Immune-mediated ocular inflammation is a common clinical diagnosis reached for horses with keratitis and uveitis. This diagnosis is made as a diagnosis of exclusion following a thorough effort to rule out an underlying cause for the inflammation, most importantly infectious and neoplastic disease. Practically, response to ophthalmic and systemic anti-inflammatory or immunomodulatory medications is used to support a diagnosis of immune-mediated ocular inflammation; however, such medications are often contraindicated in the face of infection or neoplasia. Regenerative therapy and biologics have the promise to treat equine ocular surface diseases, including corneal ulceration or immune-mediated keratitis, or intraocular diseases such as uveitis. The use of blood-derived products such as serum or platelet-rich plasma, mesenchymal stem cells, or amniotic membrane grafts may be beneficial for the treatment of ulcerative and chronic keratitis in horses. Ophthalmic problems account for up to 20% of emergencies in equine practice. Presenting problems may involve the periocular region, adnexa, or globe. Practitioners must have the experience to restrain horses with painful ocular conditions and the knowledge of how to perform a thorough eye examination. A range of clinical skills is required for the necessary diagnostic tests such as corneal cytology, ultrasound and tonometry, and common standing surgical procedures such as eyelid repair or instillation of a subpalpebral lavage system. Equine recurrent uveitis (ERU) is characterized by recurrent bouts of inflammation interrupted by periods of quiescence that vary in duration. There is little consensus on the clinical manifestations, the underlying causes, or the management. The 3 commonly recognized syndromes of ERU (classic, insidious, and posterior) do not accurately separate the clinical manifestations of disease into distinct categories. An accurate diagnosis and early intervention are essential to minimizing the effects of disease and preserving vision. Infectious conditions of the equine head are commonly encountered in clinical practice. Pathogenic bacterial, viral, and fungal organisms may localize in the extensive nasal passages, paranasal sinuses, and guttural pouches, creating a range of clinical signs and conditions that can be severe enough to lead to unexpected fatality. Renewed interest in equine dentistry has led to a greater recognition of dental disease that is associated with infection. This article focuses on bacterial and fungal infections of the main anatomic regions of the equine head, where advances in diagnosis and management have been made or consolidated in recent years. It also addresses recent advances made in the area of infectious equine corneal disease, including bacterial, viral, and fungal etiologies. Recent developments in equine recurrent uveitis as it relates to infectious diseases and ocular manifestations of systemic disease are also discussed.

Equine recurrent uveitis (ERU) is one of the most common causes of blindness in horses. Until recently, treatment of this condition consisted only of symptomatic therapy, typically with steroidal and nonsteroidal medications. A better understanding of the disease process(es) has permitted new medical and surgical therapies that have recently been described. This article highlights clinical features of ERU, the causes of ERU, and new management and treatment options for horses with ERU.

Equine clinical leptospirosis has been primarily related to uveitis and the often recurrent sequelae of ocular changes. Reports of equine hepatic and renal involvement are sporadic. More recently, serovar bratislava has been identified as a host-adapted serovar in the horse. More significantly, reports of equine abortion and stillbirth have become more frequent in the literature. This is more than likely a result of improved diagnostic techniques, not of increased prevalence of disease. In addition to abortion, equine neonatal disease is becoming more frequently recognized in association with leptospira infection. Whether leptospiral infection results in abortion or diseased foals may depend upon the stage of gestation when the mare is exposed and host immune status. Antibiotic of choice for treatment of equine leptospirosis remains speculative, as specific equine studies have not been performed. Extrapolation from other species suggests that the use of streptomycin remains a good choice of therapy for the chronic shedding state and may be used in combination with other antimicrobials for treatment of acute disease. Penicillin or potentiated penicillins and tetracycline at appropriate to high end dosages are logical choices for the treatment of acute leptospirosis.

Uveitis (inflammation of the iris, ciliary body, or choroid) is a potentially blinding condition with a significant economic impact on the horse industry. Variable symptoms are described, as well as a considerable range of structural and functional sequelae. Known causes of initial episodes include bacterial, viral, traumatic, and parasitic insults, with recurrence by immunologic mechanisms. Treatment strategies and management recommendations that may reduce the incidence or severity of episodes are discussed.
