Objective To explore the effectiveness of different knee arthroscopic medial collateral ligament release techniques in the treatment of patients with medial meniscus posterior horn injury and their prognoses. Methods A total of 54 patients with medial meniscus posterior horn injury who underwent surgical treatment at the Department of Sports Medicine in our hospital from June to August 2021 were enrolled. They were divided into group A (who underwent knee arthroscopic percutaneous outside-in, superficial release) and group B (who underwent knee arthroscopic inside-out, anteromedial release) based on the conditions and wishes of the patients. The two groups of patients were recorded and compared for surgical duration and follow-up outcomes of the following indices at 1, 3, and 12 months after surgery: local symptom duration, visual analogue score of knee pain, Lysholm score of knee joints, International Knee Documentations Committee Rating System-based score, and knee joint range of motion score. Results There was no significant difference in surgical duration between the two groups of patients (P>0.05), but the local symptom duration in group B was significantly shorter than that in group A (t=11.950,P0.05). Conclusion For patients with simple medial meniscus posterior horn injury, it is recommended to use knee arthroscopic anteromedial inside-out, superficial release if the surgeons are confident of the condition.

Image encryption technology plays an important role in today’s multimedia applications and Internet information security transmission.However, most image encryption algorithms still have problems, for example, cipher images occupied high bandwidth during transmission, image encryption speed is slow, image encryption algorithms are not associated with plain image, and there is no complete ciphertext feedback mechanism.These problems all affect the security and easy use of image encryption algorithms.In order to solve the above problems, the compressive sensing technology and chaotic system were studied, and a new image encryption algorithm based on double chaotic system and compressive sensing with plaintext association was proposed.The plaintext image association key was used to associate the plain image hash value with the Logistic chaotic system parameters.Discrete wavelet transform, was used to sparse the plain image.Then the random measurement matrix was generated by the Logistic chaos system, and the image was encrypted once by combining the compressive sensing technique and the random measurement matrix to obtain the intermediate image.The intermediate image was hashed again, and the Rucklidge chaos system initial value was associated with the plain image hash value together.The Rucklidge chaotic system and encryption algorithm were used to control the intermediate image for secondary encryption and accordingly obtain the cipher image.The encryption algorithm was a new plaintext correlation encryption algorithm, which used the image’s own pixel value to control the scrambling of the intermediate image.It also enhanced the plaintext association and established a ciphertext feedback mechanism.Simulation results and performance analysis show that the algorithm has good encryption performance.The encrypted image can be compressed according to the compression ratio, effectively reducing the size of the cipher image.Moreover, it is resistant to common attacks such as known plaintext attack, selective plaintext attack and differential attack, which is better than other common image encryption algorithms.

Objective To analyze the changes in intestinal fungi and compositions in patients with cholestatic liver disease from healthy individuals. Methods Fresh feces from 20 patients with cholestatic liver disease and 22 healthy individuals were collected. The intestinal fungi were determined by ITS2 DNA sequencing, and the differences of fungal flora diversity, structure and functional characteristics between the 2 groups were analyzed by bioinformatics methods, and the correlation between intestinal fungi and clinical parameters was analyzed. Results There were no significant differences in age, sex composition and smoking between the 2 groups (P>0.05). The serological indexes were significantly higher in the patients with cholestatic liver disease than the healthy controls (P < 0.05). No significant difference was seen in the Alpha diversity of intestinal fungi between the 2 groups (P>0.05), but significant differences were observed in Beta diversity (P < 0.01). The relative abundance of Myceliophthora was increased (P < 0.01) and that of Diutina was decreased (P < 0.01) in the patients with cholestatic liver disease. LEfSe analysis showed that Myceliophthora was significantly enriched in the patients, and the abundance of Saprotroph-Symbiotroph was significantly lower in the patients than the healthy controls. Spearman correlation coefficient analysis showed that Myceliophthora was positively correlated with alanine transaminase (ALT, P < 0.05), and aspartate transaminase (AST), alkaline phosphatase (ALP), gamma-glutamyl transferase (GGT) and total bilirubin (TBIL) (P < 0.01). Conclusion Characteristic changes are seen in intestinal fungal compositions between the patients with cholestatic liver disease and the healthy controls. Myceliophthora is closely associated with the severity of the disease.

This paper showed the effect of co-incineration of domestic waste and dewatered feces, by comparing and analyzing the operation conditions of the incineration system and the emission indicators. Under the condition that the proportion of dewatered feces blended was 5%, i.e. 48.44 t/d of dewatered feces was co-disposed in a single incinerator, the furnace temperature of the incineration system was reduced by about 5 ° on average. The incineration flue gas met the standards, while the amount of slaked lime used in the flue gas de-acidification system was increased by more than 30%. The emission concentrations of flue gas and fly ash chelate were lower than the prescribed limits. The concentration of nitrogen oxides in the incineration flue gas was increased accordingly, while the mass concentrations of heavy metals were able to meet the emission standards. This research showed that co-incineration could be used as a harmless treatment and a kind of emergency disposal method of the feces. [ABSTRACT FROM AUTHOR]

Based on field investigation and pollution detection results in a diesel contaminated site, the groundwater flow and typical non-aqueous phase liquid (NAPL) benzene, toluene and ethylbenzene migration model of the study area were established, which considered the convection-diffusion, dissolution, adsorption and microbial degradation process, and used MODFLOW and SEAM3D module of GMS to study the migration of NAPL. The results showed that NAPL plume migrate rapidly from the center to the surrounding in 100 days, and then gradually spread along the flow direction of groundwater. The pollution area of toluene is larger than that of benzene and ethylbenzene. The mass fraction of component, dissolution rate and initial concentration of Fe (III) are the key factors affecting the migration of NAPL by sensitivity analysis. The higher the mass fraction of NAPL components, the higher the equilibrium concentration in groundwater. The dissolution rate mainly affects the time when NAPL reaches equilibrium. The larger the dissolution rate, the shorter the time to reach equilibrium. The initial concentration of Fe (III) mainly affects the biodegradation of toluene. The higher the initial concentration of Fe (III), the stronger the biodegradation. However, the biodegradation of benzene and ethylbenzene is almost independent on the initial concentration of Fe (III). Objective: To investigate whether the outlet of the femoral tunnel will cause iatrogenic injury to the medial collateral ligament (MCL) during posterior cruciate ligament reconstruction (PCLR) and estimate the safe angle of femoral tunnel placement., Methods: Thirteen formaldehyde-soaked human knee joint specimens were used, 8 from men and 5 from women; the donors' age ranged from 49 to 71 years, with an average of 61 years. First, the medial part of the femur was carefully dissected to clearly expose the region of the MCL course and attachment on the femoral medial aspect and to outline the anterior margin of the region with a marked line. The marked line divided the medial femoral condyle into an area with an MCL course and a bare bone area which is regarded relatively safe for no MCL course. Then, the posterior cruciate ligament (PCL) was cut to identify the femoral attachment of the PCL. After the knee joint was fixed at a 120° flexion angle, the process of femoral tunnel preparation for the PCL single-bundle reconstruction was simulated. The inside-out technique was used to drill the femoral tunnel from the PCL femoral footprint inside the knee joint with an orientation to exit the medial condyle of the femur, and the combination angle of the two planes, the axial plane and the coronal plane, was adapted to the process of drilling femoral tunnels at different orientations. The following 15 angle combinations were used in the study: 0°/30°, 0°/45°, 0°/60°, 15°/30°, 15°/45°, 15°/60°, 30°/30°, 30°/45°, 30°/60°, 45°/30°, 45°/45°, 45°/60°, 60°/30°, 60°/45°, 60°/60° (axial/coronal). The positional relationship between the femoral tunnel outlet on the femoral medial condyle and the marked line was used to verify whether the tunnel drilling angle was a risk factor for MCL injury or not, and whether the shortest distance between the femoral exit center and the marked line was affected by the various angle combinations. Furthermore, the safe orientation of the femoral tunnel placement would estimated., Results: When creating the femoral tunnel for PCLR, there was a risk of damage to the MCL caused by the tunnel outlet, and the incidence was from 0 to 100%; when the drilling angle of the axial plane was 0° and 15°, the incidence of MCL damage was from 69.23% to 100%. There was a significant difference in the incidence of MCL damage among femoral tunnels of 15 angle combinations ( χ 2 =148.195, P <0.001). By comparison between groups, it was found that when drilling femoral tunnels at 5 combinations of 45°/45°, 45°/60°, 60°/30°, 60°/45°, and 60°/60° (axial/coronal), the shortest distances between the tunnel exit and the marked line were significantly different than 0°/45°, 0°/60°, 15°/45°, 15°/60°, and 30°/30° (axial/coronal) ( P <0.05). Additionally, after comparing the median of the shortest distance with other groups, the outlets generated by these 5 angles were farther from the marked line and the posterior MCL., Conclusion: The creation of the femoral tunnel in PCLR can cause iatrogenic MCL injury, and the risk is affected by the tunnel angle. To reduce the risk of iatrogenic injury, angle combinations of 45°/45°, 45°/60°, 60°/30°, 60°/45°, and 60°/60° (axial/coronal) are recommended for preparing the femoral tunnel in PCLR.

Objective: To evaluate the influencing factors that affect early pain after arthroscopic rotator cuff repair., Methods: A clinical data of 592 patients who met the selection criteria and underwent arthroscopic rotator cuff repair between June 2018 and October 2020 were retrospectively analyzed. There were 239 males and 353 females, with an average age of 58.1 years (range, 32-81 years). Before operation and at 3 days, 6 weeks, and 3 months after operation, the pain degree of patients was evaluated by visual analogue scale (VAS) score; and the patients were divided into no pain or mild pain group and moderate to severe pain group according to the postoperative VAS score. Preoperative and intraoperative related factors were included for univariate analysis, including age, gender, body mass index, preoperative VAS score, history of frozen shoulder, history of hypertension, history of diabetes, history of smoking, affected tendons (supraspinatus, infraspinatus, or subscapularis tendon injury), supraspinatus muscle atrophy, fatty infiltration, operation time, degree of rotator cuff tear, number of anchors, and whether to perform acromioplasty. The influencing factors of postoperative pain were screened; further logistic regression was used to conduct multivariate analysis to screen for risk factors., Results: Moderate to severe pain occurred in 440 patients (74.3%) at 3 days after operation, 382 patients (66.2%) at 6 weeks, and 141 patients (23.8%) at 3 months. Multivariate analysis showed that the women, partial-thickness rotator cuff tear, and acromioplasty were risk factors for pain at 3 days after operation ( P <0.05); the women, combined with fatty infiltration, partial-thickness rotator cuff tear, and acromioplasty were the risk factors at 6 weeks ( P <0.05); and the women, combined with fatty infiltration, and partial-thickness rotator cuff tear were risk factors at 3 months ( P <0.05)., Conclusion: Among patients undergoing arthroscopic rotator cuff repair, women, those with smaller rotator cuff tears, combined with fatty infiltration, and acromioplasty have more severe pain within 3 months after operation, and attention should be paid to postoperative analgesia in these patients management, providing an individualized approach to rehabilitation, and closer follow-up.
