HEALTH insurance companies , *HEALTH insurance costs , *INPATIENT care , *MEDICAL care costs , *OUTPATIENT medical care , *PSYCHIATRIC nursing - Abstract
The cost of psychiatric care reflects the psychiatric, psychological and psychotherapeutic care provided by all segments of the system, i.e. outpatient, acute inpatient and follow-up inpatient care. Health insurance companies pay for outpatient care and psychopharmacology treatment prescribed by prescription. The costs of mental health are not the final cost to insurance companies, that go into psychiatric care. At present there is no record of the amount consumed services, especially psychotherapy, that are paid directly by patients. Other direct costs to patients go towards the purchase of over-the-counter products that are used to alleviate various types of mental health problems. Total costs spent by health insurance companies on psychiatric for psychiatric care in 2020 was approximately 16 billion CZK, with the largest share of financial of the funds goes to providers of follow-up care (CZK 7 billion). For medicinal products, health insurance companies paid approximately CZK 3 billion. In the framework of the gradual changes related to the psychiatric reform of the reform of psychiatric care, there will be changes the proportion of funds spent on individual areas. Currently, it can be stated that, there is an increase in all segments, but in no case is there shifting of funding to the detriment of any other area. Highest increase has been recorded in the outpatient segment psychiatric care, with the largest increase in the outpatient sector compared to the previous year 2017 increased by 44%. CHILD psychiatry , *ADOLESCENT psychiatry , *MEDICAL students , *PATIENT-professional relations , *MEDICAL school graduates - Abstract
Introduction: There has been a low interest in pursuing career in Psychiatry and Child and adolescent psychiatry among medical school graduates. The aim of our study was to identify factors that influenced choice of these medical disciplines among psychiatric trainees. Method: In this qualitative study we conducted semi-structured interviews with purposively sampled trainees in Psychiatry and Child adolescent psychiatry programs in the Czech Republic. An analytical framework for this study is based on the concept of dynamics of career choice among medical students. Results: Out of 21 respondents there were 14 trainees in Psychiatry program (8 women, 6 men) and 7 in Child and adolescent psychiatry program (4 women, 3 men). The length of their training was between 4 months and 5 years. We identified 15 factors, that were cathegorized into 4 groups: characteristics and values, career needs during decision making proces of specialty choice, psychiatry and child and adolescent psychiatry perception and factors of remaining in profession. Conclusion: Motivating factors were oportunity for working with patients based on communication and therapeutic relationship; complexity of psychiatry that includes somatic, pharmacological and psychological care as well as social and forencis aspects. From a practical point of view psychiatry seemed to allow better work-life balance and was viewed as less stresfull with more time available for doctor-patient interaction, including diagnostics and treatment. Specific factor for child psychiatry was relationship to children, which motivated respondents for entering medical school. Trainees in Child and adolescent psychiatry appreciated spontaneity and playfulness of children and early developmental phase of life with a chance for behavioral change. There were also pragmatic reasons, namely chance for finding a job. Another reason was opportunity for intelectual development as there are many possible topics for research. De-motivating factors included societal prejudice, which was indentified in parents of half of respondents, regardless of their profession. Students were exposed to stigmatizing comments by academic staff after declaring their interest in psychiatry during medical school. Fundamental problem was lack of information about the existence of Child and adolescent psychiatry as a unique speciality with training available right after graduation and practically no exposure to this medical speciality during medical school. The bachelor's thesis "De-escalation of aggressive patient behavior in the psychiatric ward" deals with the issue of aggression, its prevention and effective solution. The aim of the work was to determine the frequency of aggression, its type, cause and method of solution. Another aim of the work was to find out the frequency of trainings that the respondents completed and their interest in a possible offer of training focused on managing aggression. The theoretical part of the thesis provides a basic overview of the issue, a description of de- escalation techniques and other options for managing aggression and its prevention. The empirical part of the work presents the results of own research and their comparison with the results of other professional research. The method of the survey was a self-designed questionnaire, which was distributed to general and practical nurses of the Psychiatric Hospital Bohnice. 108 duly completed questionnaires were included in the study itself. Results: It was found that more than 58% of respondents experience verbal aggression at least once a week and almost 29% of respondents experience brachial aggression at least once a week. Due to such a frequent occurrence of aggression, only less than 59% of respondents have been trained in the issue at least once in the...

- Abstract
Kyrgyzstan is one of the republics of the former Soviet Union, situated in the Central Asia. Mass riots related to unsatisfactory political and economic situation have occurred several years ago. The number of physicians related to the total population is about half as compared to the Czech Republic. They mostly work in big cities, whilst there is a lack of physicians in rural areas. Only the basic medical care is paid for by the state, otherwise the patient should cover it, or ask nongovernmental organizations for help. The health care system is financed insufficiently. Former Soviet mental hospitals, which have not been reconstructed for decades, create the base of the inpatient mental health care in Kyrgyzstan. Simultaneously, modern methods of care like outpatient departments, day hospitals, sheltered housing and sheltered works are being developed. The Mental Health Care Law was enacted in Kyrgyzstan in 1996. The number of psychiatrists, re-calculated to the total population, is about one-third compared to the Czech Republic. Only several preparations out of the main psychotropic groups are covered by the state and available, another possible treatment should be paid for by the patient. Brain imaging is rarely used in psychiatry, more demanding methods are not available at all. Psychotherapy is not applied very much, if yes, it is usually performed in an inpatient setting. Similarly to other countries in the world, psychiatry and mental disorders are significantly stigmatized. Psychiatry is not preferred as a job by young physicians, somatic branches of medicine are considered as more attractive. At an international level, psychiatry in Kyrgyzstan is particularly connected to Russia. The government as well as health care professionals make efforts to apply mental health care in accordance with world standards, but this is a demanding and long-range process. [ABSTRACT FROM AUTHOR]

Theoretical basis: Therapeutic restrictions or limitations are often taboo, although they are also part of child psychiatry. It is used for restless, aggressive patients who endanger the health of themselves or others. Therapeutic limitations use must follow given regulations. The role of the nurse is irreplaceable and important in these situations. Goals: The aim of this work was to chart the issue od therapeutic restrictions in children psychiatry department. Four research questions were used to accomplish goals. Questions are: What kind of restraints are used in the children psychiatric ward? How is the safety of pediatric pacient ensured during the therapeutic restriction in children psychiatric ward? Are there some specifics in nursing care for the patient with therapeutic restrictions? How are nurses educated in the issue of therapeutic restriction in pediatric psychiatric department? Methods: The empirical part of the work was processed in the form of qualitative reseacher. Data were collected as a semi-structured interviews. Another metod of data collection was covert observation. Based on this data collection, there were created five categories and eleven subcategories. Reseacher file: The research group was consisted of twenty three nurses, who work in the department of child and adolescent psychiatry. The selection of patiens was purposeful. Results: Research proved that restraints are an integral part of child psychiatry. The most commonly used therapeutic restrictions are courts. Respondents have shown that the work is according to standards and adhere to the principles in terms of safety. The research has shown that proper nursing documentation and education are required. Research has shown that nurses act professionally, they also take care about patient needs and privacy. It turned out that nurses are on better level of knowledge about practical use than a knowledge of theoretical nursing standard. Conclusion: Therapeutic limitations of patients in the pediatric psychiatric ward cannot be neglected or omitted. This thesis could serve as an educational materiál for many nurses, especially if they are new to the field. Continuing education in this area is desirable for nurses. This work could serve as an educational material for the general public, who could possibly encouter therapeutic limitations, for example with their own child.

Current situation: Suicide is one of the main problems in the public health field. This thesis focuses on the role of a nurse in the care of suicidal patients. The theoretical framework concentrates on the origination of this particular situation, the risk factors, how to prevent suicidal behaviors, how the nurse manages this conduct and monitors the state of the patient, how to create a safe environment and how to intervene in a crisis situation. Objectives of the thesis: First, to study the different responsibilities of the nurse in the duty of care of suicidal inpatients in the psychiatric wing under maximum safety. Second, to explore the diverse security restrictions used during the treatment of this type of patients. Methodology: The research is based on a survey of eighteen questions in which all the participants had the opportunity to answer to open questions. These eighteen questions are also divided into eight categories and, at the same time, these eight are classified into different subcategories. Analysis of the collected data was processed using the pencil-paper method, the so called coding. Research group: Thirteen nurses selected from the psychiatric wing under maximum safety. Results of the thesis: From this investigation could be assumed that the role of a nurse in the care of suicidal patients is a crucial factor. The medical treatment is not enough, but it is important to include the family circle. The conditions are not always optimum. Recommendations include focusing on the development of education and awareness, also medical management so that nurses can devote time to the patient with these problems, and a technical equipment such as a surveillance system, which is not always available for every medical center. Practical use: The present final thesis could be used as a case of study for nurses and nursing students. The results could be published in scientific magazines and conferences.

This thesis explores the position of Person-Centred Approach (PCA) in the practice of Czech clinical psychologists and psychiatrists who went through the PCA psychotherapy training and they use this approach in their current clinical practice. The main principles of PCA theory (1), principles of biomedical model in which clinical practice is rooted (2) and the new forms of person-centred psychopathology and psychodiagnosis (3) are the theoretical background of the following research. Based on the qualitative analysis of five recorded interviews (three clinical psychologists, two psychiatrists) the experience of the respondents is organised as comprehensive concepts concerning various areas and topics of the clinical practice (diagnosis, psychotherapy, dealing with PCA, dealing with biomedical model, therapeutic relationship, relation to the profession). Several common mechanisms are identified in the concept's dynamics. Through these mechanisms Person-Centred Approach is applied in areas and activities mentioned. The concepts based on experience and the PCA application mechanisms enrich the theories and ongoing research of PCA practice in the Western mental health care. Ethical aspects of involuntary hospitalization of psychiatric patients My thesis is a brief introduction to the topic of ill-defined ethical situations with patients, who are hospitalized involuntarily at a psychiatric ward. My work seeks support in valid laws and also in clerical documents so it can detect complexities that doctors as well as patients must face. Involuntary hospitalization seems like a marginal phenomenon, but by taking a closer look we can find out that annually there are thousands of people hospitalized involuntarily in the Czech Republic alone. After defining initial basic concepts my work shortly addresses history of care of mentally ill patients, legislature and its evolution, and current situation, which it strives to connect to clerical documents that deal with human liberty and dignity. This thesis is aware of changes within the frame of reform of psychiatric care that is taking place in the Czech Republic at the moment and it offers only limited insight into the issue as a whole. Theme of the bachelor thesis is a utilization of quantification of nursing care in psychiatric hospitals. Psychiatry provides a treatment to persons with mental disorder. In most cases there is nobody to help the patient with his problems. So he searches some specialiste, in this case a psychologist or a psychiatrist. The doctor determines hospitalization of the sick man if the patient is not able to visit an ambulant treatment. For these patients there is performed a basic and a special nursing care. The basic nursing care is composed of feeding, hygiene, etc. The special nursing care provides diagnostic, terapeutic and monitoring operations. The thesis has theoretical and scientific parts. The theoretical part deals with discipline of psychiatry, with form and methodes of care, with legislative definition and with education and competences of nurses. The scientific research was realized by methode of quantification of nursing care by Pochylá and Pochylý (2008), it is a qualitative and quantitative methode. The first part deals with collecting of datas and subsequent recording datas to a record sheet. Next step was to determine activities of special nursing care. There were determined averages and standard deviations for operations thereby we gained average periods of all operations. The purpose of the thesis was to determine average time difficulty of all operations and to determine time values of nursing care of selected groups of patients. The research finds that the nursing care is the most used operation for sick men. Nursing care changes in patients according to the current state of health. Other specifics of nursing care include monitoring patients who may endanger themselves or their surroundings. The most often performed operation in psychiatric hospital in medication is per os. We chose 3 goals for the job. The first objective was to inventory special nursing care in psychiatric hospitals. Another objective is to determine the average time values for individual nursing activities of special nursing care in a psychiatric hospital. The final goal was to determine the time values of special nursing care in selected patient groups. Based on the first goal, a research question has been identified: What is the most common performance in a psychiatric hospital? Based on the second objective, the research question was determined: What are the average time values for individual nursing activities of special nursing care in a psychiatric hospital? Based on the third objective, the hypothesis was chosen: The time values of special nursing care are different for selected patient groups.

The aim of the submitted thesis is description of the contemporary form of art therapy in the czech psychiatric facilities. Historical development of the approach to people suffering from mental illnesses is also outlined with respect to their art work. The investigation included visits of seven ateliers of psychiatric hospitals and interviews with local art therapists about the form of their art therapeutic approach, about the art therapeutic work in the psychiatric hospital and about the rhythm of the atelier. The results were processed by means of the case study and completed with direct observation findings. Particularly, the forms of Roznov intervention art therapy were compared. Overall image of the art therapeutic practice in the Czech Republic is completed by information from websites of all psychiatric inpatient facilities in the Czech Republic. Common signs of the visited ateliers (i. e. long-term working of one art therapist, exhibition of the patient's art works) are pointed out in the results as well as their differences (i. e. different education of art therapists, spatial conditions of the ateliers, sets of art work themes). Art therapy has longstanding tradition in the czech psychiatric environment which develops individually around current art therapists in the particular psychiatric facilities. Also Roznov art therapy approach found its place and modifications in this environment. From the outside view, integrity in the therapeutic approach, by extension in art therapists's education is missing as well as the establishment of art therapist as a profession.

Basic theoretical background The theoretical part of the thesis explores the issue of supervision and focuses on psychiatric nursing. It is divided into several parts. The first part is focused on supervision, briefly characterizes supervision as such, then it describes its divisions, functions and methods. It also explores the participants of supervision, the agreement on supervision, transcultural supervision and Balint groups. The following part describes historical development of psychiatric nursing, briefly presents the personality of Joyce E. Travelbee. In the conclusion of the theoretical part of the thesis the problems of education and competence of nurses in psychiatric care are introduced, marginally mentioning the Turku declaration. The aim of the work The main aim of the thesis is to investigate the awareness of nurses working in psychiatric care of the clinical supervision options. The task of the second objective is to find out the opinion on supervision of nurses working in psychiatric care. The third objective is focused on means to assist nurses working in psychiatric care in their personal development and growth within their practices. The purpose of the fourth objective is to find experience with supervision of nurses working in psychiatric care. The methods used Qualitative investigation was chosen for the empirical part of the thesis. The method used for data collection was in the form of semi-structured interview. The interviews with the respondents were taken down using a voice recorder and transcribed, qualitative data was then analyzed by pencil and paper encoding technique and semantically organized into 27. groups of categorization. The selection of a research sample was intentional, consisting of twelve nurses working in psychiatric care in the Czech Republic. Results The research focused on nurses who do not have personal experience with supervision revealed that they have certain awareness of the supervisory process, however not quite sufficient to be able to describe the nature and function of supervision in its entirety. Their knowledge is rather theoretical. They understood supervision as help to solve disharmonious relationships in the workplace, as well as it could improve communication skills of the nurses, it would also assist in dealing with crisis intervention and prevent burnout syndrome. The results regarding nurses who do not participate in the supervisory process clearly indicated that they have insufficient support and the opportunity for both their professional and personal growth in their workplace, while this important could be solved by supervision. The research, which covered only nurses who have had personal experience with supervision and regularly participate in it, showed that their knowledge and skills related to the supervision process are quite sufficient to be able to understand and use supervision practically in the whole range. These nurses sees supervision as a benefit that would allow them not only to understood themselves, but also to learn how to manage challenging situations that are a part of their profession. Supervision gives them confidence, provides support and teaches them to deal with people who are part of their work process. Supervision has become a tool for preventing burnout and showed them how to use their own potential effectively. Conclusion This thesis is focused on the use of supervision in psychiatric nursing. Working with mentally challenged patients is very demanding, therefore supervision plays its justified role there. If you do not know yourself and you are not able to solve your own problems, you can never effectively and comprehensively address the problems of others, who are often desperately waiting for your help. The results will be presented at professional seminars designed not only to nurses working in psychiatric care.

This bachelor thesis is focused on the history of nursing care for the mentally ill in the Czech Republic and Finland. It is written chronologically from ancient history to the present time. The whole thesis summarizes the development of nursing for the mentally ill, the system of care and education for nurses in psychiatry in the Czech Republic and Finland. The aim is to describe the development of nursing care for patients with mental disorders. The work deals with the system of mental health care and education for nursing staff in these countries. This thesis is theoretical. It was necessary to visit the Prague City Archives and the National Archives to obtain historical sources. Other important sources used in this thesis were professional literature, literature dealing with the issue in the present and historical books. In past, people thought that mental disorders are natural phenomena. In ancient times, the temples were established for mentally ill people. During that time, Greek physician and philosopher Hippocrates had great significance. In the Middle Ages, people with mental disorders were considered to be possessed by evil spirits, so treatment was mainly in isolation. In modern period, the first insane asylums were built. In the twentieth century psychiatry was influenced by Sigmund Freud with his theory about the origin of mental illness. The turning point of this time was also in the use of shock therapy and psychopharmacological discovery. Psychiatry deals with the development and strengthening of mental health and its prevention. The Czech Republic has a system of interconnected services in psychiatry, such as psychiatric clinic, clinical-psychological clinic, inpatient facilities and community care. Psychiatry in our country is under-funded, care is unsatisfactory and development is delayed compared to other countries of the European Union. Strategy for the reform of mental health care in our country has a goal to increase the quality of life of people with mental disorders. The current state is that outpatient psychiatrists are overloaded, and there is materially and technically outdated equipment in psychiatric hospitals. Our psychiatric care has much more institutional position, because it failed to systematically develop community psychiatry. But perspective is quite dense network of psychiatric and clinical-psychological clinics, which could help in moving the focus of care from inpatient facility into the natural environment of the client. The reform will also try to improve the education of nurses in psychiatry. According to Czech legislation, there is not a condition to be specialized in nurses working in psychiatry. Nurses, in the best possibility, should have university education that enables students to acquire the necessary knowledge for the profession. Even competencies are determined. Finland has a health care system based on taxes. Important medical centres provide prevention, medical examinations and screenings. Specialized care is provided in hospitals. Formerly more institutional care dominated, however today outpatient and community services greatly extended their service. Psychiatric hospitals were abolished, leaving only the psychiatric departments of general hospitals. Nursing education in Finland is carried out at the Universities of Applied Sciences. The nurse's role is health promotion, disease prevention and rehabilitation. The nurse in Finland is independent and responsible for the decisions made during the day which also supports the Code of Ethics for Nurses which is the last chapter of this thesis. The aim of the thesis is to map the development of nursing care for patients with mental disorders. One of the major findings is a significant evolution of psychiatry, when, at any time, psychiatry was viewed from different perspective. The work may serve as study material for students in medical fields, for professionals or for anyone interested in the history of psychiatry nursing.

Psychophysiological research of bipolar disorder reveals physiological correlates of each episode of the disorder. Neuroimaging methods come with findings of abnormal brain corticolimbic activity, deficit in cognitive functioning, including senzorimotor processes, and dysregulation in balance of sympathetic and parasympathetic activity. Differences are observed in bipolar disorder type I and type II and also in gender of patients. Knowledge about physiological processes may be enforced in treatment and therapy of bipolar disorder, including biofeedback. Powered by TCPDF (www.tcpdf.org)

The aim of the dissertation is to treat psychiatry and mental illness from the anthropological point of view. The topic is treated at three levels. First, I describe psychiatry as a specific culture's product, and I explore how it constitutes itself as an unbiased science. I conclude that in spite of developing its scientific aspect, its human-science aspect is neglected, stuck in the 19th century's paradigm of evolutionism. Although there is a number of sources of a new less scientistic and ethnocentric and more reflexive paradigm nowadays, it still seems to be more a vision than reality due to the institutional toughness of the present paradigm. Second, I study "mental illness" as an abstract concept. I ask if the Western psychiatry's classification is universal and I explore various culturally specific forms of mental illness and its conceptualizations. Third, partially grounded in my fieldwork, I ask how abstract psychiatric terminology is embodied in a concrete human being and how the sufferer's point of view differs from the one of the professional in the mental health care. The dissertation intends to show that despite all the three levels being interlocked, their unity is not self-evident but is always constructed and contested.

Diploma work "Mental Ilnes in the beginning of institualization czech psychiatry in 19th and 20th century" inform about slowly change in view on Madness phenomen in Europe history. This change from religious to medical care is very clear in the period if Enlightenment. Reflexion of this change from religious to medical care is also vivid in czech environment in 1869-1914. In my aproach to this problem were very important the medicals reports from Mental Sanatorium in Kosmonosy (this reports are from 1908-1910). In this materials I could study this medical turn in medical care about madmen. Diploma thesis deals with the issue of community care in psychiatry and its objective is to describe the importance of a nurse in providing this care. Community psychiatric care is a very wide area of an intermediary patient assistance designed to help the patients in any areas of their lives. Although in the Czech Republic community care has not achieved the appropriate development yet, its benefits for patients are important now and its importance has been growing. In the theoretical part, after a brief introduction to community care issues, history, principles, but also community care connection with nursing care, are mentioned. Subsequently, attention is paid to particular areas of community care that are important for the mentally ill and the role of a nurse in these areas is described. The approach to the mentally ill, the issue of stigmatization and psychiatric care organizations are mentioned. In the theoretical are part, psychiatric illnesses that may occur in community care are also analyzed and it is explained how community care may be beneficial for psychiatric patients. In the practical part, the objective was to find out the level of psychiatric nurses´ awareness of community care and to determine if they see the sense of community care in psychiatry. Making a survey of community care in which nurses can be involved was another objective of the work. The nurses´ responses to the stated hypotheses were statistically analyzed within quantitative research. The purpose of hypotheses stated was to determine whether nurses with the work experience longer than ten years are more likely to believe that psychiatric care is more beneficial for patients than hospitalization., And also, if nurses with an education higher than secondary have better awareness of community services provision. Other hypotheses were focused on finding out whether nurses consider the most common community care services issue in the Czech Republic to be housing promotion and if nurses aged over thirty years are more aware of the nurses´ role importance in community care. None of those hypotheses were confirmed. The hypothesis assuming that nurses obtain more information on community care from the Internet and literature than at workshops was confirmed. Psychiatric nurses working in mental homes in the South Moravian region and the Vysočina region took part in this research. A component of the practical part is also an analysis of community services in these regions. In both the regions 13 civic associations and community centers were mapped.

This bachelor work deals with problems of an integrated rehabilitation system at the persons with mental diseases. The integrated rehabilitation system is actually a present-day conception of the rehabilitation. The rehabilitation is solely not understood only as a medical activity in the sense of physiotherapy, but as an inter-disciplinary domain containing therapeutic, social, pedagogical and working components. An aim of the integrated rehabilitation system is to make possible for the people with mental diseases to reach or to keep an optimal level of all functions. To provide them means so they could achieve a higher living standard and means to integrate them into an active life in the society. For a wide spectrum of the mental diseases I concentrated on persons with the diagnosis of schizophrenia, obsessively compulsive alienation and a dementia of Alzheimer type. In the theoretical part I devote to the characteristics of concepts - the integrated rehabilitation, health, mental health, psychiatry. Further I describe causes, characterization, cure, therapy and social impacts of individual mental diseases. Other chapters I mention in my work are focused on the integrated rehabilitation system with particular means, a legal anchorage and a social work with the mentally ill people. For an elaboration of the experimental part of the bachelor work I chose a quantitative research combining processes of qualitative and quantitative methodology. The chosen technique is a standardized questionnaire for workers in psychiatrical hospitals and a technique of the contentual analysis of data from the medical documentation of persons hospitalized in psychiatrical hospitals. The research set was made up by 52 workers in the psychiatrical hospitals and 6 people with chosen mental diseases. As the main aim I determined to analyze a situation in the area of the integrated rehabilitation system at the people with mental diseases in psychiatrical hospitals. To describe how individual factors of the integrated rehabilitation system come through and to find out what kind of knowledge the respondents about the integrated rehabilitation system have. From the results of my work follows this conclusion: The rehabilitation of the people with mental diseases, hospitalized in mental hospitals, is aimed to a medical factor of the rehabilitation. Workers taking care of the people with mental diseases have not any concrete imagination about the integrated rehabilitation system.
