Erectile dysfunction (ED) is a common male disease and frequently encountered in Andrology. PDE5 inhibiter can competitively inhibit the hydrolysis of cyclic guanosine monophosphate (cGMP), increase the cGMP concentration in the smooth muscle cells of the corpus cavernosum penis, and achieve the therapeutic effect on ED. Tadalafil can elevate the level of cGMP in the corpus cavernosum and effectively improve ED of various causes and degrees. Tadalafil therapy for ED, characterized by long efficacy, high safety, easy acceptance by patients and their partners, and obvious promotion of patients'self-confidence and natural experience in sexual activities, can achieve satisfactory physiological and psychological results and effectively improve the quality of patients' sexual life. Therefore tadalafil deserves an extensive clinical application in the treatment of ED.

Objective: To evaluate the effects of tadalafil administered on demand or once a day in the treatment of erectile dysfunction (ED)., Methods: We randomly assigned 61 ED patients to three groups to receive tadalafil on demand, at 5 mg once daily, and at 10 mg once daily, respectively. After 42 days of medication, we compared the therapeutic effects among different groups using the patients' sexual encounter profile (SEP) diaries, detected the adverse reactions and assessed the safety of tadalafil., Results: Fifty-three (86.7%) of the patients completed the investigation, and all responded well to tadalafil medication, with a significantly improved success rate of sexual intercourse and a low rate of mild adverse effects. The mean positive rates of SEP were basically similar between the on-demand and once-daily groups., Conclusion: There are no significant differences in the improvement of penile erection and sexual satisfaction of ED patients treated by on-demand and once-daily administration of tadalafil.

Objective: Erectile dysfunction (ED) is now recognized as a comorbid condition, especially in men with cardiovascular disease or diabetes mellitus. This randomized controlled trial was to examine the effect of long-term small-dose tadalafil in the treatment of ED., Methods: A total of 98 men older than 18 years with at least a 6-month ED history were enlisted and divided into two groups to receive once-daily treatment with tadalafil at 5 mg (n = 60) and 20 mg (n = 38), respectively, for 12 months. The effects of medication were analyzed and compared using IIEF, Global Assessment Questionnaire (GAQ) and Sexual Encounter Profile (SEP), and so were the safety and tolerance of the two doses., Results: There were no statistically significant differences in the therapeutical results between the 5 mg and 20 mg groups (P < 0.05). The IIEF-5 score was raised by 8.1 points in the former and 7.9 points in the latter; the YES answers to SEP2 in the two groups were 51.3% and 49.2% before the treatment and 82.6% and 84.9% after it. No serious adverse events were observed, except some common ones, such as rubeosis (11.9% vs 8.7%) and headache (5.3% vs 4.9%) in the 5 mg and 20 mg groups., Conclusion: Oral tadalafil at 5 mg once daily is efficacious with good tolerance in the treatment of ED, and it can be an alternative to on-demand medication for some men to eliminate the inconvenience of planned intercourse within a limited timeframe.

The benefit of tadalafil should be justified by its safety and tolerance in the chronic treatment of erectile dysfunction (ED) in general and high-risk population. The main treatment-emergent adverse events, chiefly induced by the interactions of tadalafil with PDE5 and isoforms, are mild or moderate in severity, transient and reversible, which may disappear without drug withdrawal. Tadalafil does not add to the risk and severity of cardiovascular problems, and can be safely co-administered with antihypertensives and selective a-receptor blockers.

As a novel phosphodiesterase type 5 (PDE5) inhibitor, tadalafil is administered orally for the treatment of erectile dysfunction (ED). In addition to its outstanding efficacy that lasts 36 hours, tadalafil shows an excellent safety and tolerance, with only a few transient mild to moderate adverse events. Usually ED patients are concomitantly afflicted with multiple pre-existing medical conditions or receive a range of drugs at the same time. Therefore, it is essential to understand its safety in general population and some special high-risk cohorts.

The cyclic nucleotide (cGMP) signalling pathway mediates the smooth-muscle relaxing effects of nitric oxide necessary for normal erectile function. Down-regulation of this pathway is the pathophysiological pivot of many forms of erectile dysfunction (ED) and leads to the development of some chronic diseases, such as hypertension and type 2 diabetes mellitus. Therefore, selective inhibition of the enzyme that catalyses the degradation of cGMP promotes erectile responses to sexual stimulation. Recently, a new phosphodiesterase type 5 (PDE-5) inhibitor tadalafil has emerged, which has a prolonged half-life. Here is a review of recent studies on the safety of tadalafil in the treatment of ED.

Erectile dysfunction(ED) is a common ailment in middle-aged and old men. Since tadalafil was introduced into the treatment of ED in October, 2002, more and more ED patients have chosen tadalafil due to its efficacy and safety. The action mechanism, pharmacokinetics, efficacy and safety of tadalafil have been reported in many basic and clinical research documents. This review is to address the safety of tadalafil in treating ED, and provide further guidance for clinicians' choice and safer use of the drug.

Objective: To observe the safety and efficacy of tadalafil in the treatment of erectile dysfunction in clinical research., Methods: Eighty patients with ED, before and after 20 mg tadalafil administered orally, received respectively the rigiscan examination with aural and visual sexual stimulation at the same time, and then Bphallic circumference variation, stiffness index, duration were compared before and after the treatment., Results: Phallic stiffness index, duration revealed a statistically (P < 0.05) obvious improvement before and after the treatment for all patients. A significant difference (P < 0.01) on the phallic stiffness index, duration were found between the patients with psychogenic ED and others with organic or mixed ED after the treatment. Overall satisfaction with tadalafil was 82.5%. There was an obvious difference on the curative rates between the patients with psychogenic ED (92.3%) and others with organic ED (68.7%) or mixed ED (58.3%). All adverse events (11 cases of headache and dizziness, 8 cases of digestive canal unwell, 5 cases of flushing, 2 cases of back muscle pain) associated with tadalafil were mild and recovered without any treatment., Conclusion: Tadalafil improves erectile function significantly in most patients. Adverse reactions are generally transient and mild to moderate in nature.
