Objective: Sepsis remains a serious clinical problem because of high morbidity and mortality. The importance of Toll-like receptors (TLRs) for the induction of immune responses against sepsis was demonstrated in humans. The present study aimed to probe the gene polymorphisms of TLR4 (Asp299Gly and Thr399Ile) and TLR2 (Arg753Gln) in patients with sepsis among Chinese Han children in Wenzhou, and investigate the correlation with sepsis., Method: This study was conducted as a case-control study. Using polymerase chain reaction and DNA sequencing, gene polymorphisms of TLR4 (Asp299Gly and Thr399Ile) and TLR2 (Arg753Gln) in 59 children with sepsis, 38 children with severe sepsis (including 20 septic shock) and 57 healthy controls were analyzed. Hardy-Weinberg method of statistics was used to compare the frequency of genotypes alleles among three groups., Result: The mutant genotypes of TLR4 gene (Asp299Gly and Thr399Ile) were not found among sepsis, septic shock and control groups. In severe sepsis group, the Arg753Gln TLR2 polymorphism occurred in 2 out of 38 severe sepsis patients and both of the subjects with the TLR2 Arg753Gln polymorphism had fatal staphylococcal infections., Conclusion: TLR4 gene (Asp299Gly and Thr399Ile) polymorphisms may not be correlated with susceptibility to sepsis among Chinese Han children in Wenzhou. The fact that only 2 out of 38 severe sepsis patients had Arg753Gln TLR2 polymorphism suggests that a larger sample size is needed because of the rarity of the TLR2 allele among Chinese Han children in Wenzhou.

Objective: The purpose of the 3rd national survey on childhood obesity was conducted not only to understand the present status and trends of childhood obesity in China since the last survey conducted 10 years ago, but also to reveal the health status of preschool children at nutrition transit period and to evaluate the efficacy and sensitivity of cited reference population, criteria and cut-off point of body mass index (BMI), adiposity rebound age, waist/hip ratio and other parameters relevant to the diagnosis of obesity made by the national task force on childhood obesity of China (NTFCOC)., Methods: A total of 84,766 children aged 0 - 7 years were recruited in the survey by the random cluster sampling which represented a 1, 414, 220 children's population from 11 cities covered north, central, south and west regions of China. The criteria of screening overweight/obesity was more than 1 Z-score/2 Z-score of the medium of reference value of weight for height made by WHO. Length-height/weight for all subjects and waist/hip/thigh circumference and blood pressure data for children 3 - 6 years of age were measured. The prevalence of overweight and obesity, overweight-obesity ratio, adiposity rebound age and BMI were calculated. The enumeration and measurement data were statistically managed by chi-square test and T-test, respectively using SPSS version 12.0 and the significance level was 0.05., Results: (1) The prevalence of obesity and overweight was 7.2% and 19.8% for all; 8.9% and 22.2% for boys, and 5.3% and 17.0% for girls, respectively, which is 3.6/4.7 times higher than that of 1996 respectively, the annual increase rate of obesity and overweight was in average 156% and 52%, respectively. The distribution pattern of prevalence of overweight and obesity in geographic areas and gender was that the northern regions had higher prevalence than the west and the central regions and the prevalence in boys was higher than in girls. The obesity/overweight ratio (OOR) was still at a high risk level. (2) BMI at 1 month after birth was higher than 16.5, then increased to the top of 17.8 at 1 year of age and decreased to the bottom of 15.7 at 5.5 years of age and increased a little since then. It was never higher than 18 of diagnosis point of obesity during the preschool age period. (3) The adiposity rebound age was 5.5 years of age in this study and delayed half a year as compared with that seen in 1999. The adiposity rebound ratio at first half year after birth (ARR1) was 0.56 and 0.97 at second half year after birth (ARR2) which is an acceptable level according to the cut-off point made by NTFCOC and lower than the level obtained 10 years ago. (4) The pseudo-overweight phenomenon can be seen in the western regions which was derived from linear growth retardation and showed that both the malnutrition and obesity are a health problem in the poverty and maintain area of the western regions.(5) The 12.9% of obese children who's blood pressure was higher than 95th percentile of reference value and 17.2% of them had a waist/hip ratio higher than 1.0 which is a warning point for the risk factor of CHD made by the NTFCOC. (6) The data showed that all the reference population and it's cut-off point for BMI, adiposity rebound age/ratio, waist/hip ratio etc. made by NTFCOC is valuable, reliable and practicable., Conclusion: The prevalence of childhood obesity and high blood pressure in obese children has been increased sharply during the recent 10 years, which is a out of control and a severe hazard to obese children. The early prevention and management of childhood obesity is urgently needed.

Objective: The reproductive goals of married couples living in impoverished areas of China affect survival rates and quality of life of female children in those areas. The goal of this study was to collect information on parents' attitudes toward reproduction, gender preferences/prejudices, and to collect relevant socioeconomic and demographic data., Methods: Random sample families were enrolled from 3 poverty counties. Sample size included parents of 1800 children from birth to 6 years old. The children were divided into 6 age groups at one-year interval. Baseline data were collected and a questionnaire (endorsed by UNICEF) was used to evaluate reproductive expectations and gender preferences of parents., Results: (1) The success of the One Child Family program has lead to a decreased overall birthrate. (2) The gender ratio (boys:girls) of the 3 counties studied was 100:87-100:95-100:97. No gender imbalance was found in this study. (3) The "Boy Preference" was still expressed by both fathers (69.0%) and mothers (65.7%). Approximately 40% of fathers and 37% of mothers interviewed expressed that only a male child would be acceptable. (4) Approximately 65% of both fathers and mothers recognize that girls are more likely to care for aging parents., Conclusion: Government initiatives have encouraged small families, yet in this region gender ratio of newborns is still normal. More educational and social programs are needed to influence perceptions regarding gender and to eliminate the suboptimal status/poorly perceived value of girls in many societies.

Objective: To study blood Leptin level of 154 (78 male, 76 female) Chinese obese/non-obese children aged 0 - 14 years during 1999 - 2001., Methods: The gender- and age-specific distribution pattern of Leptin and its relationship with anthropometric parameters (waist circumference, waist/hip ratio, lean body mass, fat mass, body fat percentage, BMI/Kaup index etc.) and blood insulin level were recorded., Results: (1) The blood Leptin level in healthy non-obese kids ranged from 1.01 - 29.92 (ng/ml), the mean values and SD were 2.99 +/- 2.13 (ng/ml) [90% confidence interval was 1.36 - 14.21 (ng/ml) in boys and 1.74 - 21.17 (ng/ml) in girls]. There was no significant difference in the blood Leptin level between serum and plasma. (2) The blood Leptin level was higher in overweight/obese kids than that in non-obese kids (P < 0.001). (3) There was significant difference in the blood Leptin levels between boys and girls groups (P = 0.023), especially in non-obese group (P = 0.004). The multiple regression analysis showed that there was no correlation between gender and blood Leptin level when body fat factor was added (P = 0.138, 0.241, 0.990), but there was still a strong correlation between blood leptin level and BMI, FM and BF% (P < 0.001). (4) There was a correlation between blood Leptin level and age (P = 0.005), especially in overweight/obese group and in girls (P = 0.001). The blood Leptin level rose from early puberty, especially in girl group (P = 0.045). There was significant difference in blood Leptin level in different age groups (P < 0.001) (5) There were strong positive correlation between blood Leptin level and BMI, BM and FM%, a weak correlation with LBM, and no correlation with W/H ratio in boys and a positive relationship in girls. The Quatatic equation was better than the linear equation in description of the correlation mentioned above. (6) There was a correlation between blood Leptin from 0 to 7 yr and birth weight (P = 0.001), after 7 yr of age this correlation disappeared (P = 0.456). (7) A positive correlation was seen between blood Leptin level and blood insulin level (P < 0.001)., Conclusion: The blood Leptin level of 0 - 14 years old children is consistent with the level of growth of adiposity tissue and the degree of adiposity rebound.
