Objective To investigate the clinical efficacy and safety of 1 565 nm non-ablative fractional laser combined with topical minoxidil and oral finasteride in the treatment of patients with androgenetic alopecia (AGA). Methods Seventy-five male AGA patients with Norwood-Hamilton classification grade Ⅱ-Ⅲ, were randomly assigned into three groups: the control group 1, the control group 2 and the experimental group, with 25 cases in each group. Patients in the control group 1 received topical 5% minoxidil (1 mL, twice daily). Patients in the control group 2 were treated with both topical 5% minoxidil and oral finasteride (1 mg, once daily). Patients in the experimental group received a combined therapy of 1 565 nm non-ablative fractional laser in addition to topical 5% minoxidil and oral finasteride. Hair overall efficacy was evaluated using a 7-point rating scale after 24 weeks of treatment. Hair diameter and density were measured using a dermoscope. Patient satisfaction was assessed post-treatment, and adverse reactions were recorded. Results The overall efficacy of hair in the experimental group was superior to the control group 1 and the control group 2. There were no significant differences in hair density and hair diameter before treatment between the three groups (P＞0.05). After treatment, hair diameter and density increased in all three groups compared to baseline values (P＜0.05), and the hair diameter and hair density of the experimental group were higher than those of the control group 1 and the control group 2 (P＜ 0.05). Patient satisfaction in the experimental group was higher than that in the control group 1 and the control group 2 (P＜ 0.05). Patients in the experimental group experienced tolerable pain and burning sensations during laser treatment, and the symptoms were self-alleviated within a few hours. There were no serious adverse reactions reported in any group. Conclusion The combination therapy of 1 565 nm non-ablative fractional laser, 5% minoxidil, and finasteride demonstrates significantly better efficacy in the treatment of AGA than minoxidil and finasteride alone drug therapy. OBJECTIVE: To evaluate the efficacy and safety of 5% minoxidil combined with finasteride in the treatment of androgenic alopecia (AGA), so as to provide evidence-based reference for clinical application of 5% minoxidil. METHODS: PubMed, the Cochrane Library, CNKI, VIP, CBM and Wanfang Data were retrieved for randomized controlled trials (RCT) on 5% minodil combined with finasteride in the treatment of AGA published before Jun. 1st, 2022. Literature was screened by title, abstract and full text in turn, and Meta-analysis was performed by using RevMan 5. 4 statistical software. RESULTS: After screening, 7 RCT were included. Results of Meta-analysis showed that after 3, 6 and 12 months of treatment, the efficacy of AGA in the combined treatment group was better than that in the 5% minoxidil group or finasteride group, the differences were statistically significant (P<0. 01). The incidence of adverse drug reactions of topical 5% minoxidil was significantly higher than that of oral finasteride, the difference was statistically significant ( P < 0. 01). CONCLUSIONS: For patients with AGA, topical 5% minoxidil combined with oral finasteride is the most effective. However, local itching, increased dandruff or dermatitis may occur during medication, and attention should be paid to the occurrence of adverse reactions on the skin and its accessories. Objective To compare the efficacy and safety of terazosin and tamsulosin in combination with finasteride in the treatment of senile benign prostatic hyperplasia. Methods A total of 40 elderly patients(more than 60 years old)with benign prostatic hyperplasia who were admitted to the department from January to December in 2017 were randomly divided into the control group and the observation group, with 20 cases in each group. The international prostate symptom score(IPSS score), maximum urinary flow rate and residual urine volume were analyzed after 3 months of drug administration. Results 40 patients met the inclusion criteria and completed postoperative follow-up. The mean age and mean duration of the two groups of patients were basically similar. After treatment, the IPSS score, maximum urinary flow rate and residual urine volume of the two groups were improved compared with those before treatment. The differences were statistically significant(P<0.05). There were no significant differences in the IPSS score, maximum urinary flow rate, and residual urine volume between the two groups before and after treatment(P>0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). The improvement of symptoms in the observation group was more significant in the observation group, and the improvement in the urinary period of the control group was more significant(P<0.05). Conclusion In patients with benign prostatic hyperplasia, the effects of terazosin or tamsulosin combined with finasteride are similar. Objective: To investigate the clinical efficacy of finasteride combined with M receptor antagonist in the treatment of patients with prostatic hyperplasia and bladder excessive activity. Methods: Retrospective study a total of 60 patients with prostatic hyperplasia and bladder excessive activity from our hospital were collected and randomly divided into experiment group and control group with 30 cases in each group. Patients in the control group were treated by M receptor antagonist, patients in the experiment group were treated on the base of control group with finasteride. Compared the international prostate symptom score (IPSS), bladder excessive activity symptom score (OABSS), prostate volume, residual urine volume, maximal urinary flow rate and the number of urgency of the two groups before and after treatment, statistics the adverse reactions of the two groups occurred during treatment. Results: Compared with the control group, the IPSS and OABSS levels of the experiment group were lower (P<0.05); prostate volume was smaller (P<0.05); residual urine volume was less, maximal urinary flow rate was larger, the number of urgency was less (P<0.05); adverse reactions occurred rate was lower (P<0.05). Conclusion: Finasteride combined with M receptor antagonist in the treatment of patients with prostatic hyperplasia and bladder excessive activity can reduce the incidence of adverse reactions, improve the clinical efficacy, it is worth to be popularized in clinical application. Objective: To evaluate the clinical effectiveness and safety of transurethral seminal vesiculoscopy (TUSV) combined with finasteride in the treatment of chronic seminal vesiculitis with recurrent hemospermia. Methods: From January 2012 to August 2014,62 patients with chronic seminal vesiculitis recurrent hemospermia were selected from our department of urology surgery, and then were randomly divided into 2 groups, the control group of 31 cases, which received the operative treatment of transurethral seminal vesiculoscopy, and experimental group of 31 cases, which received the treatment of transurethral seminal vesiculoscopy combined with finasteride. The efficient, recurrence rate, symptoms improved and seminal plasma fructose were compared between two groups after treatment. Results: TUSV was successfully accomplished in all patients. The efficient was 96.77% in experimental group, significantly higher than that of the control group (64.52%), and the difference was statistically significant (P<0.05). The recurrence rate was 10%in experimental group, significantly lower than that of the control group (50%), and the difference was statistically significant (P<0.05); the level of fructose was significantly increased in both two groups, and it was more obviously in the experimental group (P<0.05); The symptoms improved in the experimental group was significantly better than that of the control group, and the difference was statistically significant (P<0.05). Conclusions: Transurethral seminal vesiculoscopy combined with finasteride is effective and safe for the treatment of chronic seminal vesiculitis with recurrent hemospermia. [ABSTRACT FROM AUTHOR]

OBJECTIVE: To conduct a systematic review with meta-analysis on the clinical safety of finasteride in treatment of benign prostatic hyperplasia (BPH), and to compare and analyze the results with related foreign studies. METHODS: Correlative literature with application of single finasteride or finasteride combined with other drugs in treatment of BPH were retrieved. RCT, CCT that related to finasteride were involved. The quality of literature was evaluated, the data were extracted and meta-analysis or qualitative systematic review were conducted. RESULTS: 10 RCT / CCT met the inclusion criteria. According to the analysis: finasteride had a curative efficacy with slight adverse reactions. Compared with tamsulosin group, finasteride group had a grater improvement in IPSS score, Qmax and prostate volume, with significant difference (P < 0. 05). Compared with finasteride group, finasteride +tamsulosin group had a greater improvement in Ru (P < 0. 01), but there was no significant difference in the improvement of IPSS score and prostate volume between the two groups (P > 0. 05). Compared with finasteride group, finasteride + terazosin group had a greater improvement in prostate volume, IPSS, Ru and Qmax, with significant difference (P < 0. 05). Also, compared with bonuo group, finasteride group had a grater improvement in prostate volume and PSA (P < 0. 01), but there was no significant difference in the improvement of IPSS, Ru and Qmaxbetween two groups (P > 0. 05). CONCLUSIONS: Finasteridecan in treatment of BPH can significantly relief the symptoms and reduce the prostate volume with slight adverse reactions. Objective To approach the clinical significance of the intraoperative and postoperative bleeding the shortterm oral finasteride in the TURP method. Methods A retrospective study of 86 patients with benign prostatic hyperplasia of TURP in Qiyang People's Hospital from June 2012 to September 2013 were divided into group A (46 cases) with 5 mg oral finasteride one week before surgery and group B (40 cases) without oral finasteride. The intraoperative and postoperative blood loss, electric cut operation time, postoperative maximum urinary flow rate measurement of every patients were recorded. Results There was no statistical significance between group A and group B before the operation in prostate volume, maximum uroflow rate (P>0.05)[(43.7±12.6), (45.2±8.9)mL vs (7.2±5.6), (7.3±3.6)mL/s]. The maximum uroflow rate of group A was (15.7±7.8)mL/s while group B being(14.8±9.2)mL/s. The difference had no statistical significance between the two group(P>0.05). There was no statistical significance between group A and group B in maximum uroflow (P>0.05). Compared the operation time [(46.0±15.1), (57.6±17.2)min],the intraoperative bleeding [(154.7±78.9), (226.0±94.5)mL],the postoperation bleeding [(89.2±42.3), (112.0± 87.6)mL] between the two groups,it had statistical significance in difference(P<0.05). Objective: To investigate the effect of Ruyi Jinhuang Plaster (RJP) on testosterone propionate-induced BPH in the rat model and its action mechanisms., Methods: Forty-eight SD male rats were randomly divided into six groups of equal number: normal control, BPH model control, finasteride, and high-, medium- and low-dose RJP. The BPH model was made in the latter five groups by hypodermic injection of testosterone propionate. From the first day of modeling, the rats of the normal control and BPH model control groups were treated with blank plasters and those of the high-, medium- and low-dose RJP groups with RJPs at 42.0, 21.0 and 10.5 cm2/kg applied to the dehaired area of the back, and those of the finasteride group by gavage of finasteride at 4.5 mg/kg, all once a day for 30 successive days. Then the prostates of the animals were harvested for observation of histopathological changes by HE staining, measurement of the areas of interstitial and epithelial cells and prostatic glandular cavity, and determination of the expressions of P38, JNK2, NF-кBP65 and STAT3 proteins in the prostate tissue by Western blot., Results: Compared with the BPH model controls, the high-dose RJP group showed significantly decreased proliferation and area proportion of prostatic epithelial cells (P < 0.05), increased area proportion of the prostatic glandular cavity (P < 0.05), and reduced expressions of P38, p-P38, NF-кBP65, P-NF-кBP65, STAT3, P-STAT3 and JNK2 in the prostate tissue (P < 0.05); the medium-dose RJP group exhibited markedly down-regulated expressions of JNK2 and NF-кBP65 (P < 0.05) but an up-regulated level of p-JNK (P < 0.05); while the low-dose RJP group displayed a remarkably reduced expression of JNK2 (P < 0.05) but an elevated level of p-JNK (P < 0.05)., Conclusions: RJP suppresses BPH in the model rat by down-regulating the expressions of P38, p-P38, NF-кBP65, P-NF-кBP65, STAT3, P-STAT3 and JNK2 or up-regulating that of p-JNK in the prostate tissue.

Objective: To investigate the effect of Kangshuailing Gao (KG) on benign prostatic hyperplasia (BPH) in rats and its action mechanisms., Methods: Fifty BPH model rats were randomized into five groups of equal number, BPH model control, finasteride control, and high-, medium- and low-dose KG, to be treated intragastrically with distilled water, finasteride solution at 0.52 mg/kg, and KG solution at 4.16, 2.08 and 1.04 g/kg respectively once a day for 30 days consecutively. Another 10 normal healthy rats were taken as blank controls. The rats were weighed once a week during the treatment. The wet weight and index of the prostate were obtained after treatment, followed by measurement of the contents of serum estradiol (E2) and dihydrotestosterone (DHT), testosterone (T) and hypoxia-inducible factor-1α (HIF-1α) in the prostatic tissue, and observation of histomorphological changes in the prostate under the light microscope., Results: Compared with the BPH model control group, high- and medium-dose KG significantly reduced the prostate wet weight (［0.84 ± 0.08］ vs ［0.69 ± 0.04］ and ［0.71 ± 0.07］ g, P < 0.01), the prostatic index (［0.28 ± 0.03］% vs ［0.20 ± 0.02］% and ［0.22 ± 0.03］%, P < 0.01), and the levels of T (［4.63 ± 1.25］ vs ［2.44 ± 0.47］ and ［2.91 ± 0.69］ ng/L, P < 0.01) and DHT (［154.44 ± 20.25］ vs ［88.23 ± 13.63］ and ［90.52 ± 16.44］ nmol/L, P < 0.01), but increased the level of E2 (［0.95 ± 0.24］ vs ［1.19 ± 0.14］ and ［1.20 ± 0.22］ nmol/L, P < 0.01) in the serum. High-dose KG remarkably reduced the overexpression of HIF-1α in the prostate tissue of the BPH model rats (P < 0.01) and alleviated such BPH-related symptoms as epithelium thinning, intraglandular secretion reduction, and interstitial substance decrease., Conclusions: Kangshuailing Gao acted effectively on BPH in the model rats by reducing the androgen level, balancing the estrogen/androgen ratio, and downregulating the expression of HIF-1α in the prostate tissue.

Objective: To investigate the effect of finasteride on the microvascular density (MVD) and the expression of the vascular endothelial growth factor (VEGF) in the seminal vesicle of rats., Methods: Forty male SD rats were randomly and equally divided into groups A, B, C and D, those in groups A and B fed with normal saline as the control and those in C and D with finasteride at 40 mg per kg of the body weight per day, A and C for 14 days and B and D for 28 days. Then the seminal vesicles of the animals were harvested for HE staining, measurement of MVD, determination of the expressions of CD34 and VEGF by immunohistochemistry, and observation of histomorphological changes in the seminal vesicle., Results: The expressions of CD34 in groups C and D were decreased by 6.7% and 15.8% as compared with those in A and B (P<0.01), and that in group D decreased by 9.3% in comparison with that in C (P<0.01). The expression indexes of VEGF in groups C and D were decreased by 6.9% and 14.1% as compared with those in A and B (P<0.01), and that in group D decreased by 9.0% in comparison with that in C (P<0.01)., Conclusions: Finasteride can inhibit the expression of VEGF in the seminal vesicle tissue of the rat and hence suppress the angiogenesis of microvessels of the seminal vesicle.
