Endoscopic resection (ER) and laparoscopic surgery are the main methods for treatment of early gastric cancer (EGC). Radical gastrectomy(RG) and lymph node dissection are required for the patients who don’t meet the indications of ER, but it may cause postoperative stomach dysfunction and reduce quality of life (QOL). Pylorus-preserving gastrectomy (PPG) is one of the function-preserving gastrectomy for EGC. The short-term complications of PPG are equal to that of RG and it is also beneficial to long-term nutritional status and QOL. The 5-year survival rate of PPG is similar to that of distal subtotal gastrectomy (DSG) according to a retrospective study of the matched cohort. But delayed gastric emptying (DGE) is a common problem in the short and long term after PPG. The limited lymph node dissection of PPG makes its application not widely promoted. Sentinel node navigation surgery (SNNS) may provide more personalized and minimally invasive surgery, in order to preserve the gastric function to the maximum extent.

In this study, the digestion-promoting function of an aqueous extract from Maojian green tea extract (MJ-GTE) was evaluated by small intestinal motility in mice as well as body mass, body mass gain, food intake, food utilization rate, gastric pepsin activity, and gastric pepsin excretion in rats. The chemical composition of MJ-GTE was then systematically analyzed using metabolomics based on ultra-high performance liquid chromatography-quadrupole electrostatic orbitrap mass spectrometry (UPLC-Q-Exactive/MS). The results of animal experiments showed that the intestinal propulsion ratio of ink in the high-dose MJ-GTE group (0.83 g/(kg·d)) was significantly increased compared with the model group (P < 0.05), and gastric pepsin excretion in the medium-dose MJ-GTE group (0.21 g/(kg·d)) was significantly increased compared with the negative control group (deionized water) (P < 0.05), which collectively indicated that MJ-GTE has a digestion-promoting effect. The metabolomics analysis identified 98 compounds, among which, flavones (apigenin and luteolin, 0.14–0.77 mg/g), flavanones (naringenin and eriodictyol, 0.49–1.49 mg/g), flavone-7-O-glycosides (0.57–9.07 mg/g), and flavanone-7-O-glycosides (4.49–38.98 mg/g) were the major components in MJ-GTE. This study will provide a theoretical basis for the promotion and development of Maojian green tea and related products in the future.

In order to explore the important role of seasonal temperature in the formation of soy sauce quality, three fermentation temperatures (37, 30 and 15 ℃) were set up to simulate the temperature changes during the four seasons in Guangdong and compared with natural fermentation. During the fermentation process, the dynamic changes of flavor indexes of base soy sauce were measured and sensory evaluation was also carried out to accesses the quality of the product. The results showed that fermentation at 30 ℃ was beneficial for the formation of alcohols, esters and phenols, imparting the highest sensory score and the best overall quality to base soy sauce. Base soy sauce fermented at 37 ℃ had high contents of acid and bitter amino acids and was rich in aldehydes, ketones, furans (ketones) and pyrazines, which formed the basis for soy sauce flavor. Fermentation at 15 ℃ was unfavorable for the formation of acids, bitter amino acids and characteristic flavor compounds in soy sauce, resulting in the lowest sensory score. Moreover, the great temperature changes during natural fermentation were detrimental to the formation of free amino acids and flavor substances. This study provides a theoretical basis for improving the production process and quality stability of Cantonese-style high-salt liquid-state soy sauce.

Collagenous gastritis （CG） is a rare disease characterized by a zone of collagen deposits of > 10 μm in thickness under the gastric mucosal epithelium with inflammatory cell infiltration in the lamina propria. The pathogenesis of CG is still unclear， which may be related to many factors， such as autoimmunity， inflammation and drugs， etc. Clinical manifestations of CG are complex and diverse with a chronic recurrence and prolonged process， which can be divided into child- and adult-onset groups of clinical symptoms. At present， there is no standard treatment for CG. The following therapeutic regimens have been employed including hypoallergenic diet， inhibition of gastric juice secretion， gastrointestinal mucosal protective agent， oral iron agent，glucocorticoid，5-aminosalicylicacid and immunosuppressive agents. However， clinical efficacy and follow-up outcomes are not completely consistent. In this article， research progresses on CG at home and abroad were reviewed.

Aiming at the problems in the field of Android malicious family detection,such as insufficient code visualization method construction information,large classification effect affected by the number of data sets and low classification accuracy,an Android malicious family classification method based on multi feature file synthetic image and Xception improved model is proposed.Fir-stly,three feature files corresponding to RGB multi-channel are selected to synthesize color images.Then,the improved Xception model introduces the focal loss function to alleviate the negative impact caused by the uneven distribution of samples.Finally,the attention mechanism is integrated into the improved model to extract the image features of malicious code from different dimensions,which improves the classification effect of the model.Experimental results show that the malicious code images synthesized by the proposed method contain richer features,have higher accuracy than the mainstream malicious family classification methods,and have better classification effect for unbalanced data sets.

Objective To evaluate the protective effect and inflammation inhibition mechanism of icariin (ICA) on lung injury in rat model with severe acute pancreatitis (SAP). Methods The rats were randomly divided into sham operation group (sham group), model group (SAP group, 5% sodium taurocholate was retrograde pumped into bile duct according to 0.1 mL/kg through micropump) and experimental group (ICA group, icariin 80 mg/kg intervention was given 2 hours before modeling). There were 6 rats in each group. Twenty-four hours after the establishment of the model, the inferior vena cava blood, pancreatic tissue and lung tissue were collected. The pathological injury of pancreas and lung was observed by HE staining; The activity of amylase, serum IL-6, IL-1β and TNF-α were measured by ELISA; The expression of inflammatory factors IL-6, IL-1β and TNF-α in lung tissue was determined by immunohistochemical staining; And the ratio of phosphorylated JNK/JNK to phosphorylated NF-κB/NF-κB in lung tissue was measured by Western blot. Results The pathological scores of pancreas and lung in SAP group were significantly higher than those in sham group; And the pathological scores of pancreas and lung in ICA group were significantly lower than those in SAP group. Serum amylase activity, serum IL-6, IL-1β and TNF-α content and the expression of IL-6, IL-1β and TNF-α in lung tissue in SAP group were significantly higher than those in sham group; While serum amylase activity, serum IL-6, IL-1β and TNF-α content and the expression of IL-6, IL-1β and TNF-α in lung tissue in ICA group were significantly lower than those in SAP group. The level of phosphorylated JNK and phosphorylated NF-κB in lung tissue of SAP group were significantly higher than those of sham group; While icariin might significantly reduce the level of phosphorylated JNK and phosphorylated NF-κB in lung tissue. Conclusions ICA ameliorates lung injury in SAP rat model by inhibiting acute inflammation response,which is potentially mediated by JNK/NF-κB signal pathway.

Convolutional neural network based network traffic classification scheme suffers many disadvantages such as the complex structure designing, gradient declines or even explodes, the deterioration of prediction accuracy, and etc. A residual network based improved traffic classification algorithm is proposed. The convolution layer and pooling layer in the traditional convolutional neural network are replaced by the residual network layer, which can alleviate the problem that the traditional convolution network is too deep to train effectively. The data feature information learned by the proposed algorithm in the training stage is more comprehensive, and the trained model can also be more accurate. Simulation results show that the improved algorithm has higher accuracy than the traditional neural network, which can be improved from 92.05%to 96.18%.

The purpose of this study was to investigate the extended publishing of conference papers in the field of management. The sample examined consisted of a total of 2,427 conference papers that were published in 2011 in nine international conferences spanning four academic fields, which were marketing, production and operations management, transportation management, and information management. This study adopted bibliometrics to analyze the extended publishing rate of the original conference papers, the document types of the extended publications, and the publication lag from the original conference papers to extended versions publishing. The differences between the originals and extended publications were also compared, including the changes in authorship, references, the length of articles, and the number of tables and figures. The results of the study reveal that the average extended publishing rate of the original conference papers was 17.06%, with journal articles being the major category. The average time lag for extended journal articles publishing was 27.99 months, and for submission was 14 months. This study suggested that the academic publishers provide relevant publication policy for extended publication. The journal editors and researchers should follow the guideline for submission and the editorial policy to avoid academic misconduct.

In recent years, some scholarly journals have indicated that they accept rewritten or extended version of a conference paper as long as new content are added to the journal article manuscript and that the original conference paper is included in the references. The extended publishing of conference papers now becomes topic worthy of investigation. This study analyzed the extended works of the conference papers from three major conferences in the fields of information management, i.e., Association for Computing Machinery (ACM)’s ACM Conference on Electronic Commerce (ACM EC), ACM Conference on Knowledge Discovery and Data Mining (ACM KDD), and ACM International Conference on Information and Knowledge Management (ACM CIKM). Papers from the 2011 meetings were used as the sample to understand to what extent conference papers have been extended into journal articles and other forms and the lag between conference and journal publishing. It also examined the differences between the conference papers and their extended versions (i.e., journal papers) by comparing the changes in authorship, references, article length, tables, and figures. It reveals the current practices of extending conference papers for journal publishing in the information management fields and may help us understand the contemporary scholarly publishing behavior. The education expenditure of a novice is a necessary cost of the healthcare system, which can deteriorate the shortage of nurses. The turnover rate of the novice in an operation room of a medical center in Taiwan was 55%. The investigation of this shortcoming revealed the following reasons: 1. stress from the workload, 2. unable to catch up to the work progress, 3. inconsistency in the tutoring skills of preceptors, 4. the frequency of preceptors in changing shifts, 5. preceptors working under pressure, and 6. impractical design of learning profile. Therefore, the purpose of this project is to reduce the turnover rate of new recruits in the operating room during the trial period. We implemented the following strategies: hold symposiums of the novice; redesign the educational program for the novice; periodic events for the surgical specialist education programs; demonstration and reverse demonstrations of surgical techniques; conduct a positive mindset camp for preceptors to improve their energy; regularly conduct clinical preceptors consensus conference; hiring of more certificated preceptors; fixed working shifts; provide stress relief programs; and revise the learning passport for the novice. After the implementation of the program, the turnover rate of novices in the operating room fell to 16.7%, thus achieving the purpose of the project. [ABSTRACT FROM AUTHOR]

Background & Problems: Patients undergoing percutaneous coronary interventions (PCIs) are restricted to bedrest and immobilization of the affected leg for sheath retention. These result in high incidences of back pain and emotional irritation, which may negatively affect cardiac rehabilitation. One-hundred percent of the post-transfemoral PCI patients in our unit had a back-pain score of 3 or higher, with an average pain score of 6. Analysis showed the main causes to be: poor awareness of nurses regarding back pain and physical activity; lack of physical activity guidelines and no regular auditing of physical activity; and lack of education and related materials. Purpose: To increase the completion rate of instruction for physical activity to over 90.0% and to reduce the incidence of back pain (score of 3 or higher) among post-PCIs to 35.0% or less. Resolutions: We conducted in-service education, revised related instruction materials, filmed the recommended physical activity for post-PCIs, and conducted a quality assurance audit. Results: The completion rate among post-PCIs for the instructions for physical activity increased from 32.5% to 90.0%. The incidence of back pain (score > 3) reduced from 100% to 26.8%. Conclusions: Implementing a physical activity education program with multimedia materials and regular audits effectively reduces the discomfort of patients following PCIs. [ABSTRACT FROM AUTHOR]

Ovarian cancer, known as a "silent killer", is the leading cause of gynecologic cancer death. Standard treatments for ovarian cancer are debulking surgery combined with platinum chemotherapy drugs to prolong the survival of patients. According to clinical trials run by the American Society of Gynecologic Oncology, patients who received intraperitoneal (IP) chemotherapy survived longer on average than patients who received intravenous chemotherapy alone. Thus, intraperitoneal chemotherapy is a new potential approach for treating ovarian cancer patients. However, the toxicities and undesirable complications of IP chemotherapy are the major challenges of this treatment approach. This article helps nurses recognize the toxicities and complications of IP chemotherapy and may be used as reference for future revisions to patient care guidelines. [ABSTRACT FROM AUTHOR]

Background: High-pressure workplace environments lead to a high prevalence of occupational burnout among nurses. This affects the physical and mental health of nurses and the quality of patient care. Purpose: This study investigates the current state of job stress and burnout and the relation between these two variables in order to explore the significant predictors of occupational burnout. Methods: A cross-sectional survey was conducted at a medical center in northern Taiwan using a convenience sample of 117 nurses. Data were collected in a structured questionnaire that included: a demographic inventory, a job content questionnaire, and an occupational burnout inventory. Data were analyzed using SPSS for Windows 20.0 software. Results: The average score for occupational burnout was 44.90, with the highest mean score in the aspect of personal fatigue (M = 48.80), followed by work-related fatigue (M = 47.74). This research found occupational burnout to be significantly associated with the presence of children (t = 2.25, p < .05), religious belief (t = 2.07, p < .05), perceived work and life stress (r = .44; .33, p < .01), workload (r = .36, p < .01), and workplace justice (r = -.21, p < .05). The presence of children, religious belief, perceived work stress, and workload were significant predictors, with these 4 predictors explaining 34% of the total variance in occupational burnout. Conclusions / Implications for Practice: Supervisors should provide resources to help nurses handle job stress. The findings of this study may be applied to develop a more effective interventional approach to decrease the job stress of nurses and to ameliorate their perceived burnout. [ABSTRACT FROM AUTHOR]

Background: A successful and satisfying childbirth experience not only gives women a sense of accomplishment but also improves their self-esteem and their adaptation to new maternal responsibilities. Purpose: This study aims to understand the impact of new mothers' perceived support and control during delivery on their childbirth experience. Findings may facilitate the development of appropriate nursing interventions to improve the childbirth experience. Methods: A cross-sectional survey was used to study 228 participants recruited from the postpartum ward of a teaching hospital in eastern Taiwan. A structured questionnaire that included a demographic survey, perceived labor support and control instrument, and childbirth experience instrument was used to collect data. Descriptive data were presented using percentage, mean, and standard deviation. Independent t-test, one-way ANOVA, Pearson correlation, and stepwise regression were used for inferential statistics. Results: Results revealed marital status, perceived support, and control during delivery to be significant predictors of childbirth experience satisfaction. These three factors accounted for 11% of total variance. Conclusions / Implications for Practice: Results support that obstetrics nursing personnel should further enhance their knowledge and skills related to labor support and help increase patients' sense of control over their delivery in order to increase the positive feelings of these patients about the childbirth experience.
