Coronavirus disease 2019 (covid-19) is an infectious disease caused by a novel coronavirus, named severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The mortality increases in patients older than 60 years. Younger patients with underlying serious chronic diseases are endangered by a fatal course of this infection as well. Cardiovascular manifestations of SARS-CoV-2 infection are diverse and include acute coronary syndromes, myocarditis, heart failure, arrhythmias, pericardial effusion, and thromboembolic complications. Direct and indirect myocardial injury is common in moderate and severe disease course. Troponin elevation is a marker of worse prognosis. [ABSTRACT FROM AUTHOR]

Diabetic kidney disease (DKD) is a common complication in type 1 and type 2 diabetics that can lead to terminal stages of renal failure in many patients. At the same time, the presence of DKD significantly increases the risk of cardiovascular morbidity and mortality, as well as the risk of hypoglycemia. Inhibitors of the sodium‑glucose transporter type 2 (SGLT‑2 inhibitors, gliflozins) are a relatively new class of antidiabetics for which prospective studies have shown a significant reduction in cardiovascular complications as well as significant nephroprotective effects. In this article, we summarize the clinical effects of gliflozins with a focus on their nephroprotective effects, possible mechanisms of this action and other perspectives of their use in the treatment of not only diabetic kidney disease. SGLT2 inhibitors - gliflozins - are a novel group of antidiabetic drugs whose action is independent of insulin secretion from pancreatic beta cells. In addition to affecting hyperglycaemia without a risk of hypoglycaemia, they contribute to reducing weight and hypertension. Large randomized clinical trials of these antidiabetic drugs have not only confirmed their cardiorenal safety, but also have shown other major benefits. In type 2 diabetic patients who more often present with cardiorenal syndrome, atherosclerotic cardiovascular disease, renal involvement, and increased risk of heart failure (also seen in non-diabetics), these drugs have a protective effect. Currently, their importance is appreciated not only in secondary, but also in primary prevention of cardiorenal involvement, and, along with GLP-1 analogues, they are recommended as drugs suitable for intensification of antidiabetic therapy second only to metformin and as the only possible oral antidiabetic agent for intensive insulin therapy of type 1 diabetes. Kidney transplantation represents a treatment of choice for end stage renal disease (ESRD) patients. ESRD is associated with several comorbidities and the long-term survival of patients treated with dialysis has been limited. The aim of pretransplant work-up in kidney transplant candidates is to eliminate surgical morbidity and mortality and to identify patients at risk for immunosuppression related diseases. In ideal situation, kidney transplantation should be performed just before the start of dialysis therapy. The evaluation of cardiovascular system and ischemic heart disease screening have been routinely performed before the placement on the waiting list, however, the benefit of this procedures have not been verified in prospective randomized trials. Urological work-up is important in anuric patients in whom disorders of lower urinary tracts are obvious with normal dieresis. Similarly, chronic infections should be treated before patient placement on the waiting list. Successfully treated malignancies have not represents the contraindication for transplantation if the patients wait for a defined period of time without the disease recurrence. After HLA antigen and panel reactive antibodies determination, patients are placed on the waiting list. The waiting time depends on blood group and HLA match. In the Czech Republic, patients wait for transplantation for about 1,5 years. When placed on the waiting list, patients should be routinely screened for concomitant diseases that may influence the outcome of kidney transplantation. [ABSTRACT FROM AUTHOR]

Diabetes mellitus is an important risk factor for the development of heart failure and presence of diabetes significantly worsens heart failure outcome. Introduction of gliflozins to the therapy of heart failure is one of the most important novelty. Gliflozins reduce glucose level by the sodium-glucose contransporter 2 inhibition in proximal tubulus in the kidney. Gliflozins are used as effective antidiabetic drugs with improvement of glycemic control without risk of hypoglycemia, gliflozins decrease blood pressure and patients weight. Recent studies have shown that gliflozins significantly reduce risk of cardiovascular complications and heart failure hospitalizations in diabetic patients. Clinical trials with dapagliflozin and empagliflozin have shown reduction of the risk of cardiovascular death and heart failure hospitalization in the patients with heart failure and reduced ejection fraction both in the patients with diabetes and in the patients without diabetes. The aim of the expert consenzus is to summarize practical aspects in the cooperation of cardiologist and diabetologist in the management of the patients with heart failure and reduced ejection fraction in the context of the current guidelines and other treatment options.

Cardiovascular complications are main cause of increased mortality in patients with type 2 diabetes. Decrease of overall cardiovascular risk and subsequently cardiovascular morbidity and mortality in type 2 diabetes patients is therefore an important treatment aim. To this end, intensive intervention of classical risk factors such as dyslipidemia, arterial hypertension, smoking along with lifestyle intervention is necessary. Good diabetes control optimally with the use of antidiabetic medication and smoking with positive effect on cardiovascular complication is of high importance as well. Incretin-based therapy includes an approach based on an increase of endogenous GLP-1 concentrations by inhibition of its breakdown by dipeptidyl-peptidase 4 (DPP-4 inhibitors or gliptins) or he use of GLP-1 receptor agonists that owing to modified structure have much longer half-life than endogenous GLP-1 and act the use through stimulation of GLP-1 receptor. The aim of this paper is to summarize the use of these two groups of antidiabetic drugs - gliptins and GLP-1 receptor agonists - in patients with type 2 diabetes focusing on the their cardiovascular effects and their influence on cardiovascular complications.
