Objective To establish a model that can predict the occurrence of acute kidney injury (AKI) in elderly patients with HBV-related acute-on-chronic liver failure (HBV-ACLF), and to conduct a preliminary evaluation of the performance of the model. Methods A total of 276 patients with HBV-ACLF hospitalised in The First Affiliated Hospital of Harbin Medical University from January 2020 to January 2023 were retrospectively included and divided into 72 (26.09%) in the AKI group and 204 (73.91%) in the non AKI group, according to whether AKI occurred during hospitalisation. The clinical data of all patients were extracted and screened for independent risk factors for AKI during hospitalisation using multivariate logistic regression. A prediction model was constructed accordingly, and then the efficacy of the prediction model was evaluated using ROC curves. Results Multivariate logistic regression analysis showed that age ＞70 years (OR=1.404, 95%CI: 1.134～1.737), procalcitonin＞1 ng/L (OR=1.473, 95%CI: 1.074～2.019), model for end stage liver disease (MELD) score ＞34 (OR=1.702, 95%CI: 1.254～2.311), and combined upper gastrointestinal bleeding (OR=1.516, 95%CI: 1.123～2.047) were independent risk factors for the occurrence of AKI in HBV-ACLF patients during hospitalisation (P＜0.05). A prediction model for the occurrence of AKI in HBV-ACLF patients during hospitalisation was established based on the above four parameters, and the ROC curve showed that the AUC predicted by the model was 0.882 (95%CI: 0.838～0.917), with a sensitivity of 66.67%, a specificity of 93.14% and an accuracy of 86.23%. Conclusion Elderly, high MELD score and procalcitonin level, and concomitant upper gastrointestinal bleeding are independent risk factors for AKI in patients with HBV-ACLF during hospitalisation, and the model constructed accordingly can predict the risk of AKI, thus assisting clinical disease management.

Objective To analyze the value of the modified early warning scoring system (MEWS) combined with body temperature adjusted shock index (ASI) in the pre-hospital emergency condition assessment in patients with upper gastrointestinal bleeding. Methods A total of 92 patients with upper gastrointestinal bleeding in prehospital emergency care in this hospital from January 2021 to January 2023 were selected, and the patients were classified into a moderately severe group (blood loss≥500 mL, 61 cases）, and a mild group (blood loss ＜ 500 mL, 31 cases) according to their different blood loss. The MEWS and ASI scores of patients in the 2 groups were compared to analyze the value of MEWS combined with ASI for pre-hospital emergency medical assessment of patients with upper gastrointestinal bleeding. Results The MEWS and ASI scores of patients in the moderately severe group were significantly higher than those of the mild group, and the differences were statistically significant (P＜0. 05）;the MEWS and ASI scores were related to the patient′s condition, and the differences were statistically significant (OR=2. 470, 2. 605, P＜0. 05）. The MEWS, ASI, and the combination of subjects used for the assessment of the condition of pre-hospital emergency care for patients with upper gastrointestinal bleeding The area under the working curve was 0. 847, 0. 714, and 0. 870, respectively, all of which had some assessment value, and the combined assessment of the patient′s condition was more valuable. Conclusion Both MEWS and ASI have certain value in prehospital emergency condition assessment of patients with upper gastrointestinal bleeding, and the value of joint assessment of patients′ condition is higher. This paper summarized the integrated traditional Chinese and western medicine treatment and nursing management of hypovolemic shock due to recurrent gastrointestinal bleeding in a patient with abnormal coagulation function. The symptomatic treatment was carried out based on the risk assessment, and comprehensive interventions such as airway management, complication prevention, psychological intervention, dietary guidance, medication care, Chinese herbal medicine intervention were implemented during the treatment. Objective: To investigate the risk factors for delayed healing of peptic ulcer in patients with upper gastrointestinal bleeding (UGIB), so as to provide evidences for clinical treatment. Methods: A total of consecutive 138 hospitalized patients with UGIB due to peptic ulcer from January 2012 to December 2021 were enrolled. The healing rate of peptic ulcer was determined under reexamination of gastroscopy after 1 week of treatment. The related indice were analyze with univariate analysis to determine possible influencing factors for peptic ulcer, including gender, age, body mass index (BMI), diabetes, history of taking steroid anti-inflammatory drugs (NSAIDS), smoking, drinking, Helicobacter pylori (Hp) infection, hemoglobin, serum albumin, ulcer location or Logistic regression analysis was performed to determine the independent risk factors. To analyze the risk factors of upper gastrointestinal bleeding (UGIB) in patients with cirrhosis portal hypertension (CPH), and to explore the predictive efficacy of red blood cell distribution width to lymphocyte ratio (RLR), total bile acid (TBA) and prothrombin time (PT). 150 patients with CPH who were admitted to our hospital from May 2019 to April 2022 were selected, and they were divided into UGIB group with 57 cases and non-UGIB group with 93 cases according to whether they were complicated with UGIB. Multivariate Logistic regression was used to analyze the risk factors for UGIB in patients with CPH, and receiver operating characteristic (ROC) curve was used to analyze the predictive efficacy of RLR, TBA and PT for UGIB in patients with CPH. There were statistically significant differences in proportion of drinking history, Child-Pugh grade, portal vein diameter, degree of esophageal varices, Hb, albumin, RDW and LC between two groups ( P＜ 0.05). RLR and TBA in UGIB group were higher than those in non-UGIB group, and PT was longer than that in non-UGIB group ( P＜ 0.05). Multivariate Logistic regression analysis showed that drinking history, Child-Pugh grade C, increased portal vein diameter, severe esophageal varicos, increased RLR, increased TBA, increased PT were the independent risk factors for UGIB in patients with CPH, and increased Hb as independent protective factors (P＜ 0.05). ROC curve analysis showed that the area under curve (AUC) of RLR, TBA and PT alone and in combination for predicting UGIB in patients with CPH were 0.804, 0.779, 0.786 and 0.920 respectively, and the sensitivities were 63.16%, 70.18%, 59.65% and 77.19%, respectively. The specificity was 94.62%, 80.65%, 88.17% and 96.77%, respectively. The AUC of patients with CPH complicated with UGIB predicted by RLR, TBA and PT combined was greater than that predicted by RLR, TBA and PT alone (P ＜ 0.05). Alcohol consumption, Child-Pugh grade C, increased portal vein diameter, severe esophageal varicos, increased RLR, increased TBA, increased PT are the risk factors for UGIB in patients with CPH, and increased Hb as a protective factors. Objective: To explore and study the therapeutic effects and related mechanisms of Chinese medicine hemostatic powder combined with hemostasis after oral administration of Taoren Chengqi Decoction on mice with upper gastrointestinal bleeding. 24 cases of C57 wild-type male mice were randomly divided into three groups-model group, hemostatic group, and combined group. The three groups established a stress gastrointestinal bleeding model. At two weeks after modeling, the combined group were treated with traditional Chinese medicine hemostatic powder combined with oral Taoren Chengqi Decoction, the hemostatic group were treated with traditional Chinese medicine hemostatic powder, and the model group were treated with the same dose of normal saline. Results: All mice were survived during the experiment. The body weight of the hemostatic group and the combined group were higher than the model group in the 2nd and 4th weeks of treatment (P<0.05), and the combined group were higher than the hemostatic group (P<0.05). The expression levels of serum interleukin (IL)-6 and IL-8 in the hemostasis group and the combination group were lower than the model group in the 2nd and 4th weeks of treatment (P<0.05), and the combined group were lower than the hemostatic group (P<0.05). The bleeding index of peptic ulcer in the hemostatic group and the combined group were lower than the model group in the 2nd week and the 4th week of treatment (P<0.05), and the combined group were lower than the hemostatic group (P<0.05). The relative expression levels of nuclear factor kappa-B (NF-κB) and Caspase-3 protein in gastric epithelial tissues in the hemostasis group and the combined group were lower than the model group in the 2nd week and the 4th week of treatment(P<0.05), The combined group were lower than the hemostatic group (P<0.05). Conclusion: The application of Chinese medicine hemostatic powder combined with hemostasis after oral administration of Taoren Chengqi Decoction in mice with stress gastrointestinal bleeding can reduce gastrointestinal tissue damage, inhibit the expression of inflammatory factors, and promote the normal development of mice. Upper gastrointestinal bleeding is one of the most common and critical diseases in the digestive department. There are varicose and non-varicose types，whose features are acute onset，high rate of mortality and re-bleeding. Early prediction of the outcome and the severity of upper gastrointestinal bleeding have significant importance in diagnosis，treatment，disease development and clinical decision-making. Currently，different scoring systems have been developed for predicting the risk of mortality and re-bleeding，each of which has advantages and disadvantages. We have described the screening performance characteristics of CTP，MELD，Rockall，Bayloy， AIMS65，Glasgow Blatchford，CSMCPI and part of the new scoring systems， and summarized the application and research progress in this review. Objective To compare Glasgow-Blatchford score (GBS) with full Rockall score system (full RS) in predicting clinical outcomes of upper gastrointestinal bleeding (UGIB). Methods Four hundred hospitalized UGIB patients were enrolled in this study in Baoji Central Hospital from March 2014 to June 2016. Patients were scored by full RS and GBS, respectively. The receiver operating characteristic (ROC) curve analysis was used to compare the predictive values of the two score systems for mortality, re-bleeding, transfusion, ICU admission and endoscopic intervention. Results Thirtyeight cases were lost in follow-up. The one-month mortality rate, re-bleeding rate, blood transfusion rate, ICU admission rate and endoscopic intervention rate of 362 patients were 11.6%, 9.4%, 17.7%, 13.3% and 16.0%, respectively. GBS showed better predictive values in ICU admission (AUC=0.731, P=0.009), re-bleeding (AUC=0.707, P=0.019), blood transfusion (AUC=0.704, P=0.021) and endoscopic intervention (AUC=0.742, P=0.006). Full RS showed better predictive values in one-month mortality (AUC=0.681, P=0.040). There was significant difference in predicting value of clinical outcomes of UGIB by ROC curve between GBS and full-RS systems (P＜0.05). Conclusion The GBS system is more suitable for predicting clinical outcomes of ICU admission, re-bleeding, blood transfusion and endoscopic intervention in patients of UGIB. Objective： To evaluate the diagnostic value of color Doppler ultrasonography in the portal vein of patients with liver cirrhosis and upper gastrointestinal bleeding. Methods： Fifty cases of patients with liver cirrhosis and upper gastrointestinal bleeding treated in our hospital from September, 2015 to September, 2016 were selected as the study group, and other 50 healthy subjects were enrolled in the control group. The color Doppler ultrasonography was used to detect the left portal vein diameter（Dpv）, portal vein blood flow（Pvf）, the average blood flow velocity of portal vein（Vpv）, right diameter of splenic vein（Dsv）, splenic vein flow（Svf）, the average splenic portal blood flow velocity（Spv） of two groups. Results： Compared with the normal control group, the Dpv was expanded and the Pvf was increased significantly while the Vpv was decreased in the study group, and the differences in the portal vein indexes in the study group was statistically significant compared with those in the control group（t=8.782, 5.428, 6.382, P=0.001, 0.026, 0.013）. The levels of Dsv and Svf in the study group were significantly higher than those in the control group while the Spv level was significantly lower than that in the control group（t=5.683, 3.243, 3.365, P=0.018, 0.037, 0.034）. Conclusion： The color Doppler ultrasonography in the detection of portal vein hemodynamics in patients with liver cirrhosis can effectively determine the progress of the disease and treatment program guidance. Objective: To investigate the clinical study of interventional therapy combined with octreotide for elderly patients with peptic ulcer induced acute upper gastrointestinal bleeding. Methods: 30 cases of peptic ulcer induced acute upper gastrointestinal bleeding patients were randomly divided into control group and combined treatment group, 15 cases in each group, control group were treated with embolization interventional therapy, combined treatment group with octreotide combined with embolization therapy, we evaluated short-term curative effect after 48 h, and visited in 1 month to observe patients bleeding recurrence and adverse reaction. Combined immunoassay kit detected fecal occult blood before treatment and after treatment 2 days, 3 days and 7 days and record occult blood clearance time and hospitalization time. Results: The patients in the treatment group the short-term effective rate was 92.9% significantly higher than 60%in the control group, and the difference was statistically significant (X2=8.30, P < 0.05); Fecal occult blood test before treatment, two groups of patients were positive, 2 days after treatment, the positive rate of occult blood in the combined treatment group and control group were 21.4%and 66.7%, respectively, there was significant difference between two groups (X2=6.66, P < 0.01); 3 days after treatment, combined treatment group of 92.8%patients with occult blood test negative, significantly higher than 60% in the control group, and the difference was statistically significant (X2=4.66, P <0.05); The control group of patients with occult blood clearance time and hospitalization time were (4.15± 2.37) days and (7.22± 1.98) days, respectively, but in the treatment group, which were (2.77 ± 1.98) days and (5.33 ± 2.07) days, respectively, the difference was statistically significant (P < 0.05). Conclusion: Qctreotide combined with interventional therapy can quickly stop bleeding, rebleeding rate is low and security features, which can effective treatment of peptic ulcer with acute upper gastrointestinal bleeding. OBJECTIVE: To compare the efficacy between omeprazole, esomeprazole and pantoprazole in treatment of upper gastrointestinal bleeding. METHODS: 180 patients with upper gastrointestinal bleeding admitted into Guangzhou Zengcheng District People’s Hospital from Jan. 2015 to Jan. 2017 were selected and divided into group A, group B and group C via random number table, with 60 cases in each group. Group A was given omeprazole injection for intravenous drip, group B was given esomeprazole injection for intravenous drip, group C was given pantoprazole injection for intravenous drip. Differences in frequency of haematemesis, frequency of tarry stool, amount of bleeding, pH of fasting gastric juice and incidences of adverse drug reactions among three groups were compared, and cost-effectiveness analysis was conducted. RESULTS: At the 1 st, 3 rd and 5 th day, there were no statistical significance in differences in frequencies of haematemesis and tarry stool(P>0.05); at the 3 rd and 5 th day, the amounts of bleeding of three groups were significantly less than that of the 1 st day, the amounts of bleeding group A at the 1 st and 3 rd day were significantly more than those of group B and group C, with statistically significant differences(P<0.05); there was no statistical significance in difference in the amount of bleeding between group B and group C(P>0.05). After treatment, the pH of fasting gastric juice of three groups were significantly better than those of before treatment, the pH of fasting gastric juice of group A was significantly lower than group B and group C, with statistically significant differences(P<0.05); there was no statistical significance in difference in pH of fasting gastric juice between group B and group C(P>0.05). There was no statistical significance in difference in incidences of adverse drug reactions among three groups(P>0.05). The cost-effectiveness of group C was the lowest in three groups. CONCLUSIONS: The efficacy of omeprazole, esomeprazole and pantoprazole in treatment of upper gastrointestinal bleeding are remarkable, of which esomeprazole is better in inhibiting gastric acid secretion, with shorter time of bleeding stopping and fewer adverse drug reactions, which can be used as the first choice for the treatment of upper gastrointestinal bleeding; while pantoprazole has the highest economic benefit. Objective: To explore the application value of endoscopic metal titanium clip hemostasis in acute non-variceal upper gastro intestinal bleeding. Methods: A total of 40 patients with acute non-variceal upper gastrointestinal bleeding, who were admitted to Hechi Hospital Affiliated to Youjiang Medical College for Nationalities from January 2014 to December 2014,were randomly divided into observation group (n=20) and control group (n=20). The observation group was treated with endoscopic metal titanium clip hemostasis, while the control group, with endoscopic drug injection.Then the hemostatic effect clinical efficacy and complications in the two groups after treatment were compared. Results: The effective hemostasis rate (95.00%) and in mediate hemostasis rate (100.00%) of observation group were higher than that (65.00%) and that (75.00%) of control group respectively, the differences were statistically significant (P<0.05). The clinical efficacy of observation group was better than that of control group, the difference was statistically significant (P<0.05). No serious complications occurred in the two groups after treatment Conclusion: Endoscopic metal titanium clip hemostasis has the advantages of good hemostatic effect, less complications in the treatment of patients with acute non-variceal upper gastrointestinal bleeding, which has an important reference value in clinical field. Upper gastrointestinal bleeding (UGIB), a common medical emergency, causes significant morbidity and mortality. Endoscopic evaluation and treatment remain the standard care in patients who can be hemodynamically stabilized. However, severe bleeding despite conservative medication treatment or medication combined with endoscopic intervention occurs in 5%-10% of patients, requiring interventional or surgical treatment. Endovascular embolization has emerged as an alternative to emergency operative intervention for high-risk patients with non-variceal UGIB and is now commonly considered the first-line therapy for refractory bleeding after endoscopic treatment. Child-Pugh class C or class B cirrhosis patients who have varicosity or active bleeding detected in endoscopy are at high risks for treatment failure, rebleeding, and death. A preemptive transjugular intrahepatic portosystemic shunt has been shown to reduce treatment failure and mortality significantly. OBJECTIVE: To probe into the clinical efficacy of omeprazole combined with comprehensive nursing intervention in treatment of upper gastrointestinal hemorrhage. METHODS: 100 patients with upper gastrointestinal hemorrhage admitted into Hanchuan People's Hospital from Aug. 2013 to Aug. 2015 were selected to be divided into observation group and control group via the random number table, with 50 cases in each. The control group were given conventional western clinical treatment combined with conventional nursing; while the observation group additionally received omeprazole combined with comprehensive nursing intervention. The clinical treatment effects of two groups were observed. RESULTS: After treatment and nursing intervention, the total effective rate of observation group was significantly higher than that of control group, the bleeding time and amount of bleeding after treatment in observation group were significantly lower than those in control group, with statistically significant difference (P < 0. 05). CONCLUSIONS: The efficacy of omeprazole combined with comprehensive nursing intervention in treatment of upper gastrointestinal hemorrhage is significant, which can effectively shorten the bleeding time of patients and reduce the patient's amount of bleeding after treatment. Objective: To investigate the incidence of hepatic encephalopathy (HE) in patients with non-cirrhotic portal hypertension (NCPH) and to explore its risk factors. Methods: The incidence rate of HE in 150 cases with NCPH was evaluated in two hospitals, and 188 cases of compensated cirrhosis patients were taken as control. Logistic regression was used to screen for independent risk factors for HE in patients with NCPH. Results: The incidence of overt hepatic encephalopathy (OHE) in patients with NCPH was not statistically significantly different from that in patients with cirrhosis (4.7% vs. 6.9%, P = 0.682). The incidence of mild hepatic encephalopathy (MHE) was significantly lower than that of cirrhosis patients (32.7% vs. 46.3%, P < 0.05). The presence of upper gastrointestinal bleeding, infection and portosystemic venous shunt were the main independent factors for HE in NCPH patients (OR > 1, P < 0.05). Conclusion: HE is one of the important complications of NCP, and may be influenced by factors such as upper gastrointestinal bleeding, infection and portosystemic venous shunt.

Objective: To discuss the application of the degree of portal systemic shunting in assessing the upper gastrointestinal bleeding in patients with hepatic schistosomiasis., Methods: Thirty-three patients with upper gastrointestinal bleeding caused by hepatic schistosomiasis (a bleeding group) and 29 schistosomiasis cirrhosis patients without bleeding (a non-bleeding group) were enrolled as investigation subjects in Jinshan Hospital. The subjects were scanned by the 128 abdominal slice spiral CT. The portal systemic shunting vessels were reconstructed by using thin slab maximum intensity projection (TSMIP) and multiplanar reconstruction (MPR). The degrees of the shunting vessels of the subjects were evaluated and compared, and the relationship between upper gastrointestinal bleeding and the degree of the shunting was analyzed., Results: In the bleeding group, the occurrence rates of the shunting vessels were found as follows: 86.4% in left gastric varices, 68.2% in short gastric varices, 50.0% in esophageal varices, 50.0% in para-esophageal varices, 37.9% in gastric varices, 69.7% in gastric-renal varices, 51.5% in spleen-renal varices, 25.8% in abdominal wall varices, 15.2% in omentum varices, 63.6% in para-splenic varices, 34.8% in umbilical varices, 40.9% in retroperitoneal-paravertebral varices, and 36.4% in mesenteric varices. In the bleeding group, the occurrence rates and the degree of shunt were significantly higher than those in the non-bleeding group in esophageal varices, esophageal vein, left gastric vein and gastric varices (all P < 0.05)., Conclusions: CT portal vein reconstruction can accurately display the location, degree and walking of all kinds of shunting vessels. Esophageal varices, esophageal vein, left gastric vein and gastric varices can accurately predict the risk of upper gastrointestinal bleeding in patients with hepatic schistosomiasis. The patents with higher degree of the shunting vessels have a higher risk of gastrointestinal bleeding.
