ObjectiveTo systematically review research of integrated dementia care models in primary care abroad, summarize the main dimensions and key elements of the models, and provide a reference for the construction of community-based integrated dementia care models in China. MethodsFollowing the scoping review methodology, literature on integrated dementia care models in primary care published from database inception through July 1, 2023, was searched in Medline, Embase, and Web of Science databases. The literature was screened and characteristics extracted and synthesized. ResultsA total of 3 357 articles were retrieved and 25 were finally included. The integrated dementia care models were categorized into collaborative care plans, memory clinics and care networks. The dimensions of integration focused mainly on the service and professional levels, with the least emphasis on the system, functional and normative levels. Effectiveness was mainly reflected in improved health outcomes for patients, caregiver burden, and accessibility of services, but was related to the form of implementation. The main barriers to the implementation of integrated care models included lack of funding, inconsistent payment methods, short of common information technology platforms, insufficient human resource development, and inadequate skills among general practitioners. The relationship between the models and the local primary care system was mainly embedded. ConclusionThe types of integrated dementia care models in primary care abroad are diverse and their effects vary. They provide a reference for building community-based integrated care models for older adults with dementia in China.

These issues may burden family caregivers and impact patient quality of life. Therefore, a comprehensive, multidisciplinary integrated care plan is needed to expedite postoperative recovery and reduce the social burden on patients. In recent years, government promotion of integrated healthcare, including the implementation of postoperative accelerated recovery care models, has led to the introduction of the enhanced recovery after surgery program as part of post-cardiac surgery integrated care. This program, providing combined care that is multidisciplinary and evidence-based, incorporates elements such as the provision of carbohydrate drinks, multimodal pain management, and fluid management. Reducing to a minimum the emotional and physiological stresses on patients facilitates a faster return to normal functionality. In this paper, a literature review is conducted to provide a reference for future post-coronary artery bypass surgery care by clinical teams, with the aim of offering an integrated approach to patient care. The persistent elevation of the prevalence of chronic non-communicable diseases and sudden pandemic of COVID-19 infections have once again attracted the attention of the whole society to prevention activities closely associated with epidemic prevention. Although the correlation between prevention and treatment of health is a long-term topic, innovation and mechanism of treatment-prevention integration proposed by Chinese health administration after the peak of COVID-19 epidemic has attracted attention and reflections of the medical and academic fields. Medical treatment and prevention are inseparable. The questions such as how to achieve the coordination and balance between treatment and prevention, what is the solution of treatment-prevention integration, are worth further study and exploration. Beginning with clarifying the concept of treatment and prevention, this paper analyzes the reasons for the separation of treatment and prevention, explores the approaches for effective connection and cooperative interaction between the two, thus providing further research directions for general practice researchers, calling on colleagues in general practice to contribute their wisdom and promote the contribution of treatment-prevention integration to healthy China.

Recently, the demand for home health care has increased in Taiwan, which has an aged society. The Taiwanese government has introduced several programs since 1995 to integrate and improve the medical care of patients who face difficulties visiting health-care facilities due to disability, illness or frailty. The Home-Based Integrated Care Program, which took effect in 2016, is a three-stage holistic program that incorporates home-based medical care, severe homebased medical care, and home-based palliative care. Traditional Chinese medicine (TCM) can be used for home-based medical care. In TCM, patients are treated with a special focus on preventive medicine; related treatments can be used to prevent disability, to slow the progression of disability, and to improve the health of older adults with moderate-to-severe disability. By improving individual health, TCM can also help improve the overall quality of health care. The Taiwanese government included TCM in the Home-based Integrated Care Program in June 2019. Patients who require TCM care can apply to receive such care at home. On the basis of our literature review, this paper examines the role and contribution of TCM in home health care with the support of empirical experience. Studies on functional integration in information and communication technology (ICT) for integrated care of older adults with chronic diseases are needed but rarely conducted. Therefore, this study gathered and analyzed information on the operational situation and expiration of ICT developed in various countries for disease management in health care and long-term care. This paper has three sections, each focusing on one ICT-telemedicine and wearable devices, applications and digital therapy, and smartphones and artificial intelligence. By reviewing journal articles, periodicals, government and international organization reports, and ICT product websites, this paper compares (using tables) success cases, ICT firms, and products and their functional introduction to update readers on the latest progress in ICT applications. This paper concludes by summarizing the contribution of ICT in the use of video, publication of newsletters, and health monitoring, focusing especially on the development of ICT in domiciliary care and its policy implications for long-term care in Taiwan. Diabetes is a huge problem that has been recognized as an important healthcare issue by most national governments. Diabetes affects patients not only in terms of physical health but also in terms of mental health. Furthermore, the medical costs that are associated with diabetes are considerable. Many countries have developed integrated care models for the treatment of diabetes and pay particular attention to assisting patients that require complicated care. Evidence from empirical studies shows that the integrated care of diabetes reduces complications, increases patients' health literacy, and decreases medical costs. Accordingly, integrated care has been applied to the treatment and care of diabetes patients. In the integrated care model, patients not only receive multidisciplinary care during hospitalization but also receive medical services after their return to the community. There has been a dearth of scholarly discussions in Taiwan related to the integrated care of diabetes. Therefore, we present in this paper the concept of integrated diabetes care and the role of the advanced practice nurse (APN) in the multidisciplinary model. It is hoped that this introduction provides practical insights for healthcare professionals into the essence of integrated care.

Emergency departments represent the first line in facing major healthcare events. During major epidemic outbreaks, patients crowding into the emergency departments increase the wait time for patients and overload the staffs that are on duty. The dengue fever outbreak in southern Taiwan during the summer 2015 presented a huge management challenge for physicians and nurses in local hospitals. We responded to this challenge by integrating resources from different hospital departments. This strategy successfully increased group cohesiveness among the medical team, ensuring that they could not only ultimately cope with the outbreak together but also effectively provide patient-centered care. This interdisciplinary care model may serve as a reference for medical professionals for the management of future epidemics and similar events.

The percentage of people aged 65 years and older is estimated to exceed 20% of the total population in Taiwan, which makes Taiwan a "super-aged" country. This rapid demographic transition, unprecedented in human history, presents unique challenges. The health characteristics of older people differ fundamentally from their younger adult peers, especially in terms of the critical impact of disability on health. Therefore, special attention to the interrelationship between multimorbidity and disability is required in order to provide healthcare services that are appropriate and effective. The integrated care approach of Europe, which concurrently addresses the needs of health and social care, has become the important foundation of social security in the European Union. The healthcare services available to older people in Taiwan, including outpatient, inpatient and intermediate care, are far from satisfactory and extensively fragmented. For older people with multimorbidities and / or disabilities, these fragmented healthcare services result in inconvenient access to healthcare and impaired quality of care and quality of life. At present, Taiwan has developed various healthcare models such as integrated outpatient services, age-friendly healthcare facilities, post-acute care, and a 10-year long-term care program. However, the development of these healthcare models clearly demonstrates the need for service integration. Integrated care promotes seamless case management, which aims to provide smooth access for older people with multiple, complex care needs to essential services and comprehensive healthcare benefits. During the process, the integration of services per se should be prioritized over the prompt merging of departments or facilities. The development and success of integrated care depend on the complete understanding of and promotion by the frontline working staff.
