Objective To evaluate the value of PLT and its parameters combined with AFP, CA199, CA125 and CEA on the preoperative differential diagnosis of hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (ICC). Methods We analyzed retrospectively 274 patients with liver cancer who underwent surgery in the Second Hospital of Lanzhou University. They were divided into 229 cases in HCC group and 45 cases in ICC group according to postoperative pathological results. The differences of PLT, its parameters and tumor markers between the two groups were compared. ROC curve was used to evaluate the differential diagnosis effect on HCC and ICC by significantly different indicators in single and combined forms. The best scheme was verified in the patients with determined and undetermined preoperative diagnosis. Results Compared with HCC group, the levels of PLT, PCT, CA199 and CA125 in ICC group were higher (P < 0.05) and the level of AFP was lower (P < 0.05). The diagnostic analysis results of ROC curve showed that in single test, the AUC of AFP for HCC diagnosis was the largest (0.827). The AUC of the combined groups was higher than the single groups of tumor markers; the AUC of the PCT+AFP+CA199+CA125 group was the highest in all combination groups, and AUC was 0.891. The verification of the best combination group showed that the AUC was 0.924 in the preoperative determined diagnosis group and 0.854 in the undetermined diagnosis group. Conclusion Tumor markers in combination with PLT and PCT can increase the preoperative differential diagnosis efficacy of HCC and ICC. The combination of PCT, AFP, CA199 and CA125 before operation is helpful to further determine the diagnosis and plan the operation scheme.

Generative adversarial networks (GAN) has become a popular research direction in the field of deep learning. The unique adversarial idea of GAN comes from the two-player zero-sum game in game theory. How to solve the problems of unstable GAN training, poor quality of generated samples, inadequate evaluation system, poor interpretability and other issues is critical and difficult in current GAN research. This paper investigates the research background and development trend of GAN. First, the basic idea and algorithm implementation of GAN are described, the advantages and disadvantages of GAN are analyzed, a more systematic classification of existing improved methods is made, and some typical optimization methods and derivative models of GAN are sorted out from two categories based on structure change and loss function variants respectively. Next, the similarities and differences between GAN and other generative models are compared, and their respective advantages and disadvantages are introduced. Then, this paper compares the performance of GAN and its derivative models, and summarizes their operation mechanism, advantages, limitations and applicable scenarios. The applications of GAN in the field of image generation are introduced. Finally, the mainstream evaluation indicators of GAN are listed, the main problems still faced in GAN research are analyzed and corresponding solutions are given. The listed mainstream solutions are compared and analyzed in terms of solution effects and applicability, and the future research direction is prospected.

This paper reviewed the clinical data of 15 children undergoing allogeneic hematopoietic stem cell transplantation (HSCT) for Fanconi anemia. All the children received comprehensive nursing measures including overall environmental protection, infection prevention, safety protection, psychological counseling and health education based on their disease characteristics and disease progression. Allogeneic hematopoietic stem cell transplantation was successfully performed in 15 children with Fankone anemia. After transplantation, the mean time of neutrophil engraftment was (11. 67±1. 67) d, time of transferred out of laminar flow room was (13. 33±1. 49) d. To improve the performance of adjusting performance for wall-press oil and gas in-pipe robot,part of the in-pipe robot adjusting mechanism is parametric modeled and optimization designed through ADAMS. The integrated adjusting mechanism model is set up by Solid Works according to the optimized data and the structure characteristics of adjusting mechanism for in-pipe robot,then imports the model and makes simulation analysis in ADAMS. The optimized nut horizontal thrust is reduced 41. 5%,besides,the impact load which acts on adjusting mechanism of in-pipe robot is simulated,the influence of the horizontal thrust and changes in the situation of compression spring when the adjusting mechanism is in the process of adjusting or suffering impact load is separately obtained,the theoretical basis for the selection of suitable motor and the compression spring is provided.

With the in-depth implementation of rural revitalization strategy, the protection and utilization of traditional villages have been an important topic of increasing concern in the academic community. From the perspective of daily life, this paper explores the complex interface of traditional village buildings, excavates its regional characteristics and humanistic connotation, and explores the local activation and transformation of the building composite interface. According to different interface types, this paper constructs the transformation principles of being-emerged-in-site, shaping-space-with-sprit and responding-to-culture, and proposes the transformation strategies of "shape", "feeling" and "structure" of the building composite interface. With the case of the transformation of traditional Chinese medicine clinics in Zhoutie traditional villages (community) in Yixing, in order to enhance the vitality of the streets of traditional villages and realize the active protection and utilization of traditional villages, different types of building composite interfaces are taken as the operating elements, and three transformation strategies are proposed according to different types, namely, breaking the boundary, establishing the boundary and extending the boundary. Non-terrestrial network (NTN) in 5G system is expected to provide communication services via near-earth space access and satellite access, which means 5G network has expanded its service area from land to space. Standardization status and key technolgies of 5G 3GPP Release 17 NTN were presented, including key differences compared 5G TN, especially in network architecture, time and frequency synchronization, HARQ, and mobility management etc. Moreover, the main candidate techniques in 3GPP Release 18 NTN and beyond were explored, aiming to outline a vision for NTN evolution. Based on the analysis of 5G NTN and its evolution, it was observed that the integration of TN and NTN would be an obvious technique trend, which paved a way to a unified system applied for space, land and sea. MIMO (multi-input multi-output) OTA (over-the-air) testing is an important method to evaluate the radiation performance of the antenna system, as well as an indispensable step of research & development (R&D) and mass production stages. With the advent of the 5G mobile communication system, the introduction of millimeter wave (mmWave) and other new technologies brings new challenges to the traditional MIMO OTA test method, and meanwhile makes OTA testing the only feasible solution for testing future 5G devices. Firstly, the challenges faced by 5G OTA testing was described. Then the applicability of the 4G mobile communication system OTA test method to the 5G OTA performance test was analyzed, and how to extend the low-frequency test method to the mmWave terminal test was studied. Then the current status of 3GPP's research on MIMO OTA testing was summarized, and the system model, test principle, and performance evaluation indexes of simple-sectored multi-probe anechoic chamber (SS-MPAC) were explained in detail. A rapid reentry trajectory planning method based on nonlinear model prediction theory is studied for the new lifting reentry vehicle with many constraints. Since the lifting reentry characters and mission requires need to satisfy strong terminal constraints, the nonlinear predictive model and the receding optimization scheme are used to convert the nonlinear optimization into the linear optimization objection, and a quadratic programming is used to solve it. The simulation results show that the proposed method can generate a high precision reentry trajectory with strong terminal constraints. [ABSTRACT FROM AUTHOR]

Objective: To explore the occupational health risk level of silicosis caused by silica dust exposure in non-ferrous metal mining enterprises., Methods: Using typical sampling method, 44 non-ferrous metal mining enterprises were selected in seven provinces.37 non-ferrous metal mining enterprises were included, and most of them were underground enterprises(97.30%). Enterprises basic information and silica dust exposure data of key positions were collected by using on-site hygiene investigation and detecting method. Finally, the International Council on Mining and Metals&apos;(ICMM) risk rating table method and occupational hazard risk index method(INDEX) were used to analyze the occupational health risk level of silicosis caused by silica dust exposure in non-ferrous metal mines from total and respirable dust views, respectively. Meanwhile, the square weighted Kappa test was performed to analyze the consistency between two risk assessment method., Results: Medium enterprises(64.69%(40.38%, 73.41%)), silver mining enterprises(84.69%(63.38%, 86.06%)) and antimony mining enterprises(72.22%) had relatively higher silica dust exposure rates. On-site hygiene detecting result showed that:(1) M(P25, P75) of free silica content was 21.18%(17.03%, 30.47%). (2) 1.60(0.86, 2.46)mg/m~3 for total dust concentration, 64.47% total dust concentration samples exceeding Chinese permissible concentration-time weighted average(PC-TWA), 0.68(0.30, 1.18) mg/m~3 for respiratory dust concentration, 50.00% respirable dust concentration samples exceeding Chinese PC-TWA. (3) Medium-sized enterprises, as well as rock drillers and crushers, had higher levels of silica dust exposure. Occupational health risk assessment result showed that:(1) The overall occupational health risk level of silicosis caused by silica dust exposure in non-ferrous metal mining enterprises was medium:(1) In term of total dust, ICMM risk rating table method and INDEX method indicated high and medium risks, respectively(S&#95;(weighted) were 3.52 and 2.79). (2) In term of respirable dust, both ICMM risk rating table and INDEX method indicated medium risks(S&#95;(weighted) were 2.78 and 2.35). (2) Medium-sized enterprises risk level was higher than other two production scales enterprises. ICMM risk rating table method and the INDEX method consistency analyses showed that these two risk assessment method had strong consistencies in terms of total dust or respirable dust(both Kappa values ≥ 0.600)., Conclusion: The overall occupational health risk level of silicosis caused by silica dust exposure in non-ferrous metal mining enterprises was medium, and the risk levels of medium production-scale mining enterprises, rock driller and crusher were higher.

Cochlear implant surgery is now recognized as the most effective intervention for patients with severe and profound sensorineural hearing loss. Due to various factors such as high altitude, low oxygen content, and ethnic differences in Tibet, the early hearing screening found that the incidence of neonatal hearing loss was significantly higher than in the mainland. With the great assistance of the China Disabled Persons' Federation, the first cochlear implant surgery was carried out in Tibet in 2011, and a total of 116 cases have been completed. Objective: To summarize the diagnosis and treatment of iatrogenic colonoscopic perforation (ICP)., Methods: Clinical data, treatment course and outcome of 17 patients who developed ICP following colonoscopic examination or operation at Department of Colorectal Surgery, Changhai Hospital from January 2000 to December 2013 were retrospectively analyzed., Results: During above 13 years, a total of 127 106 patients underwent colonoscopic examination or operation, of whom 17 cases (0.013%) had ICP. There were 8 males and 9 females with an average age of 65.2 (32-85) years. The interval between the onset of ICP and clinically diagnosed ICP was 0 to 6 days after performance. ICP occurred in 8 patients following colonoscopy operations, including simple colonic polyp excision, endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD), while in 9 patients following simple colonoscopy examination. Except for one patient who was immediately diagnosed with ICP through the finding of "yellow adipose tissue visible in the vision field" during operation,7 early cases (41.2%) were diagnosed by abdominal X-ray examination, and 9 later cases were confirmed by abdominal CT examination. The perforation sites included sigmoid colon in 5 cases, caecum in 3 cases, descending colon in 3 cases, descending and sigmoid junction in 2 cases, ileum in 1 case, splenic flexure in 1 case, sigmoid and rectum junction in 1 case, retum in 1 case. One case with ICP following ESD after resection of polyp in caecum was cured successfully with conservative treatment, including fasting, gastrointestinal decompression, fluid infusion, anti-infection and nutritional support. One case with ICP, which was found during colonoscopic operation and the perforation was immediately closed with titanium clip, received conservative treatment, including anti-infection and then was cured. Fifteen patients underwent surgery, including 8 patients with primary intention intestinal perforation repair, 4 patients with primary intention resection of associated intestine and anastomosis, 2 patients with primary intention resection of associated intestine and ostomy, 1 patient with primary intention intestinal perforation repair and ostomy. Postoperative abdominal incision infection occurred in 4 cases, pulmonary infection in 1 case, incision infection with cardiovascular event or urinary tract infection in 1 case each. All the patients were cured and discharged. Average hospital stay was 18.6(3-45) days., Conclusions: ICP should be diagnosed by physical examination and imaging examination as soon as possible. For perforation during colonoscopic performance, colonoscopic titanium clip can be used for closure. Perforation repair is still the main procedure for ICP. If necessary, partial intestinal resection and anastomosis or ostomy can be selected.

Objective: To compare the surgical safety and short-term efficacy of minilaparotomy and laparoscopic approach for curative resection of rectal cancer., Methods: The retrospective cohort study was adopted. A review of patients scheduled to undergo a curative resection of rectal cancer via minilaparotomy or laparoscopic approach at Department of Colorectal Surgery of Changhai Hospital from June 2016 to May 2017 was carried out. All the patients were confirmed as rectal cancer by postoperative pathology. The following patients were excluded from the study: patients who had acute complete obstruction or perforation; patients underwent Miles or Hartmann procedure; patients who required an elongation of the skin incision in minilaparotomy or a conversion from laparoscopic to open surgery. Finally, 216 patients were enrolled in this study, of whom 143 were performed with minilaparotomy approach (minilaparotomy group) and 73 with laparoscopic approach (laparoscopic group) for curative resection of rectal cancer. For the minilaparotomy technique, a 7 cm longitudinal midline incision was made between the pubic symphysis and umbilicus; a wound retractor was applied to the edge of the wound; lymph node dissection around the inferior mesenteric and artery high ligation of inferior mesenteric artery were performed; by moving the minilaparotomy wound laterally and caudad or cephalad with the S-shaped hook, cautious mobilization of the relevant segment of the bowel loop was performed; bowel anastomosis was achieved by using the double-stapled technique; the gap of the pelvic floor peritoneum and mesentery were routinely closed by the absorbable surgical suture in cases with middle and low position rectal cancer. The surgical safety, the condition of resuming and the morbidity of postoperative complication were compared between the two groups., Results: There were 145 men and 71 women. Age ranged from 26 to 87 years, with of mean age of 61 years. According to the TNM stage grouping, there were 61 patients with stage I(, 62 with stage II(, 85 with stage III(, and 8 with stage IIII( disease, respectively. These two groups did not differ significantly in terms of age, sex, body mass index, site of tumor, TNM stage(all P>0.05). All the patients completed the operation successfully. The median operation time of minilaparotomy group was significantly shorter than that of laparoscopic group [164(80-296) minutes vs. 230(90-665) minutes, Z=4.410, P=0.000]. The intraoperative medical consumable expense [11000(7000-22000) yuan vs. 23000(12000-47000) yuan, Z=11.759, P=0.000] and the total hospitalization expense [44000(22000-146000) yuan vs. 57000(45000-126000) yuan, Z=9.637, P=0.000] were significantly lower in the minilaparotomy group. There were no significant differences between the two groups in terms of operative blood loss, number of harvested lymph nodes, distance of distal resection margin, positive rate of circumferential resection margin (all P>0.05). The rate of postoperative complication in minilaparotomy group was 7.0%(10/143) and in laparoscopic group was 9.6%(7/73) without significant difference (χ 2 =0.449, P=0.503). There were 2 patients in each group who required readmission to the hospital within postoperative 30 days. The cause of readmission was ileus or acute hyponatremia in minilaparotomy group, and ileus or pevic infection in laparoscopic group. One patient died of brain death caused by acute pulmonary embolism during the perioperative period in minilaparotomy group., Conclusions: The minilaparotomy approach for curative resection of rectal cancer is safe and feasible. As compared with laparoscopic approach, it is advantageous to achieve minimal invasiveness and early recovery, but much cheaper and less time consuming.

The purpose of this study is to investigate the change of the whole brain event-related potentials(P300) in normal brain aging based on N-back cognitive tasks. The P300 of 15 normal young people and 10 normal old people were evaluated based on N-back cognitive tasks and analyzed. The results showed that the P300 latency of old people was longer in whole brain than young people, and amplitude was increased in the frontal-central region, while significantly increased in the pre-frontal region in the same load cognitive tasks. With the cognitive task load increasing, the amplitude of old people in high-load task was higher in the whole brain than that in low-load task, mainly in in the frontal region, but the difference was not statistically significant. The latency in the high-load task was shorter in the frontal-central region of right brain than the low-load task, and the difference was statistically significant. Thus, P300 showed that the normal brain aging process is mainly reflected in the pre-frontal region, and the high-load cognitive task could better reflect the change of brain function compared with the low-load cognitive task. The finding is of revelatory meaning for diagnosis of early dementia in patients.

High performance liquid chromatography (HPLC) has been widely considered as the most effective way for the separation and preparation of optically pure enantiomers. In the resolution by HPLC, the separation ability of a column strongly depends on the properties of a chiral stationary phase (CSP). Among many CSPs, the immobilized CSPs, which are becoming one of the most important kinds of CSPs, have the advantages of good solvent durability and enormous method flexibility. In this work, a novel optically active acrylamide derivative (S)-APACP was synthesized by two-step reactions, and its chemical structure was characterized by 1H NMR. The polyacrylamide derivatives were immobilized on silica gel by three-step reactions to prepare immobilized CSPs, and the immobilization amount of the polymers was tested by thermogravimetric analysis. The chiral recognition ability of the immobilized CSPs was evaluated by HPLC, and the effects of Lewis acid and mobile phase on the chiral recognition ability were investigated. The results showed that APACP polymer was successfully immobilized on silica gel to prepare immobilized-type CSP with better solvent durability, and the amount of immobilized polymer was 10. 2% to 11. 8%. The immobilized-type CSP showed good chiral recognition ability for several enantiomers.

Objective: To investigate the effects of different processing methods and durations of blood specimens on the level of soluble CD100 (sCD100) in peripheral blood., Methods: We included a total of 15 healthy individuals without any significant signs or symptoms of microbial infection. Blood samples were collected in tubes with and without additives, as well as tubes containing citrate, EDTA and heparin, respectively, and processed for different durations of time. The serum samples were allowed to clot for 1 hour, 4, 8 hours after venipuncture before centrifugation. Then the sCD100 levels were measured with ELISA., Results: Serum sCD100 levels were higher than those in plasma, and no significant differences were observed among the citrate, EDTA or heparin treated plasma groups. The sera in the tubes without additives showed lower levels of sCD100 than those in the tubes with additives. The concentrations of sCD100 in sera increased with the prolonged processing time in room temperature., Conclusion: The measurement of sCD100 in blood samples may be interfered by the in vitro release from platelets. Thus, pre-treating methods for the collection of serum or plasma samples are critical in the assessment of sCD100 concentrations and should be carefully considered for the measurement of this important biomarker in both basic and clinical studies.

Objective: To explore the application of minimally invasive percutaneous plate osteosynthesis (MIPPO) the clinical curative effect of treatment of fractures of the distal tibia., Methods: From 2010 June to 2014 June, the application of MIPPO technology combined with LCP (low bend medial distal tibial plates) for treatment of 67 cases of distal tibial fracture patients, to evaluate the healing of fracture, function etc., Results: 67 patients with effective follow-up, time was 6-20 months, mean 15 months. The incision healed in one stage, 3 weeks. The effect of 12-16 weeks to fracture healing standard weight, nomalunion and nonunion cases, no fracture, bone plate and screw loosening, exit and other phenomena. The postoperative function was evaluated according to Johner-Wruch criteria, excellent in 45 cases, good in 16 cases, 6 cases, poor in 0 cases., Conclusion: MIPPO technique combined with LCP (low bend medial distal tibial plates) in treatment of distal tibial fractures in accord with biological osteosynthesis requirements, is a recommendable method to treat fracture extension.

Objective: To evaluate the long-term outcomes of pylorus-vagus-preserving partial gastrectomy for early gastric cancer in middle third of stomach., Methods: Between January 2004 and June 2009, 46 patients with early gastric cancer in middle third of stomach underwent pylorus-vagus-preserving partial gastrectomy (PPG) while another 85 patients had conventional distal gastrectomy (DG). Clinicopathologic data and follow-up results of two groups were analyzed retrospectively, including the results of subjective nutritional assessments, laboratory blood biochemical data, endoscopic findings of remnant stomach and total 5-year survival rates., Results: Postprandial dumping syndrome occurred in 7 patients (8.2%) in DG group while no syndrome occurred in PPG group. The incidence of gallbladder stones at 18 months after operation in DG group was higher than that in PPG group. Significant difference existed between two groups (P<0.05). Even though no significant difference existed in laboratory blood biochemical data and endoscopic findings, PPG group recovered better and regurgitation was frequently found in DG group. Food residue in gastric remnant was frequently observed in PPG (31.1%) than in DG (10.8%, P<0.05) by endoscopic findings. At 2 years post-operation, the postoperative 5-year recurrence rate was 6.5% (2/46) in PPG group versus 8.2% (7/85) in DG group. However no significant difference existed between 2 groups (P=0.724). No significant difference existed between PPG group (91.3%) and DG group (90.6%) in overall 5-year survival rate., Conclusion: For early gastric cancer in middle third of stomach, pylorus-vagus-preserving partial gastrectomy is effective in maintaining postoperative function. And it has the same postoperative survival rate as conventional distal gastrectomy.

Objective: To investigate the role of NF-κB/survivin signal pathway in the intima hyperplasia of rat carotid balloon injury restenosis model., Methods: NF-κB siRNA lentivirus vector (titer was 1 × 10⁸ TU/ml) was established. Carotid balloon injury restenosis model was made in 33 SD rats. The rats were divided into 4 groups according to different processing methods, including negative control (NC) group (n = 11), NF-κB siRNA group (n =11), NF-κB siRNA+YM155 (survivin inhibitor) (n = 11), the uninjured carotid artery served as the normal control group (n = 11). After 7 days, the carotid sample (n = 5 each group) were harvested to detect the NF-κB and survivin mRNA expression by RT-PCR.The carotid sample were harvested on 28 days (n = 6 each group) for HE staining and measuring intima hyperplasia. Immunohistochemical method was also used to detect the expression of intima proliferation cell nuclear antigen (PCNA) and media α-SM-actin., Results: (1) After 7 days, NF-κB and survivin mRNA expression was significant higher in NC group than in normal control group (P<0.05), the NF-κB mRNA expression was significantly lower in NF-κB siRNA group than in NC group (P<0.05) and similar between NF-κB siRNA group and NF-κB siRNA+YM155 group. The survivin mRNA expression was significantly lower in NF-κB siRNA group compared to NC group (P<0.05) and significantly higher in NF-κB siRNA group than in NF-κB siRNA+YM155 group (P<0.05). (2) After 28 days, intima hyperplasia was observed in NC (0.13 ± 0.01), NF-κB siRNA (0.11 ± 0.01) and NF-κB siRNA+YM155 group (0.09 ± 0.01) mm² (P<0.05). Media area was similar among NC group, NF-κB siRNA group and NF-κB siRNA+YM155 group (P>0.05). I/M ratio was gradually reduced among NC group (1.55 ± 0.07), NF-κB siRNA group (0.92 ± 0.08), NF-κB siRNA+YM155 group (0.76 ± 0.06, all P<0.05). Similar results were found in the residual restenosis rate: NC group (58.71 ± 0.02) %, NF-κB siRNA group (32.13 ± 0.05) %, NF-κB siRNA+YM155 group (26.42 ± 0.03) % (all P<0.05) and expression of vascular smooth muscle cell PCNA: NC group (45.32 ± 7.21) %, NF-κB siRNA group (36.54 ± 6.42) %, NF-κB siRNA+YM155 group (28.57 ± 6.31) % (all P<0.05). On the contrary, the IOD of α-SM-actin in media increased gradually: NC group (0.055 ± 0.006), NF-κB siRNA group (0.072 ± 0.011), NF-κB siRNA+YM155 group (0.084 ± 0.008, all P<0.05)., Conclusion: Inhibiting NF-κB expression can significant decrease intima hyperplasia in this model, and this effect may be mediated by inhibiting survivin and reducing the proliferation of vascular smooth muscle cells.

Objective: To explore effects of exercise and conjugated linoleic acid (CLA) on PPARy in adolescent obese male SD rats., Methods: obese rats were modeled with high fat feeding, 32 obese rats were selected and randomly divided into control group, static + CLA group, exercise-treated group, exercise + CLA treated group. Blood and adipose tissue. were collected after 8 weeks, and blood lipid was measured. PPARγ mRNA gene expression in adipose tissue was tested using qRT-PCR, PPARγ protein expression in adipose tissue by immunohistochemistry, concentration of PPARγ in plasma by ELISA., Results: (1) TC level of static + CLA group and exercise group were lower than the control group (P <0. 05). TG level of the exercise group and exercise group + CLA were lower than the control group, static + CLA group (P <0. 01). LDL-c level of exercise group and exercise + CLA group was higher than the control group (P <0. 05), HDL-c level have no difference in groups. (2) PPAR-γ concentration in plasma in exercise group, exercise + CLA group was higher than the control group and static + CLA group (P <0. 01). Expression of PPARy mRNA in adipose tissue in exercise group, exercise + CLA group was higher than the control group and static + CLA group (P <0. 01). PPARγ concentration in plasma and expression of PPARy mRNA in adipose tissue in static + CLA group were higher than control group but with no statistical significance. (3) The situation of PPARγ protein expression was the same with the expression of PPARγ mRNA., Conclusion: 8 weeks of different intervention methods can reduce the concentration of TG and TC in blood lipid index in adolescent obese rats, TG of exercise and exercise + CLA is better decreasing than simply add CLA. Exercise and exercise + CLA can improve the expression of PPARy mRNA and protein in adipose tissue and plasma PPARγ concentration in rats.

Objective: To exploer effects of conjugated linoleic acid and exercise on RBP4 of liver and adipose tissues in adolescent obese rats., Methods: 8 adolescent SD rats were selected as blank control (C) with normal diet. Obese rats were modeled with high fat feeding for 7 weeks. 32 obese rats were selected and randomly divided into 4 groups: control group (OC), CLA group ( OCC), exercise group (OM), exercise combined with CLA group (OMC). Exercise treatment were performed on animal treadmill with running speed of 21 - 25 m/min, 60 min/every time. For CLA treatment, each rat was fed 1.6 g/kg, one time/day, 5 times/week, for 8 weeks. Animals were anesthesia treated after experiment, blood and body tissues were collected. The content of blood glucose was measured, the insulin sensitivity was calculated, testing the expression of RBP4 mRNA in visceral adipose tissue, expression of RBP4 protein in liver cells and plasma RBP4 concentration., Result: (1) Weight, weight growth rate and body fat percentage of OM, OMC groups were lower than OC, OCC groups (P < 0.01). (2) Blood glucose content of OM, OMC groups were significantly lower than OC, OCC groups (P < 0.01), while insulin sensitivity of OM, OMC groups was higher than OC, OCC groups (P < 0.01). (3) Expression of RBP4 mRNA of OM, OMC groups in visceral adipose tissue was lower than OC, OCC groups (P < 0.01). Expression of RBP4 protein of OM, OMC groups in liver cells was significantly lower than OC, OCC groups. RBP4 concentration in plasma of OM, OMC groups was lower than OC, OCC groups (P < 0.01). (4) Weight, weight growth rate, insulin sensitivity of OMC group were higher than OM group, but the percentage of body fat, blood glucose content and the expression of RBP4 were lower than OM group, there were no significant differences between them., Conclusion: Simple exercise and exercise combined with CLA conld significantly reduced adolescent obese rats weight, weight growth rate, percentage of body fat, blood glucose content and also reduced RBP4 mRNA expression in visceral adipose tissue, RBP4 protein expression in liver tissue, RBP4 level in plasma,but can increase sensitivity to insulin, in a word, the treatment of exercise and exercise combine with CLA is better than the simple complement of CLA to adolescent obese rats.

Objective: To evaluate and compare the physical and chemical properties of four different dental cements under long-term water storage., Methods: A glass-ionomer cement (A:Fuji I), a resin reinforced glass-ionomer (B: Fuji Plus), a self-adhesive resin cement (C:G-Cem), and an etch & rinse resin cement (D: Duolink) were taken as samples. According to ISO 4049, water sorption and solubility of four resin cements under different storage times (1 week, 1, 3, 6 and 12 months) were calculated (n = 5), meanwhile their surface Knoop micro hardness values were measured., Results: Surface cracks were detected on sample B and C after 12 months. Sample A was fragmentized after 6 months. Sample B showed significantly lower surface hardness after 12 months than it did after the first 24 hours of water storage (P < 0.05). Sample D showed significantly lower water sorption than other cements did at all exam time point. The highest water sorption values were observed in Sample D after 1 month [(40.8±2.5) µg/mm(3)], in Sample B after 3 months [(551.3±22.5) µg/mm(3)], in Sample C after 12 months [(147.5±8.3) µg/mm(3)]. The highest solubility values were detected in Sample B after 3 months [(105.3±10.5) µg/mm(3)], in Sample C after 12 months [(79.3 ± 6.2) µg/mm(3)], and in Sample D after 12 months [(23.9 ± 6.9) µg/mm(3)]., Conclusions: Among the four types of cements, the etch & rinse resin cement showed the best stability of physical and chemical properties under long-term water storage.

Objective: To discuss the effects of different intensity exercise on blood glucose, adolescent obesity rats insulin sensitivity and RBP4., Methods: The model of rats after 7 weeks, 32 rabbits of obesity prone rats were randomly divided into 4 groups, 8 rats in each group, respectively, control group, low intensity exercise group, moderate intensity exercise group, high intensity exercise group. High strength low in animal treadmill running speed respectively for 15 - 18, 21 - 25 and 28 - 32 m/min, 5 times/week, 60 min/times, for 8 consecutive weeks. Anesthesia for blood and tissue collection after exercise intervention, blood glucose, blood insulin, blood RBP4 concentration, liver RBP4 protein expression and visceral adipose tissue RBP4 mRNA expression., Result: The 8 week exercise intervention, low, high strength, in the sugar group was significantly lower than the control group (P < 0.01). Insulin sensitivity was higher than that of the control group (P < 0.01), insulin was lower than the control group difference. Expression of visceral adipose tissue RBP4 mRNA was low, medium and high strength group was lower than the control group, the expression was significantly down regulated (P < 0.01), in the high intensity group, low intensity group lowered significantly (P < 0.01). RBP4 higher than exercise group as control group in the expression of liver tissue protein, in the high intensity group, less the amount of expression. Plasma RBP4 concentration of high strength group was significantly lower than that of the control group (P < 0.01), high strength, low intensity group were lower than those in group (P < 0.01)., Conclusion: Through the different intensity exercise intervention can reduce blood glucose in obese rats, the plasma RBP4 concentration, improve insulin sensitivity, lower expression of RBP4 mRNA in adipose tissues, liver protein expression decreased, with the exercise intensity increased significantly.

Objective: To explore the clinical characteristics and prognostic value of monosomal karyotype (MK) patients in adult acute myeloid leukemia (AML)., Methods: We retrospectively studied 45 patients of MK⁺ in newly-diagnosed adult AML in our center from Oct 2000 to Dec 2012. Clinical characteristics, cytogenetic data and prognostic features were analyzed in the cohort of MK⁺ patients., Results: MK was found in 45 patients (19.0%) of 237 newly-diagnosed adult AML with cytogenetic data available at diagnoses. Among these 45 cases, there were 28 male (62.2%) and 17 female (37.8%). Median age of MK⁺ patients at diagnose was 58(18-91) years old. The presence of -5(31.1%) and -7(17.8%) were the most common chromatid among MK⁺ AML patients. MK was much more prevalent among elderly patients. Among AML patients, the proportions of MK⁺ patients younger than 30, 30 to 59 and older than 60 years old groups were 11.5%, 17.7% and 22.4%, respectively. There was no difference between MK⁺ and MK⁻ patients in gender distribution (P=0.545). There was also no difference between MK⁺ and MK⁻ patients in the distribution of FAB castigation (P=0.239). Median survival of MK⁺ AML patients was 6.5 months. Cumulative 5-year overall survival (OS) of was 5.2%. Forty-three MK⁺ patients (43/45, 95.6%) also had a complex karyotype (CK). Two cases that did not meet the CK had not achieved complete remission (CR), and died within 6 months. There were 12 patients who were CK⁺ in 192 MK⁻ patients. The differences of OS and CR rates between MK⁺CK⁺ patients and MK⁻CK⁺ were statistically significant (P<0.05)., Conclusion: The increased detection rate of MK with age was associated with lower CR and OS in AML patients.

Objective: To explore the efficacies of neoadjuvant chemotherapy plus nutritional supports for gastric cancer complicated with pyloric obstruction., Methods: Retrospective analyses were performed for a total of 116 patients of gastric cancer complicated with pyloric obstruction undergoing exploratory laparotomy from January 2004 to June 2013., Results: Sixty-two patients (group A) received neoadjuvant chemotherapy (regimen of FOLFOX) plus preoperative nutritional support. And parenteral (PN, n = 30) and enteral (EN, n = 32) nutritional supports were provided. Another 54 patients (group B) underwent exploratory laparotomy alone. The serum level of albumin and score of quality of life in group A at the last preoperative day improved significantly. And EN was better than PN. The rate of excision/radical excision of group A (85.5%, 45.2%) was much higher than group B (64.8%, 18.5%) (both P < 0.05)., Conclusion: Nutritional support, especially EN, can improve the nutritional status and quality of life in patients with gastric cancer complicated with pyloric obstruction. And nutritional support plus neoadjuvant chemotherapy increase the rate of tumor excision.

Objective: To investigate the application of home enteral nutrition (HEN) in patients with advanced gastric cancer and its impact on the quality of life., Methods: Data of 60 consecutive patients with advanced gastric cancer, who could not underwent operation and had relapse metastasis, from June 2010 to June 2012 were retrospectively analyzed. According to familial nutritional pattern, these 60 patients were divided into HEN group (25 cases) receiving home enteral nutritional support and control group (35 cases). HEN patients were supported through jejunostomy tube or nasal gastric tube. Control patients were supported through total parental nutrition or purely eating respectively. All the patients received intravenous chemotherapy and evaluated by Karnofsky index and Spitzer system in the first, third, sixth and twelfth month. In the sixth month, patients were also examined by EORTC QLQ-C30., Results: No significant differences were found between the two groups according to 8 elements containing age, sex, BMI, etc. A total of 53 patients died within one year, including 21 in HEN group and 32 in control group. The Karnofsky scales showed that HEN group scored meanly 57.4, 39.6 and 28.2 in the third, sixth and twelfth month respectively, which were significantly higher than those of control group (45.3, 29.2 and 20.1, P=0.041, P=0.012 and P=0.015 respectively). The Spitzer scales showed that HEN group scored meanly 5.12, 4.04 and 2.54 on average in the third, sixth and twelfth month respectively, which were significantly higher than those of control group (4.32, 3.01 and 1.97, P=0.048, 0.035 and P=0.024 respectively). The EROTC QLQ-C30 scales showed that HEN group scored higher than control group in functional scales (P<0.05), and lower in the symptom scales of short breathing, pain and tired (P=0.025, P=0.044, P=0.036 respectively), while higher in diarrhea (P=0.047)., Conclusions: The quality of life of patients with advanced gastric cancer declines gradually with the nutritional status deteriorating. HEN can be applied to improve the nutritional status and quality of life.

Galacto-oligosaccharides (GOS) are promising non-digestible oligosaccharides recognized as prebiotics. Commercial GOS containing galactose as subunit, are synthesized from lactose using the galactosyl-transferase activity of beta-galactosidase. A strain producing beta-galactosidase with transglycosylation activity was screened from the soil. Phenotypic analysis including morphology and physiology characteristics and 16S rDNA sequence analysis were carried out. Based on taxonomy results, the strain was identified as Enterobacter agglomerans B1. Medium and fermentation conditions were optimized by single factor and orthogonal experiments. The enzyme reached 9.7 U/mL in the medium (pH 7.5) containing 1% lactose, 1% yeast extract, and 0.5% peptone when cultured at 25 degrees C for 26 h. Effects of pH, temperature, lactose concentration, and reaction time on transgalactosylation by whole cells were studied. Yield of GOS reached 40.7% in 30% lactose (pH 7.5) at 50 degrees C for 12 h, as analyzed by HPLC and TLC. The results of an MS analysis showed that GOS were composed of di, tri-, and tetrasaccharides.
