Virtual reality (VR) is becoming an affordable technology and has the potential to be used in psychiatry. Cue-exposure therapy (CET) is used as an effective method in craving management in people with addiction. VR is applied for exposure simulations in CET. We are presenting a continuation of a review article published in 2019. In order to make such a review we searched for articles in the database PubMed according to key words „virtual reality & addiction"; „virtual reality & cue exposure therapy". We found 15 relevant articles published until January 10, 2021. Those articles were further divided into 3 groups. The first group includes 3 individual studies to verify the applicability of VR in opioid, cocaine and cannabis addiction. The second group consists of 7 studies to verify the applicability of VR in alcohol addiction whereas the third group consists of 5 studies to verify the use of VR in alcohol addiction treatment. Conclusion: VR allows to identify craving as well as coping with craving in a safe environment in individuals with addiction. It is necessary to prove the efficacy of VR CET in the treatment of addiction in randomized, controlled trials. [ABSTRACT FROM AUTHOR]

AIMS: The aim of this research study was to verify if there are differences in motivation among inpatients in Czech psychiatric hospitals and clients of therapeutic communities. The research was conducted among all the new referrals to the Jihlava Psychiatric Hospital (n = 255) and the clients of the Sejrek, Renarkon, Kladno-Dubi, and Vrsicek therapeutic communities (n = 50). DATA COLLECTION: The data was collected between 1 March 2015 and 31 March 2016. To measure motivation, we always distributed the Czech version of the CMRS Scale among the respondents within their first week of treatment. METHODS AND RESULTS: In the statistical data processing, we performed a frequency distribution of the answers to each statement on the scale and then calculated their mode. In relation to circumstances, the frequency distribution demonstrated that the clients of the therapeutic communities scored slightly better than the inpatients in the psychiatric hospital. Their opinions were also more convincing with regard to motivation and readiness. With regard to suitability for treatment, the clients of the therapeutic communities displayed higher awareness of their addiction problem. CONCLUSION: The patients in the psychiatric hospital emphasized rather the need to change the environment in order to achieve abstinence and the importance of individual sacrifice in completing the therapeutic programme. When the results of the frequency distribution from the psychiatric hospital and therapeutic communities were compared, a remarkable consensus was identified, despite the significant difference in the number of respondents from various types of rehab. This result was also confirmed by the mode calculation. There are no fundamental differences in motivation between the inpatients of psychiatric hospitals and clients of therapeutic communities. Background: Penitentiary care for clients with addictology issues is still an insufficiently discussed topic. Especially if we emphasize that a significant number of clients, ie users of addictive substances (especially illegal ones) have experience in serving a custodial sentence. The fact that more attention needs to be paid to the care of users of illegal addictive substances in prisons was confirmed by the Prison Service of the Czech Republic by creating a position for addictologists in prisons in 2018, following a successful pilot project. Targets: The core of the final work are case studies with the aim of examining the issue of matching needs in individual addictology services offered by Bělušice Prison. Given the prison's limited capacity to provide addictology services, also answer the question of whether the programs and interventions provided are sufficient or whether the possibility of setting up or amending one of the intervention programs could not be considered in the future. The presented bachelor thesis describes drug addiction and its effect on clients in the therapeutic community of Karlov. This bachelor thesis aims to examine the motivation of said clients to start their treatment provided by the therapeutic community of Karlov. Furthermore, this bachelor thesis also discusses the possibility of the motivation to start the treatment being influenced by the types of drugs used by the clients. Lastly, the overall functioning of the therapeutic community is also described in detail within this thesis. This bachelor thesis is divided into two parts. The theoretical part focuses on various addictive substances and their categorization. The theoretical part also describes the treatment concerning the abuse of such addictive substances, the motivation that leads to treatment and the methods used by social workers. The theoretical part concludes with the description of Karlov's therapeutic community characteristics. The practical part was conducted following the qualitative research strategy, which included the use of semistructured interviews. The sample for the semistructured interviews consists of clients of the Karlov therapeutic community. In the empirical part of the work, it was found that to join the therapeutic community Karlov, even though each client was motivated by various reasons, they began to solve their situation only when they began to lose the last thing that was important to them. Furthermore, it was found that clients began to experience drugs at a very young age and the problems associated with this addiction did not come until many years later, when the drug changed the original scale of life values for all respondents and completely disrupted personal relationships. This bachelor thesis provides insight into the issues associated with addictive substances and their subsequent treatment performed in the Karlov therapeutic community. The results of the research conducted in this bachelor thesis could be used by the Karlov therapeutic community as a basis for an improvement of their current practices.

The aim of this thesis is to describe and to evaluate the importance of group therapy during the treatment of addictions in the process of healing and social functioning of an individual and to identify the risks of group therapy. The theoretical part describes possible causes and personal presuppositions concerning the origin of addictions. It focuses on the importance of influence from one's family and raising, and how they affect addictions, codependency, psychotherapeutic approaches and models of family systems. The theoretical part furthermore characterizes group therapy, its techniques and methods, and specifies the matters, which can accompany the participants. In the practical part are processed data gained from questionnaires of respondents from therapeutical communities and after-treatment centers, who designated each group therapy, according to the perceived benefits and popularity in an ongoing treatment. The point of the research is to evaluate the relevancy of group therapy by addicted persons, to map the relevancy of each group therapy during the resolution of social issues and the acquirance of skills, and to summarize which therapeutical approaches are expected by the clients during their treatment, if they consciously approach the treatment, which factors act as healing, and which risks can accompany the treatment.

The Outcome of Addiction Treatment Programme: 5 years Follow-up In the Czech Republic, there is a long lasting tradition of an institutional treatment for alcohol and substance dependence, based on the residential programme that combines behavioral modification and psychotherapy. Publications of treatment outcomes use different methods of assessment, which make comparisons difficult. We report on 5- years of follow up in 468 patients addicted on alcohol, illegal drugs, addictive medicines or pathological gambling. They underwent the residential treatment programme at the Department of Addictive Disorders at Nechanice. They had to participate in the programme for at least 2-months. Outcome data were collected by a questionnaire sent to persons close to patients (relatives, spouses, mates) and supplemented with telephone-calls, if necessary. The outcome assessment is based on 366 correspondence or telephone contact and add additional information to data gained by questionnaires mailed in the previous years. Respondents reported total abstinence in 30% of cases throughout the 5-year follow-up. The abstinence lasting at least half a year at the time of responding, was reported in 53% of patients. The rate of those abstaining remains stable anytime after the treatment. However, the course and outcome of abstinence differs according to the addictive substance or activity. Remarkable finding is the high death rate among patients who relapsed. The assessments of treatment programmes can be useful in comparing different treatment programs and their fit to particular addiction type. [ABSTRACT FROM AUTHOR]

10. Nutriční návyky a změny ve skladbě výživy u pacientů léčících se ze závislosti na alkoholu ve vybraných psychiatrických nemocnicích ČR: Pilotní studie.

BACKGROUND: Recent studies indicate that persons dependent on alcohol may show deficiencies in their intake of specific vitamins and minerals. These deficits can subsequently be associated with other somatic, physiological, and psychological problems. AIM: The aim of the study was to identify dietary habits and nutrition among patients in addiction treatment at the Bohnice Psychiatric Hospital in Prague and the Opava Psychiatric Hospital. A pilot study was conducted to examine whether the nutrition the patients receive meets the general dietary guidelines and look for changes in dietary habits before and after the initiation of the treatment. METHODS: The study sample included 49 patients. The test battery consisted of the Alcohol Depending Scale (ADS) and the Food Frequency Questionnaire (FFQ). RESULTS: The results indicate significant changes in the patients' dietary habits and nutritional preferences before and after the institutional treatment was initiated. In the course of the treatment the quality of the patients' nutrition improved in some areas: their intake of foods such as quality carbohydrates and vegetables, the consumption of which was insufficient during their alcohol consumption, increased. On the other hand, there was also a major increase in their intake of monosaccharides and disaccharides. CONCLUSION: Treatment for alcohol dependence and abstinence affect patients' dietary habits and nutritional preferences. This bachelor thesis summarizes the current state of knowledge in the field of mindfulness-based relapse prevention. Over the last few decades, mindfulness has become part of several psychotherapeutic programs and has shown promising results in the treatment of a wide range of chronic diseases, including addiction. The starting point for writing this work is the absence of a document that would summarize the current state of knowledge in this field and the possibility of applying a relapse prevention program based on mindfulness in the Czech Republic. In the first part of the work, we briefly deal with cognitive-behavioral therapy and its relationship to the issue of addiction. Subsequently, we present the theory of relapse prevention according to Marlatt and Gordon, which is the most commonly used treatment intervention so far. The main part of the work is a description of the 8-week manualized program Mindfulness-based relapse prevention (MBRP) and a proposal for the implementation of this program in the system of care for addicts in the Czech Republic. 14. Význam diferenciální diagnostiky poruch osobnosti u pacientů s ADHD v léčbě závislostí.

Personality disorders and ADHD represent one of the most common comorbidities among substance users. At the same time, recent studies generally show relatively high prevalence of ADHD in addicted patients. In combination with more "traditional" comorbidity as personality disorders, represents a high risk complicating factor for proper diagnosis, indications for treatment, procedures and overall treatment outcome. The incidence of symptoms or residues of ADHD in adulthood represents for the individual often significant difficulties in personal, social and working lives. The patients may experience a higher level of frustration, aggression and interpersonal conflicts. All this makes them, inter alia, more vulnerable for substance abuse. The diagnostic criteria for ADHD in adulthood in some areas overlap with diagnostic guides for personality disorders, and thus may be difficult in clinical practice to reliably distinguish and determine what kind of comorbidity that is, or in what etiological relationship they are. The review focuses on the differential diagnosis of these three mental disorders. The authors reflect the prevalence of comorbidities, describe the diagnostic aspects according to DSM-V and ICD-10 and present developmental aspects of ADHD. Based on the review of assessment methods for the Czech environment and experience from psychiatric and psychological practice are outlined recommendations for clinical work with this group of patients. - Abstract
BACKGROUND: Both clinical practice and research have shown that the ADHD can be a significant risk factor for residential treatment in therapeutic communities. It may strongly affect the safety and quality of treatment of people dependent on psychoactive substances. While having a major impact on psychological functions, ADHD in adults has received little research interest with relevance to addiction treatment-related clinical psychology. The initial experience suggests that it is a complicating factor that may impair compliance with treatment and its outcomes. AIMS: To test the specific effects of ADHD on psychological resilience, stress management, emotional lability, and life skills in general among clients of therapeutic communities (TCs) for addicts. SAMPLE: An experimental study will be used to conduct a thorough psychological examination of a representative sample comprising approximately 170 clients of eight selected TCs in the Czech Republic. METHODS: The test battery comprises specific ADHD diagnostic tools and methods intended to check for any other conditions. The clients will be divided into two groups: (A) with the ADHD diagnosis and (B) without it. Clients in treatment will then be examined using a specially designed battery in order to show a higher rate of expected signs and complications in Sample A and to understand and describe their impact on treatment. RESULTS: The study should clearly show whether we can effectively and reliably diagnose the ADHD in this group of clients and whether this client population may really differ from other clients as regards their behaviour and responses in specific areas. It should also be demonstrated whether ADHD does indeed have a negative impact on the process and outcome of addiction treatment. CONCLUSION: The successful implementation of the project should provide opportunities for targeted development and testing of a specific intervention that will increase the chances of the successful completion of treatment and its better outcome. Background: The psychology of behavior change and addictive substance users' motivation to get treatment is a topical issue in addictology. The reasons why patients enter treatment vary; they have different goals, and they differ in the degree to which they are able to reflect these goals. To understand addictive behavior, it is necessary to pay attention to all aspects that determine it, i.e. to recognize the abilities, opportunities and motivations of their behavior and their interaction. The topic of patient motivation at the very beginning of the institutional treatment process of addiction, i.e. when entering detox, was not given enough attention in the Czech Republic, despite the fact that detoxification is in many cases the first, indispensable step in starting medium-term and long-term residential treatment of addiction, and work with motivation is a method of direct work with patients. The topic of my bachelor thesis is Hagiotherapy and its benefits regarded by women who underwent institutional alcohol addiction treatment in Psychiatric hospital Bohnice and who abstain now. Hagiotherapy is a psychotherapeutic method, its name comes from Greek "therapeia" - care, treatment and "hagios" - saint, pure, holy. Hagiotherapy is a form of bibliotherapy which uses Holy Scripture as a mirror. Unlike the speech-based therapy ordinarily used among psychological means it carries a strong expressive context consisting in parallels from chosen biblical stories and the therapy is also based on the therapist himself who uses those stories and appropriate situational rituals. The aim of my thesis is to find the possible benefits of hagiotherapy for alcohol addicted women during their institutional treatment and our aim is to determine whether the women consider the method beneficial and effective in relation to their life in abstinence. In the theoretical part we first clarify some basic terms connected with addiction and its treatment. This thesis focuses on the eating habits of drug users. The aim of the study is to compare changes in eating habits among clients in therapeutic communities before entering into treatment with the subsequent state of eating habits in the first month of treatment and in the third month of treatment. Data were obtained in therapeutic communities completing food frequency questionnaires, which monitor the weight of clients and consumption of major food groups - grains, vegetables, fruits, dairy products, meat, eggs, legumes, sweets and tea. Questionnaires were administered immediately after entering treatment, to obtain retrospective information on eating habits before treatment. Clients completed the questionnaire after the first and third months of addiction treatment. Based on the analysis data obtained from the questionnaires, it was found that the average weight among treatment clients grows. Larger increases were recorded for the clients, women by an average of 2 points on a scale of BMI. The data show that consumption of commodities, cereals, fruit, dairy products and sweets among the periods before treatment, in the first month of treatment and in the third month of treatment decreased. BACKGROUND - This thesis works on the assumption that one's own experience with smoking cessation along with the habit-breaking patterns that one gains from this experience, might have a positive influence on addiction treatment. It is assumed that these patterns help client with the overall treatment, the relapse prevention, motivation issues and continued abstinence. AIMS - The aim of this thesis is to investigate the effect of smoking cessation experience and the role it plays in the therapeutic community. I also focused my research on the clients' perception of the tobacco using in relation to their health, life and complete abstinence concept. METHODS - Both current smokers and ex-smokers as well as clients of two therapeutic communities were subjected to qualitative analysis. Purposive sampling method was used to select the respondents for semi-structured interview. Content analysis and partly interpretative phenomenological analysis were used to process the collected data. In this thesis I deal with the influence of personal lived experience of ex-users on their therapeutic work with the clients in addiction treatment. The personal experience in this case is meant an experience with the substance abuse or playing on the slot machines which led to addiction. For this work it is also important the personal experience of following treatment of addictive behavior by ex-uers themself. In the theoretical part I address the former career of ex-user in addiction services, the role of ex-user in the therapeutic team, the motivation of ex-user to work with addicted clients, but also the opportunities of his education and anchoring the work of ex-user in the Standards of services for drug users and pathological players. As the goal of the practical part I set myself to describe how the ex-user's own experiences with addiction and its treatment participate when working with clients. This Bachelor thesis is a sub research of the wider project focused on the impact of ADHD on selected mental functions, life skills and quality of life of the clients recovering from an addiction in the therapeutic communities. ADHD is associated with inferior adaptability and as well with more serious psychopathological symptoms. The aim of the research is to determine whether ADHD occurs in clients of the therapeutic communities and what is the prevalence. Furthermore, we search for connections with other serious clinical manifestations and evolution of these symptoms during treatment. We are also interested in retention of the clients with these symptoms in treatment process. The sample was compiled on the basis of the two-step procedure. Fundamental file in the first phase were all certified therapeutic communities for the addictions treatment. Among them were by computer randomly selected eight who participated in the research. At the second level was used a total choice of new coming clients and all of them were tested. The number of participants who filled their gender is 199. 133 men and 56 women. Title: Application and Verification of a Self-defence Program for Patients in a Psychiatric Hospital Objectives: The aim of this thesis is application of a self-defence program, created in a bachelor thesis, during an addiction treatment in Červený Dvůr, a psychiatric hospital. The thesis surveys the suitability of the educational program during addiction treatment, verification of acquired motoric skills and its possible integration in patients' free time, recommendation of the exercise to friends, and last but not least it surveys a possible interest in the program in other psychiatric hospitals. Methods: There are several methods used in the thesis. Content analysis is used for working with literature, interview is used in the conversation held with the department head and the custodian. Observation is used for watching the participants doing the exercise. Comparing is used for evaluation of the final outputs. Opinion poll is used for finding the asset of the educational program. In the thesis I focused on a process of residential treatment in a therapeutic community. Such a process is mostly divided into four stages in all therapeutic communities in the CR where different needs and tasks are required from clients. The procedure in the treatment process is evaluated by the group as well as by staff. The fulfillment of criteria in each phase is a condition needed for shifting the clients in the treatment process towards successful completion of treatment. These phases are clearly defined by therapists and clients are acquainted with them through the operating manual. The aim of the thesis is to describe the process of treatment and common elements with regard to how it is perceived and experienced by the clients during the treatment in the therapeutic community WHITE LIGHT I. Rituals can be described as ceremonial acts, that have typical rules and own significance. They have their place not only for substance abusers within therapeutic communities, but in a daily life also. Usually, rituals are bonded to traditions of a specific community and they provide a possibility for an individual to identify with this community. They provide also strong emotional experience and they concern spiritual parts of the individual as well. The main objective of this study is to provide a possible view of clients of each stages of final ritual in specific therapeutic community and to find out how it is perceived and valued in a case of a given attitude on their process of own treatment. The elaborated work is focused on emotions which are experienced by clients, whether and in a case how they sense the person, to whom is the ritual addressed, and how they sense therapists and objects which are used during the ritual. This work puts in aim to describe some of the specifics in working with adolescent drug users. It describes a profile of a problematic drug user and the working system in the only one specialized facility targeted on problematic drug users treatment, Cesta Řevnice. This work follows the history, structure and the program in this facility. It closely studies therapies such as individual and group therapy, profession consultancy and outdoor activities. In the second part of work are analysed the commonest abused drugs used by teenagers comming to Cesta Řevnice, characteristics of these drugs and its acute and also long-term effects on their users.

This work focuses on the concept of art therapy in Aftercare center, in the service of dealing with follow-up care for people who are recovering from addiction, especially on non-alcohol drugs. Given topic is treated in the theoretical and practical. The theoretical part of the paper summarizes the basic definitions related to drug addiction and dependent personality description, followed by an after-care services and focuses on the principles and goals of treatment of addiction. In the treatment of addiction is the default selected using psychotherapy and expressive psychotherapy - art therapy. Art therapy is described as a field, along with historical context, the definition of guidelines that define the field and determining the place of art therapy in the treatment of addiction. The introduction of practical work is described in detail Aftercare center Prevent, focusing on the course and composition of the art therapy program and representation in it. Then are presented work in the background in art therapy Aftercare center Prevent. The main place of work is to present specific case studies of clients with analysis and interpretation of artifacts that illustrate ways of working within art therapy group with respect to the method Rožnov art therapy. The conclusion summarizes the specifics of the use of art therapy in the type of equipment described and obvious benefits for the field of art therapy.

This thesis is focused on phenomenon called relapse. Specifically relapse in drug users who have an experience with after-treatment. In the theoretical part, I provide the view on drug addiction, treatment, after-treatment, PREVENT after-treatment centre and relapse itself. The empiric part contains target of research that deals with getting more information about facts related to relapse, how it is reflected by the addicted during the treatment and after it. It is used the method of semistructurated interviews whose transcription I consequently analyzed in accordance with the embedded theory. The analysis went first on the open coding level, later on the axial coding level.
