Background How to implement effective and reasonable support and comprehensive services for patients with advanced cancer during chemotherapy is an issue worthy of attention in community health services at present. According to the latest data, the mortality rate of malignant tumors among residents in Shanghai has become the second place after cardiovascular and cerebrovascular diseases. The current attention to patients with advanced cancer during chemotherapy is also gradually increasing. Therefore, this study tries to understand the demand for primary care among such patients and the influencing factors. Objective To investigate the demand and influencing factors for community health services during chemotherapy of partial patients with advanced cancer in Shanghai, and to explore how to develop community interventions and services to address the demand of such patients. Methods Patients with advanced cancer during chemotherapy who regularly visited or hospitalized at Shanghai Ninth People's Hospital, Wusong Hospital and Shanghai Baoshan Hospital of Integrated Traditional Chinese and Western Medicine from December 2021 to March 2022 were selected as the study subjects. Based on previous relevant questionnaires, government specifications and interview results, the final version of the demand for community health services of partial patients with advanced cancer during chemotherapy in Shanghai was formed, with a total of 3 demand dimensions (psychological, medical care, social support) and 38 demand items. The contents include patients' general information (demographic and sociological information: gender, age, religious beliefs, education level, marital status, source of medical expenses, disposable monthly household income, whether to participate in a patient group; tumor diagnosis: type and time of diagnosis, pain score, combining with other diseases), 6 items of psychological and psychological demand, 24 items of medical care demand, and 8 items of social support demand. A 3-point scale was adopted: 1 was unnecessary, 2 was necessary, and 3 was very necessary. The higher the score, the higher the degree of patients' demand, which was ranked according to the average score of each demand item. Univariate and multivariate Logistic regression analyses were used to explore the influencing factors of the demand for community health services of patients with advanced cancer during chemotherapy. Results The average scores of each demand dimension were ranked in order of psychological demand (2.31 points), medical care demand (2.27 points) and social support demand (2.18 points). The top five average scores of all demand items were "preparation of biochemical tests such as routine blood, liver and kidney functions before chemotherapy" (2.48 points), "education on chemotherapy knowledge" (2.48 points), "care of PICC catheterization during chemotherapy" (2.45 points), "management of myelosuppression after chemotherapy" (2.43 points) " and "providing of updated information on treatment, examination and rehabilitation" (2.42 points), mainly focus on the medical care demand dimension. [Objective] The synergistic relationship between land use carbon balance and human activity intensity were studied in order to provide a reference for regional collaborative carbon emission reduction planning in the Tuojiang River basin, and thereby meet the goal of “carbon peak and carbon neutrality” in China. [Methods] Using land use and socio-economic data from 2000 to 2020, carbon emission economic contribution coefficient, carbon ecological carrying coefficient, human activity intensity indicators, a coupled coordination model, and regression analysis, we calculated carbon emissions from land use in the Tuojiang River basin. We then analyzed the spatiotemporal evolution characteristics and synergistic relationship between land use carbon balance and human activity intensity. [Results] Carbon emissions in the Tuojiang River basin increased by 5.13×107 t from 2000 to 2020, with construction land accounting for over 90% of carbon emissions. The small change in carbon absorption mainly resulted from forest land showing a change pattern of initially decreasing and then increasing. The growth rate of net carbon emissions showed a downward trend. The coordination of land use carbon balance in each district and county was improving, but more than 70% of the region was in a state of imbalance and decline. The overall intensity of human activities in the Tuojiang River basin was at a moderate level, with a spatial pattern of “low-high-low-high” from north to south. [Conclusion] There was a significant positive correlation between human activity intensity and per capita GDP and per capita carbon emissions. The negative correlation between land use carbon balance coordination and human activity intensity decreased with technological progress and increased energy utilization efficiency. Improving carbon sequestration capacity and carbon productivity is an effective way to improve the coordination level of regional land use carbon balance. The results of this study provide a reference for regional collaborative carbon emission reduction planning in the Tuojiang River basin.

Objective: To investigate the role of NOD-like receptor protein 3 (NLRP3) inflammatory vesicle activation in the progression of endometriosis (EMT) to endometriosis-associated ovarian cancer (EAOC) and the mechanism. Methods: EOAC tissues (n=15), EMT tissues (n=15), and normal endometrium tissues (CON, n=15) resected from patients during April 2018 and June 2019, as well as the clinical data of patients, were collected from the Shanghai Changning Maternity and Infant Health Hospital. Expression of NLRP3, BRCA1/BRCA2 containing complex subunit 3 (BRCC3), caspase-1 and IL-1β in the tissues of each group was detected by immunohistochemistry and WB methods. BRCC3 overexpression plasmids and si-BRCC3 plasmids were constructed and transfected into EMT CRL-7566 cells. The expression level of BRCC3 protein in the transfected cells was detected by WB method. The changes in cell proliferation, apoptosis, migration and invasion of transfected cells were detected by MTT method, flow cytometry and Transwell test, respectively. The BRCC3-overexpressing cells were further interfered with NLRP3. The expression levels of BRCC3 and NLRP3 proteins after the interference were detected by WB method, and the changes in cell proliferation, apoptosis, migration and invasion after interference were also detected. Results: The expression levels of NLRP3, caspase-1, IL-1 p and BRCC3 in EAOC and EMT tissues were higher than those in CON group (all P<0.01), and there was a positive correlation between the expression of NLRP3 and BRCC3 in EAOC tissues (r=0.65, P<0.01). Overexpression of BRCC3 significantly promoted cell proliferation, migration and invasion and inhibited cell apoptosis in CRL-7566 cells (all P<0.01); While knockdown of BRCC3 had the opposite effects (all P<0.01). Over-expression of BRCC3 increased the expression level of NLRP3 (P<0.01), while interference with BRCC3 inhibited the expression of NLRP3 (P<0.01). Interference with NLRP3 partially attenuated the inhibition on cell apoptosis (P<0.01) and promotion on cell migration (P<0.05) and invasion (P<0.01) brought by BRCC3 overexpression. Conclusion: Both NLRP3 and BRCC3 are highly expressed in EAOC and EMT tissues. Over-expression of BRCC3 can promote the proliferation, migration and invasion but inhibit apoptosis of CRL-7566 cells. Objective: To understand the status and problems of microbial pollution in shopping malls and supermarkets in China., Methods: Microbial pollution in shopping malls and supermarkets was assessed by literature search, key information extraction and analysis. The strengths, weaknesses, opportunities and threats(SWOT) of risk control of pathogenic microorganisms in shopping malls and supermarkets were analyzed by SWOT analysis., Results: Common bacteria in the indoor air of shopping malls and supermarkets included staphylococcus and Bacillus, and common fungi include Aspergillus and Penicillium. The bacteria detected in dust samples, escalator surfaces and floor surfaces were mainly Proteobacteria and Actinomyces. The complete public places laws and regulations, standards and health supervision system were the advantages of the risk prevention and control countermeasures of microbial contamination in shopping malls and supermarkets. At the same time, it also had the disadvantages of incomplete microbial-related indexes in the premises, and insufficiently detailed countermeasures for prevention and control in the premises. There were opportunities for multi-sectoral participation and post-licensing risk prevention, and it was also facing challenges brought by many factors affecting the health microenviroment and over-disinfection., Conclusion: The main sites for microbial risk prevention and control in superstore-type public places included high-frequency contact areas, key public supplies and utensils, indoor air, etc. , which could be prevented and controlled through a variety of measures such as controlling the release of the source, dilution and reduction, disinfection and denaturation, etc. , and exploring a comprehensive prevention and control system that involves the autonomy of the organization, industry self-regulation, collaboration of multi-government departments, and participation of the whole society.

Objective: To investigate the clinical features and implication of restless legs syndrome (RLS) in ischemic stroke patients., Methods: A total of 199 ischemic stroke patients were enrolled and assessed by polysomnography (PSG). RLS was identified according to criteria of International Restless Legs Syndrome Study Group. Epworth Sleepiness Scale (ESS), Mini-mental State Examination (MMSE) and Patient Health Questionnaire (PHQ-9) were used to evaluate the sleep quality, cognitive function and post-stroke depression, respectively. The National Institute of Health Stroke Scale (NIHSS) was used to evaluate the neurological function 3 months after stroke onset. Gender-and age-matched non-ischemic stroke patients with RLS (primary PLS) were selected as controls., Results: Twenty-two cases of RLS were identified among 199 ischemic stroke patients (11.1%). Generalized linear model and logistic regression showed that low serum ferritin level ( β =-133.3 mg/L, 95% CI :-200.4--0.1, P <0.01), subcortical infarction ( OR =4.05, 95% CI :1.15-14.18, P <0.05) and female ( OR =2.54, 95% CI :1.04-6.23, P <0.05) were identified as the risk factors of RLS in ischemic stroke patients. Compared with ischemic stroke patients without RLS, ESS increased by 4.37 (95% CI :2.33-6.41, P <0.01), PHQ-9 increased by 2.17 (95% CI :0.39--3.94, P <0.05), and reduced NIHSS from the baseline deceased by 0.97 (95% CI :-1.79--0.15, P <0.05) in ischemic stroke patients with RLS. In addition, the incidence of moderate-severe depression increased ( OR =4.27, 95% CI :1.40-13.10, P <0.05) in ischemic stroke patients with RLS. The index of periodic leg movements of sleep (PLMS) with arousal in ischemic stroke patients with RLS was significantly higher than that in patients with primary RLS ( β =12.85, 95% CI :2.04-23.67, P <0.05)., Conclusions: RLS is common in ischemic stroke patients and has adverse influences on patients.

A method was established for the simultaneous determination of 23 volatile organic compounds (VOCs) in ambient air with combination of thermal desorption (TD) and gas chromatography-triple quadrupole tandem mass spectrometry (GC-MS/MS). The air samples were collected by active sampling method using Tenax-TA sorbent tubes, and desorbed by thermal desorption. The analytes were determined by GC-MS/MS in selected reaction monitoring (SRM) mode, and internal standard method was applied to quantify the VOCs. The results of all the 23 VOCs showed good linearities in low level (0. 01-1 ng) and high level (1-100 ng) with all the correlation coefficients (r2) more than 0. 99. The method quantification limits were between 0. 000 08-1 µg/m3. The method was validated by means of recovery experiments (n = 6) at three spiked levels of 2, 10 and 50 ng. The recoveries between 77% and 124% were generally obtained. The relative standard deviations (RSDs) in all cases were lower than 20%, except for chlorobenzene at the low spiked level. The developed method was applied to determine VOCs in ambient air collected at three sites in Shanghai. Several compounds, like benzene, toluene, ethylbenzene, m-xylenes, p-xylenes, styrene, 1, 2, 4-trimethylbenzene and hexachlorobutadiene were detected and confirmed in all the samples analyzed. The method is highly accurate, reliable and sensitive for monitoring the VOCs in ambient air.

Background and Objective: It has been proven that chemotherapy combined with epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) could not increase response for advanced non-small cell lung cancer (NSCLC), but its cellular mechanism was not well known. The aim of this study is to assess the effects of sequential administration of docetaxel and gefitinib on the cell proliferation and signal pathway of lung adenocarcinoma cell SPC-A1 and its cellular mechanism., Methods: The mutation of EGFR and K-ras gene were examined by qPCR-HRM. MTT assay was used to measure the cell proliferation. The expression and phosphorylation of EGFR, ERK, AKT and IGF-1R were determined by Western blot., Results: No EGFR or K-ras gene mutation was found in SPC-A1 cells. Compared with docetaxel or gefitinib alone, no synergistic effects on the cell proliferation were observed in cells treated with docetaxel and gefitinib concomitantly or gefitinib followed by docetaxel. However, sequential administration of gefitinib following docetaxel could remarkably increase the inhibition of docetaxel on cell proliferation. Docetaxel increased, and gefitinib decreased, the phosphorylation of EGFR and ERK respectively. The suppression of pEGFR and pERK induced by gefitinib could not be activated by docetaxel, whether simultaneously or subsequently. No significant effects on the expression of AKT and p-AKT were found when docetaxel and gefitinib were administered simultaneously or sequentially. Docetaxel decreased the expression of IGF-1R., Conclusion: The phosphorylation of both EGFR and ERK, not the phosphorylation of AKT or the expression of IGFR, may contribute to the synergistic effects of EFGR-TKI following chemotherapy on the cell proliferation of NSCLC.

Background: To determine the antigenicity of HGV NS5 recombinant proteins expressed in E.coli., Methods: HGV NS5a,NS5b and core/NS5b fusion genes were cloned into pThioC vector. Three expression plasmids were transformed into JM109(DE3) competent cells then expressed with induction by IPTG. Western blot and ELISA were used to determine the antigenicity after the three recombinant proteins were purified., Results: After identification by restriction enzyme and sequencing, it was confirmed that the expressed was target proteins espected. Purified expression proteins were found strongly immunoreactive among anti HGV positive sera by Western blot and ELISA. Compared with mixed recombinant antigen (including core, NS5a synthetic peptide and NS3 recombinant proteins), in the 22 positive sera detected with mixed antigen, 68%(15/22), 90%(20/22) and 73%(16/22) were positive by P5a,P5b and Pc?5b antigens; In the 70 negative samples with mixed antigen, 7%(5/70), 1%(1/70) and 6%(4/70) were positive by P5a, P5b and Pc?5b antigens. The positive alone was found among RTPCR positive specimen using these recombinant antigens., Conclusions: NS5 gene expressed in E.coli?which couldn't be covered with other regions of antigens was one of the essential epitopes to HGV immunologic diagnosis.
