Background The incidence of acute ischemic stroke (AIS) remains high, and a timely restoration of cerebral blood flow is crucial for its prognosis. There are less therapeutic approaches to promote recovery of cerebral blood flow in AIS patients treated beyond a limited time window. The analysis of remote ischemic postconditioning (RIPostC) on the efficacy, complications and prognosis of AIS patients beyond the time window is of great significance. Objective To investigate the role of RIPostC on the prognosis of AIS beyond time window, thus providing a safe and effective cerebral blood flow restoration way for AIS beyond time window. Methods It was a randomized, parallel group, placebo-controlled trial involving AIS patients beyond time window (onset time > 6 h) of thrombolysis who were hospitalized in the Department of Neurology, Beijing Aerospace General Hospital from September 2, 2021 to August 31, 2022. They were randomly assigned into the control group and experimental group, and treated and followed up for 90 days. General treatment and conventional treatment of cerebrovascular disease were performed in both groups. RIPostC and simulated RIPostC were respectively given 28 times within 14 days in the experimental group and control group, respectively. Before the intervention, and 30 days and 90 days after the intervention, neurological function was assessed using the modified Rankin Scale (mRS) and the National Institutes of Health Stroke Scale (NIHSS). Cognitive function was assessed by the Mini-mental Status Examination (MMSE) and Montreal Cognitive Assessment (MoCA). Daily living ability was assessed by the Instrumental Activity of Daily Living (IADL). Mental status was assessed by the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). Cerebral blood flow velocity was assessed by the transcranial Doppler ultrasound (TCD). Inflammatory response was assessed by measuring interleukin 6 (IL-6) levels. Results Ninety-nine out of 122 AIS patients finally completed the trial and follow-up, including 49 patients in the experimental group and 50 in the control group. There were no significant differences in gender, age, underlying diseases (hypertension, diabetes, coronary heart disease) and baseline NIHSS scores between the two groups (P>0.05). Repeated measures analysis of variance showed that there was an interaction between time and group on MMSE, MoCA, mRS, NIHSS, cerebral blood flow velocity, and IL-6 (P0.05). The incidence of skin ecchymosis [4.00% (2/50) vs. 12.24% (6/49) ] and the overall incidence of adverse events [12.00% (6/50) vs. 34.69% (17/49) ] in the control group were significantly lower than those of the experimental group (P

As a device for missile-borne system to transmit and receive electromagnetic waves, the functions of missile-borne antenna are becoming more and more abundant. The shared-aperture antenna can improve the aperture utilization rate of missile seeker and can avoid radiation interference between antennas. This paper proposes a low profile circularly polarized shared-aperture antenna for missile seeker antenna based on the theory of microstrip circular polari-zed antenna. More specifically, the sequential rotation feed technology is adopted for the rectangular microstrip antenna in the high-frequency C-band part, while the orthogonal double feed technology is adopted for the cross microstrip antenna in the low-frequency S-band part. The nested shared-aperture antenna structure is adopted to realize the design of shared aperture antenna of 63 mm×63 mm×1.574 mm. The reflection coefficient is less than -10 dB in the frequency bands of 4.87~5.13 GHz and 2.49~2.51 GHz. The relative bandwidth of high-frequency antenna is 8.1% and that of low-frequency antenna is 0.4%. The axial ratio of the two antennas in the maximum radiation direction at the center frequency is less than 3 dB. The experimental data show that the shared-aperture antenna designed and applied to the missile-borne antennacan significantly improve the aperture utilization of the missile seeker, and can also eliminate the deterioration of the radiation performances caused by the mutual coupling of antennas.

Target detection and recognition are popular issues in the field of high-resolution Synthetic Aperture Radar (SAR). As a typical target, aircraft detection and identification has certain uniqueness. This paper reviews the development of detection and recognition techniques for a typical target in SAR imagery, analyzes the scattering mechanism and technical difficulties of aircraft in SAR imagery, describes the system flow, technical routes, and key scientific problems of target aircraft detection and recognition in SAR imagery, summarizes the research progress from traditional methods to deep-learning-based methods for aircraft detection and recognition, discusses the characteristics and existing problems of various methods, and predicts the future development trend. This paper proposes that combining target electromagnetic scattering mechanism with deep convolutional neural network to improve the generalization capability of the model is the key to improve SAR detection and recognition performance. Moreover, this paper establishes an aircraft detection method based on the fusion of scattering information and deep convolutional neural network.

Deep learning such as deep neural networks has revolutionized the computer vision area. Deep learning-based algorithms have surpassed conventional algorithms in terms of performance by a significant margin. This paper reviews our works in the application of deep convolutional neural networks to target recognition and terrain classification using the SAR image. A convolutional neural network is employed to automatically extract a hierarchic feature representation from the data, based on which the target recognition and terrain classification can be conducted. Experimental results on the MSTAR benchmark dataset reveal that deep convolutional network could achieve a state-of-the-art classification accuracy of 99% for the 10-class task. For a polarimetric SAR image classification, we propose complex-valued convolutional neural networks for complex SAR images. This algorithm achieved a state-of-the-art accuracy of 95% for the 15-class task on the Flevoland benchmark dataset.

Traditional methods of non-contact human respiratory rate measurement usually require complex devices or algorithms. Aiming at this problem, a non-contact respiratory rate measurement method based on only the RGB video information was proposed in this paper. The method consisted of four steps. Firstly, spatial filtering was applied to each frame of the input video. Secondly, a gray compensation algorithm was used to compensate for the gray level change caused by the environmental light. Thirdly, the gray levels of each pixel over time were filtered separately by a low-pass filter. Finally, the region of interest was determined based on the filtering results, and the respiration rate of the human is measured. The physical measurement experiments were designed, and the measurement accuracy was compared with that of the biological radar. The error of the proposed method was between - 5.5% and 3% in different detection directions. The results show that the non-contact respiration rate measurement method can effectively measure the human respiration rate.

Objective: To inquire into the influence of silencing HMGB1 expression by small interfering RNA (siRNA) on cell growth, proliferation, invasion and metastasis of colorectal cancer LoVo cells both in vitro and in vivo., Methods: Lentivirus-mediated HMGB1 siRNA was transfected into LoVo cells to silence the HMGB1 expression. The HMGB1 mRNA and protein expression after siRNA transfection was detected by RT-PCR and Western blot. MTT assay was used to observe the cell proliferation and to draw a growth curve. Cell cycle was measured by flow cytometry. The ability of invasion and speed of cell migration were evaluated by transwell chamber invasion and cell scratch assay. The influence of HMGB1 silencing on the proliferation of LoVo cells in vivo was observed in LoVo tumor-bearing nude mice., Results: Lentivirus-mediated siRNA was successfully transfected into colorectal cancer cell line LoVo. The expression of HMGB1 mRNA and protein in the HMGB1-siRNA group were 0.24±0.04 and 0.21±0.03, respectively. Compared with the HMGB1-siRNA-Neg group (0.82±0.13, 1.15±0.18) and control group (0.93±0.15, 1.21±0.20), the difference was significant (P<0.05). MTT assay showed that the cell proliferation in the HMGB1-siRNA group was significantly inhibited when compared with that in the HMGB1-siRNA-Neg group and control group (P<0.05). Flow cytometry showed that the proliferation index (PI) of HMGB1-siRNA group was 38.27±1.32, significantly lower than 54.66±1.74 in the HMGB1-siRNA-Neg group and 57.43±1.29 in the control group (P<0.05). The transwell assay showed that the number of penetrated cells in the HMGB1-siRNA group was 14.0±3.5, significantly lower than 51.0±6.7 in the HMGB1-siRNA-Neg group and 68.0±5.3 in the control group (P<0.05). Similarly, the scrape wound recovered significantly slower in the HMGB1-siRNA group (83.61±23.21) µm than that in the other two groups (202.86±46.46) µm and (214.58±57.38) µm(P<0.05). The nude mouse xenograft tumor experiment showed that the final tumor volume was (521±34) mm3 in the HMGB1-siRNA group, significantly smaller than that in the HMGB1-siRNA-Neg group of (763±46) mm3 and control group of (802±51) mm3 (P<0.05)., Conclusions: Lentivirus-mediated HMGBl-siRNA can effectively inhibit the HMGB1 expression in colorectal cancer LoVo cells both in vitro and in vivo. HMGB1 gene silencing can slow the growth of colorectal cancer cells, extend the cell proliferation cycle, decrease their invasion and migration, and significantly inhibit the growth of xenograft tumor in nude mice.

Objective: To investigate the expression level of high mobility group box-1 (HMGB1) in human colorectal cancer and its relation with different clinicopathological characteristics and prognosis of colorectal cancer patients., Methods: Immunohistochemical method was used to detect the HMGB1 expression in tissue samples of 86 colorectal cancer patients and 32 normal colorectal tissue samples. Positive rates of HMGB1 expression were compared among different clinicopathological characteristics. Relation of HMGB1 expression with survival was analyzed., Results: HMGB1 expression was mainly in colorectal cancer cell nucleus, with a few appearance of co-expression in nucleus and cytoplasm. Positive rate of HMGB1 expression in normal tissues was significantly lower than that in colorectal cancers [9.4% (3/32) vs. 66.3% (57/86), P=0.000], and it was much higher in large cancers, lower differentiation, invasion to outside serosa, advanced clinical stage and lymph node metastasis (all P<0.05), but was similar in terms of age and gender (P>0.05). Survival analysis showed that 3-year survival rate of patients with positive HMGB1 expression was significantly lower as compared to those with negative HMGB1 expression (56.1% vs. 85.7%, P=0.021), meanwhile it was significantly lower in patients with co-expression in nucleus and cytoplasm as compared to those with simple nuclear expression (41.4% vs. 75.0%, P=0.013)., Conclusions: HMGB1 expression in colorectal cancer is high, and its positive rate increases with the low differentiation, invasion and metastasis. HMGB1 co-expression in nucleus and cytoplasm indicates poor prognosis of colorectal cancer patients.

Objective: To evaluate the effectiveness of repairing the deltoid ligament with ankle fracture., Methods: Between January 2010 and January 2013, 11 patients with ankle fractures associated with deltoid ligament injury were treated. There were 7 males and 4 females, with an average age of 38.2 years (range, 18-72 years). The interval between injury and operation was 6 hours to 7 days (mean, 4 days). According to Lauge-Hansen classification, ankle fracture was rated as pronation-external rotation type in 5 cases, as supination-external rotation type in 4 cases, and as pronation-abduction type in 2 cases. The MRI and color Doppler ultrasound showed deltoid ligament rupture. The results of valgus stress test, talus valgus tilt test, and anterior drawer test after anesthesia were all positive. Fracture was treated by open reduction and internal fixation, and deltoid injury was repaired., Results: All incisions healed primarily. All patients were followed up 12-18 months (mean, 13.3 months). The X-ray films showed anatomical reduction, good position of internal fixation and stable distal tibiofibular syndesmosis. The mean fracture union time was 7.6 weeks (range, 6-8 weeks). MRI at 3 months after operation showed normal shape of the deltoid ligament. According to American Orthopaedic Foot and Ankle Society (AOFAS) score, the results were excellent in 6 cases, good in 3 cases, fair in 1 case, and poor in 1 case, with an excellent and good rate of 81.8%., Conclusion: It is an effective method to treat ankle fracture with deltoid ligament injury by open reduction and internal fixation of ankle fracture and repair of the deltoid ligament injury, which can effectively rebuild medial instability and has satisfactory effectiveness.

Objective: To objectively evaluate the voice rehabilitation status in different period after treatment of Tis--T1 glottic carcinoma by CO2 laser with voice parameters., Method: A retrospective review of 41 cases with Tis--T1 glottic carcinoma treated by CO2 laser was performed, 23 cases were stage Tis (Tis group) and 18 cases with stage T1N0M0 (T1 group). The range of excision of the lesion by CO2 laser was according to the different stages of the tumor, and ensured theoperation negative margin was by intraoperative frozen pathological examination. We tested and compared the actual voice (coritaine F0, Jitter, Shimmer, NNE and MPT) of 30 cases of healthy middle-aged and old male(normal group) and all the patients at one day prior to operation, three months, six months and one year after operation respectively, which was to evaluate the voice rehabilitation status in different period after operation objectively., Result: Postoperative pathological examination revealed, 23 cases were squamous epithelium severe atypical hyperplasia, 16 cases were well differentiated squamous cell carcinoma, and 2 cases were moderately differentiated squamous cell carcinoma. Palatoglossal arch mucosal tear occurred in 3 patients. Respiratory difficulties were not seen in all cases, and normal oral feeding was obtained in all cases in postoperative three days. All patients were followed up for one year. There was statistical significance in F0, Jitter, Shimmer of both Tis group and T1 group after operation in different periods(P<0.05). But there was no statistical significance in NNE and MPT between six months and one years after operation in the two groups(P>0.05)., Conclusion: CO2 laser surgery is an effective treatment for early glottic carcinoma. Postoperative vocal function was improved in varying degrees, and voice quality gradually improved with the rehabilitation time. Partly objective parameters reflecting the vocal function gradually stabilized after half a year after operation.

Objective: To explore the application of polysomnography (PSG) and lateral cephalometric radiographs (LCR) in the diagnosis and treatment of sleep-disordered breathing (SDB) in children., Method: To select 157 cases of children suffering from SDB, 115 cases with primary snoring (PS) and 42 cases with obstructive sleep apnea hypopnea syndrome (OSAHS). After bilateral tonsillectomy and adenoidectomy, preoperative and postoperative LCR and PSG of measure were observed for statistical analysis., Result: Compared the group PS and group OSAHS; the thickness of adenoids was significantly different(P<0. 01). The preoperative oblique diameter of nasopharyngeal airway was significantly different (P<0. 05). There were significant differences on the preoperative and postoperative nasopharyngeal airway oblique diameter and oropharyngeal airway anteroposterior diameter in group PS and group OSAHS (all, P<0. 01). There were significant differences on the preoperative and postoperative AHI and LSaO2 in the group OSAHS (both, P<0. 01). The total effective rate of group PS was 97. 4% (112/115), and the total effective rate of group OSAHS was 97. 6% (41/42)., Conclusion: PSG and LCR have a very important role in the diagnosis and treatment of SDB in children. Bilateral tonsillectomy and adenoidectomy are the primary means of treatment in children with SDB.

Objective: To objectively evaluate the effectiveness of the ventricular fold pull-down combined with strip myofascial flap to repair laryngeal defect after early glottic carcinoma operation with glottic morphological parameters and voice parameters., Methods: Between January 2008 and December 2012, 47 patients with early glottic carcinoma and anterior commissure involvement underwent partial laryngectomy. All patients were male, aged from 60 to 75 years (mean, 68.5 years). The disease duration was 4-11 months (mean, 7.2 months). According to American Joint Committee on Cancer (AJCC) TNM criteria, 28 cases were classified as T1aN0M0, 14 cases as T1bN0M0, and 5 cases as T2NoMo. Laryngeal defect after resection of tumor was repaired by ventricular fold pull-down combined with strip myofascial flap. At 1 day before operation and at 1 year after operation, multilayer spiral CT was used to scan larynx, to measure and compare the anteroposterior diameter of vocal area, the distance between both sides of the vocal process, and the thickness of soft tissue of vocal area, and the effect of combined soft tissue flap was objectively assessed in laryngeal morphology reconstruction. The actual voice parameters [including Fo, Jitter, Shimmer, normalized noise energy (NNE), and maximum phonatory time (MPT)] were tested and compared, and the effect of the combined soft tissue flap on postoperative laryngeal pronunciation was evaluated., Results: Postoperative pathological examination revealed well-differentiated squamous cell carcinoma in 38 cases, and moderately-differentiated squamous cell carcinoma in 9 cases; no tumor was found in the resection margin. Healing of neck incision was obtained in all patients at 7-9 days after operation. Forty-four cases were decannulated at 9-11 days after operation and the remaining 3 cases were decannulated at 3 weeks after operation. Oral feeding usually started in all cases at 3-4 days after operation. All patients were followed up 1 year. At 1 year after operation, the anteroposterior diameter of vocal area was significantly reduced when compared with preoperative one (t = 15.161, P = 0.000); the distance between both sides of the vocal process and the thickness of soft tissue of vocal area had no significant changes (P > 0.05). Compared with preoperative ones, there were significant differences in Shimmer, NNE, and MPT (P < 0.05), but no significant difference was found in Fo and Jitter (P > 0.05) at 1 year after operation., Conclusion: Ventricular fold pull-down combined with strip myofascial flap can repair laryngeal defect effectively after partial laryngectomy and maintain the effective airway after operation. It not only has no effect on postoperative laryngeal morphology, but also can be used as new laryngeal voice vibration body.

Liquid chromatography-mass spectrometry (LC-MS) is the mainstream of high throughput protein identification technology. Peptide retention time in reversed-phase liquid chromatography (RPLC) is mainly determined by the physicochemical properties of the peptide and the LC conditions (stationary phase and mobile phase). Retention time can be predicted by analyzing these properties and quantifying their effects on peptide chromatographic behavior. Prediction of peptide retention time in LC can be used to improve identification of peptides and post translational modifications (PTM). There are mainly two methods to predict retention time: i.e., retention coefficients and machine learning. The coefficient of determination between observed and predicted retention times can reach 0.93. With the development of LC-MS technology, retention time prediction will become an important tool to facilitate protein identification.

Objective: To investigate the epidemiological and clinical characteristics of breast cancer associated with multiple primary malignant neoplasmas (MPMNs)., Methods: The data of 519 patients of breast cancer associated with MPMNs admitted to the Cancer Hospital, CAMS and diagnosed by operation and pathology in the period of 1958 to 2001 were studied retrospectively to analyze the morbity, age of onset, sex ratio, predilection site of tumor, and the interval time between sequential tumors. The 519 patients, all female, were divided into four groups: bilateral primary breast cancer (1st group), synchronic Paget's disease (PD) associated with breast cancer in the same breast (2nd group), breast cancer as the first tumor associated with MPMNs (3rd group), and breast cancer as the second or third tumour associated with MPMNs (4th group)., Results: The constituent rate of bilateral primary breast cancer, synchronic Paget's disease (PD) associated with breast cancer in the same breast, breast cancer as the first tumor associated with MPMNs, and breast cancer as the second or third primary tumour associated with MPMNs was 3.0%, 0.8%, 3.2%, and 2.5% respectively. The median age of onset was 42.5, 47.3, 51.4, and 51.8 years respectively for the four groups. The mean interval time between the appearance of the first tumor and the appearance of the second tumor was 5.4, 0, 8.6, and 7.6 years in the four groups respectively. The predilection sites of second and third primary cancers in the third group were lung, breast, esophageal, ovary, and large intestine. The predilection sites of first primary cancer in the 4th group were uterus, ovary, large intestine, lung, lymphatic tissue, and esophagus successively. The involvement rate of breast in breast cancer associated with MPMNs was 72.4%., Conclusion: (1) Breast is one of the predilection organs of MPMNs. (2) The predilection organs of breast associated with MPMNs are the target organs of female hormone (eg, breast, ovary and uterus) and the organs involved by radiotherapy for breast cancer (eg, lung and esophagus). (3) Early diagnosis and timely and proper treatment have satisfactory effect.
