3. Párová terapie v roce 2020: Současný stav a nový vývoj.

- Abstract
This paper provides a critical analysis and synthesis of the current status and emerging developments in contemporary couple therapy. Its narrative centers on the evolution of couple therapy into a prominent intervention modality and coherent body of practice. The review begins with the consideration of the field's strong empirical underpinnings derived from research on couple therapy and basic relational science. Couple therapy comprises the widely accepted method for reducing relationship distress and enhancing relationship quality. Moreover, both as a stand-alone intervention and in conjunction with other treatment formats, couple-based interventions have garnered considerable empirical support for their effectiveness in addressing a broad spectrum of specific relational dysfunctions as well as individual emotional and physical health problems. We highlight the convergence of methods through common factors, shared strategies, and remarkably similar arrangements across approaches. Our review also points to key differences among approaches, the importance of recognizing respective strengths and limitations linked to these differences, and building on differences across models when selecting and tailoring interventions for a given couple. The discussion concludes with a consideration of recent trends in the field including the impact of telehealth and related digital technologies, the expansion of specific treatments for specific problems and diverse populations, the interface of couple therapy with relationship education, and enduring challenges as well as new opportunities addressing broader systemic and global dynamics. Emotion-Focused Therapy is (understood as) an integrative, neo-humanistic, therapeutic approach whose formation and further development is substantially bound with empirical research of psychotherapeutic processes and mechanisms of change in therapy. This paper deals with empirical studies that were published during the second decade of the 21st century, focused on the efficacy of EFT used in the therapy of anxiety disorder, as well as other mental disorders. Current findings are being presented in the context of an individual-focused, family-focused and group-focused form of EFT. Particular attention is paid to selected new directions and innovations in the field of research that emerged only in recent years, during the verification process of the EFT – especially the transdiagnostic approach. Pain, from its origin through its treatment and in its subsequent maintenance, is influenced by psychosocial factors. For this reason, psychotheraphy is also used in pain treatment. The use of cognitive-behavioral therapy (CBT) is dominant. The paper describes the history of CBT and its three basic approaches: behavioral, (operant), cognitive, and cognitive-behavioral. The indications to CBT and the most frequent problems in treatment are isolated. Basic CBT techniques in chronic pain treatment, namely the cognitive restructuration and the practice of skills for pain and stress management (education, adaptive positive self-instruction, relaxation and imagination, increasing activity, exposition, structured problem solving, and relapse coping) are specified. Particular fields of chronic pain which represent the goal of multidisciplinary program of pain treatment including CBT are examined. The author structures basic assumptions of successful program: direct and indirect positive reinforcement of pain behavior, positive reinforcement of required behavior, improvement of physical condition, cognitive reframing, education, training in CBT in pain treatment, and conforming to the CBT principles. The paper sumarizes the efficiency of CBT in chronic pain treatment in adult patients, describes problems in its monitoring and suggests some innovations for increasing its use [ABSTRACT FROM AUTHOR]

The paper gives an overview of current developments in discourse analysis, including its most prominent trends, protagonists, theories, and concepts. The paper emphasizes diversity of approaches to discourse analysis and gives specific examples of their application to research in health psychology, feminist psychology, and counselling and psychotherapy. In this way, the paper aims to document the relevance of discourse analysis for psychological research and, by implication, for psychological theory and practice. 15. [Comparison of 3 forms of psychosocial rehabilitation in patients with epilepsy].

The submitted paper is based on experience 1. with enlisting epileptic patients in programmes for neurotic patients during hospitalization (1968 - 1983, a total of 20 patients), 2. an ambulatory psychotherapeutic group for epileptic patients only (period 1983 - 1989, a total of 41 subjects, i.e. 39 patients and two mothers of patients) and 3. a club for epileptic patients (1990 - 1993, a total of 56 subjects, incl. 44 patients and relatives, partners and friends). The majority of patients are not only treated at the neurological department but are also psychiatric patients. The author mentions also excursions made with patients who attend the club and on two arts exhibitions of work made by the patients. He presents also results and discusses positive and negative of these tested forms of psychosocial work. The advantage of the programme for neuroses is the relatively intensive action. The advantage of the ambulatory group is the possibility to discuss in detail specific problems of patients with epilepsy. The club is acceptable for a much wider circle of patients than the other two forms. The author recommends a combination of the described three procedure and the application of the widest possible range of other rehabilitation methods.

Aggressiveness is one of the basic characteristics of human beings. It can be defined as a relatively constant individual disposition or group characteristic manifested by the frequency, intensity and mode of aggressive behaviour. Aggressiveness has an inborn basis but its frequency, intensity and form are acquired during life and can therefore be influenced by the environment, i.e. also by training and psychotherapy. With regard to the genesis and purpose we differentiate between reflex, instrumental aggression, aggression with an end in itself and aggression resulting from affection. Psychological theories on the genesis of aggression can be divided into endogenous, exogenous and the interaction approach. The author discusses in his paper also the relationship between aggression and the type of psychotherapeutic treatment, and expands in more detail on aggressiveness and its control within the framework of Rogers' approach (person-centered approach).

A comprehensively conceived behaviourally oriented therapeutic programme of influencing undesirable sexual behaviour with creation of socially acceptable patterns of sexual behaviour, using in addition to group, regime and sociotherapeutic methods and individual behavioural techniques, (frequently under phallographic control, described in the paper), appears to be most effective in the framework of institutional treatment as regards rate of relapses and economic effectiveness. This is confirmed by ten years of experience with this therapeutic approach at the sexuological department of the psychiatric sanatorium (comparable with results of similar departments abroad), achieved despite difficulties as regards personnel and shortage of professional information. The relapse rate of sexual delinquency during the ten-year investigation, assessed by available methods, was only 9% of all treated subjects.

In the first part the author summarizes views of 95 Czechoslovak psychiatrists as regards the position of psychotherapy in the Czechoslovak health services and the position as regards psychotherapeutic training. From the summary ensues that the number of specialized departments does not correspond to the needs of our health services and the author emphasizes the need of better organization of psychotherapy in practice. At present there are opportunities of training in psychotherapy in particular in the psychotherapeutic section of the Psychiatric Society of the Czech Medical Society and in the Institute for Postgraduate Medical and Pharmaceutical Training in Prague. Both alternatives comprise compulsory participation in psychotherapeutic training groups. To make psychotherapeutic training more available, the author informs in the subsequent part of the paper on the possibility of education and training of health workers in psychotherapy. In this connection the author emphasizes the selection of participants, the principles of group psychotherapy training and the demand of selection of experienced supervisors and tutors. Training in psychotherapy should be differentiated to make differently oriented programmes possible; e.g. the programme for specialists in psychotherapy and another programme for doctors working in clinical medical branches. Finally the author deals with the perspective problem of specialization examinations in psychotherapy and indicates problems associated with institutionalization of psychotherapy.

Forgiveness is one of the psychological phenomena, which did not received adequate attention in psychological studies for a long time. The situation is changing due the rise of applied positive psychology and thus the topic is the subject of interdisciplinary studies in the world. This paper points to the considerable potential of the process of forgiveness as one of the intervention techniques in counseling and psychotherapy, which aims to improve the psychological health of the client. Forgiveness can be one of the options how to reduce the negative affective aspect of suffering - the psychological injury of a victim. First, it is pointed to the essence of forgiveness in counseling and psychotherapy, then, the forgiveness is defined regarding the phases of forgiveness and the process of forgiveness is described according R. D. Enright. The process models of forgiveness in counseling and psychotherapy are presented regarding the health aspect of forgiveness and non-forgiveness. In the conclusion, some results of studies dealing with intervention programs of process model of forgiveness in the field of quality of life, namely physical and mental health, are mentioned. Besides that, the author tries to outline the strengths and weaknesses of the process of forgiveness in counseling and psychotherapy based on three approaches to forgiveness in psychotherapy. The hope turns out that this phenomenon will become the subject of deeper psychological study not only abroad but also in the Czech and Slovak Republics. [ABSTRACT FROM AUTHOR]

The paper introduces psychotherapeutic experience with alcohol addicted homeless clients. The focus of the article is motivation of a client from time perspective, i.e. a development of early, continuous, and final motivation of a client to change one’s life style after an intervention of the centre in Havířov. The author describes pitfalls of such motivation and refers to case studies of various clients. Readers can observe specifics of work with addicted clients without homes via symptoms of individual addiction development. The paper shows influences on clients’ motivation as well as specific features of the cooperation between a therapist and a client.

This work focuses on the concept of art therapy in Aftercare center, in the service of dealing with follow-up care for people who are recovering from addiction, especially on non-alcohol drugs. Given topic is treated in the theoretical and practical. The theoretical part of the paper summarizes the basic definitions related to drug addiction and dependent personality description, followed by an after-care services and focuses on the principles and goals of treatment of addiction. In the treatment of addiction is the default selected using psychotherapy and expressive psychotherapy - art therapy. Art therapy is described as a field, along with historical context, the definition of guidelines that define the field and determining the place of art therapy in the treatment of addiction. The introduction of practical work is described in detail Aftercare center Prevent, focusing on the course and composition of the art therapy program and representation in it. Then are presented work in the background in art therapy Aftercare center Prevent. The main place of work is to present specific case studies of clients with analysis and interpretation of artifacts that illustrate ways of working within art therapy group with respect to the method Rožnov art therapy. The conclusion summarizes the specifics of the use of art therapy in the type of equipment described and obvious benefits for the field of art therapy.

This paper presents 12 individual case studies representing three different psychiatric diagnoses: depression, schizophrenia, and phobia. The collected data suggests that aspects of each one of the disorders covered are somehow reflected in the dreams and dream existence of the patients. Based on repeated in-depth interviews (n=48) with each of the participants (n=12), it is clear that dream content often repeats in each individual. Further phenomenological and qualitative content analysis revealed repetition of characteristic dream content (feelings, phenomenon, and existentials) in each of the three groups. While more research is needed, the results suggest dream content could be a significant tool in establishing a diagnosis and in aiding the psycho-therapeutic process.

In the crisis centre for children cases of force used on children cumulate, incl. sexually abused children. The interdisciplinary team of the centre is faced with the task to diagnose and treat this phenomenon and with the protection of the child from possible further damage (secondary victimization). In the submitted paper the author analyzes the period from October 1992 to the middle of September 1994, using uniform indicators. Special attention is devoted to psychic and somatic symptoms of the sexually abused child.

The submitted paper is based on experience 1. with enlisting epileptic patients in programmes for neurotic patients during hospitalization (1968 - 1983, a total of 20 patients), 2. an ambulatory psychotherapeutic group for epileptic patients only (period 1983 - 1989, a total of 41 subjects, i.e. 39 patients and two mothers of patients) and 3. a club for epileptic patients (1990 - 1993, a total of 56 subjects, incl. 44 patients and relatives, partners and friends). The majority of patients are not only treated at the neurological department but are also psychiatric patients. The author mentions also excursions made with patients who attend the club and on two arts exhibitions of work made by the patients. He presents also results and discusses positive and negative of these tested forms of psychosocial work. The advantage of the programme for neuroses is the relatively intensive action. The advantage of the ambulatory group is the possibility to discuss in detail specific problems of patients with epilepsy. The club is acceptable for a much wider circle of patients than the other two forms. The author recommends a combination of the described three procedure and the application of the widest possible range of other rehabilitation methods.

Aggressiveness is one of the basic characteristics of human beings. It can be defined as a relatively constant individual disposition or group characteristic manifested by the frequency, intensity and mode of aggressive behaviour. Aggressiveness has an inborn basis but its frequency, intensity and form are acquired during life and can therefore be influenced by the environment, i.e. also by training and psychotherapy. With regard to the genesis and purpose we differentiate between reflex, instrumental aggression, aggression with an end in itself and aggression resulting from affection. Psychological theories on the genesis of aggression can be divided into endogenous, exogenous and the interaction approach. The author discusses in his paper also the relationship between aggression and the type of psychotherapeutic treatment, and expands in more detail on aggressiveness and its control within the framework of Rogers' approach (person-centered approach).

A comprehensively conceived behaviourally oriented therapeutic programme of influencing undesirable sexual behaviour with creation of socially acceptable patterns of sexual behaviour, using in addition to group, regime and sociotherapeutic methods and individual behavioural techniques, (frequently under phallographic control, described in the paper), appears to be most effective in the framework of institutional treatment as regards rate of relapses and economic effectiveness. This is confirmed by ten years of experience with this therapeutic approach at the sexuological department of the psychiatric sanatorium (comparable with results of similar departments abroad), achieved despite difficulties as regards personnel and shortage of professional information. The relapse rate of sexual delinquency during the ten-year investigation, assessed by available methods, was only 9% of all treated subjects.

In the first part the author summarizes views of 95 Czechoslovak psychiatrists as regards the position of psychotherapy in the Czechoslovak health services and the position as regards psychotherapeutic training. From the summary ensues that the number of specialized departments does not correspond to the needs of our health services and the author emphasizes the need of better organization of psychotherapy in practice. At present there are opportunities of training in psychotherapy in particular in the psychotherapeutic section of the Psychiatric Society of the Czech Medical Society and in the Institute for Postgraduate Medical and Pharmaceutical Training in Prague. Both alternatives comprise compulsory participation in psychotherapeutic training groups. To make psychotherapeutic training more available, the author informs in the subsequent part of the paper on the possibility of education and training of health workers in psychotherapy. In this connection the author emphasizes the selection of participants, the principles of group psychotherapy training and the demand of selection of experienced supervisors and tutors. Training in psychotherapy should be differentiated to make differently oriented programmes possible; e.g. the programme for specialists in psychotherapy and another programme for doctors working in clinical medical branches. Finally the author deals with the perspective problem of specialization examinations in psychotherapy and indicates problems associated with institutionalization of psychotherapy.

Velký prostor je věnován soudobé psychoanalýze, dynamické terapii, KBT, gestalt terapii, prožitkové terapii, rodinné terapii, systemickým přístupům i integraci psychoterapeutických směrů. Zařazeny jsou kazuistiky pěti terapeutů a jejich vzájemné komentáře ke své práci. Mezi další témata knihy patří: situace oboru v Evropě, práce s emocemi, práce s tělem, příběhy v terapii, psychoterapie v medicíně a v sociální rehabilitaci, kombinace psychoterapie a farmakoterapie, platby za psychoterapii, etika v psychoterapii, východní a kulturní vlivy, výzkumy účinnosti, expresivní metody a alternativní terapie. Doc. Zbyněk Vybíral byl od roku 1990 šéfredaktorem časopisu Konfrontace, nyní řídí časopis Psychoterapie. Vede katedru psychologie na Fakultě sociálních studií Masarykovy univerzity v Brně, kde založil magisterské studium psychoterapie. MUDr. Jan Roubal je lektorem, supervizorem a praktikujícím psychoterapeutem v gestalt terapii, přednáší rovněž na Fakultě sociálních studií Masarykovy univerzity v Brně. K sepsání kapitol byli přizváni přední čeští a slovenští představitelé jednotlivých směrů.

Psychoterapeutická pomoc dětem má řadu specifik, daných především nutností respektovat ještě více než u dospělých vývojovou situaci dítěte a kontext jeho rodiny. Učebnice podává přehled obecných otázek spojených s psychoterapeutickou pomocí dětem, představuje jednotlivé přístupy a formy terapie i možnosti jejich použití u vybraných skupin poruch (děti nemocné, týrané, užívající drogy aj.). Nové, přepracované a rozšířené vydání knihy tří renomovaných českých autorů vedle základů oboru odráží i vývoj v posledním desetiletí. Výklad je doplněn řadou příkladů a kazuistik.
