ObjectiveTo explore the feasibility of the International Classification of Functioning, Disability and Health Rehabilitation Set (ICF-RS) in evaluating the overall functional level, rehabilitation treatment effect, and the relationship between rehabilitation efficacy and cost-effectiveness of patients with knee osteoarthritis (KOA) in the northwest Gobi region.MethodsA total of 50 patients with KOA who were hospitalized in the Acupuncture and Moxibustion Department of Jimsar County Hospital of Traditional Chinese Medicine from March 2022 to March 2023 were selected. Due to reasons such as patients' own comprehension deviation and low assessment compliance, 8 cases were discontinued or dropped out, and finally 42 cases were included. Personalized rehabilitation programs were formulated based on the severity of functional disorders of the patients, referring to the "Clinical Practice Guidelines of Traditional Chinese Medicine Rehabilitation for Knee Osteoarthritis", once a day, 6 days a week, for a total of 2 weeks. ICF-RS was used to evaluate the overall functional level, rehabilitation efficacy and rehabilitation treatment costs of the patients.ResultsThe main physical functional disorders of KOA patients were b280 pain sensation, b455 exercise tolerance, b130 energy and drive function, and b134 sleep function in sequence; the main activity functional disorders were d410 changing basic body postures, d455 moving around, and d415 maintaining a body posture; the main participation functional disorders were d850 paid employment and d920 recreation and leisure. There was no statistically significant difference in the composition ratio of the frequency of the limit values of all categories of ICF-RS (except d240 controlling stress and other psychological needs) between Han and ethnic minority KOA patients (P>0.05). ObjectiveThis study explores the use of the International Classification of Functioning, Disability, and Health Rehabilitation Set (ICF-RS) to evaluate the physical, activities, and social participation levels of patients with chronic neck, shoulder, waist, and leg pain, and to explore its influencing factors.MethodsWe selected 162 patients with chronic neck, shoulder, hip, and leg pain from the outpatient departments of the Central Hospital of Xiangyang City from July to December 2023, who had meet the diagnostic criteria and inclusion and exclusion criteria for chronic pain. A trained assessor used a questionnaire to collect detailed information on the patients' basic demographic data (sex, age, education level, medical expenses, body mass index), lifestyle habits (smoking, drinking), medical history (hypertension, hyperlipidemia, diabetes), and use of pain medication. The patients were assessed for their functional levels (physical function, activity function, social participation) using the ICF-RS. A multivariate ordered Logistic regression analysis was used to identify independent risk factors for the patients' body function, activity function, and social participation function (mild, moderate and severe impairment).ResultsAmong the three dimensions of physical function, daily activities, and social participation of patients with chronic pain in outpatient clinics, about 1/3 of them with mild functional impairments being the main type. There were 10 ICF-RS categories (33.33%) with a functional impairment proportion > 50%, involving 4 categories in the physical function dimension, 2 categories in the daily activity function dimension, and 4 categories in the social participation dimension. The differences in exercise tolerance and mobility function among patients with chronic pain of different age groups were statistically significant (P = 0.040, P = 0.030); the differences in the ability to help others among patients with chronic pain of different age groups and whether they drank alcohol were statistically significant (P = 0.008, P = 0.021); the differences in entertainment and leisure ability among patients with chronic pain of different age groups and whether they had hyperlipidemia were statistically significant (P = 0.028, P = 0.004).ConclusionPatients with chronic pain have varying degrees of functional impairments in their body function, daily activities function, and social participation function, with mild functional impairments being the predominant; Age, alcohol consumption, and hyperlipidemia are independent influencing factors for functional impairment in body function, daily activity function, and social participation function.

ObjectiveTo examine the prevalence of multiple chronic conditions (MCC) among community-dwelling elderly in a district of Shenzhen city for providing evidence to life quality promotion of the population. MethodsAt 62 community health care centers in Nanshan district of Shenzhen city and with random cluster sampling and a self-designed questionnaire, face-to-face interview and physical examination were conducted among 4 475 physical examinees aged 65 years and above from 2018 through 2019 to collect information relevant to MCC prevalence. The physical function of the elderly was assessed using grip strength tests and Short Physical Performance Battery assessments. Unconditional multivariate logistic regression analysis was performed to explore the relationship between MCC and physical function decline. ResultsAmong the 4 297 participants included in the final analysis, 2 728 (63.49%) were identified as having MCC; of the MCC sufferers, 1043 (38.23%)，793 (29.07%)，and 892 (32.70%) had two, three, and four and more chronic conditions. Physical function decline was assessed for 745 (17.34%) of all the participants, and the number (percentage) of the participants assessed as having decreased gait speed, prolonged sitting up time, and decreased grip strength were 1 761 (40.98%), 1 475 (35.33%), and 1 761 (40.98%), respectively. After adjusting for sex, age, education, marital status, registered residence, smoking, and alcohol drinking, the results of logistic regression analysis showed that the participants with MCC were at significantly increased risk of reduced physical function (odds ratio [OR] = 1.52, 95% confidence interval [95% CI]:1.26 – 1.82), decreased gait speed (OR = 1.83, 95% CI:1.45 – 2.30), and prolonged sitting up time (OR = 1.22, 95% CI:1.06 – 1.39) compared to those without the conditions. ConclusionThe study findings indicate a high prevalence of MCC and the prevalence is associated with increased risks of physical function decline, reduced gait speed, and prolonged sitting up time among the community-dwelling elderly people in Nanshan district of Shenzhen city.

Background Frailty can increase the risk of negative health-related outcomes in older adults. Protein supplementation may be an effective way to improve frailty, but there is disagreement about its effects on frailty. Objective To systematically evaluate the effects of protein supplementation on muscle mass, strength, and physical function in frail/pre-frail older adults. Methods Electronic databases of CNKI, Wanfang Data, CQVIP, PubMed, Embase, Web of Science, Cochrane Library, CINAHL and Medline were retrieved for randomized controlled trials (RCTs) of the effects of protein supplementation on muscle mass, strength and physical function in frail/pre-frail older adults published from inception to June 2022. After literature screening, the quality of eligible RCTs was evaluated, and from which relevant data were extracted. RevMan 5.4 was performed to explore the effects of protein supplementation on muscle mass, muscle strength and physical function in frail/pre-frail older adults. And for the outcome indicator of muscle strength (grip strength) , due to large amount of reported literature, this study will be based on the amount of protein supplementation (0.05) . The results of subgroup analysis showed that the effect of protein supplementation on the grip strength of the Asian population subgroup was significantly different from that of the European population subgroup in between-group comparisons (χ2=5.76, P=0.02) . Conclusion Protein supplementation may improve gait speed in frail/pre-frail older adults, but it does not show a significant advantage in improving their muscle mass, muscle strength and other physical functions. It is recommended to further investigate the effects of longer durations of supplementation, different types of protein supplemented, different amounts of supplementation and different regional populations on older adults with different frailty states, in order to find the best pattern of protein supplementation and provide a more sufficient evidence-based basis for frailty management.

Objective To investigate the effect of home-based resistance training in cardiac rehabilitation for patients with coronary heart disease (CHD). Methods Totally 122 CHD patients requiring home-based cardiac rehabilitation were divided into the control group (n=61) and the observation group according the sequence of hospital admission. All patients received community care and health education by telephone follow-up, and patients in the observation group were given health education on cardiac rehabilitation and home-based resistance training. The physical function, motor ability, quality of life and cardiac parameters of patients were measured. Results After a three-month intervention, The scores of Chinese Mini Physical Performance Test(CM-PPT) and Chinese Questionnaire of Quality of Life in Patients with Cardiovascular Diseases(CQQC) in the observation group were higher than those in the control group (P＜0. 01). Patients in the observation group achieved higher outcomes of 30-second Arm Curl Test (30-ACT), longer distance of 6-Minute Walking Test and shorter time of 10x Sit-To-Stand Test (STS10) compared with those in the control group after intervention (P＜0. 05). Higher level of Cardiac output (C0) and Left Ventricular Ejection Fraction(LVEF) and lower level of Left Ventricular End Diastolic Volume (LVEDV) and Left Ventricular End Systolic Volume(LVESV) were reported in the observation group than those in the control group(P＜0. 05). Conclusion Application of health education on cardiac rehabilitation and home-based resistance training could improve the physical function, motor ability, cardiac function and quality of life in CHD patients. Objective: To investigate the clinical characteristics of overlapping syndromes of low muscle mass in Chinese patients with rheumatoid arthritis (RA) and their impact on physical function., Methods: Consecutive patients with RA were recruited from September 2019 to April 2024 at Department of Rheumatology and Immunology, Sun Yat-Sen Memorial Hospital. Clinical data including disease acti-vity, physical function and radiographic assessment were collected. All patients also finished measurement of body composition, grip strength, and gait speed, and overlapping syndromes of low muscle mass as well as malnutrition, sarcopenia, sarcopenic obesity, and cachexia were evaluated. The Stanford health assessment questionnaire- disability index (HAQ-DI) was used to evaluate physical function. Logistic regression was used to analyze the related factors of physical dysfunction., Results: A total of 1 016 RA patients were recruited. Their mean age was (52.4±12.5) years, and 82.5% were female. There were 557 cases (54.8%) with overlapping syndromes of low muscle mass and all of them were malnutrition. On this basis, 326 cases (32.1%) exhibited sarcopenia, 124 (12.2%) sarcopenic obesity, and 33 (3.2%) cachexia. There were 584 (57.4%) of RA patients having physical dysfunction, with varying degrees of severity 421 (41.4%) mild, 124 (12.2%) moderate, and 39 (3.8%) severe. Compared with patients without overlapping syndromes of low muscle mass ( n =459) or with malnutrition only ( n =231), RA patients with both malnutrition and sarcopenia ( n =326) had significantly higher core disease activity indicators and higher rate of physical dysfunction (69.6% vs . 42.0% vs . 56.6%). However, compared with patients without overlapping syndromes of low muscle mass, patients with malnutrition only had lower HAQ-DI score (median 0.0 vs . 0.1) and lower rate of physical dysfunction (42.0% vs . 56.6%). Multivariate Logistic regression analysis showed that simultaneously overlapping malnutrition and sarcopenia were associated factors of physical dysfunction ( OR =2.021, 95% CI : 1.067-3.828), but malnutrition only was not., Conclusion: Simultaneously overlapping malnutrition and sarcopenia can deteriorate disease activity and physical dysfunction in RA patients. The screening and evaluation of overlapping syndromes of low muscle mass, especially sarcopenia should be emphasized in patients with RA.

Intensive care unit (ICU)-acquired weakness is a common neuromuscular complication of critical illness that is considered to be associated with prolonged duration on mechanical ventilation and systemic inflammatory response syndrome. In addition, nutrition and metabolic alternations, which are commonly seen in patients in the ICU, may further accelerate muscle wasting and increase the incidence of ICU-acquired weakness. The clinical features of ICU-acquired weakness include acute generalized muscle weakness that develops after the onset of critical illness. Diaphragmatic dysfunction, post-extubation dysphagia, and functional decline also are common in patients with ICU-acquired weakness. As the recovery of these physical functions is lengthy and difficult, a multidisciplinary team management is recommended. This mini-review was conducted to provide a scientific overview for ICU-acquired weakness, including its definition, etiology, diagnosis/screening, impacts, and potential intervention strategies. We hope that increasing the understanding of frontline staff will promote the timely planning and implementation of related screenings and interventions to enhance the functional recovery of patients receiving care in the ICU. Purpose: The purpose of this study was to systematically review the instruments utilized to assess physical impairment in post-intensive care syndrome (PICS) of intensive care unit (ICU) survivors. Method: Online databases searched were MEDLINE, Cochrane, CINAHL, and Embase. Studies that met the following criteria were included: 1) the study population exclusively had experience with ICU admission; 2) the study assessed pulmonary, neuromuscular, and physical functions; and 3) the study was published in English language journals after 2007. Results: A total of 56 instruments (2 pulmonary, 25 neuromuscular, 29 physical function) from 94 studies were reviewed. They were classified into self-report, observation, and measurement according to the type of assessment. No instrument measured all 3 areas of physical impairment. Five instruments were originally developed for the ICU patients. The most frequently applied instruments were the Medical Research Council and the 36-item Short Form Survey (physical component summary), which were used in 23 studies each. Only 13.8% of reviewed studies reported the reliability or validity of the instruments. Conclusion : Our results suggest that the appropriateness of instruments assessing physical impairment in PICS cannot be guaranteed. Despite the multidimensional concept of physical disabilities, most studies measured only one area, and studies that reported psychometric properties were limited. Objective: To examine the application effect of risk management combined with precision nursing in the maintenance of physical functions and the safety management of older patients with chronic diseases and to provide the basis for the formulation of physical function maintenance and fall prevention programs for older inpatients with chronic diseases., Methods: The control group and the observation group were randomly selected by cluster sampling from geriatric wards of similar conditions in West China University, Sichuan University. The control group received conventional nursing care and the observation group had risk management combined with precision nursing on top of the conventional nursing care. The primary outcome indicator was activities of daily living (ADL). The secondary outcome indicators were the whole-day ambulation time and infusion time, the incidence of falls, and length-of-stay among patients., Results: A total of 109 patients in the control group and 110 patients in the observation group completed the study. At the time of admission, there was no statistically significant difference between the ADL scores of the two groups ( P >0.05). Among the patients in the intervention group, the post-intervention ADL scores of the observation group (75.74±22.71) were higher than the pre-intervention ADL scores (68.74±17.32) and the difference was statistically significant ( t =2.435, P <0.05). Among the patients in the control group, the post-intervention ADL scores of the control group (70.36±21.90) were higher than the pre-intervention ADL scores (69.14±18.29) and the difference was not statistically significant ( t =0.549, P >0.05). In addition, the post-intervention ADL score of the observation group was higher than that of the control group, and the difference was statistically significant ( t =2.973, P <0.05). After intervention, patients in the observation group had higher whole-day ambulation time during hospitalization (7.35±2.07) than those in the control group did (4.11±2.62), showing significant difference ( t =1.508, P <0.05). The whole-day infusion time during hospitalization of the observation group (1.68±0.19) was lower than that of the control group (2.39±0.31), showing significant difference ( t =-2.339, P <0.05). There was no significant difference in the incidence of falls or the length-of-stay between the two groups ( P >0.05)., Conclusion: Risk management combined with precision nursing helps improve patients' ability to perform activities of daily in older patients with chronic diseases, increase their whole-day ambulation time, and reduce the whole-day infusion time without increasing the incidence of falls, which has a positive effect on physical function maintenance and fall prevention in hospital safety management of older patients with chronic diseases., (Copyright© by Editorial Board of Journal of Sichuan University (Medical Sciences).)

Objective To explore the effect of empathy nursing on the anxiety and nursing satisfaction of cesarean parturients. Methods Seventy-two cases of cesarean parturients treated in our hospital between September 2013 and October 2014 were selected as the study object. These women were randomly divided into an observation group and a control group，with 36 cases in each group．Those in the control group were given routine nursing and those in the observation group were given routine nursing and empathy nursing．Results After nursing， the anxiety and depression of patients in both groups were alleviated. The anxiety score and depression score in the observation group were respectively (31.16±3.94) and (18.21±2.97)， lower than those in the control group， with the anxiety score of (43.19±4.45) and the depression score of(43.19±4.45) ．The differences between the two groups were statistically significant (P<0.01). After nursing， improvement on life quality of the patients was registered in both groups．The scores of cognitive function， physical function，emotional function，role function and social function of patients in the observation group higher than those in the control group．The comparative difference was statistically significant (P<0.01). Besides， the total score of nursing satisfaction in the observation group was (118.53±12.41)，significantly higher than that in the control group which was (97.65±6.81)，and the difference was statistically significant (P<0.01). Conclusion For cesarean parturients， empathy nursing can better help to identify the patients'psychological changes and to establish closer bond between the nursing staff and the patients，which can provide guidance for them to cultivate positive mental state， help the staff to better understand their needs， pains and their pressure， thus lessen their anxiety and improve the nursing effect and satisfaction. [Purpose] The purposes of this study were to assess the relationships between the angular velocity of knee extension using a gyro-sensor and physical functions of the frail elderly, and to confirm the safety of this protocol. [Subjects] The participants of this study were 170 older people (age ± standard deviation 78.6 ± 5.6) who participated in an exercise program for frailty. [Method] Angular velocity and strength of knee extension and other functions such as walking ability were measured. Pearson correlation coefficients and multiple linear regression analysis were used to examine the relationships between maximal knee joint angular velocity and the functional performance measurements. [Result] The angular velocity of knee extension was significantly correlated with all the physical functions measured, except for grip strength, and was found to be more highly correlated with the physical functions measured than knee extension strength in multiple linear regression analysis. [Purpose] We investigated changes in perioperative gastrointestinal cancer patients' physical function between before and after surgery, and the influence of gender and surgical procedure on the changes. [Subjects] The subjects were 50 perioperative gastrointestinal cancer patients, 27 males and 23 females with an average of 62.5±10.7 years, who were categorized by gender and two surgical procedures. [Method] We measured the 6-min walking distance (6MWD) of each patient before and after surgery, and calculated the postoperative distance as a percentage of the preoperative distance (%6MWD). [Results] For all patients, and by gender and surgical procedure, postoperative 6MWD significantly decreased. Further, for %6WMD we found main effects of both gender and surgical procedure. [Conclusion] A significant postoperative decrease in physical function of perioperative gastrointestinal cancer patients was found after surgery, and our results suggest the possibility that gender and surgical procedure are factors influencing the changes in postoperative physical function. We consider that information on gender and surgical procedure of perioperative gastrointestinal cancer patients is important information, because it indicates changes in postoperative physical function. Objectives: In order to explore whether being widowed was a negative life event for older women, we employed the stress process model to examine the associations between widowhood, leisure activities, and physical function among middle-aged and older Taiwanese women. Methods: This study was based on data from the Taiwan Longitudinal Study of Aging (TLSA), a nationally representative sample (n=1,070) of middle-aged and older women aged 50-66, begun in 1996 and with 7-year follow-ups. Generalized linear modeling with GEE estimates was used due to repeated measures. Results: The results showed a significant, positive association between leisure activities and physical function in middle-aged and older women. The widowed women were more likely than married women to participate in social and religiously related leisure activities, and their physical function was not significantly poorer than that of th married women. Conclusions: The findings suggested that participation in leisure activities promoted physical function in the study population. Being widowed was not a negative life event in terms of function for middle-aged and older Taiwanese women. Social welfare and health-related policies that aim to promote well-being should consider participation in leisure activities as an important mechanism through which widows can maintain their physical function.
