the transmission of speech information via the implant, with the implicit assumption that this will also result in improved spatial-hearing abilities. However, binaural sensitivity is reduced by interaural place-of-stimulation mismatch, a likely occurrence with a standard CI frequency-to-electrode allocation table (FAT). As a step toward reducing interaural mismatch, this study investigated whether a test of interaural-time-difference (ITD) discrimination could be used to estimate the acoustic frequency yielding the best place match for a given CI electrode. ITD-discrimination performance was measured by presenting 300-ms bursts of 100-pulses-per-second electrical pulse trains to a single CI electrode and band-limited pulse trains with variable carrier frequencies to the acoustic ear. Listeners discriminated between two reference intervals (four bursts each with constant ITD) and a moving target interval (four bursts with variable ITD). For 17 out of the 26 electrodes tested across eight listeners, the function describing the relationship between ITD-discrimination performance and carrier frequency had a discernable peak where listeners achieved 70% to 100% performance. On average, this peak occurred 1.15 octaves above the CI manufacturer's default FAT. ITD discrimination shows promise as a method of estimating the cochlear place of stimulation for a given electrode, thereby providing information to optimize the FAT for SSD-CI listeners.

Little is known about the perception of artificial spatial hearing by hearing-impaired subjects. The purpose of this study was to investigate how listeners with hearing disorders perceived the effect of a spatialization feature designed for wireless microphone systems. Forty listeners took part in the experiments. They were arranged in four groups: normal-hearing, moderate, severe, and profound hearing loss. Their performance in terms of speech understanding and speaker localization was assessed with diotic and binaural stimuli. The results of the speech intelligibility experiment revealed that the subjects presenting a moderate or severe hearing impairment better understood speech with the spatialization feature. Thus, it was demonstrated that the conventional diotic binaural summation operated by current wireless systems can be transformed to reproduce the spatial cues required to localize the speaker, without any loss of intelligibility. The speaker localization experiment showed that a majority of the hearing-impaired listeners had similar performance with natural and artificial spatial hearing, contrary to the normal-hearing listeners. This suggests that certain subjects with hearing impairment preserve their localization abilities with approximated generic head-related transfer functions in the frontal horizontal plane.

The ability to process binaural temporal fine structure (TFS) information was assessed using the TFS-AF test (where AF stands for adaptive frequency) for 26 listeners aged 60 years or more with normal or elevated low-frequency audiometric thresholds. The test estimates the highest frequency at which a fixed interaural phase difference (IPD) of ϕ (varied here between 30° and 180°) can be discriminated from an IPD of 0°, with higher thresholds indicating better performance. A sensation level of 30 dB was used. All listeners were able to perform the task reliably, giving thresholds well above the lowest allowed frequency of 30 Hz. The duration of a run averaged 5 min. Repeated testing of the normal-hearing listeners showed no significant practice effects. Thresholds varied markedly across listeners, but their ranking was fairly consistent across values of ϕ. Thresholds decreased (worsened) with decreasing ϕ and were lower than for a group of young listeners tested in an earlier study. There were weak to moderate, negative correlations between TFS-AF thresholds and audiometric thresholds at low frequencies (125–1000 Hz) but not at high frequencies (4000–8000 Hz). In conclusion, the TFS-AF test yielded a graded measure of binaural TFS sensitivity for all listeners. This contrasts with the TFS-LF (low-frequency) test, which measures the smallest detectable shift in IPD for a fixed frequency. The absence of practice effects and a reasonably short administration time make the TFS-AF test a good candidate for the assessment of sensitivity to changes in binaural TFS for older listeners without or with hearing loss.

The aim of this study was to evaluate the performance of a visually guided hearing aid (VGHA) under conditions designed to capture some aspects of "real-world" communication settings. The VGHA uses eye gaze to steer the acoustic look direction of a highly directional beamforming microphone array. Although the VGHA has been shown to enhance speech intelligibility for fixed-location, frontal targets, it is currently not known whether these benefits persist in the face of frequent changes in location of the target talker that are typical of conversational turn-taking. Participants were 14 young adults, 7 with normal hearing and 7 with bilateral sensorineural hearing impairment. Target stimuli were sequences of 12 question-answer pairs that were embedded in a mixture of competing conversations. The participant's task was to respond via a key press after each answer indicating whether it was correct or not. Spatialization of the stimuli and microphone array processing were done offline using recorded impulse responses, before presentation over headphones. The look direction of the array was steered according to the eye movements of the participant as they followed a visual cue presented on a widescreen monitor. Performance was compared for a "dynamic" condition in which the target stimulus moved between three locations, and a "fixed" condition with a single target location. The benefits of the VGHA over natural binaural listening observed in the fixed condition were reduced in the dynamic condition, largely because visual fixation was less accurate.

This study investigated the effects of unilateral hearing loss (UHL), of either conductive or sensorineural origin, on stereo sound localization and related visual bias in listeners with normal hearing, short-term (acute) UHL, and chronic UHL. This performance gap is due to the degradation of binaural and monaural sound localization cues, caused by a combination of device-related and patient-related issues. In this study, we targeted the device-related issues by measuring sound localization performance of 11 NH listeners, listening to free-field stimuli processed by a real-time CI vocoder. The use of a real-time vocoder is a new approach, which enables testing in a free-field environment. For the NH listening condition, all listeners accurately and precisely localized sounds according to a linear stimulus-response relationship with an optimal gain and a minimal bias both in the azimuth and in the elevation directions. In contrast, when listening with bilateral real-time vocoders, listeners tended to orient either to the left or to the right in azimuth and were unable to determine sound source elevation. When listening with an NH ear and a unilateral vocoder, localization was impoverished on the vocoder side but improved toward the NH side. Localization performance was also reflected by systematic variations in reaction times across listening conditions. We conclude that perturbation of interaural temporal cues, reduction of interaural level cues, and removal of spectral pinna cues by the vocoder impairs sound localization. Listeners seem to ignore cues that were made unreliable by the vocoder, leading to acute reweighting of available localization cues. We discuss how current CI processors prevent CI users from localizing sounds in everyday environments.

Persons with aphasia (PWA) often report difficulty understanding spoken language in noisy environments that require listeners to identify and selectively attend to target speech while ignoring competing background sounds or “maskers.” This study compared the performance of PWA and age-matched healthy controls (HC) on a masked speech identification task and examined the consequences of different types of masking on performance. Twelve PWA and 12 age-matched HC completed a speech identification task comprising three conditions designed to differentiate between the effects of energetic and informational masking on receptive speech processing. The target and masker speech materials were taken from a closed-set matrix-style corpus, and a forced-choice word identification task was used. Target and maskers were spatially separated from one another in order to simulate real-world listening environments and allow listeners to make use of binaural cues for source segregation. Individualized frequency-specific gain was applied to compensate for the effects of hearing loss. Although both groups showed similar susceptibility to the effects of energetic masking, PWA were more susceptible than age-matched HC to the effects of informational masking. Results indicate that this increased susceptibility cannot be attributed to age, hearing loss, or comprehension deficits and is therefore a consequence of acquired cognitive-linguistic impairments associated with aphasia. This finding suggests that aphasia may result in increased difficulty segregating target speech from masker speech, which in turn may have implications for the ability of PWA to comprehend target speech in multitalker environments, such as restaurants, family gatherings, and other everyday situations.

Speech perception in complex sound fields can greatly benefit from different unmasking cues to segregate the target from interfering voices. This study investigated the role of three unmasking cues (spatial separation, gender differences, and masker time reversal) on speech intelligibility and perceived listening effort in normal-hearing listeners. Speech intelligibility and categorically scaled listening effort were measured for a female target talker masked by two competing talkers with no unmasking cues or one to three unmasking cues. In addition to natural stimuli, all measurements were also conducted with glimpsed speech—which was created by removing the time–frequency tiles of the speech mixture in which the maskers dominated the mixture—to estimate the relative amounts of informational and energetic masking as well as the effort associated with source segregation. The results showed that all unmasking cues as well as glimpsing improved intelligibility and reduced listening effort and that providing more than one cue was beneficial in overcoming informational masking. The reduction in listening effort due to glimpsing corresponded to increases in signal-to-noise ratio of 8 to 18 dB, indicating that a significant amount of listening effort was devoted to segregating the target from the maskers. Furthermore, the benefit in listening effort for all unmasking cues extended well into the range of positive signal-to-noise ratios at which speech intelligibility was at ceiling, suggesting that listening effort is a useful tool for evaluating speech-on-speech masking conditions at typical conversational levels.

In patients with bilateral hearing loss, the use of two hearing aids (HAs) offers the potential to restore the benefits of binaural hearing, including sound source localization and segregation. However, existing evidence suggests that bilateral HA users’ access to binaural information, namely interaural time and level differences (ITDs and ILDs), can be compromised by device processing. Our objective was to characterize the nature and magnitude of binaural distortions caused by modern digital behind-the-ear HAs using a variety of stimuli and HA program settings. Of particular interest was a common frequency-lowering algorithm known as nonlinear frequency compression, which has not previously been assessed for its effects on binaural information. A binaural beamforming algorithm was also assessed. Wide dynamic range compression was enabled in all programs. HAs were placed on a binaural manikin, and stimuli were presented from an arc of loudspeakers inside an anechoic chamber. Stimuli were broadband noise bursts, 10-Hz sinusoidally amplitude-modulated noise bursts, or consonant–vowel–consonant speech tokens. Binaural information was analyzed in terms of ITDs, ILDs, and interaural coherence, both for whole stimuli and in a time-varying sense (i.e., within a running temporal window) across four different frequency bands (1, 2, 4, and 6 kHz). Key findings were: (a) Nonlinear frequency compression caused distortions of high-frequency envelope ITDs and significantly reduced interaural coherence. (b) For modulated stimuli, all programs caused time-varying distortion of ILDs. (c) HAs altered the relationship between ITDs and ILDs, introducing large ITD–ILD conflicts in some cases. Potential perceptual consequences of measured distortions are discussed.

For bilateral cochlear implant (CI) patients, electrodes that receive the same frequency allocation often stimulate locations in the left and right ear that do not yield the same perceived pitch, resulting in a pitch mismatch. This pitch mismatch may be related to degraded binaural abilities. Pitch mismatches have been found for some bilateral CI users and the goal of this study was to determine whether pitch mismatches are prevalent in bilateral CI patients, including those with extensive experience with bilateral CIs. To investigate this possibility, pitch matching was conducted with 16 bilateral CI patients. For 14 of the 16 participants, there was a significant difference between those electrodes in the left and right ear that yielded the same pitch and those that received the same frequency allocation in the participant’s clinical map. The results suggest that pitch mismatches are prevalent with bilateral CI users. The results also indicated that pitch mismatches persist even with extended bilateral CI experience. Such mismatches may reduce the benefits patients receive from bilateral CIs.

The ability to detect a target signal masked by noise is improved in normal-hearing listeners when interaural phase differences (IPDs) between the ear signals exist either in the masker or in the signal. To improve binaural hearing in bilaterally implanted cochlear implant (BiCI) users, a coding strategy providing the best possible access to IPD is highly desirable. In this study, we compared two coding strategies in BiCI users provided with CI systems from MED-EL (Innsbruck, Austria). The CI systems were bilaterally programmed either with the fine structure processing strategy FS4 or with the constant rate strategy high definition continuous interleaved sampling (HDCIS). Familiarization periods between 6 and 12 weeks were considered. The effect of IPD was measured in two types of experiments: (a) IPD detection thresholds with tonal signals addressing mainly one apical interaural electrode pair and (b) with speech in noise in terms of binaural speech intelligibility level differences (BILD) addressing multiple electrodes bilaterally. The results in (a) showed improved IPD detection thresholds with FS4 compared with HDCIS in four out of the seven BiCI users. In contrast, 12 BiCI users in (b) showed similar BILD with FS4 (0.6 ± 1.9 dB) and HDCIS (0.5 ± 2.0 dB). However, no correlation between results in (a) and (b) both obtained with FS4 was found. In conclusion, the degree of IPD sensitivity determined on an apical interaural electrode pair was not an indicator for BILD based on bilateral multielectrode stimulation.

In many daily life communication situations, several sound sources are simultaneously active. While normal-hearing listeners can easily distinguish the target sound source from interfering sound sources-as long as target and interferers are spatially or spectrally separated-and concentrate on the target, hearing-impaired listeners and cochlear implant users have difficulties in making such a distinction. In this article, we propose a binaural approach composed of a spatial filter controlled by a direction-of-arrival estimator to track and enhance a moving target sound. This approach was implemented on a real-time signal processing platform enabling experiments with test subjects in situ. To evaluate the proposed method, a data set of sound signals with a single moving sound source in an anechoic diffuse noise environment was generated using virtual acoustics. The proposed steering method was compared with a fixed (nonsteering) method that enhances sound from the frontal direction in an objective evaluation and subjective experiments using this database. In both cases, the obtained results indicated a significant improvement in speech intelligibility and quality compared with the unprocessed signal. Furthermore, the proposed method outperformed the nonsteering method.

Users of bilateral cochlear implants (CIs) show above-chance performance in localizing the source of a sound in the azimuthal (horizontal) plane; although localization errors are far worse than for normal-hearing listeners, they are considerably better than for CI listeners with only one implant. In most previous studies, subjects had access to interaural level differences and to interaural time differences conveyed in the temporal envelope. Here, we present a binaural model that predicts the azimuthal direction of sound arrival from a two-channel input signal as it is received at the left and right CI processor. The model includes a replication of a clinical speech-coding strategy, a model of the electrode-nerve interface and binaural brainstem neurons, and three different prediction stages that are trained to map the neural response rate to an azimuthal angle. The model is trained and tested with various noise and speech stimuli created by means of virtual acoustics. Localization error patterns of the model match experimental data and are explicable largely in terms of the nonmonotonic relationship between interaural level difference and azimuthal angle.

In patients with bilateral cochlear implants (CIs), pairing matched interaural electrodes and stimulating them with the same frequency band is expected to facilitate binaural functions such as binaural fusion, localization, and spatial release from masking. Because clinical procedures typically do not include patient-specific interaural electrode pairing, it remains the case that each electrode is allocated to a generic frequency range, based simply on the electrode number. Two psychoacoustic techniques for determining interaurally paired electrodes have been demonstrated in several studies: interaural pitch comparison and interaural time difference (ITD) sensitivity. However, these two methods are rarely, if ever, compared directly. A third, more objective method is to assess the amplitude of the binaural interaction component (BIC) derived from electrically evoked auditory brainstem responses for different electrode pairings; a method has been demonstrated to be a potential candidate for bilateral CI users. Here, we tested all three measures in the same eight CI users. We found good correspondence between the electrode pair producing the largest BIC and the electrode pair producing the maximum ITD sensitivity. The correspondence between the pairs producing the largest BIC and the pitch-matched electrode pairs was considerably weaker, supporting the previously proposed hypothesis that whilst place pitch might adapt over time to accommodate mismatched inputs, sensitivity to ITDs does not adapt to the same degree.

There has been continued interest in clinical objective measures of binaural processing. One commonly proposed measure is the binaural interaction component (BIC), which is obtained typically by recording auditory brainstem responses (ABRs)-the BIC reflects the difference between the binaural ABR and the sum of the monaural ABRs (i.e., binaural - (left + right)). We have recently developed an alternative, direct measure of sensitivity to interaural time differences, namely, a following response to modulations in interaural phase difference (the interaural phase modulation following response; IPM-FR). To obtain this measure, an ongoing diotically amplitude-modulated signal is presented, and the interaural phase difference of the carrier is switched periodically at minima in the modulation cycle. Such periodic modulations to interaural phase difference can evoke a steady state following response. BIC and IPM-FR measurements were compared from 10 normal-hearing subjects using a 16-channel electroencephalographic system. Both ABRs and IPM-FRs were observed most clearly from similar electrode locations-differential recordings taken from electrodes near the ear (e.g., mastoid) in reference to a vertex electrode (Cz). Although all subjects displayed clear ABRs, the BIC was not reliably observed. In contrast, the IPM-FR typically elicited a robust and significant response. In addition, the IPM-FR measure required a considerably shorter recording session. As the IPM-FR magnitude varied with interaural phase difference modulation depth, it could potentially serve as a correlate of perceptual salience. Overall, the IPM-FR appears a more suitable clinical measure than the BIC.

Hearing-impaired adults, including both cochlear implant and bilateral hearing aid (HA) users, often exhibit broad binaural pitch fusion, meaning that they fuse dichotically presented tones with large pitch differences between ears. The current study was designed to investigate how binaural pitch fusion can be influenced by amplitude modulation (AM) of the stimuli and whether effects differ with hearing loss. Fusion ranges, the frequency ranges over which binaural pitch fusion occurs, were measured in both normal-hearing (NH) listeners and HA users with various coherent AM rates (2, 4, and 8 Hz); AM depths (20%, 40%, 60%, 80%, and 100%); and interaural AM phase and AM rate differences. The averaged results show that coherent AM increased binaural pitch fusion ranges to about 2 to 4 times wider than those in the unmodulated condition in both NH and bilateral HA subjects. Even shallow temporal envelope fluctuations (20% AM depth) significantly increased fusion ranges in all three coherent AM rate conditions. Incoherent AM introduced through interaural differences in AM phase or AM rate led to smaller increases in binaural pitch fusion range compared with those observed with coherent AM. Significant differences between groups were observed only in the coherent AM conditions. The influence of AM cues on binaural pitch fusion shows that binaural fusion is mediated in part by central processes involved in auditory grouping.

Aversiveness of loud sounds is a frequent complaint by hearing aid users, especially when fitted bilaterally. This study investigates whether loudness summation can be held responsible for this finding. Two aspects of loudness summation should be taken into account: spectral loudness summation for broadband signals and binaural loudness summation for signals that are presented binaurally. In this study, the effect of different symmetrical hearing losses was studied. Measurements were obtained with the widely used technique of Adaptive Categorical Loudness Scaling. For large bandwidths, spectral loudness summation for hearing-impaired listeners was found to be greater than that for normal-hearing listeners, both for monaurally and binaurally presented signals. For binaural loudness summation, the effect of hearing loss was not significant. In all cases, individual differences were substantial.

The ability to process binaural temporal fine structure (TFS) information, which influences the perception of speech in spatially distributed soundscapes, declines with increasing hearing loss and age. Because of the relatively small sample sizes used in previous studies, and the population-unrepresentative distribution of hearing loss and ages within study samples, it has been difficult to determine the relative and combined contributions of hearing loss and age. The aim of this study was to survey published and unpublished studies that assessed binaural TFS sensitivity using the TFS-low frequency (LF) test. Results from 19 studies were collated, yielding sample sizes of 147 to 648, depending on the test frequency. At least for the test frequency of 500 Hz, there were at least 67 listeners in each of four adult age groups and the distribution of audiometric thresholds at the test frequency within each group was similar to that for the population as a whole. Binaural TFS sensitivity declined with increasing age across the adult lifespan and with increasing hearing loss in old adulthood. For all test frequencies, both audiometric threshold and age were significantly negatively correlated with TFS-LF sensitivity ( r ranging from −0.19 to −0.64) but the correlation was always significantly higher for age than for audiometric threshold. Regression analyses showed that the standardized regression coefficient was greater for age than for audiometric threshold, and that there was a significant interaction; the effect of increasing age among older listeners was greater when the hearing loss was ≥30 dB than when it was

Differences in the temporal fine structure (TFS) of sounds at the two ears are used for sound localization and for the perceptual analysis of complex auditory scenes. The ability to process this binaural TFS information is poorer for older than for younger participants, and this may contribute to age-related declines in the ability to understand speech in noisy situations. However, it is unclear how sensitivity to binaural TFS changes across the older age range. This article presents data for a test of binaural sensitivity to TFS, the “TFS-adaptive frequency” (AF) test, for 118 listeners aged 60 to 96 years with normal or near-normal low-frequency hearing, but a variety of patterns of hearing loss at higher frequencies. TFS-AF scores were significantly lower (i.e., poorer) than those for young adults. On average, scores decreased by about 162 Hz for each 10-year increase in age over the range 60 to 85 years. Individual variability increased with increasing age. Scores also declined as low-frequency audiometric thresholds worsened. The results illustrate the range of scores that can be obtained as a function of age and may be useful for the diagnosis and management of age-related hearing difficulties.

The simulation framework for auditory discrimination experiments (FADE) was adopted and validated to predict the individual speech-in-noise recognition performance of listeners with normal and impaired hearing with and without a given hearing-aid algorithm. FADE uses a simple automatic speech recognizer (ASR) to estimate the lowest achievable speech reception thresholds (SRTs) from simulated speech recognition experiments in an objective way, independent from any empirical reference data. Empirical data from the literature were used to evaluate the model in terms of predicted SRTs and benefits in SRT with the German matrix sentence recognition test when using eight single- and multichannel binaural noise-reduction algorithms. To allow individual predictions of SRTs in binaural conditions, the model was extended with a simple better ear approach and individualized by taking audiograms into account. In a realistic binaural cafeteria condition, FADE explained about 90% of the variance of the empirical SRTs for a group of normal-hearing listeners and predicted the corresponding benefits with a root-mean-square prediction error of 0.6 dB. This highlights the potential of the approach for the objective assessment of benefits in SRT without prior knowledge about the empirical data. The predictions for the group of listeners with impaired hearing explained 75% of the empirical variance, while the individual predictions explained less than 25%. Possibly, additional individual factors should be considered for more accurate predictions with impaired hearing. A competing talker condition clearly showed one limitation of current ASR technology, as the empirical performance with SRTs lower than −20 dB could not be predicted.

Spatially separating speech maskers from target speech often leads to a large intelligibility improvement. Modeling this phenomenon has long been of interest to binaural-hearing researchers for uncovering brain mechanisms and for improving signal-processing algorithms in hearing-assistive devices. Much of the previous binaural modeling work focused on the unmasking enabled by binaural cues at the periphery, and little quantitative modeling has been directed toward the grouping or source-separation benefits of binaural processing. In this article, we propose a binaural model that focuses on grouping, specifically on the selection of time-frequency units that are dominated by signals from the direction of the target. The proposed model uses Equalization-Cancellation (EC) processing with a binary decision rule to estimate a time-frequency binary mask. EC processing is carried out to cancel the target signal and the energy change between the EC input and output is used as a feature that reflects target dominance in each time-frequency unit. The processing in the proposed model requires little computational resources and is straightforward to implement. In combination with the Coherence-based Speech Intelligibility Index, the model is applied to predict the speech intelligibility data measured by Marrone et al. The predicted speech reception threshold matches the pattern of the measured data well, even though the predicted intelligibility improvements relative to the colocated condition are larger than some of the measured data, which may reflect the lack of internal noise in this initial version of the model.

This article describes a model of loudness for time-varying sounds that incorporates the concept of binaural inhibition, namely, that the signal applied to one ear can reduce the internal response to a signal at the other ear. For each ear, the model includes the following: a filter to allow for the effects of transfer of sound through the outer and middle ear; a short-term spectral analysis with greater frequency resolution at low than at high frequencies; calculation of an excitation pattern, representing the magnitudes of the outputs of the auditory filters as a function of center frequency; application of a compressive nonlinearity to the output of each auditory filter; and smoothing over time of the resulting instantaneous specific loudness pattern using an averaging process resembling an automatic gain control. The resulting short-term specific loudness patterns are used to calculate broadly tuned binaural inhibition functions, the amount of inhibition depending on the relative short-term specific loudness at the two ears. The inhibited specific loudness patterns are summed across frequency to give an estimate of the short-term loudness for each ear. The overall short-term loudness is calculated as the sum of the short-term loudness values for the two ears. The long-term loudness for each ear is calculated by smoothing the short-term loudness for that ear, again by a process resembling automatic gain control, and the overall loudness impression is obtained by summing the long-term loudness across ears. The predictions of the model are more accurate than those of an earlier model that did not incorporate binaural inhibition.

Recent physiological studies in several rodent species have revealed that permanent damage can occur to the auditory system after exposure to a noise that produces only a temporary shift in absolute thresholds. The damage has been found to occur in the synapses between the cochlea’s inner hair cells and the auditory nerve, effectively severing part of the connection between the ear and the brain. This synaptopathy has been termed hidden hearing loss because its effects are not thought to be revealed in standard clinical, behavioral, or physiological measures of absolute threshold. It is currently unknown whether humans suffer from similar deficits after noise exposure. Even if synaptopathy occurs in humans, it remains unclear what the perceptual consequences might be or how they should best be measured. Here, we apply a simple theoretical model, taken from signal detection theory, to provide some predictions for what perceptual effects could be expected for a given loss of synapses. Predictions are made for a number of basic perceptual tasks, including tone detection in quiet and in noise, frequency discrimination, level discrimination, and binaural lateralization. The model’s predictions are in line with the empirical observations that a 50% loss of synapses leads to changes in threshold that are too small to be reliably measured. Overall, the model provides a simple initial quantitative framework for understanding and predicting the perceptual effects of synaptopathy in humans.

In a collaborative research project, several monaural and binaural noise reduction algorithms have been comprehensively evaluated. In this article, eight selected noise reduction algorithms were assessed using instrumental measures, with a focus on the instrumental evaluation of speech intelligibility. Four distinct, reverberant scenarios were created to reflect everyday listening situations: a stationary speech-shaped noise, a multitalker babble noise, a single interfering talker, and a realistic cafeteria noise. Three instrumental measures were employed to assess predicted speech intelligibility and predicted sound quality: the intelligibility-weighted signal-to-noise ratio, the short-time objective intelligibility measure, and the perceptual evaluation of speech quality. The results show substantial improvements in predicted speech intelligibility as well as sound quality for the proposed algorithms. The evaluated coherence-based noise reduction algorithm was able to provide improvements in predicted audio signal quality. For the tested single-channel noise reduction algorithm, improvements in intelligibility-weighted signal-to-noise ratio were observed in all but the nonstationary cafeteria ambient noise scenario. Binaural minimum variance distortionless response beamforming algorithms performed particularly well in all noise scenarios.

This special issue contains a collection of 13 papers highlighting the collaborative research and engineering project entitled Advancing Binaural Cochlear Implant Technology—ABCIT—as well as research spin-offs from the project. In this introductory editorial, a brief history of the project is provided, alongside an overview of the studies.

We present the first portable, binaural, real-time research platform compatible with Oticon Medical SP and XP generation cochlear implants. The platform consists of (a) a pair of behind-the-ear devices, each containing front and rear calibrated microphones, (b) a four-channel USB analog-to-digital converter, (c) real-time PC-based sound processing software called the Master Hearing Aid, and (d) USB-connected hardware and output coils capable of driving two implants simultaneously. The platform is capable of processing signals from the four microphones simultaneously and producing synchronized binaural cochlear implant outputs that drive two (bilaterally implanted) SP or XP implants. Both audio signal preprocessing algorithms (such as binaural beamforming) and novel binaural stimulation strategies (within the implant limitations) can be programmed by researchers. When the whole research platform is combined with Oticon Medical SP implants, interaural electrode timing can be controlled on individual electrodes to within ±1 µs and interaural electrode energy differences can be controlled to within ±2%. Hence, this new platform is particularly well suited to performing experiments related to interaural time differences in combination with interaural level differences in real-time. The platform also supports instantaneously variable stimulation rates and thereby enables investigations such as the effect of changing the stimulation rate on pitch perception. Because the processing can be changed on the fly, researchers can use this platform to study perceptual changes resulting from different processing strategies acutely.

This study compared spatial speech-in-noise performance in two cochlear implant (CI) patient groups: bimodal listeners, who use a hearing aid contralaterally to support their impaired acoustic hearing, and listeners with contralateral normal hearing, i.e., who were single-sided deaf before implantation. Using a laboratory setting that controls for head movements and that simulates spatial acoustic scenes, speech reception thresholds were measured for frontal speech-in-stationary noise from the front, the left, or the right side. Spatial release from masking (SRM) was then extracted from speech reception thresholds for monaural and binaural listening. SRM was found to be significantly lower in bimodal CI than in CI single-sided deaf listeners. Within each listener group, the SRM extracted from monaural listening did not differ from the SRM extracted from binaural listening. In contrast, a normal-hearing control group showed a significant improvement in SRM when using two ears in comparison to one. Neither CI group showed a binaural summation effect; that is, their performance was not improved by using two devices instead of the best monaural device in each spatial scenario. The results confirm a “listening with the better ear” strategy in the two CI patient groups, where patients benefited from using two ears/devices instead of one by selectively attending to the better one. Which one is the better ear, however, depends on the spatial scenario and on the individual configuration of hearing loss.

In studies investigating binaural processing in human listeners, relatively long and task-dependent time constants of a binaural window ranging from 10 ms to 250 ms have been observed. Such time constants are often thought to reflect "binaural sluggishness." In this study, the effect of binaural sluggishness on binaural unmasking of speech in stationary speech-shaped noise is investigated in 10 listeners with normal hearing. In order to design a masking signal with temporally varying binaural cues, the interaural phase difference of the noise was modulated sinusoidally with frequencies ranging from 0.25 Hz to 64 Hz. The lowest, that is the best, speech reception thresholds (SRTs) were observed for the lowest modulation frequency. SRTs increased with increasing modulation frequency up to 4 Hz. For higher modulation frequencies, SRTs remained constant in the range of 1 dB to 1.5 dB below the SRT determined in the diotic situation. The outcome of the experiment was simulated using a short-term binaural speech intelligibility model, which combines an equalization-cancellation (EC) model with the speech intelligibility index. This model segments the incoming signal into 23.2-ms time frames in order to predict release from masking in modulated noises. In order to predict the results from this study, the model required a further time constant applied to the EC mechanism representing binaural sluggishness. The best agreement with perceptual data was achieved using a temporal window of 200 ms in the EC mechanism.

Besides improving speech intelligibility in background noise, another important objective of noise reduction algorithms for binaural hearing devices is preserving the spatial impression for the listener. In this study, we evaluate the performance of several recently proposed noise reduction algorithms based on the binaural minimum-variance-distortionless-response (MVDR) beamformer, which trade-off between noise reduction performance and preservation of the interaural coherence (IC) for diffuse noise fields. Aiming at a perceptually optimized result, this trade-off is determined based on the IC discrimination ability of the human auditory system. The algorithms are evaluated with normal-hearing participants for an anechoic scenario and a reverberant cafeteria scenario, in terms of both speech intelligibility using a matrix sentence test and spatial quality using a MUlti Stimulus test with Hidden Reference and Anchor (MUSHRA). The results show that all the binaural noise reduction algorithms are able to improve speech intelligibility compared with the unprocessed microphone signals, where partially preserving the IC of the diffuse noise field leads to a significant improvement in perceived spatial quality compared with the binaural MVDR beamformer while hardly affecting speech intelligibility.

Even though hearing aid (HA) users can respond very differently to noise reduction (NR) processing, knowledge about possible drivers of this variability (and thus ways of addressing it in HA fittings) is sparse. The current study investigated differences in preferred NR strength among HA users. Participants were groups of experienced users with clear preferences (“NR lovers”; N = 14) or dislikes (“NR haters”; N = 13) for strong NR processing, as determined in two earlier studies. Maximally acceptable background noise levels, detection thresholds for speech distortions caused by NR processing, and self-reported “sound personality” traits were considered as candidate measures for explaining group membership. Participants also adjusted the strength of the (binaural coherence-based) NR algorithm to their preferred level. Consistent with previous findings, NR lovers favored stronger processing than NR haters, although there also was some overlap. While maximally acceptable noise levels and detection thresholds for speech distortions tended to be higher for NR lovers than for NR haters, group differences were only marginally significant. No clear group differences were observed in the self-report data. Taken together, these results indicate that preferred NR strength is an individual trait that is fairly stable across time and that is not easily captured by psychoacoustic, audiological, or self-report measures aimed at indexing susceptibility to background noise and processing artifacts. To achieve more personalized NR processing, an effective approach may be to let HA users determine the optimal setting themselves during the fitting process.
