Objective·To systematically integrate relevant influencing factors of advanced cancer patients' engagement behavior in advance care planning (ACP).Methods·The systematic search of Chinese and English literature on factors influencing ACP engagement behavior in advanced cancer patients from inception to December 2022 in China National Knowledge Infrastructure (CNKI), Wanfang, China Biomedical Literature Database (Sinomed), PubMed, Cochrane Library, Embase, CINAHL, and PsycINFO was conducted. After the literature quality evaluation, content extraction and summary were conducted by two researchers, and the data of quantitative research and qualitative research were extracted and integrated respectively. The final influencing factors of ACP engagement behavior of advanced cancer patients were obtained. With the help of the theoretical domain, they were mapped to the capability, opportunity, motivation-behavior (COM-B) model step by step.Results·A total of 21 studies were included and 27 factors were summarized, including 9 theoretical domains. Mapping to the COM-B model included 9 capability factors (knowledge of ACP, education level, accurate knowledge of prognosis, knowledge of the time of disease diagnosis, prior experience, subjective life expectancy, age, cancer site, and disease symptom burden), 13 opportunity factors (gender, marital status, race/ethnicity, religious belief, dependent children, family economic condition, place of living, housing type, family support, social support, doctor-patient relationship, acculturation, and whether or not to establish a hospice service center) and 5 motivational factors (ACP attitude, ACP belief, ACP motivation, anxiety and depression, and death attitude). Among them, doctor-patient relationship, religious belief, ACP attitude, educational level, marital status, family support, knowledge of ACP, accurate knowledge of prognosis, age, place of living, attitude toward death, prior experience, and race/ethnicity were more influential factors on ACP engagement behavior.Conclusion·Based on the COM-B model, the influencing factors of ACP engagement behavior in advanced cancer patients can be comprehensively summarized. Future studies can use the above factors as an entry point to design continuous, multifaceted, and comprehensive interventions based on the COM-B model to promote the practice of ACP engagement behavior in advanced cancer patients.

Objective To investigate the predictive effect of circulating tumor cell (CTC) combined with neutrophil to lymphocyte ratio (NLR) on efficacy of advanced gastric cancer. Methods The clinicopathological data of 36 patients with advanced gastric cancer who received firstline treatment in Jiangning Hospital Affiliated to Nanjing Medical University and Jiangsu Cancer Hospital from June 2022 to January 2023 were collected. The optimal cutoff values of CTC and NLR were calculated using X-tile software, and the patients were divided into high CTC group (CTC>15, n=21) and low CTC group (CTC≤15, n=15), high NLR group (NLR>2.32, n=20) and low NLR group (NLR≤2.32, n=16). COX regression analysis and survival curve were used to analyze the relationship between CTC, NLR and progression free survival (PFS). Receiver operating characteristic (ROC) curves compared the predictive power of CTC, NLR, and the combination in predicting the outcome of advanced gastric cancer. Results Multivariate COX regression analysis showed that CTC (HR=7.441, P=0.045) and NLR (HR=4.690, P=0.011) were associated with PFS in patients with advanced gastric cancer. Objective: To explore the efficacy of tislelizumab combined with pemetrexed and carboplatin chemotherapy in elderly patients with advanced lung adenocarcinoma and its effects on serum insulin-like growth factor 1 (IGF-1) and insulin-like growth factor binding protein 3 (IGFBP-3) levels. Methods: Ninety-four driver gene-negative elderly patients with advanced lung adenocarcinoma admitted to the Fourth Affiliated Hospital of Nanjing Medical University from July 2021 to December 2022 were selected as the study subjects. They were divided into chemotherapy group (pemetrexed combined with carboplatin, n=48) and tislelizumab group (pemetrexed+carboplatin+tislelizumab, n=46) by the random number table method, and both groups were treated for 4 cycles, with 21 days as a cycle. The clinical efficacy, immune function ［T lymphocyte subsets (CD4+, CD8+, CD4+/CD8+］ and serum indicators (IGF-1, IGFBP-3) before and after treatment were recorded in both groups, and the adverse reactions were assessed. Results: The clinical efficacy and objective response rate (58.70% vs 29.17%, χ2=8.329, P＜0.01) in tislelizumab group were significantly higher than those in chemotherapy group. After 4 cycles of treatment, the CD4+, CD4+/CD8+ and IGFBP-3 levels in both groups were significantly increased compared with those before treatment (P＜0.05), and the indicators in tislelizumab group were higher than those in chemotherapy group (P＜0.05); CD8+ and IGF-1 levels were significantly decreased in both groups compared with those before treatment (P＜0.05), and the indicators in tislelizumab group were lower than those in chemotherapy group (P＜0.05). Adverse reactions were mainly grade 1-2 in both groups, and there was no significant difference in the incidence of adverse reactions between two groups (P＞0.05). Conclusion: The first-line therapy of tislelizumab combined with platinum-containing dual-drug for elderly advanced lung adenocarcinoma can improve the clinical efficacy, improve the balance of CD4+/CD8+, regulate the expression of serum IGF-1 and IGFBP-3 levels. The adverse reactions are controllable, and the treatment regimen is safe and reliable.

Objective: To investigate the effects of telizumab injection combined with albumin paclitaxel and carboplatin on intestinal flora and prognostic nutritional index (PNI) and systemic immune inflammatory index (SII) in patients with advanced non small cell lung cancer (NSCLC). Methods: 115 patients with advanced NSCLC who were treated in Nanjing Jiangning Hospital and Zhongda Hospital Affiliated to Southeast University from May 2020 to February 2023 were selected. Patients were divided into control group and study group according to the random number table method, 57 cases and 58 cases respectively. Control group was treated with albumin paclitaxel and carboplatin, and study group was treated with tirelizumab injection on the basis of control group. The objective response rate (ORR), disease control rate (DCR), serum tumor markers [cytokeratin 19 fragment (CYFRA21-1), carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125)], intestinal flora (bifidobacterium, lactobacillus, Enterococcus), SII and PNI were compared between two groups, and the incidence of adverse reactions during treatment was observed. Results: The ORR (44.83%) and DCR (77.59%) in study group were higher than those in control group (24.56%, 50.88%) (P<0.05). Compared with control group after treatment, CYFRA21-1, CEA, CA125, Enterococcus and SII in study group were lower, and bifidobacterium, lactobacillus and PNI were higher (P<0.05). There was no difference in the incidence of adverse reactions between two groups (P>0.05). Conclusion: Tirelizumab injection combined with albumin paclitaxel and carboplatin in the treatment of patients with advanced NSCLC, which can improve clinical efficacy and improve SII, PNI and intestinal flora. Objective: To investigate the effect of Camrelizumab combine with chemotherapy on immune function, neutrophil to lymphocyte ratio (NLR), C-reactive protein to albumin ratio (CAR) in patients with advanced non-squamous non-small cell lung cancer (NSCLC). Methods: Used retrospective analysis, 100 patients with advanced non-squamous NSCLC who were admitted to the first affiliated hospital of Anhui medical university north district from January 2020 to December 2022 were selected, and patients were divided into control group (n=48) and observation group (n=52) according to the treatment methods. Control group received chemotherapy with pemetrexed disodium combined with cisplatin injection or nedaplatin for injection, and observation group received Camrelizumab treatment on the basis of control group. The efficacy, immune function, tumor markers, NLR, and CAR were compared between two groups, and the incidence of adverse reactions during treatment were observed simultaneously in two groups. Results: Compared with control group, the clinical total effective rate in observation group was higher (P<0.05). 4 cycles after treatment, compared with control group, the carcinoembryonic antigen (CEA), cytokeratin 19 fragment antigen 21-1 (CYFRA21-1), carbohydrate antigen 125 (CA125), NLR, CAR and CD8+ in observation group were lower, while CD3+, CD4+ and CD4+/CD8+ were higher (P<0.05). There was no difference in the incidence of adverse reactions between two groups (P>0.05). Conclusion: Patients with advanced non-squamous NSCLC combine with Camrelizumab treatment on the basis of chemotherapy, which is helpful to control disease progression, improve clinical efficacy, reduce tumor markers levels, improve immune function, and reduce NLR and CAR. Objective To explore the preoperative predictors of lymphovascular invasion (LVI) in patients with advanced gastric cancer, and establish the corresponding nomogram prediction model and conduct internal validation. Methods A total of 246 cases of advanced gastric cancer who underwent surgical resection in the Department of Gastrointestinal Surgery of Hengshui People's Hospital from January 2018 to December 2021 were selected. Patients were divided into the LVI positive group and the LVI negative group according to postoperative pathological diagnosis. The age, gender, tumor differentiation, tumor size, tumor site, Borrmann classification, Lauren's classification, cT stage, cN stage and systemic immune-inflammation index (SII) of patients were collected and compared between the two groups. The predictors that were statistically different between the two groups were subjected to multivariate Logistic regression and further developed into a visual prediction model. Bootstrap method was applied for internal validation of the prediction efficiency of the model. Results The differences of tumor size, Borrmann classification, tumor differentiation, Lauren classification, cT staging, cN staging and SII were statistically significant between the two groups ( P＜0.05). Multivariate Logistic regression analysis showed that tumor size (OR=2.184, 95%CI: 1.224-3.898), Borrmann classification (OR=2.517, 95%CI: 1.294- 4.896), cT staging (OR=1.860, 95%CI: 1.045-3.308), cN staging (OR=1.816, 95%CI: 1.004-3.285) and SII (OR=1.001, 95%CI: 1.000-1.002) were independent predictors of LVI in advanced gastric cancer. A preoperative nomogram prediction model for advanced gastric cancer LVI was developed based on results of multivariate analysis. By internal validation, the area under curve (AUC) value of the subject operating characteristic (ROC) curve of the nomogram was 0.735, which was higher than that of tumor size (0.599), Borrmann staging (0.564), cT staging (0.604), cN staging (0.582) and SII (0.615), respectively. The calibration curve showed that the probability of predicted LVI by the nomogram was in a good agreement with the probability of actual LVI occurrence. The Hosmer-Lemeshow test showed good model fit (χ² =4.387, P=0.821). Conclusion The established nomogram prediction model can help to predict the probability of LVI in advanced gastric cancer preoperatively, which can provide a guideline for clinical individualized treatment. With the continuous development of immunotherapy, the treatment of patients with advanced non-small cell lung cancer (NSCLC) is gradually becoming more and more precise. The clinical benefit of immune checkpoint inhibitors in patients with NSCLC with positive programmed cell death-ligand 1 (PD-L1) expression is obvious, but the effect in patients with negative expression remains inexact. This article reviews the treatment options available for advanced NSCLC patients with negative PD-L1 expression, aiming at providing references and options for their treatment.

Objective To explore the clinical efficacy and immunomodulatory effects of anlotinib capsule combined with GP regimen in the treatment of advanced non-small cell lung cancer(NSCLC). Methods Forty patients with advanced NSCLC admitted to Nanjing Jiangbei Hospital from October 2020 to October 2022 were divided into two groups using a random number table method. The chemotherapy group ( n=20) was treated with GP regimen, and the combined group ( n=20) was treated with anlotinib capsule on the basis of the chemotherapy group. The short-term efficacy, immune function indexes (CD3+, CD4+, CD4+/CD8+, NK cells) and FACT-L scale scores of the two groups were detected before treatment and 4 months after treatment, and the incidence of adverse reactions was recorded. Results After treatment, the disease control rate (95.00% vs 70.00%, χ2=4.329, P=0.037) and the total effective rate (70.00% vs 30.00%, χ2=6.400, P=0.011) of the combined group were better than those of the chemotherapy group. After treatment, CD3+, CD4+, CD4+/CD8+ , NK cells and FACT-L and KPS scores in the two groups were increased, and those in the combined group were higher than those in the chemotherapy group (P0.05). Conclusion Anlotinib capsule+GP regimen can improve the overall therapeutic effect of patients with advanced NSCLC, not only improve the function of the patient ‘s immune system, but also has high safety, and has certain clinical application value.

Objective: To investigate the effects of Jiawei Bazhen decoction combined with m FOLFOX6 regimen on tumor markers, immune function and peripheral blood phosphatidylinositol3-ki-nase (PI3K) -serine protein kinase (Akt) signal pathway in patients with advanced colorectal cancer. Methods: According to the random number table method, 96 patients with advanced colorectal cancer who were treated in Beibei Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Traditional Chinese Medicine from September 2020 to January 2022 were divided into control group (48 cases, treated with m FOLFOX6 regimen) and study group (48 cases, treated with Jiawei Bazhen decoction combined with m FOLFOX6 regimen) . The efficacy, TCM syndrome score, Karnofsky score (KPS), tumor markers, immune function and peripheral blood PI3K-Akt signal pathway related indexes were compared between the two groups. Results: Compared with the control group, the total clinical effective rate in the study group was higher (P＜0.05). After treatment, The total of TCM syndrome score in the study group after treatment was lower than that in the control group, and the KPS score was higher than that in the control group (P＜0.05). After treatment, CD4+, CD4+/CD8+, immunoglobulin (Ig) M and Ig A in the study group were higher than those in the control group, and CD8+was lower than that in the control group (P＜0.05). After treatment, the serum carcinoembryonic antigen (CEA), carbohydrate antigen 199 (CA199) and carbohydrate antigen 72-4 (CA72-4) in the study group were lower than those in the control group (P＜0.05). After treatment, the expression levels of PI3K, Akt m RNA in the peripheral blood of study group were lower than those in the control group (P＜0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P＞0.05). Conclusion: Jiawei Bazhen decoction combined with m FOLFOX6 regimen has a significant effect in the treatment of advanced colorectal cancer, which can effectively prevent the disease progression of advanced colorectal cancer, enhance immunity, and reduce the level of serum tumor markers. Objective　To explore the relationship between quantitative analysis of real-time ultrasound elastography and the prognosis of percutaneous transbiliary drainage ( PTCD) in elderly patients with advanced malignant obstructive jaundice. Methods　Retrospectively selected 112 elderly patients with advanced malignant obstructive jaundice who underwent PTCD treatment in our hospital between May 2017 and May 2021 were divided into poor prognosis (n = 42) and good prognosis groups (n = 70) according to their prognosis. We compared the clinical data of the two groups, the quantitative indexes of real-time ultrasound elastography before and after PTCD, analyzed the correlation between the quantitative indexes of real-time ultrasound elastography and Child-Pugh score, the quantitative indexes of real-time ultrasound elastography after PTCD and the prognosis of PTCD in elderly patients with advanced malignant obstructive jaundice, and drew the receiver operating characteristic (ROC) curve, and evaluated the value of real-time ultrasound elastography quantitative indicators for the prognosis of elderly patients with advanced malignant obstructive jaundice after PTCD. Results　There were statistically significant differences in the site of obstruction, Child-Pugh score, and total bilirubin level between the two groups ( P < 0.05). The mean value of strain of real-time postoperative ultrasound elastography quantitative index of PTCD in the poor prognosis group was lower than that in the good prognosis group, and the percentage of blue field was higher than that in the good prognosis group (P < 0.05). The mean value of strain and blue field percentage were associated with Child-Pugh score (P < 0.05). Strain mean value and blue field percentage were independent factors influencing PTCD prognosis in elderly patients with advanced malignant obstructive jaundice ( P < 0.05) .The AUC for the combined diagnosis of strain mean value and blue field percentage in elderly patients with advanced malignant obstructive jaundice for PTCD prognosis was 0.862. Conclusions　Real-time ultrasound elastography quantitative measurement of mean strain and percentage of blue area in elderly patients with advanced malignant obstructive jaundice are related to the liver function and prognosis. Objective: To evaluate the short-term efficacy of pan-immune-inflammatory value (PIV), neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR) in patients with advanced non-small cell lung cancer (NSCLC) who were treated with programmed cell death-1 (PD-1) inhibitors and the evaluation value of prognosis. Methods: The clinical data of 132 patients with advanced NSCLC who were treated with PD-1 inhibitors in the Affiliated Hospital of Xuzhou Medical University from January 2018 to January 2020 were collected. According to the optimal threshold grouping of PIV, NLR, PLR and MLR before immunotherapy, the relationship between them and disease control rate (DCR) was analyzed. Kaplan-Meier method was used to draw the survival curve, and Cox regression analysis was used to determine the risk factors affecting survival and prognosis. Results: The optimal thresholds of PIV, NLR, PLR and MLR were 286.17, 3.07, 250.97 and 0.33 respectively. Patients with NLR ≤3.07 had higher DCR (P＜0.05). The median overall survival (OS) were longer in PIV ≤286.17 group, NLR ≤3.07 group, PLR ≤250.97 group and MLR ≤0.33group. Cox regression analysis showed that PIV, NLR, PLR and MLR were the risk factors affecting the prognosis of patients (P＜0.05). Conclusion: Patients with NLR ≤3.07 have better short-term efficacy, and patients with low levels of PIV, NLR, PLR and MLR have better prognosis. Due to the immunosuppressive microenvironment as well as the lack of medicine that targets the driver gene alterations, the prognosis of patients with pancreatic ductal adenocarcinoma(PDAC)is worse than that of other malignant tumors.At present, chemotherapy is still the main stay of treatment for most patients with advanced PDAC. In recent years, with the continuous development of anti-cancer drugs and the wide application of next-generation sequencing technology, increasing number of studies have shown that genetic alteration could guide the selection of chemotherapy regimen and molecular targeted therapy for some patients with advanced PDAC.Guidelines of the National Comprehensive Cancer Network and Chinese Society of Clinical Oncology recommend that all patients with advanced PDAC should undergo somatic and germline genetic profiling to help determining the optimal treatment strategy and to prolong patients' survival. Therapy targeting KRAS mutation, cancer vaccines and therapy of anti-tumor metabolism are still in clinical research. This article will review current status and progress of medical treatment for advanced PDAC.

To investigate the relationship between serum tumor abnormal protein (TAP), programmed cell death molecule 5 (PDCD-5), tumor necrosis factor-α (TNF-α ) and anti-programmed death receptor 1 (PD-1) in patients with advanced non-small cell lung cancer (NSCLC). 87 patients with advanced NSCLC who were admitted to Department of Oncology, Shuyang Hospital Affiliated to Xuzhou Medical University from March 2018 to March 2021 were selected. All patients were treated with anti-PD-1, and they were divided into disease control group (67 cases) and disease progression group (20 cases) according to treatment efficacy. Serum TAP, PDCD-5 and TNF-α levels were detected and compared between two groups before and after treatment, and clinical data were collected. Multiple Logistic regression was used to analyze the influencing factors of anti-PD-1 therapy efficacy in patients with NSCLC. Receiver operating characteristic (ROC) curve was drawn to analyze the evaluation value of TAP, PDCD-5 and TNF-α levels for anti-PD-1 therapy in patients with NSCLC. After treatment, the serum TAP level of patients in disease control group decreased, the PDCD-5 and TNF-α levels increased (P<0.05). There was no significant differences in serum TAP, PDCD-5 and TNF-α levels in patients with disease progression group before and after treatment (P>0.05). After treatment, the serum TAP level of patients in the disease control group was lower than that in the disease progression group, while the PDCD-5 and TNF-α levels were higher than those in the disease progression group (P<0.05). Logistic regression analysis showed that the of serum TAP levels and distant visceral metastasis were risk factors for anti-PD-1 treatment efficacy in NSCLC patients (P<0.05), and PDCD-5 and TNF-α were protective factors before treatment (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of the combined detection of TAP, PDCD-5 and TNF-α in serum for evaluating the efficacy of anti-PD-1 treatment in patients with NSCLC was 0.892, which was higher than 0.649, 0.647 and 0.787 of the above three indicators alone. The serum TAP, PDCD-5 and TNF-α before treatment of patients with advanced NSCLC before and after anti-PD-1 therapy are correlated with therapeutic efficacy, which could be used as indicators for evaluation of anti-PD-1 efficacy. OBJECTIVE: To probe into the efficacy of carrelizumab combined with albumin paclitaxel and cisplatin in the first-line treatment of patients with advanced esophageal cancer and effects on peripheral blood programmed death receptor 1 ( PD-1) and programmed death receptor ligand 1 ( PD-L1 ). METHODS: Eighty patients with advanced esophageal cancer admitted to the hospital from Jul. 2019 to Oct. 2020 were extracted to be divided into the observation group and the control group at the ratio of 1 ∶1 according to the random number table method, with 40 cases in each group. The control group was treated with albumin paclitaxel and cisplatin, while the observation group received carrelizumab combined with albumin paclitaxel and cisplatin. The efficacy and levels of tumor markers [ squamous cell carcinoma-associated antigen ( SCC-Ag ), carbohydrate antigen 19-9 ( CA19-9), cytokeratin 19 fragment antigen 21-1(CYFRA21-1), carcinoembryonic antigen(CEA)], immune function indicators[T cell subsets (CD3 +, CD4 +, CD4 + / CD8 + ), dendritic cells ( DC), DC / mononuclear macrophages ( PBMC)], serum PD-1, PD-L1, the core quality of life scale(QLQ-C30) score before and after treatment and the incidence of adverse reactions during treatment were compared between two groups. The survival of patients in both groups was counted at 1 year of follow-up. RESULTS: The disease control rate of the observation group was 77. 50% (31 / 40), significantly higher than 55. 00%(22 / 40) of the control group, with statistically significant difference(P<0. 05). To investigate the effects of Aidi injection combined with capecitabine and oxaliplatin regimen (XELOX regimen) on Th1/Th2 immune balance and serum tumor markers in patients with advanced colorectal cancer. 80 patients with advanced colorectal cancer who were treated in our hospital from July 2018 to June 2021 were selected, and they were randomly divided into control group (XELOX scheme) and study group (Aidi injection combined with XELOX scheme), with 40 cases each. The efficacy, Karnofsky (KPS) score, Th1/Th2 immune balance and serum tumor markers were compared between the two groups, and the incidence of adverse reactions was observed. The objective remission rate (ORR) and disease control rate (DCR) in the study group were higher than those in the control group (P<0.05). OBJECTIVE: To probe into the efficacy of carrelizumab combined with amlotinib in the treatment of advanced gastric cancer and its effect on short-term prognosis, so as to provide reference for clinical treatment. METHODS: From Jan. 2019 to Jan. 2020, 100 patients with stage ill B to N gastric cancer who failed the second-line treatment in this hospital were extracted to be divided into the control group and the observation group via the random number table. Patients in the control group ( n = 50) received anlotinib 10 mg orally from the 1st to 14th day of treatment for 4 cycles( 3 weeks as a treatment cycle). The observation group ( n = 50) was additionally given 200 mg carrelizumab intravenously on the 1st day of treatment for 4 cycles( 3 weeks as a treatment cycle), and follow-up was maintained after discharge. The efficacy, safety, survival time, serum vascular endothelio genie factor( VEGF) level and programmed cell death receptor 1 ( PD-1) expression on CD4 + T cells and CD8+ T cells in peripheral blood were compared between two groups. The above indicators of patients with different microsatellite instability ( MSI) status were observed( MSI-H for high MSI, MSI-L for low MSI, and MSS for no MSI). RESULTS: Eighty-eight patients were enrolled for the results analysis. ( 1) The disease control rate in the observation group [ 79. 07% ( 34/43 )] was significantly higher than that in the control group [ 57. 78% ( 26/ 45) J, the difference was statistically significant ( PO. 05). To observe the effect of Kanglixin capsule combined with oxaliplatin, calcium folinate and 5-fluorouracil (FOLFOX4) on immune function, quality of life and serum tumor markers in patients with advanced gastric cancer. 80 patients with gastric cancer in our hospital from July 2018 to January 2021 were prospectively selected. They were divided into two groups: control group and observation group according to the drawing, 40 cases in each group. The control group received FOLFOX4 regimen treatment, and the observation group received Kanglixin capsule combined with FOLFOX4 regimen treatment, 3 weeks as a course of treatment, 2 courses of treatment. The efficacy, quality of life at 2 courses after treatment and adverse reactions during treatment of the two groups were observed, the changes of tumor markers and immune function indexes before treatment and 2 courses after treatment were compared between the two groups. objective: To investigate the effect of Fuzheng Jiedu formula combined with endu on lung function, T cell subsets and quality of life in patients with advanced non-small cell lung cancer (NSCLC). Methods: 116 patients with advanced NSCLC who were admitted to our hospital from March 2016 to October 2018 were randomly selected, patients were divided into two groups: control group (n=58, endu combined chemotherapy) and study group (n=58, combined with Fuzheng Jiedu formula on the basis of control group). The chemotherapy cycle was 21 days, with a total of 4 cycles. The curative effect and the incidence of adverse reactions were compared between the two groups. Lung function, T cell subsets and quality of life of the two groups before and 4 cycles after treatment were compared. Objective: To observe the effect of intensity modulated radiation therapy (IMRT) synchronous TP chemotherapy (cisplatin combined with paclitaxel) on immune function, systemic inflammatory response index and serum tumor markers in patients with locally advanced non small cell lung cancer (NSCLC).Methods:86 patients with locally advanced NSCLC in Qingyuan People’s Hospital were selected from August 2017 to August 2019.According to the coin tossing, they were divided into control group and experimental group,43 cases in each group. The control group was treated with TP chemotherapy. The experimental group was treated with combined with IMRT on the basis of the control group. The clinical efficacy, immune function, systemic inflammatory response index and serum tumor markers were compared between the two groups. The incidence of adverse reactions,1-year survival rate and median survival time in the two groups were observed. Objective: To observe the efficacy of tegafur combined with apatinib in the treatment of advanced recurrent and metastatic esophageal cancer and its effect on T cell subsets and serum tumor marker levels. Methods: The case collection time was from March 2015 to March 2018, and the case collection scope was 70 patients with advanced recurrent and metastatic esophageal cancer in our hospital. The patients were divided into control group and experimental group by envelope lottery, with 35 cases in each group. The control group was treated with tegafur, and the experimental group was treated with apatinib on the basis of the control group. Both groups received chemotherapy for 2 cycles. Objective: To investigate the effect of bevacizumab combined with paclitaxel plus cisplatin (TP) chemotherapy on the immune function, quality of life and serum tumor markers in patients with advanced non-squamous non-small cell lung cancer (NSNSCLC). Methods: April 2016 2 80 cases of advanced NSNSCLC 019 patients admitted to our hospital during August. The patients were divided into a control group (n=40, TP chemotherapy treatment) and a study group (n=40, combined with bevacizumab treatment on the basis of the control group) using a random number table method to compare the efficacy, immune function, and immune function of the two groups. Objective: To investigate the relationship between red blood cell distribution width (RDW), Neutrophils to lymphocytes ratio (NLR) and clinicopathological characteristics and prognosis of advanced non-small cell lung cancer (NSCLC) patients. From May 2017 to May 2019, 106 patients with advanced NSCLC (NSCLC group) in our hospital and 102 patients who were normal in physical examination (control group) were selected as the research object. Detection of RDW and NLR was performed, the relationship between RDW, NLR and clinicopathological characteristics and prognosis of NSCLC patients were analyzed. RDW, NLR in the NSCLC group were higher than those in the control group (P<0.05), RDW in age ≥ 60 years and lymph node metastasis with NSCLC patients were higher than those of age < 60 years and no lymph node metastasis with NSCLC patients (P<0.05), NLR in TNM staging Ⅳ stage, lymph node metastasis with NSCLC patients were higher than those in TNM staging Ⅲ stage, no lymph node metastasis with patients (P<0.05). Kaplan-Meier survival curve analysis showed that the surose in the low RDW group and the low NLR group (P<0.05). Univariate COX regression analysis showed that the differentiation degree, TNM stage, lymphatic metastasis, RDW and NLR were correlated with the prognosis of NSCLC patients (P<0.05), while multivariate COX regression analysis showed that lymphatic metastasis, RDW and NLR were correlated with the prognosis of NSCLC patients (P<0.05). RDW and NLR of advanced NSCLC patients are higher, RDW is related to age and lymph node metastasis, NLR is related to TNM stage and lymph node metastasis. Objective: To evaluate the efficacy and adverse reactions of Jiedu Granules combined with apatinib in patients with advanced liver cancer. Methods: A retrospective study was conducted on the clinical data of 27 patients with liver cancer who were treated in the First Affiliated Hospital of Naval Military Medical University from December 2018 to June 2019 with oral Jiedu granules and apatinib. Patients with unresectable or recurrent liver cancer were included in the study, Jiedu granules were combined with apatinib until the disease progressed or its toxic and side effects could not be tolerated, follow-up observation was made on the treatment effect, survival time, disease progression time, inflammatory factor indexs and adverse reactions. Results: 4 patients (14.81%) with complete remission (CR), 4 patients (14.81%) with partial remission (PR), 8 patients (29.63%) with stable (SD), 11 patients (40.74%) with progression (PD), disease control rate (DCR) was 59.26% (16/27), the objective response rate (ORR) was 29.63% (8/27). The median progression-free survival (PFS) was 3.630 months, the median overall survival (OS) was 13.667 months. The most common adverse reactions were hypertension 59.26% (16/27), proteinuria 59.26% (16/27), diarrhea 74.07% (20/27), and hand-foot syndrome 62.96% (17/27). C-reactive protein and interleukin 2 levels were decreased in the inflammatory factor indexs after treatment, there were statistical differences (P<0.05). Objective: To investigate the effects of transcatheter arterial chemoembolization (TACE) combined with radiofrequency ablation (RFA) on serum tumor activity factors, liver function and prognosis in patients with advanced hepatocellular carcinoma. Methods: 103 patients with advanced hepatocellular carcinoma who were admitted to our hospital from February 2012 to June 2016 were selected, they were randomly divided into control group (n=51) and study group (n=52), the control group was treated with RFA, and the study group was treated with TACE combined with RFA. The clinical total effective rate was compared between the two groups. Liver function and serum levels of tumor active factors in two groups were compared before and after treatment. The complications and prognosis of the two groups were observed. Results: After treatment, the total clinical effective rate of the study group was 53.85% (28/52), which was significantly higher than that of the control group 33.33% (17/51)(P<0.05). After treatment, total bilirubin (TBIL), Alanine aminotransferase (ALT) and amino acid aminotransferase (AST) were lower than those before treatment (P<0.05), but there were no significant differences in TBIL, ALT and AST between the two groups (P>0.05). After treatment, the alpha-fetoprotein (AFP), carbohydrate antigen 199 (CA199) and matrix metalloproteinase (MMP) in both groups were lower than those before treatment, and those in the study group were lower than those in the control group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). The recurrence rate of 1 year and 2 years after treatment in the study group was lower than that in the control group, and the survival rate was higher than that in the control group (P<0.05). Conclusion: TACE combined with RFA is effective in the treatment of advanced hepatocellular carcinoma. Patients with advanced gastric cancer have a poor prognosis, which remains the clinical concerned hot topic. The main previous treatments for advanced gastric cancer were adjuvant chemotherapy and palliative surgery, however, the application of conversion therapy has improved the survival in recent years. BACKGROUND: Preliminary experimental studies have shown that after rabbit chondrocytes are induced with advanced glycation end products (AGEs), the expression levels of tumor necrosis factor-α, matrix metalloproteinase 13 and matrix-degrading enzymes are significantly increased. However, the underling mechanism of AGEs-induced chondrocyte injury remains unclear. OBJECTIVE: To explore the mechanism of AGEs-induced chondrocyte injury by observing the expression levels of inflammatory factors, matrix-degrading degrading enzymes and nuclear factor (NF) κB. METHODS: The human chondrocytes were harvested by mechanical separation and improved two-step enzyme digestion method, and then identified by collagen type II immunohistochemical staining and immunocytochemistry. Human chondrocytes were stimulated with AGEs (1, 10, 25, 50 and 100 mg/L) for 24 hours, and those incubated with BSA and normal chondrocytes were as controls. Subsequently, the mRNA and protein expression levels of interleukin-1, matrix metalloproteinase 13 and tumor necrosis factor-α were detected by real-time PCR and western blot assay. The contents of inhibitor protein of NF-kB α (IκBα) in cytoplasm and NF-kB (p65) in nuclei were detected by western blot assay. RESULTS AND CONCLUSION: Compared with control groups, AGEs, especially at the concentration of 100 mg/L, significantly up-regulated the expression levels of tumor necrosis factor-α, matrix metalloproteinase 13, interleukin-1 α and interleukin-1 β in the chondrocytes in a concentration-dependent manner. Moreover, AGEs significantly decreased the content of IκBα in cytoplasm and significantly increased the content of NF-kB (p65) in nucleus. Objective: To observe the clinical efficacy of Gefitinib tablets combined with Addie injection in the treatment of ad­vanced non-small cell lung cancer (NSCLC) with epidermal growth factor receptor (EGFR) positive. Methods: 62 cases of advanced NSCLC with EGFR positive patients treated in Aerospace Central Hospital and Puai Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology from January 2012 to December 2015 were selected. They were divided into experi­mental group (n=31) and control group (n=31) according to the randomized double blind method. The experimental group were treated with Addie injection combined with Gefitinib Tablets, and the control group were only treated with Gefitinib Tablets, the two groups were treated for 2 courses. The treatment effect and adverse reactions of the two groups were compared. Followed up for 1 year, the survival rate of the two groups was compared. Results: The effective rate and disease control rate in the experimental group were higher than those in the control group (P<0.05). The pain relief rate, improvement rate of sleep quality and diet improvement rate in the experimental group were higher than those in the control group (P<0.05). The incidence of adverse reactions in the experimental group was slightly lower than that in the control group, but the difference was not statistically significant (P>0.05). Followed up for 1 years, 28 patients in the experimental group survived, and 22 cases in the control group survived. The survival rate in the experimental group was higher than that in the control group (P<0.05). Conclusion: The curative effect of Addie injection combined with Gefitinib Tablets is better than sim­ple application of Gefitinib Tablets in the treatment of EGFR positive advanced NSCLC, can improve sleep quality and diet condition, re­lieve pain, better safety, so as to improve the prognosis of patients, it is worthy of clinical promotion. Background and objective The aim of this study is to integrate with literature review, and explore the value of treatment of advanced non-small cell lung cancer with second line. Methods For the metastatic progressive non-small cell lung adenocarcinoma patient, the evaluation of efficacy for complete response (CR) with endostar combined GC, the sequential treatment with gefitinib, used pemetrexed combined DDP as the second line treatment, followed up and observed with the progression free survival (PFS) and survival time of patient. Results Pemetrexed combined DDP in the treatment of 5 cycles, the evaluation of lung cancer efficacy for CR, bone metastasis was steady, PFS was 6.6 months, survival time is 22 months now, improved the quality of living life. Conclusion For advanced non-small cell lung adenocarcinoma recurrence and metastasis, after the treatment of first line and maintenance therapy, selecting adequately pemetrexed combined DDP, as the second line treatment, can prolong the lifetime and improve the quality of life.

Background and objective Erlotinib has begun to show its unique advantage in the past several years. The aim of this study was to evaluate the efficacy and adverse reactions of erlotinib on patients with advanced non-small cell lung cancer (NSCLC) of stage IIIb/IV. Methods Patients with advanced NSCLC ( IIIb/IV stage) were recruited in this study. They had recurrence after receiving at least one or two regimens of platinum-based chemotherapy from March 2007 to January 2009. Erlotinib was given orally to the patients at dose of 150 mg/d, once daily until disease progression. The clinical outcomes and adverse reactions were observed. Patients were followed up. Kaplan-Meier method was used to perform survival analysis. Results Among the 16 patients, 0 patients got complete response (0%); 2 patients got partial response (12.5%); 5 stable disease (31.2%) and 9 progressive disease (56.2%). The total response rate (CR+PR) was 12.5% and the disease control rate (including tumor response and stable disease) was 43.7%. All patients were followed up for 1-22 months; 2 cases are still alive until January 2009; the follow-up ratio was 100%. The average survival time could reach 10.25 (3-22) months. The median overall survival period were 9 months and one year progression free survival rate were 13.3%. The main toxicities were rash (93.7%), diarrhea (68.7%), reduced appetite (43.7%) and liver dysfunction (18.7%). Conclusion Erlotnib demonstrated significant anti-tumor activity and a favorable tolerability profile in this series of NSCLC patients.

OBJECTIVE:To probe into the clinical effects of imatinib in treatment of advanced malignant melanoma with C-kit and B-raf gene mutation .METHODS:124 patients with advanced malignant melanoma tumors admitted from Jan.2010 to Jan.2014 were selected to be divided into observation group and control group via the random number table, with 62 cases in each .The control group were treated with dacarbazine , while the observation group additionally received imatinib based on the control group .All patients were followed up for 2 years.The toxic reactions, efficacy, disease-free survival ( DFS) of two groups were observed , and the one-year and two-year survival rate of two groups were compared .RESULTS:There was no statistical significance between two groups in the adverse drug reactions during the treatment (P>0.05).The disease remission rate of observation group was 6.29%(38/62), significantly higher than that of control group [22.58%(14/62)], with statistically significant difference (P<0.05).The DFS of observation group was (10.2 ±1.1) months, significantly higher than that of control group [(5.1 ±0.3) months], with statistically significant difference (P<0.05).Meanwhile, the one-year and two-year survival rate of observation group was significantly higher than that of control group, with statistically significant difference(P<0.05).CONCLUSIONS:Imatinib in treatment of advanced malignant melanoma with C-kit and B-raf gene mutation can effectively control the rate of disease progression and prolong patients'survival time, without any additional tadverse drug reactions during treatment . Objective: Patients with advanced gastric cancer have a poor prognosis and a possibility of peritoneal metastasis even if receiving gastrectomy. Hyperthermic intraperitoneal chemotherapy (HIPEC) can effectively kill free cancer cells or small lesions in the abdominal cavity. At present, preventive HIPEC still lacks safety evaluation in patients with locally advanced gastric cancer. This study aims to explore the safety of radical resection combined with HIPEC in patients with locally advanced gastric cancer. Methods: A descriptive case series study was carried out. Clinicopathological data of 130 patients with locally advanced gastric cancer who underwent radical resection + HIPEC at the Department of Gastrointestinal Surgery, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology from January 2020 to February 2021 were retrospectively analyzed. Inclusion criteria: (1) locally advanced gastric adenocarcinoma confirmed by postoperative pathology; (2) no distant metastasis was found before surgery; (3) radical resection; (4) at least one HIPEC treatment was performed. Exclusion criteria: (1) incomplete clinicopathological data; (2) tumor metastasis was found during operation; (3) concomitant with other tumors. HIPEC method: all the patients received the first HIPEC immediately after D2 radical resection, and returned to the ward after waking up from anesthesia; the second and the third HIPEC were carried out according to the patient's postoperative recovery and tolerance; interval between two HIPEC treatments was 48 h. Observation indicators: (1) basic information, including gender, age, body mass index, etc.; (2) treatment status; (3) perioperative adverse events: based on the standard of common adverse events published by the US Department of Health and Public Health (CTCAE 5.0), the adverse events of grade 2 and above during the treatment period were recorded, including hypoalbuminemia, bone marrow cell reduction, wound complications, abdominal infection, lung infection, gastroparesis, anemia, postoperative bleeding, anastomotic leakage, intestinal obstruction, pleural effusion, abdominal distension, impaired liver function, and finally a senior professional title chief physician reviewed the above adverse events and made a safety evaluation of the patient; (4) association between times of HIPEC treatment and adverse events in perioperative period; (5) analysis of risk factors for adverse events in perioperative period. Results: Among the 130 patients, 79 were males and 51 were females with a median age of 59 (54, 66) years and an average body mass index of (23.9±7.4) kg/m(2). The tumor size was (5.4±3.0) cm and 100 patients (76.9%) had nerve invasion. All the 130 patients received radical resection + HIPEC and 125 (96.2%) patients underwent laparoscopic surgery. The mean operative time was (345.6±52.3) min and intraoperative blood loss was (82.0±36.5) ml. One HIPEC treatment was performed in 54 patients (41.5%), 2 HIPEC treatments were in 57 (43.8%), and 3 HIPEC treatments were in 19 (14.6%). The average postoperative hospital stay was (13.1±7.5) d. A total of 57 patients (43.8%) had 71 cases of postoperative complications of different degrees. Among them, the incidence of hypoalbuminemia was 22.3% (29/130), and the grade 2 and above anemia was 15.4% (20/130), lung infection was 3.8% (5/130), bone marrow cell suppression was 3.7% (4/130), abdominal cavity infection was 2.3% (3/130), and liver damage was 2.3% (3/130), wound complications was 1.5% (2/130), abdominal distension was 1.5% (2/130), anastomotic leakage was 0.8% (1/130), gastroparesis was 0.8% (1/130) and intestinal obstruction was 0.8% (1/130), etc. These adverse events were all improved by conservative treatments. There were no statistically significant differences in the incidence of adverse events during the perioperative period among patients undergoing 1, 2, and 3 times of HIPEC treatments (all P >0.05). Univariate and multivariate logistic analyses showed that age > 60 years (OR: 2.346, 95%CI: 1.069-5.150, P =0.034) and neurological invasion (OR: 2.992, 95%CI: 1.050-8.523, P =0.040) were independent risk factors for adverse events in locally advanced gastric cancer patients undergoing radical resection+HIPEC (both P <0.05). Conclusions: Radical surgery + HIPEC does not significantly increase the incidence of perioperative complications in patients with advanced gastric cancer. The age >60 years and nerve invasion are independent risk factors for adverse events in these patients.

Objective: To investigate the prognosis-related factors and its predictive value in patients with hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). Methods: Sixty-three cases with HBV-ACLF were enrolled. According to the prognosis of 4-weeks, patients were divided into survival and death group. Univariate and multivariate analyses were performed on the clinical data of the two groups of patients to screen the risk factors affecting prognosis, evaluate its predictive value, and compare them with the MELD score, CTP score, and CLIFACLF score. The data were analyzed using t-test, Mann-Whitney U test, χ (2) test. Multiple logistic regression analysis was used for multiple risk factors. Results: There were 63 cases with HBV-ACLF, with 16 cases (25.40%) in the 4-week survival group, and 47 cases (74.60%) in the death group. The survival group age was 38.38 ± 14.50 years, which was significantly lower than the age of the death group 52.28 ± 12.51 years ( P < 0.001). The survival group alpha-fetoprotein (AFP) level was 91.21 (8.38 ~ 154.10)μg/L, which was significantly higher than the level of the death group [12.60 (5.70 ~ 33.80) μg/L, P = 0.039]. The survival group alanine aminotransferase (ALT) level was 925.65 (523.43 ~ 1 364.80) U/L, which was much higher than that of the death group [371.60 (117.30 ~ 895.30) U/L, P = 0.040]. The survival group serum sodium level was (136.59 ± 4.03) mmol /L, which was significantly higher than the level of the death group [(132.22 ± 6.37) mmol/L, P = 0.013]. The survival group ascites severity level was much lower than that of the death group ( P = 0.008). The survival group creatinine level was 56.50(49.43 ~ 86.25) μmol/L, which was much lower than the level of the death group [86.20 (68.00 ~ 143.00) μmol/L, P = 0.003]. Multivariate logistic regression analysis showed that ascites ( OR = 0.470, 95% CI : 0.226 ~ 0.977) and age ( OR = 0.941, 95% CI : 0.888 ~ 0.996) were risk factors affecting the HBV-ACLF prognosis. The area under the curve predicted liver failure prognostic score for ascites and age was 0.821, and the sensitivity and specificity were 68.8% and 87.2%, which was higher than the area under the curve predicted by the MELD score, CTP score, and CLIFACLF score, respectively. Conclusion: Age and ascites can be used to predict the clinical outcome in patients with HBV-ACLF. Younger patients without ascites have a higher survival rate at 4-weeks, but older patients with ascites are more likely to have a lower survival rate.

Objective: To investigate the prognostic factors associated with unresectable (stage Ⅲa-Ⅳ, according to the 7th edition of the AJCC cancer staging manual) lung squamous cell carcinoma. Methods: We retrospectively analyzed 350 patients with inoperable locally advanced, recurrent or metastatic lung squamous cell carcinoma who were admitted to the First Affiliated Hospital of Chinese Medical University from January 2005 to June 2018. The clinical pathological data, treatment and survival follow-up information of the patients were collected. Kaplan-Meier survival was used to compare the overall survival rate of different risk groups. Univariate analysis and multivariate Cox regression analysis were used to determine the independent prognostic factors. Results: A total of 350 patients were enrolled. The median overall survival (OS) of these patients was 16.7 months. Univariate analysis showed the stage, Eastern Cooperative Oncology Group(ECOG), first-line chemotherapy evaluation (RECIST version 1.1), radiation therapy, number of systemic chemotherapy lines, carcinoembryonic antigen (CEA), cancer antigen 125 (CA125), C reactive protein (CRP), lactate dehydrogenase (LDH), whether liver, brain, boneor metastasis were associated with the OS of patients with advanced lung squamous cell carcinoma (all P <0.05). Multivariate analysis showed that ECOG score ( HR =1.855, 95% CI: 1.063-3.239, P =0.030), whether underwent lung resection ( HR =0.476, 95% CI: 0.302-0.751, P =0.001), first-line chemotherapy evaluation [stable disease (SD): HR =0.293, 95% CI: 0.159-0.540, P <0.001; complete response (CR)+ partial response (PR): HR =0.223, 95% CI: 0.120-0.413, P <0.001], CRP ( HR =1.715, 95% CI: 1.080-2.723, P =0.042), LDH ( HR =1.116, 95% CI: 0.780-1.596, P =0.002) and CEA ( HR =1.855, 95% CI: 1.361-2.528, P <0.001) before chemotherapy, liver metastasis ( HR =2.453, 95% CI: 1.461-4.120, P =0.001) are independent prognostic factors for patients with unresectable lung squamous cell carcinoma. Conclusion: The ECOG score, surgical treatment history, first-line chemotherapy, LDH, CEA and CRP before chemotherapy, liver metastasis are independent prognostic factors for patients with advanced lung squamous cell carcinoma.

Perioperative treatment is critical to improve the outcomes of patients with advanced gastric cancer. There are three therapeutic modes of perioperative treatment for resectable gastric cancer: neoadjuvant chemotherapy+ D1/D2 surgery+ adjuvant chemotherapy, D0/D1 surgery+ adjuvant radiochemotherapy, and D2 surgery+ adjuvant chemotherapy. Over the decades, a large number of clinical studies had been conducted to optimize the perioperative treatment mode of gastric cancer, including the postoperative radiotherapy and chemotherapy, and perioperative chemotherapy, and to explore the feasibility of preoperative radiochemotherapy, targeted therapy, and immunotherapy in advanced gastric cancer. After nearly 20 years of development and exploration, although the perioperative treatment mode for advanced gastric cancer has become standardized, there are still some core issues that need to be solved urgently, including the selection of population for perioperative treatment, the limitation of efficaly evaluation criteria, insufficient emphasis on laparoscopic exploration before neoadjuvant treatment, and lack of exploration in esophagogastric junction cancer. We should fully integrate the current clinical research data into clinical practice, adopt a multidisciplinary diagnosis and treatment mode, and follow the principles of standardized diagnosis and treatment based on a multi-dimensional analysis of patient characteristics, and formulate the most reasonable treatment strategy to ultimately benefit patients.
