Background Physical and medical integration is essential for promoting the health of community residents and is an important guarantee for the high-quality development of community medical services, and practitioners are the foothold and focus of physical medicine integration services. However, there is currently a lack of relevant research on the impact of community medical institutions in carrying out physical medicine integration services from the perspective of practitioners. Objective To study the understanding and views of practitioners on how to carry out the integrated service of physical medicine in the community to provide ideas and references for the community to better carry out the integrated service of physical medicine. Methods In March and April 2023, 11 practitioners from 5 community health service centers in Zhuzhou city, Hunan Province, were selected as survey subjects by means of an objective sampling method. The descriptive research method was adopted to conduct "one-on-one" semistructured in-depth interviews with community practitioners. With the help of the content analysis software Nvivo 12.0, text transcription recording, analysis unit formation, content coding and theme extraction were carried out for the interview content. Finally, the interview data were logically analyzed using descriptive phenomenological analysis to capture GPs' understanding and perceptions of the influences on the development of body-health integration services in the community, and to summarize the themes of the interviews. Results The factors affecting the development of physical and medical integration services in communities can be summarized into 4 themes and 10 subthemes. There was an urgent need to optimize the service environment for the integration of physical medicine (insufficient site supply, lagging atmosphere, and insufficient publicity) , improve the ability of practitioners (insufficient cognitive ability for the integration of physical medicine and the ability to issue sports prescriptions) , lack of support in community hospitals (lack of training activities related to the integration of physical medicine, insufficient fund allocation and shortage of human resources) , and lack of patient cognition of the integration of physical and medicine (patients have poor cognition of the efficacy of body-medicine integration, and patients have poor cognition of the risk of body-medicine integration) . Conclusion There are still many shortcomings in the service ability of community practitioners. To promote better development of community integrated services, it is urgent to optimize the service environment of community integrated services, to improve practitioners' integrative cognition ability and exercise prescription ability, to strengthen the support and guarantee of community health service institutions, and to improve patients' awareness of the efficacy and risk of physical and medical integration services.

Exercise has been confirmed by most studies to be effective in preventing and treating depression, and has become a recommended treatment by many researchers. However, previous research results on exercise interventions for depression have not been effectively integrated, and there is still a lack of uniform clinical exercise therapy guidelines across countries. In this paper, we systematically and comprehensively discussed the effects of exercise on depression, including the effects of exercise types, intensities, frequencies, and amounts of exercise on the efficacy of depression interventions in different populations, and summarized the neurobiological mechanisms involved in the development of depression as well as the antidepressant effects of exercise by sorting out relevant literature. This paper shows that in exercise interventions for depression, aerobic exercise is the most frequently chosen type of exercise, the intensity of which is usually moderate to high intensity, and the frequency and amount of exercise are recommended at high frequency and dosage. The neurobiological mechanisms of exercise resistance to depression are mainly that exercise can improve the morphological structure of central nervous system tissue, increase the levels of a series of neurotrophic factors, thereby enhancing neuronal plasticity, and improve the function of the neurosecretory system, reducing damage to brain tissue caused by neuroinflammatory responses and oxidative stress. This paper can provide a certain reference for the development and implementation of clinical exercise prescription for depression in China, and draw on the in-depth development of exercise antidepressant research.

This study analyzed the regional climate variations in heavy rainfall across South China by using the IMERG precipitation data during 2001--2020. The research focused on the spatiotemporal distribution characteristics of heavy rainfall and the diurnal variations of extreme heavy rainfall before, during, and after the onset of the South China Sea summer monsoon. The results showed that: (1) The rainfall center in South China was influenced by diverse weather systems at different times of the year. The ratios of rainfall during the two rainy seasons to the annual precipitation were inversely distributed, and the proportion of heavy rainfall along the coast of Guangdong and Guangxi was significantly higher than those of other regions. Heavy rainfall contributed the most to the overall precipitation in the rainfall center of South China. (2) The coastal areas of Guangdong experienced two peaks of heavy rainfall in June and August, while Hainan Island observed a single peak in October. Other regions of Guangdong and Guangxi exhibited a single peak in June, which was closely related to the onset of the South China Sea summer monsoon. During the onset of the summer monsoon, the strong summer monsoon intensified the heavy rainfall in Zone I (Beibu Gulf), Zone II (area near Yangjiang), and Zone III (Pearl River Delta) along the coastal land areas of Guangdong and Guangxi. In the second rainy season, the summer monsoon continued to contribute to increased heavy rainfall across the entire coastal area and Hainan Island. In early June, Zone III and Zone IV (Shanwei and its eastern region) experienced a high frequency of heavy rainfall, primarily due to warm-sector heavy rainfall induced by the southwest summer monsoon. (3) Hainan Island exhibited the highest occurrence of extreme heavy rainfall in South China, predominantly in the afternoon. Guangdong followed with a lower occurrence, while Guangxi experienced the lowest occurrence. The diurnal variation of extreme heavy rainfall was most pronounced in the morning along the coastal area of Guangdong and Guangxi, especially from early morning to forenoon (5--11 am), which corresponded to the peak period in Zone II and Zone III. BACKGROUND: At present, there are relatively few studies on the closed reduction of intramedullary nail treatment for femoral shaft fractures. OBJECTIVE: To investigate the effect of closed reduction and intramedullary nailing in the treatment of femoral shaft fractures in the lateral position. METHODS: From January 2015 to October 2018, 54 patients with femoral shaft fractures were treated with intramedullary nailing at Department of Orthopedics and Traumatology, the Third Affiliated Hospital of Zunyi Medical University. The patients were randomly assigned to three groups, including 17 cases in the supine mechanical traction group, 17 cases in the lateral artificial traction group, and 20 cases in the lateral mechanical traction group. All patients signed the informed consent. This study was approved by the Hospital Ethics Committee. Closed reduction success rate, operation time, intraoperative blood loss, fracture healing time, and hospital for special surgery knee score at 6 months after surgery were com pared in the three groups. RESULTS AND CONCLUSION: (1) All patients were followed up for 6 to 15 months. (2) The success rate of fracture closure was 100% in the lateral mechanical traction group, 82% in the lateral artificial traction group, and 59% in the supine mechanical traction group. There was no significant difference between supine mechanical traction group and lateral artificial traction group (P > 0.05). The success rate was significantly higher in the lateral mechanical traction group than in the supine mechanical traction group (P=0.002). There was no significant difference between the lateral mechanical traction group and the lateral artificial traction group {P > 0.05). (3) No significant difference was found in operation time between the supine mechanical traction group and the lateral artificial traction group (P > 0.05). The operation time was significantly shorter in the lateral mechanical traction group than in the supine mechanical traction group and lateral artificial traction group (P < 0.05). (4) Intraoperative blood loss was significantly more in the supine mechanical traction group than in the lateral artificial traction group (P=0.02) and lateral mechanical traction group (P=0.001 ). No significant difference was determined in intraoperative blood loss between the lateral artificial traction group and lateral mechanical traction group (P > 0.05). (5) Fracture healing time was significantly longer in the supine mechanical traction group than in the lateral artificial traction group (P=0.030) and lateral mechanical traction group (P < 0.001 ). There was no significant difference in fracture healing time between the lateral artificial traction group and the lateral mechanical traction group (P > 0.05). (6) No significant difference in hospital for special surgery knee score at 6 months after surgery was detected among the three groups (P > 0.05). (7) These results suggested that application of lateral recumbent traction frame closure and intramedullary nailing for the treatment of femoral shaft fractures has the advantages of high success rate, short operation time, less intraoperative blood loss and short fracture healing time. The water ecological carrying capacity status and change trend of Yuehai Lake were studied, which would provide a basis for water quality improvement and self-repair ability. Water and aquatic biological samples were collected in winter (January), spring (April), summer (July), autumn (October) of 2015-2017 from the Lake. On the basis of 10 water quality indicators such as CODMn aquatic organisms density and biodiversity index, the water ecological carrying capacity evaluation index system was constructed. By the method of analytic hierarchy process, the hierarchical structure model was established to evaluate the water ecological carrying capacity of the Lake from the perspectives of water ecological support and water environmental pressure. Results indicated that the variation rule of the pressure index, load index and water ecological carrying capacity index of the Lake were the same, with significant seasonal changes, which were the largest in summer and the smallest in winter. The Lake has a high pressure and low load state in summer and a low pressure and weak load state in winter. The ecological carrying capacity indexes of the Lake was 0.32, 0.34 and 0.33 respectively from 2015 to 2017. The overall ecological carrying capacity of the Lake was weak, and the ability of the ecosystem to resist external interference and self-recovery needed to be improved. Nitrogen was the most important index affecting the ecological carrying capacity, so nitrogen and other nutrients should be reduced from the source to enter the Lake. [ABSTRACT FROM AUTHOR]

Objective: To determine expression levels of ubiquitin C-terminal hydrolase-L1 (UCH-L1) and serum glial fibrillary acidic protein (GFAP) in patients with acute cerebral infarction and their clinical significance.
 Methods: A total of 80 patients with acute cerebral infarction in Chongqing Cancer Hospital from January 2014 to February 2016 were enrolled as an observation group. Another 80 healthy people served as a control group. The expression levels of UCH-L1 and GFAP in the 2 groups were detected.
 Results: Sensibility and specificity for UCH-L1 and GFAP were 75.0%, 87.5% and 81.3%, 90.0%, respectively. Receiver operating characteristic curve areas of UCH-L1 and GFAP were 0.670 and 0.757, respectively. There were no significant significance in age, gender, drinking, smoke, diabetes, and hyperlipidemia in the 2 groups (P>0.05). High blood pressure rate in the observation group was higher than that in the control group (P<0.05). Spearson/Pearson analysis showed that serum UCH-L1 and GFAP levels were positively correlated with hypertension, but they were negatively correlated with sex, age, diabetes, hyperlipidemia, alcohol consumption, smoking, and other factors. General data at different time in the observation group was not statistically different (P>0.05). The expression levels of UCH-L1 and GFAP in the observation group was higher than that in the control group (P<0.05). UCH-L1 and GFAP levels at different time in the 2 groups were not statistically different (P>0.05). UCH-L1 and GFAP levels in the light, medium, and heavy groups were higher than those in the control group (P<0.05), while UCH-L1 and GFAP levels in the medium and heavy groups were higher than those in the light group (P<0.05). There was significant difference between levels of UCH-L1 or GFAP and infarction size at different time in the observation group (P<0.05). The results of Pearson correlation analysis showed that the levels of serum UCH-L1 and GFAP were positively correlated (r=0.634, P=0.001).
 Conclusion: The levels of serum UCH-L1 and GFAP are significantly increased at the early stage of acute cerebral infarction, and they have a certain correlation with the severity of cerebral infarction, which can provide a basis for early clinical diagnosis and treatment.
