Trazodone hydrochloride sustained -release, as a multi-receptor binding drug, can exert different pharmacological effects at different doses and can improve a variety of psychiatric disorders. To further standardize the clinical application of trazodone hydrochloride sustained release, the expert writing group integrated domestic and international clinical trials, clinical practice guidelines, prescribing guidelines and clinical medication experience to prepare this expert recommendation for clinical application. This article mentions that trazodone hydrochloride sustained release is clinically applicable to depression, insomnia caused by various reasons, and also applied to generalized anxiety disorder, sexual dysfunction, post-traumatic stress disorder, substance dependence and withdrawal reactions and obsessive-compulsive disorder, and describes drug dosage, interactions, and precautions for use in special populations, with a view to providing clinicians with scientific and comprehensive medication guidance.

OBJECTIVE: To probe into the clinical effects of modified Tiaoqi Yangshen decoction combined with trazodone in the treatment of anxiety insomnia. METHODS: A total of 123 patients with anxiety insomnia admitted into Chongqing Fuling District Hospital of Traditional Chinese Medicine from Mar. 2019 to Mar. 2022 were extracted to be divided into the research group (n = 62) and control group (n = 61) via the random number table method. The control group was routinely given trazodone, while the research group received Tiaoqi Yangshen decoction combined with trazodone. The efficacy, adverse drug reactions, psychological characteristics and sleep quality [Hamilton depression scale (HAMD), Hamilton anxiety scale (HAMA) and Pittsburgh sleep quality index (PSQI) scores, polysomnography and neurotransmitter [neuropeptide Y (NPY), substance P (SP), 5-hydroxytryptamine (5-HT)] levels of two groups were observed. RESULTS: The total effective rate of the research group was 95. 16% (59/62), higher than 78. 69% (48/61) of the control group, the difference was statistically significant (P < 0. 05). After treatment, the scores of restlessness, night terrors, restless sleep, chest discomfort or chest heavy load decreased significantly in both groups, the research group were significantly lower than those in the control group, the differences were statistically significant (P<0. 05). After treatment, the HAMD, HAMA and PSQI scores of the research group were lower than those of the control group, with statistically significant differences (P<0. 05). After treatment, the total sleep time of the research group was longer than that of the control group, sleep latency was shorter than that of the control group, and the number of awakenings was less than that of the control group, with statistically significant differences (P<0. 05). After treatment, the levels of NPY and 5-HT in the research group were higher than those in the control group, and the level of SP was lower than that in the control group, with statistically significant differences (P< 0. 05). The incidence of adverse drug reactions in the research group was 6. 45% (4/62), which was not statistically different from 4. 92% (3/61) in the control group (P > 0. 05). CONCLUSIONS: Modified Tiaoqi Yangshen decoction combined with trazodone in the treatment of anxiety insomnia can further improve the psychological characteristics and sleep quality of patients, regulate nerve conduction, and has significant effect and high safety. Objective To determine the efficacy and safety of trazodone in the treatment of benzodiazepine dependence. Methods Forty insomnia patients who met the Classification and Diagnostic Criteria of Mental Disorders in China Third Edition (CCMD-3) of dependence syndrome due to benzodiazepine were involved in the study. Patients were randomly assigned to trazodone group and placebo group for 3 months. The efficacy were assessed by Withdrawal Symptoms Checklist, Hamilton Anxiety Rating Scale (HAMA) and polysomnography (PSG). Adverse events were assessed by Treatment Emergent Symptom Scale (TESS). Results The Withdrawal Symptoms Checklist of trazodone group was significantly lower after 7 d treatment than that of placebo group (P = 0.000), and HAMA score of the trazodone group was also significantly lower after 15 d treatment than that of placebo group (P = 0.000). There were no difference in Withdrawal Symptoms Checklist and HAMA of placebo group before and after treatment. Withdrawal Symptoms Checklist and HAMA of the trazodone group were decreased after treatment (P = 0.000). In comparison with placebo group, sleep parameters of the trazodone, including total sleep time (TST), sleep efficiency (SE), sleep latency (SL) and slow wave sleep (SWS) time presented improvement after 7 d treatment (P = 0.000, for all). After trazodone treatment, total sleep time, slow wave sleep time, sleep efficiency and sleep latency were improved (P = 0.000, for all). No obvious adverse reaction occurred. There were no significant differences in TESS scores between pre- and post-treatment in both groups (P > 0.05). Conclusion Trazodone is markedly effective and safe in the treatment for benzodiazepine dependence.

Objective ：To establish a HPLC-MS／MS method for the determination of trazodone （TRZ） in human plasma ． Methods ：The plasma samples were treated by protein precipitation and separated in CAPCELL PAK C １８ MG column with a mobile phase of methanol-water-ammonium acetate-formic acid under gradient conditions ．The flow rate was ０ ．４ ml／min and column temperature was ５０ °C．Detection was performed in positive ion and multiple reaction monitoring （MRM） acquisition mode with electrospray ion source（ESI） as the ionization technique ．Results ：TRZ displayed good linearity within the ranges of 10.0-３０００ ng／ml（r ＝ ０．９９６ ９ ）．Mean extraction recovery rate was ９７．５％ ．The intra-batch and inter-batch relative standard deviation （RSD） was ⩽ ８ ．２％ （n＝ ６）．Conclusion ：The HPLC-MS／MS method is specific ，sensitive ，accurate ，simple and rapid ，which could be used for analysis and research of TRZ in biospecimens. Objective: To explore the clinical efficacy of trazodone alternative therapy (AT) on benzodiazepines (BZDs)- dependent elderly patients and the effect on cognition of the patients involved. Methods ： A prospective research was made on 62 BZDs-dependent elderly patients admitted into the Sichuan Yibin Second People，s Hospital from Sep 2012 to Sep 2013. The patients received treatment with decreased dosage and trazodone AT. The depression，sleep status, changes of cognitive function in the patients and incidence of adverse drug reactions (ADRs) were observed before AT and 6 months after AT. Results ： There were no significant differences in fall-asleep time, sleep duration and sleep efficiency of the patients measured by the scales of Pittsburgh Sleep Quality Index (PSQI)，when comparisons were made between before AT and after AT (P > 0.05). Following AT， the scores of sleep quality，sleep disorder and the disorder of daily activities，as well as total scores [ (1.96士 0.43), (1.85±0.33)，（2.22 ± 0.46) and (12.81 ± 1.15)] were significantly lower than those [ (2.31 ± 0.41), (2.42 士 0.39)，(2.64±0.35) and (14.18±1.08)] before AT (P<0.05). After AT，the scores of all the indicators in Wechsler Scale of the 62 patients were significantly higher than those before AT (P<0.05). Conclusion: The AT could produce relatively good therapeutic effect on BZDs-dependent patients，and at the same time it could improve the cognitive function of the patients.
