Aimed at the problem of synchronizing duplication in mobile database, this paper introduced the technology of multi-agent and transaction result set into synchronizing duplication of transaction level, and provided the model of synchronizing duplication in mobile database based on the technology of multi-agent. It compared with the existing models, this model had the following characteristics:a) improving the scalability of system; b) reducing the usage of mobile devices and network resources; c) proposing a series of synchronization strategy aiming at the harsh network environment. Purpose: To investigate the effects of hypoxia on the expression of osteoprotegerin (OPG) and receptor activator of nuclear factor kappa B ligand (RANKL) mRNA in rat bone marrow mesenchymal stem cells (rBMSCs)., Methods: rBMSCs were isolated and cultured by whole bone marrow cell adherent method, and an optimal hypoxic preconditioning model was established with CoCl 2 (cobalt chloride). rBMSCs were incubated in cell culture mediums with different concentrations of CoCl 2 (final concentrations of CoCl 2 were 0, 50, 100, 200, 400 μmol/L) and incubated for different times. MTT assay was applied to detect the effect of CoCl 2 on cell proliferation. mRNA and protein expression of HIF-1α of rBMSCs was detected by real-time PCR and Western blot. After treated with 100 μmol/L CoCl 2 for 0, 12, 24, 48, 72, 96 h, the expression of rBMSCs OPG/RANKL mRNA were detected by real-time PCR. The differences in distribution of each genotype were analyzed with SPSS 18.0 software package., Results: Compared with the control group, 200, 400 μmol/L CoCl 2 inhibited the proliferation of rBMSCs (P<0.05). However, 50, 100 μmol/L CoCl 2 had no significant impact on the proliferation of rBMSCs (P>0.05). Real-time PCR and Western blot showed that HIF-1α expression in 50 μmol/L and 100 μmol/L CoCl 2 groups was significantly higher than the control group; the effect of 100 μmol/L CoCl 2 was significantly greater than 50 μmol/L CoCl 2 . After cultivated in hypoxia condition for 12 h, the expression of OPG and RANKL mRNA in rBMSCs didn't change significantly (P>0.05). After cultured hypoxia condition for 24, 48, 72, 96 h, the expression of OPG mRNA in rBMSCs increased while the RANKL decreased, thus the ratio of OPG/RANKL increased and the difference was significant (P<0.05)., Conclusions: Hypoxia can regulate the mRNA expression of OPG and RANKL mRNA in rBMSCs and significantly promote osteogenic differentiation.

Objective: To explore the improvement of modified radical neck dissection in clinical node positive (cN(+)) low risk papillary thyroid cancer patients., Methods: Seventy-one cases of papillary thyroid cancer with cN(+) underwent selective neck dissection (IIa, III, IV) from 2007 to 2010 were reviewed, including 10 men and 61 women. All patients were at stage I, including 15 T1, 46 T2, 10 T3. Twenty-six patients were found Positive nodes were found by palpation in 26 cases and by ultrasonagraphy in 45 cases., Results: Cervical lymph node metastasis were confirmed pathologically in 63 cases, with metastatic rate of 88.7%; 100% in cN(+) cases by palpation and 82.2% in cN(+) cases by ultrasonagraphy, respectively. Metastatic nodes existed in one level in 13 cases, two levels in 31 cases, and three levels in 19 cases. Recurrence occurred to 4 patients during follow up, with a recurrent rate of 5.6%, and 2 cases of them were found recurrence in the anterior part of Vb, 2 cases in the carotid sheath., Conclusion: Selective neck dissection (IIa, III, IV) is acceptable for cN(+) low risk papillary thyroid cancer patients, which can decrease complications in neck and shoulder greatly.

Objective: To evaluate the salvage central neck dissection (CND) for papillary thyroid cancer in the patients with suspicious residual positive lymph nodes in the central compartment., Methods: A total of 85 cases undergoing salvage CND between January 2006 to January 2009 was reviewed., Results: Fifty-nine (69.4%) of the 85 patients showed residual positive lymph nodes in the central compartments. The number of dissected lymph nodes for each dissection ranged 2 - 13 nodes, with an average of 5.7 nodes. A total of 159 positive lymph nodes were found in the 59 cases, with an average of 2.7 positive nodes in each case. Four of 6 patients who underwent initial CND in other hospitals were found with residual positive nodes in the central apartments. The incidence of postoperative complications for the salvage CND was 10.6% (9/85), of them 5 cases with hoarseness, 2 cases with transient hypoparathyroidism, 1 case with postoperative bleeding, and 1 case with subplatysmal effusion. The median of follow-up after salvage CND was 44 months (3 - 5 years), showing contralateral central recurrence in 1 case, lateral neck recurrence in 4 cases, lung metastasis in 1 case, and no death case., Conclusions: Salvage CND should be done for those patients without undergoing initial CND but with positive nodes found after thyroidectomy or the high risks (T3, T4, extra thyroid extension, residual in primary site, and vascular or lymphatic vessel invasion); and also for those patients with initial CND but insufficient extent.

Objective: To analyze the infection of human papillomavirus in lip squamous cell carcinoma patients., Methods: The pathological samples of 9 clinical diagnosed lip cancer patients were collected. Lunimex and PCR techniques were used to detect the HPV gene infection and immunohistochemistry method was used to analyze the HPV protein expression in the samples., Results: In the 9 cases, 1 was positive for HPV16 gene and 7 were positive for HPV16/18 E6 protein expression. The total positive rate was about 8/9., Conclusion: The high HPV infection rate in lip cancer patients in the study indicated indirectly that the importance of the direct contact to the infection of HPV what was the basis for pathogenesis of the lip squamous cell carcinoma.

Objective: To evaluate the function autotransplanted parathyroid tissues in thyroid surgery., Methods: The control group comprised 120 volunteers with normal parathyroid glands in whom serum parathyroid hormone (PTH) levels in blood samples taken from the bilateral elbow head veins. The experimental group comprised 34 patients in whom the parathyroid glands, which were mistakenly cut or could not be preserved during thyroid surgery because of ischemia, were fragmented and autografted into the brachioradialis muscle of the nondominant forearm. On the day of surgery and at 3 days, 7 days, and 2 months postoperatively, contrast imaging and detection of serum PTH levels were performed to evaluate the function of the transplanted parathyroid tissues., Results: In the control group, the mean value of the difference between the bilateral brachiocephalic vein PTH levels was (1.19 ± 0.98) ng/L (maximum 4.52 ng/L). In this study, twice the maximum value (9.04 ng/L) was taken as the effective standard. Based on the 9.04 ng/L effective value, the effective number was 26 patients and the effective rate was 76.5% at 2 months postoperatively in the 34 patients of the experimental group., Conclusions: The autotransplanted parathyroid tissues showed their functions in most patients, therefore, parathyroid gland autotransplantation is an effective method of preserving function of parathyroid glands that are free or severely ischemic in thyroid surgery.

Objective: To detect the serum proteomic fingerprints in patients with hypopharyngeal squamous cell carcinoma (HPSCC) by surface enhanced laser desorption/ionization time of flight mass spectrometry (SELDI-TOF-MS) protein chip array technique., Methods: The serum samples were obtained from 58 HPSCC patients for protein expression analysis using SELDI-TOF Protein Chip technique and cation-exchange (CM10) protein array. All the spectra were compared and the qualified mass peaks with mass-to-charge ratios (m/z) between 1 and 70 kD were autotimatically detected. The tree analysis pattern was generated using Biomarker Patterns Software., Results: The protein profiles of HPSCC serum were analyzed according to the clinical and pathological features of the patients and their treatment response. No significant difference was noted in the serum proteins between HPSCC patients with different statuses of cervical lympha node metastasis (P>0.05), and the difference between well differentiated and poorly differentiated HPSCC was only minor. No significant difference was found in the serum proteins between chemotherapy-sensitive patients and the insensitive patients (P>0.05), but 5 proteins were identified to be overexpressed in the sensitive patients (P < / = 0.05). Radiotherapy-sensitive HPSCC patients were segregated from the insensitive group with a sensitivity of 86.67% and specificity of 100%., Conclusion: The serum protein at the m/z value of 6115.74 is overexpressed in radiotherapy-sensitive HPSCC patients. Serum protein profiling allows the prediction of radiotherapy response in HPSCC patients, and the identified proteins may serve as candidate biomarkers for predicting the radiotherapy sensitivity of HPSCC.

Background and Objective: Many thyroid cancer patients need to receive second operation because of tumor residue. This study was to explore the resection extent of primary lesion in re-operation of thyroid carcinoma., Methods: Clinical data of 55 thyroid carcinoma patients who received re-operation were reviewed. The patients, including seven men and 48 women, were aged of 18-56. All patients received ipsilateral residual lobectomy, isthmectomy, ipsilateral anterior cervical muscle and level VI lymph node dissection. The optimal extent for radical re-resection of primary lesion was explored., Results: Tumor residue was proved by pathology in 29 patients, with a rate of 52.73%. Of the 29 patients, eight had primary lesion residue, seven had level VI lymph node residue, and 14 had both. Tumor cells were connected with muscle in four patients. Forty-five metastatic lymph nodes were found., Conclusion: Ipsilateral residual lobectomy, isthmectomy, ipsilateral anterior cervical muscle and level VI lymph node dissection is the basic re-resection extent of primary lesion for thyroid carcinoma patients.

Objective: The study is to explore the extent, main-point and use of en bloc principle in re-operation of thyroid carcinoma., Methods: Clinical data of 75 patients received re-operation from 2002 to 2006 because of nonstandard operation were reviewed, including 10 men, 65 women, age ranging from 21-56. Ipsilateral completing lobectomy, isthmectomy, ipsilateral anterior cervical muscle, and level VI dissection were done in all the patients by en bloc principle., Results: Residual tumor was found in 39 cases. There were, 10 in primary site, 8 in VI level, and 21 in both. Therefore tumor residual rate was 52.0%. The total metastatic lymph nodes in VI level were 63. Among the eight patients with recurrent laryngeal nerves paralyses which were hurt in the first operation, 4 were found been cut completely, 4 were ligated. The ligated locations were all near the place which the nerve enter the larynx. The ligated nerves were released, and the patients voice was improved greatly., Conclusions: Ipsilateral completing lobectomy, isthmectomy, ipsilateral anterior cervical muscle and VI level dissection is adequate for thyroid carcinoma patients who received nonstandard operation. The principle of en bloc resection can be used in the reoperation of thyroid carcinoma.

Background & Objective: The prognosis of stage IV glottic cancer is poor. This study was to explore the impacts of different treatment modalities, cervical lymph node status and surgical margin on the prognosis of stage IV glottic cancer., Methods: Clinical data of 88 patients with stage IV glottic cancer were reviewed. The correlations of different treatment modalities, cervical lymph node status, and surgical margin to the prognosis of stage IV glottic cancer were analyzed., Results: The overall 3-and 5-year survival rates of the 88 patients were 71.6% and 63.0%. There was no significant difference in survival rate among the patients received operation, operation plus postoperative radiotherapy, chemoradiotherapy, and operation plus chemotherapy (P=0.729). The overall survival rate was significant lower in patients with lymph node metastasis than in those without lymph node metastasis (P=0.015); for stage cN0 patients, there was no significant difference between the patients with and without occult lymph node metastasis (P=0.474). There was no significant difference between the patients with positive surgical margin and those with negative surgical margin (P=0.016)., Conclusions: N stage is the important prognostic factor for stage IV glottic cancer. The prognosis of patients with lymph node metastasis is poor. The survival rate of the patients with positive surgical margin is lower than that of those with negative surgical margin.

Background & Objective: Cervical lymph node metastasis rate of glottic carcinoma is low. How to deal with cervical adenopathy remains controversial. This study was to explore the factors relate to cervical lymphatic metastasis of glottic carcinoma., Methods: Clinical data of 333 patients with glottic carcinoma, treated at Cancer Center of Sun Yat-sen University from Jan. 1, 1992 to Dec. 31, 2000, were reviewed. Distribution of cervical adenopathy, prognosis, and neck management were analyzed., Results: The overall lymphatic metastasis rate was 9.61% (32/333); the occult lymphatic metastasis rate was 6.24% (20/321). Most metastatic lymph nodes located at ipsilateral levels II, III and IV (28/32). The pathologic grade had no correlation to the overall lymphatic metastasis rate (P=0.092), and occult lymphatic metastasis rate (P=0.067). The overall lymphatic metastasis rate (P=0.002) and occult lymphatic metastasis rate (P=0.015) rose up following with increased T stage. Neck selective radiotherapy for the patients at stage cN0 had no significant impact on occult lymph node metastasis rate (P=0.363). The 3-and 5-year survival rates were significantly lower in cN+ patients than in cN0 patients (56.25% vs. 88.70%, 46.67% vs. 85.37%, P<0.001), significantly lower in the cN+ patients with occult lymph node metastasis than in the cN+ patients without lymph node metastasis (68.18% vs. 89.00%, 63.31% vs. 85.55%, P=0.005), and significantly lower in naive cN+ patients than in the naive cN0 patients with occult lymph node metastasis (41.67% vs. 68.18%, 16.67% vs. 63.31%, P=0.004)., Conclusions: Most metastatic lymph nodes of glottic carcinoma locate at ipsilateral levels II, III and IV, especially at level II. Cervical lymphatic metastasis affects the prognosis of glottic carcinoma.

Aim: To construct the eukaryotic expression vector of the cDNA sequence encoding bioactive N-terminal fragment of human bactericidal/permeability-increasing protein (BPI) and express it in CHO cells., Methods: Total RNA was extracted from human polymorphonuclear leukocytes (PMN) and subjected to reverse transcription, then the human BPI cDNA gene was amplified by nested PCR. The PCR product was cloned into pUC19 plasmid and confirmed by restriction enzyme digestion and DNA sequencing. Then the specific BPI encoding fragment was subcloned into pcDNA3 plasmid to form pcDNA-BPI(N) eukaryotic expression vector. CHO cells were transfected with the recombinant plasmid and the stable clones were selected by G418. The expression of BPI was identified by immunofluorescent assay., Results: Restriction enzyme digestion and DNA sequence analysis revealed that the sequence encoding signal peptide and bioactive N-terminal fragment of BPI had six nucleotide substitutions, compared with that of the established human BPI sequence. The expression products of the selected CHO positive cell clones were detected by anti-BPI monoclonal antibody., Conclusion: The construction of the eukaryotic expression vector of bioactive fragment of human BPI and its successful expression in CHO cells are helpful to further study of the role of BPI.

Background & Objective: The cervical lymphatic metastasis rate of glottic cancer is low, and has seldom been reported. This study was to explore the factors related to cervical lymphatic metastasis of T3-T4 glottic cancer, and analyze its correlation to prognosis., Methods: Clinical data of 83 patients with T3-T4 glottic cancer, treated in Cancer Center of Sun Yat-sen University from 1992 to 2000, were reviewed retrospectively. The lymphatic metastasis rate, distribution of metastatic lymph nodes, influence factors of neck recurrence of cN0 glottic cancer, and correlation of cervical lymphatic metastasis to prognosis of T3-T4 glottic cancer were analyzed., Results: Overall lymphatic metastasis rate was 20.5%. The neck recurrence rate of cN0 patients was 14.3%. Most metastatic lymph nodes located at the ipsilateral levels II, III, and IV, while only 1 located at the contralateral level II. For cN0 patients, there was no difference in the neck recurrence rate between observation group and prophylactic cervical radiation group (P=0.772). Histopathologic differentiation affected the neck recurrence of cN0 patients (P=0.028); while T stage did not relate to the neck recurrence (P=0.217). The prognosis of cN+ patients was poorer than that of cN0 patients (P<0.001). The neck recurrence of CN0 patients did not affect the prognosis (P=0.460)., Conclusion: Most metastatic lymph nodes of T3-T4 glottic cancer locate at the ipsilateral levels II, III, and IV. Poor differentiation is significantly correlated to high risk of neck recurrence among cN0 patients. However, the recurrence does not affect the prognosis. Close observation should be done to the cervix of cN0 patients; while neck dissection should be done to cN+ patients.

Background & Objective: The occult lymph node metastasis rate of supraglottic cancer is high, and the treatment of occult lymph node metastasis is important. This study was to explore the related factors, treatment, and prognosis of occult lymph node metastasis of supraglottic cancer., Methods: Clinical data of 104 patients with cN0 supraglottic cancer, treated in Cancer Center of Sun Yat-sen University from 1992 to 1999, were retrospectively reviewed. The occult lymph node metastasis rate, distribution of metastatic lymph nodes, related factors, and treatment of the neck were analyzed., Results: The occult lymph node metastasis rates were 23.1% (24/104) in the whole group, and 23.9% (11/46) in stage T2 patients, 30.8% (8/26) in stage T3, 18.5% (5/17) in stage T4. The metastatic lymph nodes mainly located at lateral levels II-III(22/24). The prognosis of the patients with occult lymph node metastasis was poor (log-rank=10.66, P=0.001). Positive margin increased occult lymph node metastasis rate (Chi(2)=10.015, P=0.002), while pathologic differentiation (Chi(2)=3.349, P=0.175), T stage (Chi(2)=2.701, P=0.440), and treatment of primary lesion (Chi(2)=1.093, P=0.296) had no effects on it. Selective neck dissection reduced occult lymph node metastasis rate (Chi(2)=4.070,P=0.044)., Conclusions: The occult metastatic lymph nodes of cN0 supraglottic cancer mainly locate at lateral levels II-III. The prognosis of the patients with occult lymph node metastasis is poor. Positive margin increases occult lymph node metastasis rate. For patients at stage T1N0, observation is enough; for patients at stage T2-4N0, lateral neck dissection (levels II-IV) is effective.

Background & Objective: Clinical treatment of stage T3 glottic laryngeal cancer still remains controversial. This study was to explore the influences of different treatment modalities, lymph node metastasis, and positive margin on prognosis of stage T3 glottic cancer., Methods: Records of 65 stage T3 glottic cancer patients, treated in Cancer Center of Sun Yat-sen University from Jan. 1, 1990 to Dec. 30, 1998, were retrospectively analyzed. The influences of different treatments, lymph node metastasis, and positive margin on survival of the patients were analyzed., Results: The 3- and 5-year overall survival rates were 75.47% and 65.07%. No significant difference was found among surgery alone, radiotherapy alone, and surgery plus adjuvant radiotherapy groups (P=0.914), between partial laryngectomy and total laryngectomy groups (P=0.710), and between positive margin and negative margin groups (P=0.176). The lymph node metastasis rate was 18.5%, and the occult lymph node metastasis rate was 10.8%. Lymph node metastasis was a poor prognostic factor (P<0.001)., Conclusions: The influences of different treatments on survival of stage T3 glottic cancer patients are similar, and the effects of partial laryngectomy and total laryngectomy are also similar. Lymph node metastasis affects prognosis, and wait-and-see is recommended to cN0 patients. The survival of patients with positive margin after postoperative radiotherapy is similar to that of patients with negative margin.

Objective: To develop a new type of feedback active noise cancellation earmuff (ANCE) for aviator's helmet., Method: A mathematical model of such a structure was established based on frequency response function and automatic control theory. The method and role for designing ANCE controllers were summarized and the optimum design method was used for designing the controllers., Result: The acoustic components and simulation circuit were well matched. One of the prototype can attenuate the noise more than 15 dB in the frequency range of 50-2000 Hz (19 dB at the highest)., Conclusion: The noise cancellation value of this earmuff has achieved international level and it can be mass-produced.

Aim: To observe the antiproliferative effect of antisense recombinant adenoviral vector for c-myc on rat thymus lymphocytes., Methods: Antisense and sense bacterial plasmids for c-myc were constructed. Bacterial plasmids and El detected adenoviral plasmid were cotransfected into 293 cells. Recombinant adenoviral vectors were obtained after cotransfection. The antiproliferative effects were assayed by MTS. The expression of c-myc mRNA was detected by RT-PCR., Results: The results showed that antisense recombinant adenoviral vector for c-myc could inhibit rat thymus lymphocytes proliferation. The expression of c-myc mRNA was decreased after antisense recombinant adenoviral vector for c-myc was transfected into cells., Conclusion: Recombinant antisense adenoviral vector for c-myc could inhibit rat thymus lymphocytes proliferation.

Background & Objectives: At present head and neck surgeons from many countries have different opinions on management of well-differentiated thyroid cancer (WDTC). We will discuss WDTC invading trachea surgical treatment and its clinical significance., Methods: Retrospectively reviewed clinical data of 15 cases WDTC invading trachea, According to WDTC invading extent and grade, there were 3 kinds of surgical approaches: 1) end to end anastomosis; 2) tissue flap reconstruction; 3)larynx-tracheal dissociation., Results: 15 cases underwent radical resection and reconstruct the defect of tracheal or larynx-tracheal dissociation. 2 cases received directly suture, 5 cases received sternocleidomastoid muscle flap reconstruction, 2 cases received pectorlis major muscle flap reconstruction, 2 cases received platysma flap reconstruction, 2 cases received free forearm flap with muticore titanium-board reconstruction, 2 cases received larynx-tracheal dissociation with larynx block out and tracheal fistula. 10 cases (10/15, 66.7%) received decannulation postoperation. Patients who were success fully decannulated could recover phonation and maintain airway breath. In the 5 patients who couldn't decannulate, underwent sternocleidomastoid muscle flap reconstruction, 1 underwent free forearm flap with muticore titanium-board reconstruction, 1 underwent pectorlis major muscle flap reconstruction, 2 underwent larynx-tracheal dissociation, but all of them could hardly utter voice by compress tracheostoma and needed permanent tracheostoma due to collapse of trachea. The recurrence rate of our group is 33.33%, 5 years survival rate is 88.89%., Conclusions: WDTC with trachea invading easily cause dyspnea or emptysis influencing on 5 years survival rate. We should take more actively surgical approach to resect all the tumor and involved organ, thus improve survival rate and reduce recurrence postoperation.

Objective: To develop a new multiple-polymerase chain reaction-single strand conformation polymorphism (multi-PCR-SSCP) system for detecting the aphC promoter, inhA, and katG gene mutations in isoniazid-resistant Mycobacterium tuberculosis isolates in the single reaction, and for the quick diagnosis of isoniazid-resistant Mycobacterium tuberculosis isolates., Methods: Three pairs of oligonucleotide primers were designed according to the aphC promoter, inhA, and katG genes of Mycobacterium tuberculosis to examine isoniazid-resistance by multi-PCR-SSCP., Results: Isoniazid-sensitivity and resistance were analyzed with general PCR and multi-PCR at the same time, and H(37) Rv was used as a control. These two protocols amplified the anticipated fragments, the rate of consistency being 100%. By single gene PCR-SSCP, the mutation rates of aphC promoter, inhA, and katG gene were 17%, 20%, and 66%, respectively. The mutation rate detected by multi-PCR-SSCP was 83%., Conclusions: Multi-PCR-SSCP is a sensitive and specific method for rapid detection of aphC promoter, inhA, and katG gene mutations in isoniazid-resistant Mycobacterium tuberculosis isolates. Drug-resistant gene detection may be clinically useful in the therapy of tuberculosis.
