Circular RNA (circRNA), an emerging non-coding RNA, is closely related to the processes of lipid deposition, inflammatory response, apoptosis, proliferation and repair in atherosclerosis (As), and affects the development of As. Traditional Chinese medicine (TCM) has an important academic value in the prevention and treatment of As, and its regulatory mechanism has not been comprehensively summarized. This paper reviewed the association of circRNA with lipid deposition, inflammatory response and macrophage in As, and summarized the research progress of the correlation between circRNA and atherosclerosis and the regulation of traditional Chinese medicine. Macrophage is a kind of immune cell which is closely related to atherosclerosis (As). With the change of internal environment, macrophage can polarize into different phenotype. Under the physiological environment, the polarization of macrophages is in dynamic balance. In the As state, the polarization balance is destroyed and the corresponding phenotypic macrophages are differentiated, which play different roles in each stage of As. The debate between Zheng and Xie exists in the whole process of As formation and development, while macrophage polarization runs through the whole stage of As syndrome differentiation and change. On account of this, this paper discusses the relationship between macrophage polarization and As based on the theory of Zheng and Xie, and propose that attention should be paid to the combination of macro and micro in the clinical prevention and treatment of As, and it can control the inflammatory response and stabilize plaque by regulating the macrophage polarization, thus providing an emerging target for the treatment of As with traditional Chinese medicine. Atherosclerosis (As) is the pathological basis of various vascular diseases, which affects the function of important organs. Recent studies have found that N 6 -methyladenosine (m6 A) modification plays a vital role in As. This paper summarizes the regulatory effects of m6 A modification on vascular endothelial cell (VEC) injury, vascular smooth muscle cell (VSMC) transformation, macrophage (Mø) differentiation, foam cell (FC) formation, pyroptosis, and lipid regulation, and summarizes the regulation function of some m6 A regulatory proteins in As. Atherosclerosis is the main pathological basis for the occurrence and development of cardiovascular and cerebrovascular diseases. Macrophage efferocytosis deficiency exists in the formation of atherosclerosis. Defective efferocytosis leads to the accumulation of uncleared apoptotic cells and secondary necrosis, leading to characteristic formation of necrotic core and plaque instability in atherosclerosis. The complete process of efferocytosis includes recognition("find me")stage, phagocytosis("eat me")stage, and the post-processing reaction (digestion) stage. It has been reported that miRNA is involved in the regulation of key signal transduction and lipid homeostasis in atherosclerotic efferocytosis. This paper discussed the important role of efferocytosis in the process of atherosclerosis and the influence of efferocytosis defect on atherosclerosis. This review focuses on the regulatory effects of "eat me" and "don􀆳t eat me" signals of efferocytosis in the overall inflammation of atherosclerosis, and meanwhile the regulation of miRNA on lipid metabolism and inflammation regression in atherosclerosis, and the fine tuning of miRNA targeting macrophage efferocytosis in the process of atherosclerosis. Macrophages play multiple roles in atherosclerosis. The progression of atherosclerosis is associated with morphological and functional changes in the lymphatic vessels of the diseased arteries, but the mechanism is not fully understood. This paper mainly reviews the origin and classification, the markers and the function of macrophage in atherosclerosis, the origin, the structure function and the markers of lymphatic vessels in atherosclerosis, the changes of arterial wall lymphangiogenesis in different stages of atherosclerotic lesions, the functional role of lymphangiogenesis in atherosclerosis, the lymphatic migration of macrophages and its mechanism involved in lymphangiogenesis, in order to provide a basis for the mechanism research and clinical treatment of atherosclerosis. Selective autophagy is specific to degraded substrate proteins, including mitophagy, lipophagy, inflammasomophagy, reticulophagy, pexophagy, aggrephagy and ribophagy, etc. Atherosclerosis (As) is a chronic inflammatory disease mainly characterized by lipid accumulation, foam cell formation, endothelial cell dysfunction, abnormal proliferation of vascular smooth muscle cell, and local inflammation. In recent years, a large number of studies have suggested that selective autophagy is involved in the development of As. This paper reviewed the regulatory mechanisms of selective autophagy, and the potential roles of mitophagy, lipophagy, inflammasomophagy, reticulophagy and pexophagy in As, which can provide new ideas for the treatment of As. 预防卒中的发生和复发是缺血性脑血管病防治的首要任务，多发脑血管狭窄合并主动脉弓溃疡斑块的患者缺血性卒中复发的风险显著升高。本文介绍1例普罗布考联合阿托伐他汀治疗多发脑血管狭窄合并主动脉弓溃疡斑块患者的发病和治疗情况。该例患者表现为轻度神经功能缺损，住院期间发现多发脑血管狭窄合并主动脉弓溃疡斑块，在序贯双联抗血小板治疗3个月后持续阿司匹林单药抗血小板治疗，并全程联合普罗布考和阿托伐他汀强化降脂、抗氧化治疗，随访25个月，无卒中复发，且各项影像学和血液学检查指标保持稳定。 Abstract: Preventing the occurrence and recurrence of stroke is the top priority of the treatment strategy for ischemic cerebrovascular disease. Patients with multiple cerebral vascular stenosis and aortic arch ulcer plaques have a significantly increased risk of recurrent ischemic stroke. This paper introduces a case of probucol combined with atorvastatin in the treatment of multiple cerebral vascular stenosis with aortic arch ulcer plaque. This acute ischemic stroke patient presented with mild neurological impairment. During his hospitalization, multiple cerebral vascular stenosis and aortic arch ulcer plaques were discovered. After 3 months of sequential dual antiplatelet therapy, aspirin therapy was continued and combined with probucol and atorvastatin throughout the course for enhanced lipid-lowering antioxidant therapy. After 25 months of follow-up, there was no recurrence of stroke in this patient, and all imaging and hematological indicators remained stable.

Cardiovascular and cerebrovascular diseases are the main causes of disability and death, and atherosclerosis is an important cause. The incidence of atherosclerosis in premenopausal women is significantly lower than that in men of the same age, while the incidence of atherosclerosis in postmenopausal women is significantly higher than that in men of the same age, which is closely related to estrogen levels. In this paper, sex differences in atherosclerosis were reviewed from the aspects of imaging examination, risk factors, intestinal microbiome and vascular inflammatory effects on atherosclerosis, to provide evidence for precision medical treatment of atherosclerosis.

Atherosclerosis is the main cause of cardiovascular diseases， while Carotid intima-media thickness is a biomarker of subclinical atherosclerosis. Numerous studies have confirmed that exercise can slow down or even reverse the formation and progression of carotid Intima-Media thickness. This paper provides a review of the effects of different exercise modalities， intensities， and populations on the vascular structure to provide a reference for the prevention of atherosclerosis with exercise.
