9. Erfolgreiche Retherapie von Relapsern nach Interferon/Ribavirin freier Kombinationstherapie

Abstract

Die Verfugbarkeit neuer antiviraler Substanzen hat die Therapie der chronischen Hepatitis C revolutioniert. Heute konnen fast alle Patienten innerhalb weniger Wochen geheilt werden. Leider gibt es aber wenige Patienten, bei denen dieser Erfolg nicht gegeben ist. In einer osterreichischen Multicenterstudie wurden 2014 – 2015 392 Patienten mit fortgeschrittenen Lebererkrankungen infolge Infektion mit dem Hepatitis C Virus mit Ribavirin (RBV)-freien Kombinationen von direkt wirkenden antiviralen Substanzen behandelt (AUSTRIAN INTERFERON/RIBAVIRIN-FREE COHORT (AURIC)). 16 (4,1%) Patienten erlitten einen Relapse nach Therapieende. Diese Untersuchung berichtet uber den Erfolg einer Retherapie bei diesen Patienten. Methoden: Den 16 Patienten (12 Manner, 4 Frauen, alle mit Zirrhose) wurde eine Retherapie angeboten, bisher wurden 11 behandelt. Die Vortherapie bestand aus Sofosbuvir (SOF) in Kombination mit Simeprevir (SMV, 12 – 24 Wochen) oder Daclatasvir (DCV, 12 – 24 Wochen) bzw. aus Ledipasvir (LDV; 12 – 24 Wochen). Die Wahl des Zweitregimes war dem Untersucher uberlassen. Der Therapierfolg wurde durch Nichtnachweisbarkeit von HCV-RNA 12 Wochen nach Therapieende definiert. Resultate: Sowohl unter der Ersttherapie als auch unter der Retherapie konnte bei keinem Patienten die HCV RNA nachgewiesen werden. Nach Ende der Retherapie. Hatten 10/11 Patienten einen SVR (siehe Tabelle). Warum der eine Pat. einen Relapse hatte ist unklar. Zusammenfassung: Relapser nach einer RBV freien DAA Kombinationstherapie konnen erfolgreich mit anderen Therapiekombinationen behandelt werden.
