Objective To investigate the application value of the facial artery perforator pedicled V-Y advancement flap in the treatment of middle and lower facial tumors in the elderly population. Methods A retrospective analysis was performed on the data of 35 elderly patients who underwent extensive resection of middle and lower facial tumors and reconstruction using the facial artery perforator pedicled V-Y advancement flap from January 2017 to December 2023 in Department of Plastic Surgery, Affiliated Hospital of Guangdong Medical University. Collect their basic information, postoperative flap survival rate, incidence of postoperative complications, and patient satisfaction, and conduct retrospective analysis. Results All 35 patients' skin flaps survived, and there were 2 cases of skin flap tip bruising after surgery, with a total complication rate of 5.71%. After a follow-up period of 3 to 6 months postoperatively, there was no significant difference in the color and texture of the recipient skin flap compared to the surrounding skin. The overall patient satisfaction rate was 94.29%. Conclusion The V-Y advancement flap of the perforating branch of the facial artery has reliable blood supply, similar color and texture to the recipient area, low cost in the donor area, and high patient satisfaction. Objective To explore to establish a stable and feasible rabbit cerebral vascular bypass（CVB）model to provide an experimental tool for studying the ischemic cerebrovascular disease（ICD） and selecting the operation mode. Methods Thirty-two New Zealand rabbits were randomized into the group A，B，C and D，8 cases in each group. The 4 groups were given only anesthesia and anatomy without conducting vascular bypass，facial artery internal carotid artery bypass at 4，8 12 weeks after autologous facial arteriovenous anastomosis respectively. The animal mobility capacity was evaluated by 2 experimental technicians before model construction and at 1，2，3 months after successfully constructing the model. The vascular situation was assessed through cerebral angiography. Results Each group was succeeded in constructing the model. The rabbits all survived. Compared with the group A，the mobility capacity in the group B，C and D was significantly decreased，the difference was statistically significant（P<0.05），but which was recovered along with the time prolonging. The animal mobility capacity at postoperative 3 months had no statistical difference between the group A and D（P>0.05）；after conducting the facial artery internal carotid artery bypass，the angiography re-examination at each observation point showed that the group B，C and D had different degrees of arterial dilation compared with the group A，while which in the group D was most obvious，the difference was statistically significant（P<0.05）；the cerebral blood vessels were unobstructed in all groups. Conclusion Adopting animal facial artery internal carotid artery bypass has the advantages of high success rate，stability and feasibility，which is a more ideal model for conducting the study and intervention on ICD. PURPOSE: To explore the use of the contralateral retroangular island flap for reconstructing midfacial defects following skin cancer ablation. METHODS: Midfacial defects in 12 patients following resection of skin cancers were reconstructed with contralateral retroangular island flap. The patients aged from 55 to 76 years. Nine of them were male and the others were female. Retroangular flaps raised from the contralateral infraorbital region, with a pedicel containing the angular vessels, were used to reconstruct the defect. The defect ranged in size from 3.0cmx3.0cm to 5.0cmx7.0 cm, and the flap skin paddle ranged in size from 2.0cmx4.0cm to 4.0cmx6.0cm. RESULTS: All flaps survived. The patients were followed for an average of 8.5 months (range: 6-12 months). The vascularity of the flaps was good, and the match in terms of color and texture was excellent. The donor site scar in the nasolabial groove was acceptable. CONCLUSION: The contralateral retroangular flap is a good alternative for reconstructing moderate to large midfacial defects following skin cancer ablation. [ABSTRACT FROM AUTHOR]

PURPOSE: To evaluate the effect of facial artery-submental artery island flap for immediate reconstruction of the tongue defect. METHODS: From January 2005 to January 2006, facial artery-submental artery island flap were used to repair the tongue defect after radical tumor resection in eleven cases, 6 patients were male and 5 were female; the flap extension varied from 8cm×3cm to 12cm×4.5cm. RESULTS: Eleven flaps survived completely. The appearance and function of the tongue were satisfying, no recurrences and metastasis was founded after 1 year follow-up. CONCLUSION: The submental flap has reliable blood supply, could be operated conveniently and safely. It can be used to reconstruct tongue defects in patients with T2 tongue cancer.
