Abstract

患者女，55岁。因“体检发现双肾肿物半个月余”入院，行双肾肿物切除。镜下显示肿瘤呈推挤样生长，与正常组织分界清，个别结节内见纤维化玻璃样变的分隔，间质极少。肿瘤细胞排列成腺管状、腺泡状、巢团状，间以丰富的薄壁血管。肿瘤细胞胞质丰富，部分嗜酸性颗粒状、部分淡染空泡状甚至透明，胞膜清晰；细胞核大，核仁清晰，可见奇异核，双核及核周空晕。免疫组织化学显示肿瘤表达PAX8、CD117及SDHB，部分表达细胞角蛋白7。患者外周血基因检测显示FLCN基因第11号外显子1285号碱基位缺失。结合患者胸部CT显示的多发肺大疱及面部的白色丘疹，诊断为Birt-Hogg-Dubé综合征相关肾细胞肿瘤。.

Abstract

原发性子宫内膜鳞状细胞癌（primary endometrial squamous cell carcinomas，PESCC）是一种罕见的子宫内膜癌。本文报道1例55岁患者，刮宫标本及经腹腔镜筋膜外全子宫切除标本组织学形态均显示不同分化程度的鳞状上皮呈浸润性生长，并见短梭形细胞，胞质丰富、透亮。刮宫标本中未见正常子宫内膜腺体及异常腺性结构；全子宫切除标本中短梭形与鳞状上皮穿插生长，侵犯浅肌层，周围子宫内膜可见慢性子宫内膜炎，局灶符合子宫内膜不典型增生；免疫表型：明确的鳞状上皮成分及短梭形细胞广谱细胞角蛋白、细胞角蛋白（CK）5/6、p63、β-catenin、CD10均阳性，CK7、CDX2阴性。患者随访8个月，身体状况良好。PESCC需要严格掌握诊断标准，除外子宫颈来源的鳞状细胞癌及子宫内膜样癌伴广泛鳞化。该文描述PESCC组织学特点、刮宫及全子宫标本诊断难点、阐述其诊断要点，并复习和总结相关文献。.

宫颈浸润性复层产黏液的癌（invasive stratified mucin-producing carcinoma，ISMC）是新近发现的一种宫颈浸润性黏液性腺癌，属于人乳头状瘤相关性腺癌，组织学特点为实性巢状，巢内细胞抱团式镶嵌排列，多层，叠瓦状，可见散在分布数量不等的产黏液细胞，周边细胞栅栏状排列。免疫组织化学染色表达腺癌标志物，p16弥漫阳性，阴性或弱阳性表达p63、p40等鳞癌标志物。ISMC可独立发生或与其他宫颈癌伴发，临床预后差于普通型腺癌，好于胃型黏液腺癌。.

Abstract

胆囊腺鳞癌是一种罕见的胆囊肿瘤。报道1例72岁男性胆囊腺鳞癌，患者于2018年7月就诊于福建省立医院。镜下示腺癌和鳞状细胞癌成分混合分布，鳞状细胞癌成分以高-中分化为主，存在明显的角化珠及细胞间桥，腺癌成分以中-低分化为主，呈腺样、巢团状生长。胆囊腺鳞癌发生率低，侵袭性大，预后差，目前手术是治疗该病的主要手段，研究胆囊腺鳞癌临床病理学和生物学特征，对于诊断和治疗有重要的指导意义。.

患者女，58岁。肛周肿胀疼痛10余年，术后1年复发。切除肿物病理示：肿瘤细胞呈不规则索状、带状或片状排列，间隔以纤维组织；肿瘤细胞体积大，细胞边界不清，胞质丰富，嗜伊红细颗粒状，并见嗜伊红小体；散在凋亡小体及凝固性坏死灶；核分裂象2/10 HPF。免疫组织化学染色：CD68、S-100蛋白阳性，Ki-67阳性指数5%。特殊染色：胞质过碘酸-雪夫（PAS）染色及淀粉酶消化-过碘酸雪夫（D-PAS）染色阳性。诊断为（会阴部）恶性颗粒细胞瘤。.

31. [Detection of HBxAg in liver specimens of patients with hepatocellular carcinoma]

Abstract

Detection of HBxAg was done by the immunogold-silver staining technique in 22 liver cancer specimens from patients with hepatocellular carcinoma and positive markers of hepatitis B were found to express HBxAg of hepatitis B virus, which was distributed in the perinuclear cytoplasm. The result suggests that HBxAg can be detected practically in all the liver specimens of the patients with hepatocellular carcinoma, who had been infected by hepatitis B virus. The relationship between X gene and its translation product and hepatocellular carcinoma is discussed.
