To assess the efficacy and safety of the drug ampasse in the treatment of patients with chronic cerebrovascular disorders (CCVD).A multicenter, randomized, double-blind, placebo-controlled, confirmatory study of the efficacy and safety of ampasse (phase III) was conducted in 124 patients aged 50 to 75 years. The main group (MG) - 62 patients, received the test drug ampasse, solution for intravenous administration, 5 mg/ml, at a dose of 5 ml (25 mg), intravenously bolus slowly, the duration of treatment was 15 days. Control group (CG) - 62 patients, received comparison drug: placebo (0.9% sodium chloride-5 ml).All 124 patients fully completed the procedures and visits, there were no dropouts from the study. The proportion of patients who reached the primary endpoint (an increase in the score by 2 or more points on the MoCA scale) was 83.87% in MG and 22.58% in CG, that is, the efficacy of therapy in MG was 61.29% higher than in CG (Ampasse has a positive effect on cognitive functions and QOL, does not increase the frequency of adverse events in patients with CCVD compared to placebo, does not cause significant side effects, and is well tolerated by patients.Оценка эффективности и безопасности препарата ампассе в лечении пациентов с хроническим нарушением мозгового кровообращения (ХНМК).Проведено многоцентровое рандомизированное двойное слепое плацебо-контролируемое исследование эффективности и безопасности лекарственного препарата ампассе (III фаза) у 124 пациентов в возрасте от 50 до 75 лет. Основная группа (ОГ) — 62 пациента получали исследуемый препарат ампассе, раствор для внутривенного введения, 5 мг/мл, в дозе 5 мл (25 мг), внутривенно струйно медленно, продолжительность лечения 15 дней. Группа сравнения (ГС) — 62 пациента получали плацебо (0,9% натрия хлорид — 5 мл).Все 124 включенных в исследование пациента прошли полностью курс терапии, выбывших из исследования не было. Доля пациентов, достигших первичной конечной точки (увеличение оценки на 2 балла и более по шкале MoCA) в ОГ составило 83,87%, в ГС — 22,58%, т.е. эффективность терапии в ОГ была на 61,29% выше, чем в ГС (В результате проведенного исследования обнаружено, что применение препарата ампассе оказывало положительное действие на когнитивные функции и качество жизни, не увеличивало частоту нежелательных явлений у пациентов с ХНМК по сравнению с плацебо, не вызывало значимых побочных эффектов, хорошо переносилось пациентами.

Neurodegeneration (ND) is the basis of the gradual increase in neurological deficit and cognitive impairment in patients with multiple sclerosis (MS). Morphometry of the brain is the method for monitoring of ND. The possibility of using neuropsychological tests for this purpose is being actively discussed.To examine the effect of treatment with interferon beta-1b (IFNb-1b) on the development of the neurodegenerative process according to morphometry and cognitive testing.Twenty-three patients with relapsing-remitting MS were examined. The control group included 10 healthy people. All patients received IFNb-1b in a dose of 9.6 million IU. EDSS, MSFC, depression and anxiety Beck test, Wechsler test, magnetic resonance imaging (MRI) of the brain, voxel morphometry with FreeSurfer 5.3 were used. The EDSS score was 4.0 before treatment.In MS patients, MSFC and Wechsler scores were worse in comparison with similar indicators of the control group. Deterioration of neurological status, motor function test and PASAT during exacerbation was identified in 7 patients. Morphometry revealed the increase in hypointense white matter lesions (HWML) (p0.05) in patients with MS. There was a downward trend in the cortical and subcortical gray matter volume and the increase in ventricular volume. A direct correlation between the amount of HWML and the degree of disability on EDSS scale (p=0.018), the inverse correlation between HWML and Wechsler test, PASAT (p0.05) were identified. After a year of therapy, there were no statistically significant changes in motor and cognitive functions and no signs of 'pseudoatrophy'.Morphometric data in MS were changed compared to healthy controls. There were correlations between morphometry results, disability and cognitive dysfunction. After a year of IFNb-1b therapy, there were no significant changes in cognitive function and EDSS.Нейродегенерация (НД) - основа постепенного нарастания неврологического дефицита и когнитивных нарушений у больных рассеянным склерозом (РС). Для мониторирования процесса НД используют морфометрию головного мозга. Возможность применения для этой цели нейропсихологических тестов активно обсуждается. Цель исследования. Изучить влияние терапии интерфероном-β-1b (ИФН-β-1b) на развитие нейродегенеративного процесса по данным морфометрии и когнитивного тестирования. Материал и методы. Обследованы 23 пациента с ремиттирующим течением РС. Контрольную группу составили 10 практически здоровых. Все пациенты с РС получали терапию ИФН-β-1b 9,6 млн МЕ через день. Использовали шкалы EDSS, MSFC, депрессии и тревоги Бека, тест Векслера, проводили магнитно-резонансную томографию (МРТ) головного мозга, воксельную морфометрию с помощью приложения FreeSurfer 5.3. До начала терапии средний показатель по шкале EDSS у больных РС был менее 4,0 баллов. Результаты и заключение. У пациентов с РС показатели тестов MSFC и Векслера были снижены по сравнению с аналогичными показателями контрольной группы. У 7 в период обострения выявлено ухудшение показателей неврологического статуса, двигательных функций и теста PASAT. При морфометрии у пациентов с РС был увеличен объем гипоинтенсивных очагов в белом веществе (ГОБВ) (p0,05), имелась тенденция снижения объема серого вещества коры и подкорковых структур, увеличение объема желудочков. Выявлена прямая корреляционная связь между объемом ГОБВ и степенью инвалидизации по шкале EDSS (p=0,018), обратная корреляция между ГОБВ и показателями теста PASAT (p0,05). При выполнении повторного исследования через год терапии не было обнаружено статистически значимых изменений показателей двигательных и когнитивных функций, были выявлены признаки 'псевдоатрофии'. Таким образом, морфометрические показатели мозга при РС были изменены по сравнению с контролем, выявлены корреляции данных морфометрии, инвалидизации и когнитивной дисфункции. После года терапии ИФН-β-1b не отмечено значимых изменений когнитивных функций и оценки по шкале EDSS.

To investigate the efficacy and safety of the exoskeleton ExoAtlet in complex therapy of patients with multiple sclerosis (MS).A pilot study within the prospective open controlled program was conducted. Eighteen patients with relapsing-remitting MS (RRMS) in remission and secondary progressive MS (SPMS) with the level of neurological deficit on the EDSS from 3 to 7 points have completed the study. EDSS, MSFC, HADS, MoCA scales were administered and the force measuring insoles F-Scan Tekscan (USA) were used to study the biomechanics of walking.Good tolerability of workload within 30-40 min. was observed. The improvement in the EDSS was detected in 9 patients, in whole, a significant positive trend (p0.01) was shown. The study of the biomechanics of the walk showed its significant impairment compared to healthy individuals: reduction of parameters of rate, speed and step length, significant instability, pronounced asymmetry, the decrease in support and shock lower limb function, high coefficient of variability of the parameters, the phenomenon of recurrence of the vertical component of support reactions. After a course of exercise of walking in the exoskeleton, the walking speed and stability increased, oscillation of the body decreased, support function increased, the phenomenon of cyclical changes of the vertical component of support reactions reduced. The results of the pilot study showed promising future research opportunities for robotic-assisted walking and maintenance of the vertical posture with the help of the exoskeleton ExoAtlet to restore the abilities of movement in MS patients with locomotor disorders.Цель исследования. Изучение эффективности и безопасности применения экзоскелета ExoAtlet в комплексной терапии пациентов с рассеянным склерозом (РС). Материал и методы. Было проведено пилотное исследование по открытой контролируемой наблюдательной программе. Эзоскелет ExoAtlet применили у 18 пациентов с ремиттирующим РС в стадии ремиссии (РРС) и вторично-прогредиентным течением (ВПРС) с уровнем неврологического дефицита по шкале EDSS от 3 до 7 баллов. Наряду с клиническими шкалами EDSS, MSFC, HADS, MoCA для изучения биомеханики ходьбы использовали силоизмерительные стельки F-Scan фирмы 'Tekscan' (США). Результаты и заключение. В процессе проведения занятий была отмечена хорошая переносимость нагрузок в пределах 30-40 мин. У 9 из 18 пациентов зарегистрировано улучшение состояния по шкале EDSS, в целом отмечена достоверная положительная динамика. При исследовании биомеханики ходьбы исходно обнаружено ее значительное нарушение по сравнению с показателями здоровых: снижение параметров темпа, скорости и длины шага, значительная неустойчивость, выраженная асимметрия, снижение опорной и толчковой функций нижних конечностей, высокий коэффициент вариативности параметров, явление цикличности вертикальной составляющей опорной реакции. После курса тренировки ходьбы в экзоскелете отмечено повышение темпа ходьбы, возросла устойчивость, уменьшились раскачивания туловища относительно фронтальной плоскости, снизились асимметрия, возросла опорная функция, уменьшилось явление цикличности изменений вертикальной составляющей опорной реакции. Полученные результаты показали перспективность дальнейших исследований возможностей роботизированного ассистирования ходьбе и поддержанию вертикальной позы с помощью экзоскелета ExoAtlet для восстановления способностей передвижения у больных РС с двигательными нарушениями.

We present the results of a one-year follow-up study of 11 patients treated with gilenya. The results of cardiomonitoring (ECG and daily arterial pressure monitoring) during the treatment with the first dose of the drug are presented. No clinically significant disturbances of functions in patients without cardiovascular diseases were noted. In these cases, no progression of disease was identified. Relapses were not observed in 82% of patients.

Five hundreds and thirty-six patients, aged from 19 to 50 years, have been studied. Occipital lobe epilepsy (OLE) have been diagnosed in 22 (4,1%) of cases. The study included evaluation of anamnesis, clinical and neurological examination, routine EEG and/or video-EEG monitoring and MRI of the brain. Duration of the study was 1-5 years (on average 3 years). The most typical clinical features were visual phenomena, mostly "irritation symptoms" represented by elementary hallucinations. Visual "symptoms of loss" (scotomas, amaurosis) are associated with ineffectiveness of antiepileptic drugs. The regional epileptiform activity in occipital leads is found in the routine EEG in 1/4 of patients, epileptogenic structural lesions of the same localization in the MRI - in 1/3 of patients. The data suggest that these diagnostic methods are not sufficient and support the validity of clinical diagnosis.

One hundred and seventy-five patients (73 men and 102 women, aged 16-76 years) with symptomatic and probable symptomatic epilepsies, were studied. The duration of disease was from few months to 38 years. The study included evaluation of anamnesis, clinical and neurological examination, routine EEG and/or video-EEG-monitoring, X-ray CT and/or MRI of the brain. The antiepileptic therapy was started or corrected. The period of observation was 1-3,5 years, on average 2 years. Duration of disease, correctness and effectiveness of the previous AED therapy, EEG and neuroimaging data, frequency and type of epileptic seizures were evaluated. A remission was achieved in 96 (54.8%) cases, a reduction of seizures frequency by 50% and more--in 45 (25.8%), no effect--in 34 (19.4%). Epileptiform activity on routine EEG, long disease duration, high frequency of seizures, focal seizures with/without secondary generalization were associated with ineffectiveness of the treatment. Neuroimaging findings and a character of the previous therapy did not impact on the prognosis of disease.

One hundred and seventy-three patients with symptomatic and probable symptomatic epilepsies, mean age 31.2 years, were studied. The study included evaluation of anamnesis, clinical and neurological examination, routine EEG and/or video-EEG-monitoring, X-ray CT and/or MRI of the brain. The antiepileptic therapy was corrected. The duration of follow-up study was 2 years. General neurologist therapy (initial therapy) and epileptologist therapy (final therapy) were compared using cost-effectiveness analysis, frequency of using different drugs, ABC- and VEN-analysis. The final therapy was effective in 93.1% of cases, a remission was achieved in 53.2%. The ineffective therapy decreased from 56.6 to 6.9%. The ineffective expenses for the initial therapy were 82.8%. The final therapy was significantly effective (p = 0.001) and expensive (p = 0.004) but the cost difference was low (3420 roubles/year).

Cases of young patients with different diseases of nervous system in which tremor was the main symptom, clinical peculiarities of trembling hyperkinesis in essential tremor, in olivopontocerebellar degeneration, in strionigral atrophia, in residual phenomena of organic brain damages as well as their differences from functional hyperkinesia are characterized. The data are presented on comparative efficiency of different methods of examination.
