Abstract

新型HER2抗体耦联药物对于HER2低表达晚期乳腺癌显示出良好的临床获益，HER2低表达指的是免疫组织化学检测HER2 1+或HER2 2+且原位杂交未扩增。HER2低表达的准确检测，不仅为乳腺癌治疗提供新的机遇，也为HER2精准的病理检测带来挑战。本文基于HER2低表达概念的提出，就HER2检测前、检测中及检测后应注意的问题，HER2低表达的异质性及人工智能辅助判读的进展展开讨论，以期促进HER2低表达病理检测的标准化和规范化。.

Abstract

Ki-67是乳腺癌常用的细胞增殖标志物，但因临床有效性较低，在治疗决策中的作用有限。2019年10月国际乳腺癌Ki-67工作组（IKWG）召开会议，讨论了Ki-67免疫组织化学检测在乳腺癌组织中的分析有效性及临床应用现状，并于2020年12月28日在线发表了Ki-67评估的更新，基本共识包括：（1）与雌激素受体（ER）和HER2一样，检测前的影响因素对于检测结果至关重要；（2）推荐采用标准化的视觉评估方法；（3）要有严格的质量评估保证和控制体系，确保分析的有效性；（4）在ER阳性、HER2阴性的T1~T2、N0~N1期患者，当Ki-67≤5%或Ki-67≥30%时可辅助判断是否辅助化疗及预测预后。综上所述，规范检测前影响因素和视觉评估方法可提高Ki-67免疫组织化学的分析效度。目前，Ki-67免疫组织化学评估仅用于Ⅰ期或Ⅱ期乳腺癌患者的预后预测。基于机器学习的自动化评估可能有助于解决目前Ki-67免疫组织化学判读的痛点。.

Abstract

报道3例甲状腺转移性透明细胞性肾细胞癌并探讨其临床病理特征、鉴别诊断、治疗及预后。3例甲状腺转移性透明细胞性肾细胞癌大体表现为灰白-灰褐色，质地硬。镜下观察见3例肿瘤细胞均呈圆形，胞质清晰，呈腺泡状或巢状排列。免疫组织化学染色：3例肿瘤细胞均表达广谱细胞角蛋白（CKpan，AE1/AE3）、PAX8、PAX2、CD10、碳酸酐酶9（CA9）、肾细胞癌标志物（RCC）和波形蛋白。荧光原位杂交（FISH）检测3例均有VHL基因缺失和染色体3p缺失。3例随访1~3年，均未见复发，例1、2因脑血管疾病死亡，例3患者仍健在。.
